Purpose: To report a case of bilateral horizontal gaze palsy associated with pontine hemorrhage. Methods: A 51-year-old man developed a sudden decrease in consciousness. Computed tomography reveal a pontine hemorrhage and infarctions of both basal ganglia. The mentality was recovered after conservative treatment, but he complained of difficulty in moving his eyes. We performed ophthalmic and radiologic evaluations to confirm a diagnosis of bilateral horizontal gaze palsy and to reveal the condition. Results: On ophthalmic examination, his eye showed complete bilateral horizontal gaze palsy, but convergence and vertical eye movements were normal. Radiologic evaluation revealed a pontine hemorrhage, and the hemorrhagic area was consistent the bilateral paramedian pontine reticular formation (PPRF).

Purpose: Iris cyst is a rare condition and there has been no Korean report of bilateral iris cyst. The authors present the first Korean report of a patient with bilateral iris cysts. Methods: A 62-year-old man presented with intermittent visual field disturbance of the left eye when exposed to bright light. The patient underwent visual field examination and slit lamp examination. On slit lamp examination, a dark-brown mass was revealed along the lateral pupillary margin in the left eye. In the right eye, the shape of the iris was convex at the 8 o`clock direction. Ultrasound biomicroscopy, confirmed the presence of a mass. Results: The patient showed no specific abnormality in visual field examination. Ultrasound biomicroscopy showed the objects to be cystic, with a thin-walled homogenous hypoechoic lesion on the posterior surface of the iris in both eyes. There were two cysts in each eye measuring 4.35 mm×1.13 mm and 1.47 mm×1.11 mm in the left eye, and 1.05 mm×1.02 mm and 1.90 mm×1.05 mm in the right eye. Conclusions: Asymptomatic iris cyst does not need to undergo treatment, but careful examination is nevertheless required required at regular intervals in case a complication develops.

Vehicle aerodynamic performance is developed using both full-scale wind tunnel as well as quarter scale model at Hyundai-Kia Nam_yang R&D Center. The model wind tunnel is a relatively useful tool compared to the full-scale wind tunnel especially when the aerodynamic performance of a new vehicle is being predicted during the styling stage for a new vehicle. Model scale test can provide valuable aerodynamic data well before the prototype or pilot vehicle construction stage if the geometry and Reynolds similarities are considered properly for the scale model. The prediction accuracy of vehicle aerodynamic performance depends on the detailing of the test model such as underbody and engine room geometry.
An Experimental Study on the Aerodynamic Characteristics around Moving Bodies near the Ground

The objective of present study is to compare the flow characteristics around moving bodies near the ground. The influence of the gap height and ground conditions, the moving and fixed grounds, on the aerodynamic forces as well as the dynamic behavior of the wake flow around the square cylinder near a ground is investigated. The model wind tunnel experiments were carried out in an open circuit low turbulence subsonic wind tunnel. The wind tunnel is equipped with moving belt system to simulate the ground effect. The mean velocity and the turbulent field around a square cylinder were measured using a two-component Particle Image Velocimetry (PlV). From the velocity profiles in wake region, the pressure recovery on a moving ground decreases due to the high momentum supply, finally causing the drag to increase from the fixed ground, while the lift has opposite pattern. Through the full scale wind tunnel testing, aerodynamic force difference occurs under ground conditions and increases as yaw angle increases. Therefore the adequate ground simulation is necessary to enhance the control stability under the transient crosswind.
망막정맥폐쇄 환자에서 유리체내 베바시주맙 주입술의 2년 임상 결과

Purpose: The authors evaluated the 2-year clinical results of intravitreal bevacizumab injection in retinal vein occlusion (RVO). Methods: Thirty-two eyes of 32 patients treated with an intravitreal bevacizumab injection of 1.25 mg (0.05 ml) for RVO (branch RVO: 22 eyes, central RVO: 10 eyes), repeated 3 times at a 6-week interval and were available for a follow-up period of at least 2 years were retrospectively reviewed. Best-corrected visual acuity (BCVA) before treatment and 6, 12, and 24 months after 3 serial injections, was recorded. The optical coherence tomography (OCT) results were analyzed to identify prognostic factors for recurrent macular edema that needed reinjection. Results: Two years after the treatment, mean BCVA was significantly improved (<em>p</em> = 0.000). Out of 32 eyes, 16 (branch RVO: 8 eyes; central RVO: 8 eyes) needed reinjection because of recurrent macular edema. In central RVO, a significantly high reinjection rate was shown in serous retinal detachment (SRD) compared with cystoid macular edema (CME) as identified in OCT findings (<em>p</em> = 0.049). Additionally, in branch RVO, a high reinjection rate was shown in SRD, although statistically not significant (<em>p</em> = 0.375). Conclusions: In patients with RVO, a significant visual improvement was maintained for at least 2 years after intravitreal bevacizumab injection. 서울지역 설사환자로 부터 분리된 Shigella flexneri의 성상과 유전적 특성

The shigellae are common etiological agents of bacillary dysentery in humans and primates. During four years from 2002 to 2005, 22 strains of Shigella spp. were isolated from the diarrheic patients in Seoul region. All of them were identified as S. flexneri by biochemical tests and serotyping. The prevalence of serotypes were variable by year, but the major serotypes were 2a and 3a. In an antimicrobial susceptibility test, all of the isolates were resistant to streptomycin and tetracycline, and susceptible to amikacin, kanamycin, cefoxitin, and gentamicin. All of the isolates showed the multi-resistant patterns over 3 drugs. By analysis of the plasmid profile the isolates were classified into 7 groups (P1~P7). Serotypes 2a and 2b were distributed to P1, P2, P3, and P4. Serotype 3a was differentiated to P5 and serotype 3b, to P6 and serotype 4a, to P7. PCR results showed that all isolates were positive for two virulence genes, ipaH and ial, but none of the strains had stx gene. The set1A and set1B genes were detected from 12 isolates (54.5%) that belonged to serotype 2a and 2b. The sen gene was detected from 19 isolates (86.4%). The 22 isolates showed 12 to 17 DNA fragments in the sizes ranging from 20.5 kb to 1135 kb, resulting in 13 patterns by the PFGE with Not I digestion. The PFGE patterns of the isolates showed the close relation with the serotypes, but no relations with year of isolation and antimicrobial resistance.
서울지역 집단식중독 환자에서 검출된 노로바이러스의 유행양상

Fecal specimens from acute gastroenteritis in Seoul from 2003 to 2007 were collected and then tested for the presence of Norovirus by RT-PCR. Among a total of 4,685 samples investigated, 383 samples (8.2%) were positive. The analysis of outbreaks related norovirus contamination occurred from 2003 to 2007 in Seoul revealed 57 cases happened during investigated period. Seasonal prevalence showed winter season predominant characteristics of pattern of epidemics by long term investigation for norovirus infections. The incidence of norovirus infection in the case of acute gastroenteritis by catering food in school were 32%, by food in general restaurant were 29%. This epidemiological investigation in Seoul was strongly needed for control and prevention of outbreaks related with norovirus by forecasting disease epidemics. 2003년부터 2007년까지 서울지역 집단식중독 환자에 대하여 RT-PCR을 이용하여 노로바이러스를 검출하였으며, 총 4,685건 중 노로바이러스가 383건(8.2%)이 검출되었다. 식중독 발생 사례는 2003년부터 2007년까지 57사례가 발생하였다. 계절별 유행양상은 겨울철에 가장 많이 검출되었다. 섭취장소별 유행양상은 학교급식 중 위탁경영에 의한 발생이 32%로 나타났으며, 일반음식점에 의한 발생이 29%로 나타났다. 본 연구는 서울지역 노로바이러스성 식중독 관리를 위한 기초자료로 활용될 수 있을 것으로 기대된다.
망막정맥폐쇄와 당뇨망막병증에서 황반부종 치료를 위한 유리체내 베바시주맙 주입술의 효과

Purpose: To evaluate the long-term results of intravitreal bevacizumab injection for macular edema (ME) due to retinal vein obstruction (RVO) and diabetic retinopathy (DR). Methods: The objects of study were patients with decreased visual acuity due to ME with RVO and DR for whom intravitreal injections of 1.25 mg (0.05 ml) bevacizumab were repeated three times with an interval of six weeks and who were available for a follow-up period of more than 12 months. The patients underwent additional bevacizumab injections if ME increased as assessed by optical coherence tomography (OCT). Best-corrected visual acuity (BCVA) and central macular thickness (CMT) were measured at baseline and follow-up visits. Results: There were 16 patients with RVO and 18 patients with DR. In the RVO group, the mean length of follow-up was 12.4±1.1 months, the mean baseline BCVA was 0.75±0.32 and the final BCVA was 0.42±0.25, a difference that was statistically significant (p<0.05). The mean CMT at baseline was 588.5±301.0 μm and this decreased to a mean of 191.8±112.0 μm at the end of the follow-up period (p<0.05). In the DR group, the mean length of follow-up was 15.4±3.2 months, the mean baseline BCVA was 0.63±0.33 and the final BCVA was 0.61±0.37, a difference that was not statistically significant (p>0.05). The mean CMT at baseline was 462.0±195.0 μm and decreased to a mean of 282.2±177.3 μm at the end of the follow-up period (p<0.05). Conclusions: In RVO and DR, three injections of intravitreal bevacizumab with an interval of six weeks and additional injections as indicated were effective in reducing ME and showed especially good results in improvement of visual acuity for ME due to RVO.
