        구개열에서 비인두강의 생리해부학적 구조와 과비음과의 연관성 연구

        비인강폐쇄란 연구개, 인두측벽 그리고 인두후벽간의 움직임이 서로 조화되어 구강과 비강을 나누어주는 괄약근 기전으로서 연하, 호흡, 발음 등의 생리적기능에 중요한 역할을 한다. 이 기능에 문제가 생긴 경우를 비인강폐쇄부전이라하며 그 원인으로는 (1) 연구개의 길이 및 움직임이상 (2) 비인두강의 해부학적 공간문제 (3) 인두 후벽과 측벽의 기능이상 등이 있다. 본 연구는 구개열 환자의 측면두부방사선계측사진을 통해 비인두강을 생리해부학적으로 분석하였으며 산출된 말소리의 과비음정도를 Nasometer로 평가하였다. 이로부터 얻은 정상군과 구개열환자군의 결과를 각각 비교하였으며, 비인강폐쇄부전과의 연관성을 알아보기 위하여 Anatomic VPI와 Nasalance score의 값을 비교분석하였다. 얻어진 결과는 다음과 같았다. 1. 측면두부방사선계측사진 결과, 연구개 길이, 연구개 두께, 비인강 깊이, 비인강 면적, Adequate ratio에서 두 그룹간 유의한 차이를 나타내었다. 2. Nasometer 결과. 모음/오/와 구강공명음문장, 구강장해음문장에서 두 그룹 간 유의한 차이를 나타내었다. 3. 구개열환자군에서 비인두강의 폐쇄부전 정도를 표현해주는 Anatomic VPI와 Nasalance score는 전반적으로 연관성이 없었다. 다만, 모음/이/와 일부 구강자음으로 이루어진 문장에서 다소의 상관성을 나타내었다. 결론적으로, 측면두부방사선계측사진과 Nasometer 각각의 검사결과에서 두 그룹간 유의한 차이를 찾아볼 수 있었으나, 구개열환자군내에서 비인강폐쇄부전을 표현하는 Anatomic VPI와 Nasalance score는 모음/이/와 구강자음을 포함한 문장을 제외하고는 전반적으로 연관성이 없었다. Velopharyngeal closure is a sphincter mechanism between the activities of the soft palate, lateral pharyngeal wall and the posterior pharyngeal wall, which divides the oral and nasal cavity. It participates in physiological activities such as swallowing, breathing and speech. It is called a velopharyngeal dysfunction when this mechanism malfunctions. The causes of this dysfunction are defects in (1) length, function, posture of the soft palate, (2) depth and width of the nasopharynx and (3) activity of the posterior and lateral pharyngeal wall. The purposes of this study are to analyze the nasopharynx of cleft palate patients using cephalometry and to evaluate the degree of hypernasality using nasometry to find its relationship with velopharyngeal dysfunction. The following results were obtained : 1. In cephalometry, there were significant differences in soft palate length, soft palate thickness, nasopharyngeal depth, nasopharyngeal area, and adequate ratio between two groups. 2. In nasometry, there were significant differences between two groups in vowel /o/ and sentences including oral consonants. 3. In cleft palate patients, though no general correlation was found between Anatomic VPI and nasalance scores, vowel /i/ and sentences including oral consonants were slightly correlated. In conclusion, cephalometry and nasometer results were significantly different between the two groups. Though in the cleft palate group. Anatomic VPI and nasalance scores, which are indices for velopharyngeal closure, excluding the vowel /i/ and sentences including oral consonants show generally no significance.

        구개열환자에서 비인두공간과 비인강폐쇄부전과의 연관성

        비인강폐쇄는 연구개, 인두측벽 그리고 인두후벽간의 움직임이 서로 조화되어 구강과 비강을 나누어주는 괄약근 기전으로서 연하, 호흡 및 발음 등의 생리적 기능에 중요한 역할을 한다. 이 기능에 문제가 생긴 경우를 비인강폐쇄부전이라하며 그 원인으로는 (1) 연구개의 길이 및 움직임이상, (2) 비인두강의 해부학적 공간문제, (3) 인두 후벽과 측벽의 기능이상 등이 있다. 본 연구는 구개월 환자의 측면두부방사선 사진을 통해 비인두강을 해부학적으로 분석하고 동시에 산출된 각 모음의 과비음 정도를 평가하여 비인강폐쇄부전과의 연관성을 비교해 본 것이며, 얻어진 결과는 다음과 같다. 1.연구개 길이는 정상인에 비해 현저히 짧았다. 2.adequate ratio는 정상인에 비해 작게 나타났다. 3.adequate ratio가 감소함에 따라 모음 조음시 anatomic mVPI가 점차 증가하였다. 4.각 모음 조음시 anatomic VPI는 과비음정도와 비례관계를 보였다. 5.고모음(/u/,/i/)의 과비음정도가 저모음(/a/)에 비하여 크게 나타났다. 결론적으로, 구개열환자에서 측면두부방사선 사진은 비인강폐쇄부전의 진단 및 평가에 유용하게 사용될 수 있으며, 비인두강의 해부학적 구조는 산출되는 과비음정도와 밀접한 연관성이 있었다. The causes of this error are defects in (1) length, function, posture of the soft palate, (2) depth and width of the nasopharynx, (3) activity of the posterior and lateral pharyngeal wall. The purpose of this study is to analyze the nasopharynz of cleft palate patients using lateral cephalograms and at the same time, evaluate the degree of hypernasality of each vowels to find its relationship with nasopharyngeal incompetence. The following results were obtained : 1.The length of the soft palate was markedly short than normal. 2.The adequate ratio was smaller than the normal value. 3.As the adequate ratio decreased, when articulating vowels, anatomic mVPI increased. 4.When articulating each vowels, anatomic VPI was in proportion with the degree of hypernasality. 5.The degree of hypernasality was greater in high vowels(/i/, /u/) than low vowel(/a/). From the above results, it can be concluded that in cleft palate patients, lateral cephalograms can be used effectively in diagnosing and evaluating nasopharyngeal incompetence. The anatomic structure of the nasopharynx has close relation to the degree of hypernasality.

      • 토끼에서 enrofloxacin과 enrofloxacin acetate의 比較藥物動態學

        This study was designed to examine the in vitro antibacterial activities and pharmacokinetics of enrofloxacin acetate (ENFXA) and enrofloxacin (ENFX). Two enrofloxacins had wide spectrum antibacterial activities against Gram positive and negative bacteria, and mycoplasma, showing potent antibacterial activities with low MICs (0.05-3.33 ㎍/㎖ for Gram positives, 0.002 ㎍/㎖ for Gram negatives and 0.02 ㎍/㎖ for mycoplasma). In order to investigate the profiles of two formulations of enrofloxacin we studied comparative pharmacokinetics of them in rabbits. Five rabbits were given 5 ㎎/㎏ body weight of each enrofloxacin formulation intravenously (i.v.) and orally (p.o.) in a cross-over study. Pharmacokinetic parameters of both formulations were calculated by the use of PCNONLIN, a computer program. Their bioavailibility in rabbits, the means of the area under the curve (AUC). After oral administration of each formulation in rabbits, the mean elimination half-lives (t_1/2,ke) were 3.25h (ENFX) and 4.32 h (ENFXA), and mean AUC 4.27 ㎍·h/㎖ (ENFX) and 6.21 ㎍·h/㎖ (ENFXA). Both enrofloxacin formulations seemed to have good tissue distribution and penetration as indicated by large volume of distribution: 4.76 1/㎏ for ENFX and 7.23 1/㎏ for ENFXA. With the results obtained in this study, ENFXA could be used in place of ENFX in rabbits.

        강화 섬유의 분산도가 SiC<sub>f</sub>/SiC 복합소재의 기계적 강도에 미치는 영향

        This paper investigates the impact of fiber dispersion on the internal structure and mechanical strength of SiC<sub>f</sub>/SiC composites manufactured using spread SiC fibers. The fiber volume ratio of the specimen to which spread SiC fiber was applied decreased by 9%p compared to the non-spread specimen, and the resin slurry impregnated between the fibers more smoothly, resulting in minimal matrix porosity. In order to compare the fiber dispersion of each specimen, a method was proposed to quantify and evaluate the separation distance between fibers in composite materials. The results showed that the distance between fibers in the spread specimen increased by 2.23 ㎛ compared to the non-spread specimen, with a significant 42.6% increase in the distance between fiber surfaces. Furthermore, the 3pt bending test demonstrated a 49.3% higher flexural strength in the spread specimen, accompanied by a more uniform deviation in test data. These findings highlight the significant influence of SiC fiber dispersion on achieving uniform densification of the SiC<sub>f</sub>/SiC matrix and increasing mechanical strength.

        Growth and Developmental Characteristics of Lettuce, Tomato and Melon Grown under Spectrum Conversion Greenhouse Films

        This experiment was carried out to determine the effect of different spectrum conversion greenhouse films (red, blue, red+blue) to changing microclimate and subsequent effect on growth and quality of lettuce (Lactuca sativa L.), tomato (Solanum lycopersicum L.), and melon (Cucumis melo L.) Transmission of PAR (photosynthetic active radiation, 400-700 nm) under red, blue, and blue+red (BR) films was higher by 7.2, 7.6, and 5.8%, respectively, as compared with control greenhouse film. Light transmission in the 300-1100 nm wave band was 91.6% and 91.3% under red and blue films, respectively, compared to 86.4% under control film. Mean daytime temperature in greenhouse was slightly higher in red and blue film and mean night time temperature was higher in BR film. There were no significant differences in relative humidity among the films. Number of leaves and plant fresh weight of lettuce were higher under BR film. Total number of marketable tomato fruits was greater under blue and BR film and fruit weight was heavier under red and blue film compared to control film. Under BR and red film melon fruit weight was increased by 140 g and 80 g respectively, as compared to control film. Sugar content in melon and tomato fruits was not affected by covering films.

        Electrochemical Synthesis of Octahedral Nanostructured PbF<sub>2</sub>

        In this work, we investigate anodization of Pb in ethylene glycol containing small amount of $NH_4F$, demonstrating that ${\beta}-PbF_2$ particles with octahedral morphology can be prepared by adjusting the applied potential and anodizing time. FE-SEM images and XRD measurements of anodic nanostructures as a function of anodizing time clearly show that PbO is first formed on Pb. Subsequently, a local dissolution of PbO leads to formation of skeleton structure of PbO, releasing $Pb^{2+}$ ions in the electrolyte. The lead ions can be precipitated on the walls or intersection of the skeleton walls when the concentration of lead ions is saturated. The method described in this article shows the feasibility of formation of metal fluoride crystal by anodization of metal in a fluoride containing solution.

        망막중심동맥폐쇄에 합병된 망막신생혈관의 유리체강 내 아바스틴 주입술에 의한 치험 1예

        목적: 중심망막동맥폐쇄에 합병된 망막혈관신생 소견을 보인 환자의 유리체강 내 아바스틴??(Bevacizumab) 주입술 시행 후 망막신생혈관의 퇴행을 경험하였기에 보고하고자 한다. 대상과 방법: 특이 과거력 없는 71세 남자 환자가 양안의 시력저하(안전수지)를 주소로 내원하였다. 안저검사상 양안 후극부에 황반부를 중심으로 망막의 혼탁 및 혈류장애 소견 보였으나, 전형적인 중심와 앵두반점은 보이지 않았다. 형광안저촬영상 양안 동정맥 통과 시간의 지연 및 우안 망막혈관신생 부위의 형광누출이 관찰되었다. 발병 2개월째 우안 안저검사상 유리체출혈 및 악화된 혈관신생 소견 보여, 유리체강 내 아바스틴?? 주입술을 시행하였다. 결과: 주입술 후 3주째, 우안의 유리체 출혈 감소되었고, 형광안저촬영상 혈관신생의 퇴행 소견 보였다. 결론: 유리체강 내 아바스틴?? 주입술은 중심망막동맥폐쇄에 합병된 망막 혈관신생에 있어서 고려할 수 있는 안전하고 효과적인 방법으로 생각한다. Purpose: To report a case of regression of retinal neovascularization following central retinal artery occlusion with a single intravitreal injection of avastin?? (bevacizumab). Methods: A 71-year-old man without significant past medical history visited our clinic because of diminished vision in his both eyes (count-finger vision). Fundus examination showed retinal opacification on the posterior pole around the center of the macula with significant reduction of the retinal circulation, but there is no cherry-red spot at the fovea. Fluorescein angiography revealed marked delay in arteriovenous transit time in both eyes, and leakage from retinal neovascularization in the right eye. Five months after the occlusion, fundus examination of the right eye showed vitreous hemorrhage and increased retinal neovascularization. We performed intravitreal avastin?? injection. Results: After three weeks, there was decreased vitreous hemorrhage and regression of neovascularization on fluorescein angiography in the right eye. Conclusions: Our experience suggests that intravitreal avastin?? injection may be a safe and effective alternative to conventional therapeutic procedures for retinal neovascularization with central retinal artery occlusion.

        20대 이하 간세포암종의 증례 : 20대 여성에서 임신 중 발견된 간세포암

        Hepatocellular carcinoma (HCC) in pregnancy is very rare. The cirrhosis which is present in the majority of patients with HCC induces infertility. The diagnostic methods and treatment modalities in HCC during pregnancy are different from those of usual types of other HCC. A 26-year-old, 32th-gestational-week pregnant female was sent to our hospital because of abnormal liver function test. A 1.5cm sized mass was identified in segment 6 of liver which was compatible to AJCC stage I. She did not have any other medical history except Hepatitis B Virus carrier and the HBs Ag of her mother also was positive. At the 40th gestational week, the female baby was delivered uneventfully. And then she underwent the transarterial chemoembolization (TACE) following the Rt. Hemihepatectomy. Since she underwent a surgical resection, the tumors have been recurred in the remnant liver only. Whenever the tumors were founded, the aggressive surgical approaches were performed including 3 times of hepatic resection with TACE or TACI. She is still alive with good general condition and normal liver function for 9 years since the first diagnosis was made. Therefore an extremely rare case of hepatocellular carcinoma in pregnancy is treated successfully because of aggressive therapies.

        발아조건과 배지종류 및 양액 농도에 따른 식물공장 재배 청경채와 상추의 묘 생육특성

        본 연구는 식물공장에서 청경채와 상추의 발아조건과 육묘배지 및 육묘 시 양액농도가 묘의 생육에 미치는 영향을 구명하고자 수행되었다. 청경채와 상추의 발아율은 광의 유무와 온도처리(15, 20, 25℃) 간에 유의적 차이가 없었으나, 육묘배지 간에는 우레탄스펀지의 경우 파종 전ㆍ후 수분관리 유ㆍ무에 따라 큰 차이를 보였다. 즉, 우레탄스펀지 배지에서 파종전ㆍ후 수분관리를 하지 않았을 경우 암면에 비해 발아율이 현저히 낮았으나, 수분관리를 해주었을 때에는 암면보다 발아율이 높았으며, 묘의 생육도 암면과 차이가 없었다. 그리고 청경채의 생체중은 EC 1.0dSㆍm?¹에서 1.5dSㆍm?¹에서보다 1.8배 무거웠고, 0.5dSㆍm?¹에서보다는 6배 무거웠다. 상추의 생체중 또한 EC 1.0dSㆍm?¹이 1.5dSㆍm?¹와 0.5dSㆍm?¹에 비해 1.7배와 3.5배 각각 무거웠다. This experiment was conducted to study the effects of germination condition, nursery media and nutrient concentration in plant factory on the growth characteristics of pak-choi and lettuce seedlings. The results showed that temperature (15, 20, 25℃) and light conditions (lighting and no lighting) had no significant effect on germination rate of pak-choi and lettuce, whereas the interaction of growing media (rockwool and urethane sponge) and moisture management before and after sowing had significant effect on germination percentage. In case of no moisture management before and after sowing, germination percentage was much higher in rockwool medium than urethane sponge medium. But germination percentage of urethane sponge was higher than that of rockwool with applying moisture before and after sowing. At the nutrient concentration of EC 1.0 dSㆍm?¹ the fresh weight of pak-choi was heavier than in EC 1.5 and 0.5 dSㆍm?¹ by 1.8 and 6.0 times, respectively. At the nutrient concentration of EC 1.0 dSㆍm?¹ the fresh weight of lettuce was also heavier than in EC 1.5 and 0.5 dSㆍm?¹ by 1.7 and 3.5 times, respectively.
