Odontomas are the most common type of odontogenic tumor. Odontomas are further subdivided into compound and complex categories. Compound odontomas are composed of multiple, small tooth-like structures. On the other hand, complex odontomas consist of a conglomerate mass of enamel and dentin that bears no anatomic resemblance to a tooth. Odontomas are treated by simple local excision, and the prognosis is excellent. The purpose of this study was the treatment of odontomas by surgical excision and tooth eruption of a successive permanent tooth.
Three types of ossifying fibroma: A report of 4 cases with an analysis of CBCT features

Ossifying fibroma is a slow-growing benign neoplasm that occurs most often in the jaws, especially the mandible. The tumor is composed of bone that develops within fibrous connective tissue. Some ossifying fibromas consist of cementum-like calcifications, while others contain only bony material; however, a mixture of these calcification types is commonly seen in a single lesion. Of the craniofacial bones, the mandible is the most commonly involved site, with the lesion typically inferior to the premolars and molars. Ossifying fibroma of the jaw shows a female predominance. Some reports of ossifying fibroma have been published in the literature; however, this report continues the research on this topic by detailing 3 types of ossifying fibroma findings on panoramic radiographs and cone-beam computed tomographic images of 4 patients. The radiographs of the presented cases could help clinicians understand the variations in the radiographic appearance of this lesion.

This study aimed to compare the re-mineralization effects of the new fluorine formulations of the 5% sodium fluoride adhesive film (NaF AF) and 38% silver diamine fluoride (SDF) with conventional 5% sodium fluoride varnish (NaF V), and whether the re-mineralization effect of each fluoride formulation is affected by the depth of the initial enamel caries. The re-mineralization effect of the SDF group was significantly greater than those of the NaF V and NaF AF groups (p<0.05). The NaF V and NaF AF groups showed a significantly greater re-mineralization effect than did the control group (artificial saliva) (p<0.05), and there was no difference between those two groups. Also, in the NaF V and NaF AF groups, there was no significant difference in the degree of re-mineralization depending on the depth of the initial enamel caries, but in the SDF group, a significantly greater amount of re-mineralization was observed in deep lesions than in shallow lesions (p<0.05). SDF is expected to expand the range of fluoride for the prevention and treatment of dental caries that were limited to initially shallow caries.
Forced eruption with customized resin tube appliance

There are various forms of dental trauma that occur in childhood. In the subgingivally crown-root fracture case, forced eruption has been depicted as one of the most conservative treatments. Generally, the forced eruption is done by using wires, power chains, elastic threads, or using removable appliances and elastic. However, due to the characteristics of pediatric patients, it is difficult to obtain the patient's cooperation and maintain clean oral hygiene status. As an alternative, forced eruption using MTA (Mini-Tube Appliance) can be proposed. This case report describes two cases of subgingivally crown-root fracture, which were treated with modified MTA to form a customized resin tube to perform a treatment more aesthetic, rapid, and stable.

Biliary atresia is a rare disease, and it is defined as partial or total absence of the biliary tract. If not treated, this disease may cause death within 5 years. Treatment methods for biliary atresia include conservative methods and surgical methods, such as a Kasai operation or/ and liver transplantation. Oral symptoms associated with biliary atresia are green pigmented teeth, enamel hypoplasia, rampant caries, enlargement of the pulp chamber and root canals, and delayed eruption of teeth. Among these symptoms, green pigmentation of teeth is the most significant and unesthetic complication affecting a child`s social psychology. The majority of case reports about green pigmented teeth caused by biliary atresia showed that it affected the deciduous dentition rather than the permanent dentition. This case report describes the dental management of a patient with biliary atresia, who had undergone liver transplantation at age two years, and presented with green pigmentation in both deciduous and permanent dentition.
Removal of a fractured needle during inferior alveolar nerve block: two case reports

The inferior alveolar nerve block is the most common method of local anesthesia for intraoral surgery at the posterior mandibular region. However, unexpected complications may occur when administering the local anesthesia. One of these uncommon complications is the fracture of the needle. If the injection needle is broken during the surgery, it should be removed immediately. However, this is one of the most difficult procedures. In this report, we present two cases of needle fracture during the procedure, and its successful removal under general/local anesthesia administration.

Subcutaneous emphysema occurs when air is forced into the fascial planes of the connective tissue leading to sudden onset of swelling. Although uncommon, a dental treatment using an air-drive handpiece and compressed air syringes has been reported as a causative factor of the subcutaneous emphysema. Due to its low prevalence, dentists often overlook the potential for developing the disease and thereby miss the opportunity for early treatment. This case report presents an occurrence of iatrogenic cervicofacial subcutaneous emphysema subsequent to maxillary primary incisor pulpectomy in a patient with brain damage after cardioplegia.

Epidermolysis Bullosa (EB) is a rare genetic disease that causes pain and results in peeling of the skin and mucous membranes, as well as easy blistering due to minor trauma or friction. EB is classified into four types according to the layer where the tissue is desquamated and blisters form. There are three genetic disorder-type EB diseases, namely, epidermolysis bullosa simplex (EBS), junctional epidermolysis bullosa (JEB), and dystrophic epidermolysis bullosa (DEB), as well as an acquired autoimmunity disorder. DEB shows autosomal recessive or dominant inheritance, and is characterized by deformities in the skin including coalescence of the fingers, blisters, scars, abnormalities of the fingertips, and milia. In extreme cases, a narrow esophagus or airway, ankyloglossia, microglossia, or squamous cell carcinoma may be seen. Dental problems associated with EB include microstomia or ankyloglossia, caused by scarring due to repeated blistering, as well as a high risk of dental caries due to difficulty in controlling a toothbrush (in turn due to hand and finger scarring). Therefore, continuous regular examination and preventive care are important. In this case report, we describe the dental treatment applied under general anesthesia for rampant caries in a 9-year-old patient with DEB who visited Chosun University Dental Hospital.

저자는 입술 및 혀에 대해 자해증상을 보이는 2명의 Lesch-Nyhan 증후군 환아에서 구강조직에 대한 자해행동을 예방하기 위해 soft mouth guard를 이용한 보존적 치료법과 전악발거의 침습적 방법을 이용하여 치료한 증례를 보고하는 바이다. Lesch-Nyhan 증후군 환자의 자해행동을 예방하기 위한 치료법은 아직 정립된 기준이 존재하지 않아 각 환자의 자해양상 및 정도에 따라 치료방법은 달라질 수 있다. 가능하다면 치아를 보존할 수 있는 장치 또는 BTX-A 제재 등을 이용한 보존적 방법을 먼저 시도해 보는 것이 좋다. 이 때 치료의 성공률을 높이기 위해서는 치과적 접근 이외에도 소아정신과와의 협진을 통한 정신심리학적 접근, 필요시 약물치료를 고려하는 것 등 여러 방향으로 접근하는 것이 더욱 바람직한 결과를 가져올 것으로 사료된다. 보존적 접근이 실패할 경우 더 이상의 조직상실과 전신적 감염 예방을 위해 극단적 방법이지만 전악발거를 고려해야 할 것이다. Lesch-Nyhan syndrome is a rare X-linked recessive genetic disorder. During purine metabolism, the hypoxanthine guanine phosphribosyl transferase (HGPRT) enzyme is deficient causing phosphoribosyl transferase to accumulate and resulting in excessive uric acid. Clinical symptoms include hypercalcemia, choreoathetosis, spasticity, mental retardation, and self-injury to lips, tongue or fingers. This results not only in pain caused by the self-injury but also secondary infection of the wound site and the esthetic damage of the soft tissue defect. Dental treatments include conservative methods using intraoral appliances such as soft mouthguards, fixed lip bumpers, and occlusal guards, and invasive methods such as extraction of all teeth or forming an artificial anterior open bite. We report two cases of Lesch-Nyhan syndrome patients with self-mutilation behavior; one was treated with a preservative method using a soft mouthguard, and the other was treated with extraction of all teeth.
