        KAERI 부지 주변의 환경선량 측정을 위한 온라인 감마선량 감시시스템

        환경감마선량 변동을 감시하기 위해 한국원자력연구소 부지 주변에 온라인 감마선량 감시망을 구성하였다. 모니터링 포스트에 설치된 전리함 검출기에 의해 측정된 자료는 무선전송방식으로 중앙통제소에 전송되어 실시간으로 감시된다. 무선전송 방식은 자료의 전송, 처리 및 자자에 신뢰도가 높고 경제적이었다. 감시시스템은 급격한 선량증가시 조기경보체제와 연결할 수 있게 하였다. On-line gamma monitoring system around KSERI-site was set up to monitor the radiation fluctuations in environment. Data on gamma exposure rates measured by the ionication chamber in the monitoring posts are transmitted to a computer of central control station with radio telemetry transmission and reliable on handing and storing of data. This monitroing system can triger an early warning system in the event of abnormal radiation levels.

        Partial Sequence Analysis of Puumala Virus M Segment from Bats in Korea

        Hantavirus is a genus of the Bunyaviridae family causing two serious diseases, hemorrhagic fever with renal syndrome (HFRS) and hantavirus pulmonary syndrome (HPS). Puumala virus is a member of hantavirus originally found in Europe, and its natural reservoir is Clethrionomys glareolus. It is also associated with the human disease nephropathia epidemica, a milder form of HFRS. To identify the hantaviruses in bats, bats were collected from Jeong-Sun, Won-Joo, Chung-Ju and Hwa-Cheon area in Korea, and nested RT-PCR was performed with serotype specific primer from M segment. Interestingly, Puumala virus was detected in bats (Rhinolophus ferrum-equinum) only from Won-Joo. The 327 bp nested RT-PCR product, was sequenced. The sequence database search indicates that the sequence is homologous to the published sequence of Puumala viruses. The sequence similarities were ranged from 71% to 97%. The highest sequence similarity was 97% with Puumala virus Vranicam strain, and the lowest was 71% with Puumala virus K27 isolate. Puumala virus Vranicam strain was isolated from a bank vole (Clethrionomys glareolus) in Bosnia-Hercegovina. Puumala virus K27 was isolated from human in Russia. This analysis confirms that bats (Rhinolophus ferrum-equinum) in Korea are natural reservoir of Puumala virus.

        Adult Patients with Congenital Muscular Torticollis Treated with Bipolar Release : Report of 31 Cases

        Objective : We assessed the surgical results of bipolar release in 31 adult patients with uncorrected congenital muscular torticollis (CMT) and more than 12 months of follow-up. Methods : Thirty-one patients underwent a bipolar release of the sternocleidomastoid muscle (SCM) and were retrospectively analyzed. The mean follow-up period was 14.9 months (range, 12-30). The mean age at time of surgery was 30.3 years (range, 20-54). Patients were evaluated with a modified Lee's scoring system, cervicomandibular angle (CMA) measurement, and a global satisfaction rating scale using patient self-reporting. Results : The modified Lee's scoring system indicated excellent results in 4 (12.9%) patients, good in 18 (58.1%), and fair in 9 (29.0%) at the last follow-up after surgery. The improvements in neck movement and head tilt were statistically significant (p<0.05). The preoperative mean CMA was $15.4^{\circ}$ (range, 5.4-29.0), which was reduced to a mean of CMA of $6.3^{\circ}$ (range, 0-25) after surgery (p<0.05). The global satisfaction rating scale was 93.7% (range, 90-100). A transient sensory deficit on the ipsilateral lower ear lobe was noted in three cases. No significant permanent complications occurred. Conclusion : Bipolar release of the SCM is a safe and reliable technique for the treatment of CMT in adults.

      • 병원급식 체계에 관한 조사 연구

        This study was conducted to evaluate the hospital foodservice management practices in Seoul and to suggest the guideline for the effective foodservice management. Questionnaire survey method were used in this study. Questionnaire were administered to dietitians in twenty hospitals. Data were analyzed for frequency, percentage, χ^(2)-test, and one-way ANOVA using SAS PC Package program. The results were summerized as follows : The number of dietition were less than the required number for efficient foodservice. Centralized foodservice method was adopted in most hospitals and conventional foodservice system was taken in all the hospitals. The space of a kitchen and a dining room were confined and sanitary facilities and working condition revealed problems. Equipped facilities in surveyed foodservice operations were assessed as the insufficient condition, especially, efficient facilities such as a cutting machine were scarecely make up.

        남성의 클래식 재킷원형에 관한 연구 : 20대 초반의 남성을 중심으로 For ages of the early in the twenties

        The purpose of this study is to improve the classic jacket pattern for Korean young men whose chest circumference is 91-94cm, status is 170-175 cm, and drop value between chest and waist circumference is 15. The study was carried out by the following procedures. 1. Through the comparative investigation on the conventional classic jacket patterns of Hyung-sook Lee's, :文化's in Japan, and SMA's and the clothing experiment, the characteristics of every three patterns were found. 2. According to correct the unsuitable point in Hyung-sook Lee's pattern which was found more suitable than other two patterns and to refer to the product size of the ready-made classic jackets for men, the new suitable classic jacket pattern was achieved. 3. The sensory evaluation for appearance and comfort and one to one comparison was applied to evaluate the new classic jacket pattern by comparing with the Hyung-sook Lee' s jacket pattern. From the result of the sensory evaluation and one to one comparison, it was found that the new classic jacket pattern was more suitable for appearance and comfort. 4. From the result of this study, the suitable surplus of men's classic jacket at present is about 23∼24cm on the chest circumference, 31∼32 cm on the waist circumference, 14∼15 cm on the hip circumference, 13∼14 cm on the upper arm circumference, and 14 cm on the wrist circumference and the ease is about 3.5 cm on the armhole circumference. This size is not the absolute size but it helps to make the surplus in any other clothes for Korean young men.

        Omental infarction, the end result of impaired perfusion to the greater omentum, is a rare benign self-limiting clinical entity. The main clinical symptom is non-specific localized abdominal pain with a moderately raised white blood cell and erythrocyte sedimentation rate. These findings often mimic an abdominal surgical emergency. This condition is often misdiagnosed as acute appendicitis or cholecystitis. The characteristic feature of CT scan and ultrasonography provide non-invasive diagnosis in most patients with omental infarction. We report a case of patient whose CT scan showed the characteristic finding of omental infarction. The patient was improved spontaneously only with conservative care.

      • 임신 제 일삼분기와 제 이삼분기 초음파의 태아기형 진단의 유용성에 대한 고찰

        Objective: Ultrasonographic screening between 9 and 13 weeks for fetal structural abnormalities using transvaginal sonography in unselected population. Visualization of detailed fetal anatomy in the first trimester has improved as a result of technological progress in ultrasound machines and the introduction of transvaginal sonography(TVS). Our study aims to evaluate the accuracy of TVS as a screening tool for detecting fetal malformations in a nonselected population of pregnant women. For this purpose we compared the results obtained by using second trimesster TA scanning with those obtained using TVS at 9-14 weeks' gestation. Methods: From March 2001 to February 2003, 620 unselected consecutive pregnant women were recruited at their initial visit at Soonchunhyang university hospital, department of obstetrics and gynecology and offered a TVS scan at the average gestational age of 9-13 weeks in addition to the TA screening usually performed between 22-26 weeks. To fulfill the technical requirements of a screening test, ultrasonographic examinations were performed by skillful operators with different degrees of experienced using adequate instruments Aloka SSD 5500 (Aloka Co. Ltd., Tokyo, Japan) provided with either a 3.5-5MHz TA probe or a 5-7.5MHZ TV probe) in a fixed scanning time of maximum 30 min, generally considered sufficient for a complete fetal anatomic survey. Results: During the study period we found in our unselected population an overall prevalence of target abnormalities of 2.2%(24 malformed fetuses out of 600 screened). In the fetuses examined by TVS, 16 abnormalities were detected whereas 8 were not; of these, 6 were detected at transabdominal rescreening at 22-26 weeks and the remaining 2 were observed after birth. A nuchal region abnormality was the most frequent type of malformation detected in the early scan, including six cases of nuchal edema, two cases of cystic hygroma associated with hygroma associated with hydrops, three cases of hydronephrosis, one case of fetal neck teratoma, one case of cleft lip. Conclusion: This study has demonstrated the potential of screening a low risk population for fetal abnormalities at 9-13 weeks of gestation using transvaginal sonography and rescreened via transabdominal sonography at 22-26 weeks. Larger studies are required to establish the clinical value of the first trimester scan.
