        기저질환이 없는 사람에서 발생한 Campylobacter fetus에 의한 재발성 심내막염 1예

        C. fetus 심내막염은 드물게 발생하는 질환으로 주로 면역능이 저하된 환자에서 발병하고 대동맥판을 침범하는 경우가 많으며 적절한 치료가 되지 않을 경우 치사율이 높다. 저자들은 기저질환이 없던 69세 남자에서 C. fetus 심내막염을 경험하였기에 문헌고찰과 함께 보고하는 바이다. 환자는 발열로 내원하여 시행한 혈액배양 검사상 C. fetus가 동정되었고 경식도 초음파 검사상 삼첨판에 증식증이 발견되어 심내막염 진단하에 ceftriaxone에 이어 meropenem으로 총 4주간 항생제를 투여받고 호전되었다가 한달 후 다시 재발하였고 판막부전 소견 진행하여 삼첨판 치환술을 시행받고 전신상태 호전되어 퇴원하였다. Campylobacter fetus is a rare cause of endocarditis. To date, only about 30 cases of C. fetus endocarditis have been reported mainly in the elderly with chronic underlying illnesses. Most cases showed tendency to involve aortic valve. This report describes a case of native tricuspid valve infective endocarditis in a 69-year-old man without any underlying disease. Despite adequate antibiotic therapy for 4-weeks, endocarditis recurred and leading to surgical valve replacement.

      • VHDL을 이용한 유도전동기의 속도제어

        최근에 VHDL을 이용한 ASIC 회로설계 기술이 개발되면서 전용 프로세서의 설계가 용이하게 되어 많은 분야에서 응용되고 있다. 본 논문은 VHDL을 이용한 ASIC 회로설계 기술을 유도전동기의 송도제어에 응용하는 연구로서 유도전동기 구동시스템의 제어부를 효율적으로 설계하고 one-chip으로 구현하는데 목적을 두고 있다. 이를 위해 유도전동기의 속도제어 방식인 슬립제어의 동작레벨을 기술하고 시뮬레이션하여 동작을 확인한 후, 회로를 합성하고 최종적으로 FPGA를 이용하여 속도제어 ASIC을 제작하였다. 본 논문에서 제시된 유도전동기의 속도제어 시스템은 시뮬레이션과 실험을 통하여 그 타당성을 입증하였다. The ASIC chip design for the speed control of an induction motor has been a subject of increasing interest since an effective system-on-a-chip design methodology is developed. This paper investigates the design and implementation of the ASIC chip for the speed control of an induction motor using VHDL which is a standarded hardware description language. The presented system is implemented using a simple electronic circuit based on FPGA. The overall drive system for the speed control of an induction motor is verified through the simulation and experiment.

        Ion exchange if the most reliable process for removing ionic impurities and is the economic operation with reuse of the resin. Ion exchange is widely used in water and wastewater treatment, especially softening and demineralization. Ion selectivity depends on the hydrated radius, charge of ions and ionic concentration. The objective of this study is to determine the selectivity order of cations with equilibrium and column ion exchanges, and to investigate the effect of the background anion on selectivity. Cation selectivity increases with decreasing concentration and increasing charge(H^+<Na^+<Cs^+<Mg^2+<Cu^2+) in equilibrium and column cation adsorptions.

        Hydrazine is a weak base and strong reducing agent in the aqueous solution and is primarily utilized as a high-energy rocket propellant and an oxygen scavenger in boiler or feedwater. The objective of this study was to investigate the physicochemical properties and reactions of hydrazine and the catalytic and thermal decomposition by the emperature change. Hydrazine was fast decomposed with the of lower activation energy and at the higher temperature.

      • 자궁탈의 임상적 고찰

        The purpose of this study was to evaluate clinical aspects of prolape uteri. This clinical and statistical study was attempted to analyze 369 cases of uterine prolapse(328 cases -operated, the other cases - not operated) from Jan. 1994 to Dec. 1998 at the Department of Obstetrics and Gynecology, Chosun University Hospital. The results were as follows. 1. The incidences of uterine prolapse was 0.62% of the total admitted gynecologic patients during the study period. 2. Age distribution at the time of operation showed that 184 cases(25.6%) were 60 to 65 years of age, and 62 cases(18.2%) were 56 to 60 years of age. 3. Age distribution at the time of development of prolapse uteri showed that 134 cases(36.3%) were 21 to 30 years of age, 126 cases(34.1%) were 31 to 40 years of age. 4. 228 cases(61.7%) were those who had suffered from uterine prolapse for more 11 years and the mean durations were 15.6 years. 5. The degree of prolapse uteri showed that 281 cases(76.1%) were 3rd degree and 61 cases(16.5%) were 2nd degree. 6. The parity showed that 297 cases(80.2%) were over 4 times and 2 cases(0.27%) was nullipara. 7. The delivery place was at home in 318 cases(86.1%) and in hospital in 51 cases(13.9%). 8. The main symptoms comprised prolapse in 311 cases(84.2%), urinary frequency in 259 cases(70.1%), and bearing down sensation in 242 cases (65.8%). 9. An operation in 328 cases was carried out of 369 cases of prolapse uteri, 301 cases were treated by the vaginal hysterectomy with either anterior &osterior repair or posterior repair, 23 cases were treated by anterior &osterior repair. 10. The postoperative complications were urinary tract infection in 7 cases(2.1%), urinary retention in 4 cases(1.2%), vaginal vault prolapse in 2 cases(0.6%). 11. The indwelling foley catheter was kept for 7 days in 279 cases and the average postoperative hospitalization was around 9 days. 12. The postoperative pathologic findings were chronic cervicitis in 211 cases(67.8%), squamous metaplasia of the cervical gland in 98 cases(31.5%), adenomyosis in 69 cases, leiomyoma in 47 cases, and dysplasia of cervix, cervical polyp, endometrial polyp were also resulted. In conclusion, clinical features of uterine prolapse were studied. The clinical evaluation of uterine prolapse include age, parity, degree of prolapse, delivery site, age of onset, symptomatology, type of operation, complications, the results of histopathological examinations. Further study is necessary before these aspects can be implicated in the clinical evaluation of uterine prolapse.

      • A case report of continuous veno-venous hemodialysis (CVVHD) in acute methanol poisoning

        Methanol is a common ingredient in many products such as illicit liquors, windshield washer fluids, and homemade alcoholic beverages. Whether accidental or intentional, the ingestion of methanol can be potentially fatal if not diagnosed and treated promptly. Methanol is oxidized first by hepatic alcohol dehydrogenase to formaldehyde then to formic acid, which results in acidosis, neurotoxicity and death in severe poisoning. Here reported is the case of a 46-year-old Korean woman who survived methanol intoxication. She was presumed to have ingested methanol six hours before arriving at the emergency department. She was drowsy with a Glasgow Coma Scale score of E2V4M5. She was afebrile with a blood pressure of 103/73 mmHg, a pulse rate of 77 beats/min and a respiratory rate of 24 breaths/min. Initial arterial blood gas showed a pH of 7.180, bicarbonate of 12.3 mmol/L, PaCO2 of 33.0 mmHg and base excess of -14.9. Her serum osmolar gap was 249 mOsm/kg H2O and the serum anion gap was 21.7 mmol/L. Based on the history and the blood test, we diagnosed the methanol intoxication and promptly initiated continuous veno-venous hemodiafiltration (CVVHDF). CVVHDF was performed for 39 hours using the Prismaflexi system (Gambro Lundia AB, Sweden) on the first day of admission at the following settings: blood flow, 150 mL/min; replacement volume, 1,000 mL/h; and dialysate, 1,000 mL/h. Two days after hospitalization, we stopped CVVHDF after the anion gap normalized. On day 5, the patient was discharged without any complications. We received the result of methanol concentrations in serum and urine from our reference laboratory when the patient discharged. The analysis detected high concentrations of methanol in blood and urine and high concentrations had decreased in both serum and urine during CVVHDF. In conclusion, our case supports that CVVHDF enhances the elimination of serum methanol by a decrease in half-life. Keywords: Methanol; Hemodiafiltration; Acidosis

        국내 건설회사의 지속가능성장률 분석을 통한 건설 산업 특성에 관한 연구

        The Ministry of Construction and Transportation announced the Earned Value Management Since the global financial crisis caused by US sub-prime mortgage turmoil in Sep. 2008, construction industry is under serious difficulties in the management at present about two years to the financial crisis, which are matured PF(Project Financing) loans and at least 50 trillion won of locked up funds by unsold houses, due to the industrial characteristic declining more rapidly than other industries in recession. For this difficulty shown in construction industry, it established the causes not only the depression in construction market overtaken followed by the global financial crisis but also the business strategy of local construction companies excessively weighted towards external growth. In today’s rapidly changing society, such business strategy overly focused on growth only may threaten even the corporate sustainability. Therefore, local construction companies must meet the changes in customer demands and the market and they must focus on the establishment of efficient strategies through the corporate strategic management at the same time. Based on this background, this study analyzed the sustainable growth rate with accounting information of 70 construction companies listed to the market of Korea Stock Exchange during the period from 2005 to 2009 and then analyzed the characteristics shown in those companies classified into the good company group and the bad company group for the corporate activity of construction companies by which the reasonable growth can be maintained while the growth-oriented strategy is still valid.

        전해질 성분 및 농도, 이온교환 수지 비율에 따른 이온교환 특성 연구

        본 연구에서는 전해질 성분 및 농도, 이온교환 수지 조성비율을 통해 이온교환 수지탑의 성능을 평가하고, 이온교환 수지에 의한 입자성 물질 제거의 능력과 입자성 물질이 이온교환 특성에 미치는 영향을 조사하였다. 이온교환 수지탑 배열 순서에 따라 파괴(돌파)시점이 연장되며, 파괴(돌파)순서는 음이온의 경우 Cr< NO₃- < F, 양이온의 경우 Na+<K+<Ca2+순 이였으며, 전해질의 농도가 증가할수록 파과시간이 단축되었다. 이용교환 수지의 조성비 변화에서는 양음이온교환 수지 조성비가 1:2의 경우 1:1 및 1:3의 경우보다 파과{돌파)시간이 연장되었으며 동일한 전해질 농도에서 입자 농도가 증가하면 20% 미만으로 파과(돌파)시간이 단축되었다. 양이온 교환수지 비율이 높으면 양이온의 파괴{돌파)시점이 늦어져 이온교환 수지탑의 수명이 연장되고, 입자성 물질은 전해질 농도와 무관하게 이온교환 수지의 세공을 막아 이온교환 용량을 감소시켜 파과{돌파)시간을 단축시키는 것으로 조사되었다. The object of this study was to investigate the influence of composition and concentration of electrolyte, ratio of cation to anion exchange resin of mixed ion exchange column in the performance of ion exchange. Also this work examined the removal capability of suspended solids by ion exchange resin and the effect of particule on the characteristics of ion exchange. Breakthrough time was extended as the amount of ions and particles present in liquid was decreased. The case of anion, the breakthrough sequence is CI<NO₃-<F, but the case of cation, the breakthrough sequence is Na+<K+< Ca2+. As for the ratio of cation to anion exchange resin of 1:2, the breakthrough time was prolonged compared with that of 1: 1 and 1 :3. For the electrolyte of equal concentration containing suspended solid, breakthrough time was contracted less than 20%. It results in the increase in the removal capacity of cation exchange resin. For the higher ratio of cation exchange resin, suspended solids are shorten the cation’s breakthrough time so that the runtime of ion exchange resin tower is increased.

This paper propose strategic operation plans for efficient global outsourcing by switching buyer-supplier competition (zero-sum) to mutually beneficial(positive-sum) buyer supplier relationship. In conclusion, this paper suggested the needs of cooperation for strengthening the core abilities and the strategic plans of supporting small & medium construction company. and Research method is that there is summary of current status and future of SCM(Supply Chain management) and "How to develop on the Supply Chain management in construction" related housing construction industries
