        Management of carbapenem-resistant Acineto-bacter baumannii epidemic in an intensive care unit using multifaceted intervention strategy

        Background/Aims: We evaluated the effects of a multifaceted intervention for controlling a carbapenem-resistant Acinetobacter baumannii (CRAB) epidemic in an intensive care unit (ICU) with no restrictions on carbapenem use. Methods: This study was conducted in a medical ICU between April 2012 and June 2016. The baseline infection control programs included surveillance cultures, contact precautions, and environmental cleaning. However, increases in the detection of CRAB isolates beginning in May 2013 led to the implementation of a new protocol, consisting of universal glove and gown use with daily chlorhexidine bathing for all patients in combination with baseline programs. The efficacy of the intervention was subjected to interrupted time series (ITS) analysis. Results: The multifaceted intervention led to a decrease in the incidence of CRAB from 17.65 to 0.89 cases per 1,000 patient-days, during the study period (p < 0.001). ITS analysis revealed a significant change in the trend of CRAB incidence (-0.413, p < 0.001). Over this same period, mean monthly use of carbapenems increased from 143.54 ± 95.73 to 204.95 ± 78.43 doses per 1,000 patient-days (p = 0.018). Conclusions: Our multifaceted intervention reduced the incidence of CRAB acquisition in an ICU where carbapenem use was not restricted.

        <P>Novel functional materials that use both the spin and charge of an electron offer many exciting opportunities to create new information processing and storage devices with ultralow power consumption. The recent discovery of magnetism in atomically thin-layered materials could boost research in the field of light element materials such as van der Waals magnetic nanostructures. Here, we report intrinsic magnetization in large-area graphene grown on Fe(100) foil structures. The optimal growth mechanisms provide high-quality graphene samples with controllable magnetic properties, such as thickness-dependent magnetization. Using first-principles calculations, we investigated several possible scenarios for controlling the magnetic properties. Moreover, the independent magnetic structures induced in graphene were identified through field-applied magnetic force microscopy (FA-MFM) followed by vibrating sample magnetometry (VSM). Our results open a new avenue for controlling the magnetic properties of graphene structures by mediating surface growth on regular magnets for applications in large-scale spintronics.</P> [FIG OMISSION]</BR>

      • S-626 Successful control of CRAB epidemic in a medical ICU using multifaceted intervention

        Background: There are few data about the impact of universal glove and gown use with chrohexidine (CHG) bathing on the decrease in the rate of carbapenem-resistant?Acinetobacter baumannii?(CRAB) infection or colonization, compared with this strategy for controlling MRSA or VRE.?Methods: This study was conducted in a medical ICU between April 2012 and June 2016. The 18-month baseline infection control programs included active surveillance cultures, contact precautions, and environmental cleaning. However, increases in the detection of CRAB isolates beginning in May 2013 led to the implementation of a new protocol, consisting of universal glove and gown use with daily CHG bathing for all patients in combination with baseline programs. Changes in the incidence of CRAB were analyzed using an interrupted time series (ITS) analysis.?Results: Only three cases of ICU-acquired CRAB were identified during first 12 months, followed by a spike in CRAB acquisition to 17.65 cases per 1,000 PD in the last quarter of baseline period. However, our new protocol led to a decrease in the monthly incidence of CRAB, with a slope of -0.071 cases per 1,000 PD. ITS analysis revealed a significant change in the trend of CRAB incidence (-0.417;?p<0.001), although the change in level was not significant (0.033;?p=0.930).?Conclusions: Our study showed that universal glove and gown use with CHG bathing may be effective to control CRAB in a MICU.Figure 1.         Poly(1-vinylpyrrolidone-co-vinyl acetate)를 이용한 나노영가철의 소수성 유기용매 친화성 향상

        목적 : 상업부지, 주택부지 등에서 기준치를 뛰어넘은 유기 오염물질이 검출되어 토양 및 지하수 오염의 심각성이 커지고 있다. 특히 지중 소수성 유기오염물질인 NAPL (Non-aqueous phase liquids)은 물에 섞이지 않아 기존 토양복원기술로 제거가 어렵다. 또한 장기간에 걸쳐 서서히 지하수층으로 용해되어 지하수를 오염시킨다. 토양・지하수복원을 위해 NAPL 제거 필요성이 커지게 되면서 지중 내 다양한 오염물질 처리에 널리 사용되는 나노 소재인 나노영가철(nanoscale zero valent iron; nZVI)에 대한 관심도 커지고 있다. 그러나 nZVI는 낮은 분산성으로 지중에서 이동성이 저하되는 문제를 가지고 있다. 본 연구에서는 nZVI의 이동성을 향상시키는 동시에, NAPL에 선택적으로 이동하게 하도록 친수성 및 소수성 말단을 동시에 가지고 있는 poly(1-vinylpyrrolidone-co-vinyl acetate) (PVP/VA) 로 표면 개질하였다. 방법 : PVP/VA 존재하에서 FeSO4·7H2O와 NaBH4의 반응을 통하여 PVP/VA로 표면 개질된 nZVI를 합성하였다. 합성 소재의 분산성을 확인하기 위하여 가시광선 분광기를 이용하여 침전실험을 실시하였으며, 모래 충진 컬럼에서의 이동성을 평가하였다. 또한 투과전자현미경을 사용하여 PVP/VA 존재 유무에 따른 입자의 크기 및 특성의 변화를 관찰하였다. 다른 PVP/VA 비율을 가지는 nZVI를 이용한 질산성 질소 환원능 평가를 통하여 분산도 향상에 따른 반응성의 변화를 관찰하였다. NAPL에 대한 선택적 이동성을 확인하기 위하여 트리클로로에틸렌 및 도데케인을 이용하여 PVP/VA 유무에 따른 이동을 평가하였다. 최종적으로 다공성 매질에서의 NAPL에 대한 선택적 이동성을 파악하기 위하여 도데케인이 흡수된 스펀지 층을 통과하는 nZVI의 비율을 평가하였다. 결과 및 토의 : PVP/VA-nZVI 합성 소재의 분산성은 합성 전과 후가 큰 차이 없었으나, 투과전자현미경을 통하여 얻어진 입자의 크기는 감소하는 것으로 평가되었다. 다공성 매질에서의 이동성은 PVP/VA 개질에 따라 크게 향상되는 것으로 나타났다. 또한 PVP/VA로 개질하였을 시에 질산성 질소의 환원 속도가 향상되는 것으로 보아, 작은 입경을 가지도록 유도하는 PVP/VA의 효과를 확인할 수 있었다. 트리클로로에틸렌 및 도데케인을 이용한 NAPL에 대한 선택적 이동성 평가 결과, PVP/VA에 의하여 소수성 오염물질에 대한 친화도가 향상되어, nZVI가 NAPL 층으로 선택적으로 이동하는 것을 확인할 수 있었다. 컬럼을 통해 수행한 PVP/VA-nZVI 합성 소재의 이동성 확인 실험 결과, NAPL인 도데케인이 존재한 컬럼의 Fe 제거율은 98.55%고 dodecane이 없는 컬럼은 88.16%로 PVP/VA-nZVI가 컬럼내에서 이동하다가 도데케인에 선택적으로 부착되는 것을 확인할 수 있었다. 결론 : 친수성과 소수성 말단을 동시에 보유하고 있는 PVP/VA를 이용하여 nZVI의 표면을 개질함으로써 더 작고 고른 입도를 가지는 nZVI를 합성할 수 있었으며, 이를 통하여 다공성 매질에서의 높은 이동성 및 오염물질과의 높은 반응성을 확보할 수 있었다. 또한, PVP/VA의 친수성 및 소수성 성질에 따라 소수성 유기용매인 NAPL에 대한 친화도를 높여 nZVI가 NAPL 층으로 이동할 수 있도록 유도하였다. 이를 통하여 토양으로 nZVI를 주입할 때, 표적이 되는 NAPL 층으로의 이동을 촉진하여 토양 정화 효율을 향상시킬 수 있을 것으로 기대할 수 있다. Objectives : Levels of organic contaminants in excess of the standard minimum have been detected in many commercial and residential sites, and the severity of soil and groundwater pollution is increasing. In particular, non-aqueous phase liquids (NAPLs) are hydrophobic organic pollutants that do not mix with water and are difficult to remove with existing soil remediation technology. These pollutants slowly dissolve into the groundwater over long periods of time, thus contaminating the groundwater. With the increasing need to remove NAPLs for soil and groundwater remediation, widespread interest has focused on the use of nanoscale zero valent iron (nZVI). However, nZVI has the disadvantage of reduced subsurface mobility. Hence, in the present study, the nZVI surface is modified with poly(1-vinylpyrrolidone-co-vinyl acetate) (PVP/VA), which has both hydrophilic and hydrophobic groups, to improve the mobility and selectivity of nZVI for the removal of NAPL. Methods : The PVP/VA modified nZVI is synthesized through the reaction of FeSO₄・7H₂O and NaBH₄ in the presence of PVP/VA. To confirm the dispersibility of the prepared material, a precipitation experiment is performed using a visible light spectrometer, and the mobility through a sand-filled column is evaluated. In addition, the variation in particle size and characteristics according to the presence of PVP/VA is examined via transmission electron microscopy. The nitrate reduction ability of nZVI with PVP/VA is also evaluated to reveal changes in reactivity depending upon the degree of dispersion. To confirm the selective mobility towards NAPL, trichloroethylene and dodecane are used to evaluate the mobility with and without PVP/VA. Finally, the ratio of nZVI passing through the sponge layer absorbing dodecane is evaluated to determine the selective mobility towards NAPL in the porous medium. Results and Discussion : Although the dispersibility of the PVP/VA-nZVI is not significantly changed, the particle size is significantly decreased. Both the mobility in porous media and the nitrate reduction rate are improved via PVP/VA modification. The affinity for hydrophobic contaminants and the selective migration of PVP/VA-nZVI towards the NAPL layer are also improved. The high affinity for the NAPL was also shown by the column with NAPL layer. Conclusions : Surface-modification with PVP/VA, which has both hydrophilic and hydrophobic ends, enabled the synthesis of nZVI with a smaller and more uniform particle size, thus providing high mobility in porous media and high reactivity towards contaminants. The combined hydrophilicity and hydrophobicity of PVP/VA is shown to increase the affinity of nZVI towards NAPL and, thus, promote its migration to the NAPL layer. Thus, it is anticipated that the efficiency of soil remediation can be improved by promoting the movement of nZVI towards the target NAPL layer.

        기업은 퇴직자, 외부용역, 협력업체 등의 감사, 내부고발 및 정보유출 등을 확인하기 위한 많은 노력을 기울이고 있다. 하지만 감사 대상 분석을 위한 인력 충원의 어려움, 비용 소모 및 소요 시간 증가, 업무 효용성 저하 등을 비롯하여, 지속적으로 늘어나는 정보량으로 인해 정보감사 수행에 어려움을 겪고 있다. 본 연구의 목적은 정보감사 수행시간의 단축 및 협업 등을 위하여 디지털 포렌식 분석도구인 AccessData社의 AD LAB을 활용하여 효율적인 정보감사를 수행할 수 있는 적용 방안을 제시한다.

      • Weight Change Prediction for Automated Depression Diagnosis

        Early detection of depression through self-diagnosis is challenging. Especially survey-based self-diagnosis can lead to incorrect results because the answer contains the subjective opinion of the respondent. To overcome this problem, the multi sensor-based method using a smartphone has been proposed as an alternative for self-diagnosis of depression. These methods diagnose depression by using various sensor data that represented typical depression symptoms from the smartphone. However, sensor data have difficulty estimating weight changes, a representative symptom of depression. In this paper, we propose a facial image-based weight change estimation method for use in diagnosing depression. In particular, we show i) simple feature engineering, designed to fast and robustly compare between two facial images using 3D face landmarks, ii) face alignment processing that makes the model accurately compare facial images which were taken from various viewpoints, and iii) a novel preprocessing with monocular depth estimation to solve scale variance according to variation of distance between the camera and face.         한국 대학생의 문화간 감수성과 영어 능력 간의 관계

        The purpose of this study was to explore the relationship between English ability and intercultural sensitivity of Korean college freshmen. The participants were 203 college students and they were divided into three groups (high, mid, and low) according to their English ability. The data for the study was collected, using the Intercultural Sensitivity Scale (ISS) and English grades of Korean College Scholastic Ability Test (CSAT). The intercultural sensitivity scale consists of 5 elements, namely, interaction engagement, respect for cultural difference, interaction confidence, interaction enjoyment, and interaction attentiveness. The results of this study show that intercultural sensitivity had a positive relationship with English ability and more positive relationships among 5 elements were found in the high English ability group. Additionally, between English majors and non-English majors, significant differences were found in intercultural sensitivity and more positive relationships were shown in non-English majors. The implications of this study were discussed.
