        알렉시딘과 차아염소산나트륨의 화학적 상호반응 후 mass spectrometry를 이용한 파라클로로아닐린의검출 분석

        근관세척제인 NaOCl과 CHX를 병행 사용하는 경우 적갈색의 발암물질로 알려진 PCA가 생성된다. 본 연구의 목적은 CHX와 유사한 buigunide 계통의 소독제 인 ALX을 NaOCl 과 흔합 반응 시 PCA가 생성되 는지 여부를 mass spectrometry를 이용하여 평가하고자 하였다. 대조군으로는 4% NaOCl 용액과 2% CHX의 혼합용액, 0.5%PCA 용액, 및 1%ALX 용액을 사용하였고 실험군으로는 5가지 농도(1%, 0.5%, 0.25%, 0.125%, 0.0625%)의 ALX 용액과 4% NaOCl의 흔합용액을 질량분석기를 이용하여 mole-cular peak를 분석한 결과 ALX과 NaOCl의 혼합물에서는 PCA(m/w=128)로 보이는 128 피크가 관찰되지 않았다. 또한 용액의 색 변화에서도 ALX이 농도가 높을수록 열은 노랑색을 띄었으나 농도가 낮아질수록 흰색으로 관찰되었으며 어떠한 침전물의 형성도 보이지 않았다. Recent studies demonstrated that the combination of chlorhexidine (CHX) and sodium hypochlorite (NaOCl) resulted in the formation of a precipitate, para-chloroaniline (PCA). Alexdidine (ALX) is a kind of biguanides like CHX, and has stronger detoxification effect against the bacterial virulence factors such as lipoteichoic acid and lipopolysacchardide compared with CHX. The purpose of this study was to determine whether PCA was formed after chemical interaction between ALX and NaOCl using mass spectrometry. Mass spectrometry was performed for the mixture of five different concentrations of ALX (1, 0.5, 0.25, 0.125, 0.0625%) and 4% NaOCl. Results showed that the peak of PCA was not detected in mixed solu tions of ALX and NaOCl in mass spectrometry analysis. The color of mixed solution of ALX and NaOCl after chemical interaction was light yellow to white, but there wasn't any precipitate found.

        한국인 배자 및 태아에서 유치 발생의 조직학적 변화

        Tooth development is usually described in four stages such as bud stage, cap stage, bell stage and crown stage. Exact time of appearance of tooth primordia is different among reports, and up to now there is no timetable regarding initial tooth development. To understand the congenital malformations and other disorders of the orofacial region, there is need to establish a standard timetable of early tooth development. Till now, studies on the tooth development were mainly on later fetuses, and only few reports on early stage. Also, there were no reports on the time when bud stage turns to cap stage, and cap stage to bell stage. In this study, external morphology of face and the early development of the tooth, and transition of bud stage to cap stage, cap stage to bell stage were studied using 27 staged human embryos and 9 serially sectioned human fetuses. the results are as follows : 1. Mandibular region was formed by union of both mandibular arch at stage 15, and maxillary region by union of maxillary arch, medial nasal prominence, and intermaxillary segment at stage 19. 2. Ectodermal thickening which represents the primordia of tooth appeared in mandibular region at stage 13, and maxillary region at stage 15. 3. Bud stage began from mandibular primary central incisor at stage 17, and maxillary primary central incisor at stage 18. And the sequence of appearance was in the mandibular primary lateral incisor at stage 19, maxillary primary lateral incisor at stage 20, mandibular primary canine at stage 22, maxillary primary canine and primary first molar at stage 23, madibular primary first molar and maxillary primary second molar at 9th week, and mandibular primary second molar at 10th week of development. 4. cap stage began from the primary anterior teeth at 9th week, and primary second molar still had the characteristics of cap stage at 12th week of development. 5. Transition to bell stage started from the primary anterior teeth at 12th week, and primary second molar started at 16th week of development. 6. Trnasition to crown stage started from primary anterior teeth at 16th week, and primary second molar at 26th week of development.

        Fine needle aspiration is usually performed as a preoperative cytologic evaluation in salivary gland tumors, and complications of the lesion after fine needle aspiration are scarcely developed. The secondary change of tissue by fine needle aspiration can make the diagnostic and subsequent therapeutic difficulties for clinicians and require a careful approach. Fine needle aspiration can cause variety of changes in Warthin tumor, and it is presumed that those changes are mainly caused by the infarction of the mass due to vascular injury and following inflammatory reactions. We would introduce our experience of granulomatous and necrotic change after fine needle aspiration on parotid Warthin tumor with consequent difficulties in diagnostic and surgical approach to the patient.

        Analyzing the Effect of River Ecosystem Fragmentation by Bridges on Changes in the Wintering Population of Whooper Swans (Cygnus cygnus): Targeting the Nakdong River, Busan

        Background and objective: Fragmentation of river ecosystems is expected to affect biodiversity loss, but bridge construction is proceeding without consideration in urban areas. This study was conducted to determine the effect of internal environmental factors and river ecosystem fragmentation caused by bridge construction on the population of wintering whooper swans (Cygnus cygnus) in the Nakdong River of Busan Metropolitan City, a key wintering site for whooper swans. Methods: To compare the wintering population according to the distance between bridges, we surveyed and analyzed the current status of the population by period, distance between bridges, and land cover. One-way ANOVA and post-hoc test were conducted to confirm whether the differences in the environmental factors of the wintering sites, such as the distance between bridges, land cover status, and the number of wintering individuals, were statistically significant. Results: 83.6-94.7% of the wintering population in the lower Nakdong River were observed in Sec. 2 (Nakdong River Estuary Bank-Seobusan Nakdong River Bridge, 5.3 km) and Sec.4 (Gamjeon-Iron Bridge-Nakdong River Hwaengdansugwan Bridge, 3.6 km). As for the distance between the feeding and resting places of whooper swans and the bridges, whooper swans used the waterside and wetlands at an average distance of 1,147.5 m (10.9-2,611.2 m) from the bridge. Conclusion: Considering the weight of male swans and the presence of young individuals, as well as disturbance factors such as the noise and speed of vehicles crossing the bridges, it is necessary to maintain at least a 4 km distance between bridges for stable wintering. In addition, since fragmentation of river ecosystems has been confirmed to have an adverse effect on biodiversity, it would be desirable to keep the ecosystem intact and connected.

        A Novel Method for Deriving Optimal Synchronism-Check Phase Angle in Transmission System

        <P>Most of faults occurring in transmission system are transient. To cope with such faults, automatic reclosing is applied and it is economic and effective method to improve the reliability and transient stability of power system. Many countries applying 3-phase and leader-follower reclosing scheme use a synchronism-check relay to limit impacts associated with automatic reclosing under Live-Bus/Live-Line (LBLL) conditions. Most utilities generally use a standardized conservative setting that is in the range of 20 degrees to 30 degrees. However, it might be sometimes not a good idea because each transmission line in system has different system conditions such as system structure, line length and load flow that might result in undesirable system impacts. Therefore, it is necessary to derive the Optimal Synchronism-Check (OSC) phase angle which is appropriate for each transmission system. In this paper, a novel method composed of three steps is proposed to derive the OSC phase angle. The proposed method is applied to transmission system based on actual data from Korea. Computer simulations are performed using ElectroMagnetic Transient Program (EMTP) and simulation results show that proposed method is valid and effective.</P>

        Relationship Between Prostate Volume and Lower Urinary Tract Symptom in Health Checkup Subjects

        Purpose: This study aimed to evaluate the relationship between prostate volume and lower urinary symptom (LUTS) in subjects undergoing health checkup and to know the usefulness of TRUS in health screening. Materials and Methods: The study was conducted in 883 men aged ≥20 years who underwent TRUS for health screening. All participants had filled in the international prostate symptom score (IPSS) and were tested for prostate-specific antigen; prostate volume, central gland volume, and transitional zone index were measured using TRUS. We analyzed the differences in the IPSS by prostate volume and differences in prostate volume by severity of LUTS and correlation between prostate volume and each component of IPSS. Results: There were differences in the total IPSS, storage score, and voiding score between the subjects with prostate volumes of ≥30 mL or not (p=0.027, p=0.037, and p=0.029, respectively). However, the differences were found only for urgency and weak stream. The volume of the severe symptom group was bigger than those of the mild and moderate symptom groups (p=0.002 and p=0.014). The correlation between prostate volume and IPSS was significant only for the between the central volume and nocturia (r=0.112, p<0.01). Conclusions: The relationship between prostate volume and urinary symptoms showed significant but low correlation and found only in some components. For the accurate diagnosis, it would be more useful to accompany various voiding-related surveys in addition to TRUS during health screening.

        Agaston Score used as indicator of quantative coronary artery calcification is calculated manually by Human being. The accuracy of the values measured using artificial intelligence programs and the manual measurement method, which is an existing conventional method, was compared. The comparison of the average value of the results of 136 people using the initial version showed a statistically significant difference, and the results of 156 people using the upgraded version showed no statistically significant difference. In the case of initial version of the program, the accuracy of the division of the anatomical structure was poor, resulting in a difference in the result values. After that, in the case of the group using the improved version, the problem was improved, resulting in no statistically significant difference. It was possible to confirm accuracy and the possibility of development of deep learning based AI.

        Gene Expression Profile in Microglia following Ischemia-Reperfusion Injury

        Microglial activation is thought to play a role in the pathogenesis of many brain disorders. Therefore, understanding the response of microglia to noxious stimuli may provide insights into their role in disorders such as stroke and neurodegeneration. Many genes involved in this response have been identified individually, but not systematically. In this regards, the microarray system permitted to screen a large number of genes in biological or pathological processes. Therefore, we used microarray technology to evaluate the effect of oxygen glucose deprivation (OGD) and reperfusion on gene expression in microglia under ischemia-like and activating conditions. Primary microglial cultures were prepared from postnatal mice brain. The cells were exposed to 4 hrs of OGD and 1 h of reperfusion at $37^{\circ}C$. Isolated mRNA were run on GeneChips. After OGD and reperfusion, >2-fold increases of 90 genes and >2-fold decrease of 41 genes were found. Among the genes differentially increased by OGD and reperfusion in microglia were inflammatory and immune related genes such as prostaglandin E synthase, $IL-1{\beta}$, and $TNF-{\alpha}$. Microarray analysis of gene expression may be useful for elucidating novel molecular mediators of microglial reaction to reperfusion injury and provide insights into the molecular basis of brain disorders.

      • Can Pleuropulmonary Paragonimiasis be Cured by Only the 1st Set of Chemotherapy? Treatment Outcome and Clinical Features of Recently Developed Pleuropulmonary Paragonimiasis

        <P><B>Purpose</B> Most patients with pleuropulmonary paragonimiasis can be cured by the initial single set of Praziquantel (PZQ) treatment. However, several cases have been reported to have unsatisfactory responses to the initial PZQ treatment. The objective of this study was to evaluate the clinical findings of patients with pleuropulmonary paragonimiasis who needed additional PZQ treatment after the 1st set chemotherapy. <B>Patients and Methods</B> Thirty-two patients who were diagnosed with pleuropulmonary paragonimiasis at our institution between 2003 and 2008 were retrospectively reviewed. <B>Results</B> All patients were treated initially with PZQ for 3 days (1st set chemotherapy). Twenty-four patients (75.0%) showed improvement in respiratory symptoms and pulmonary involvements. However, eight patients (25.0%) suffered from relapsed respiratory symptoms and pleural effusion. For these patients, an additional 2nd set PZQ treatment resulted in the resolution of the symptoms and pulmonary involvements. The characteristics of patients who needed multi-set treatments were as follows; longer duration of respiratory symptoms (single vs multi-set treatment group; 6.67 ± 8.08 vs 17.86 ± 11.84 weeks, p=0.009), higher IgG titer (optical density, O.D.) for <I>Pargonimus westermani</I> (ELISA O.D. for PW, 0.54 ± 0.19 vs 0.88 ± 0.16 O.D., p=0.001) and higher frequency of multiple pulmonary lesions (% of patients with multiple lesions; 16.7% vs 50.0%, p=0.059). <B>Conclusion</B> The patients who had a longer duration of respiratory symptoms, higher ELISA titer for PW and/or multiple pulmonary lesions needed an additional PZQ treatment after the 1st set of chemotherapy. Close follow-up after the initial treatment is necessary especially for such patients.</P>
