        Development of Dilated Esophagus, Sigmoid Esophagus, and Esophageal Diverticulum in Patients With Achalasia: Japan Achalasia Multicenter Study

        Background/AimsPatients with achalasia-related esophageal motility disorders (AEMDs) frequently present with dilated and sigmoid esophagus, and develop esophageal diverticulum (ED), although the prevalence and patients characteristics require further elucidation. MethodsWe conducted a multicenter cohort study of 3707 patients with AEMDs from 14 facilities in Japan. Esophagography on 3682 patients were analyzed. ResultsStraight (n = 2798), sigmoid (n = 684), and advanced sigmoid esophagus (n = 200) were diagnosed. Multivariate analysis revealed that long disease duration, advanced age, obesity, and type I achalasia correlate positively, whereas severe symptoms and integrated relaxation pressure correlate negatively with development of sigmoid esophagus. In contrast, Grade II dilation (3.5-6.0 cm) was the most common (52.9%), while grade III dilation (≥ 6 cm) was rare (5.0%). We found early onset, male, obesity, and type I achalasia correlated positively, while advanced age correlated negatively with esophageal dilation. Dilated and sigmoid esophagus were found mostly in types I and II achalasia, but typically not found in spastic disorders. The prevalence of ED was low (n = 63, 1.7%), and non-dilated esophagus and advanced age correlated with ED development. Patients with right-sided ED (n = 35) had a long disease duration (P = 0.005) with low integrated relaxation pressure values (P = 0.008) compared with patients with left-sided ED (n = 22). Patients with multiple EDs (n = 6) had lower symptom severity than patients with a single ED (P = 0.022). ConclusionsThe etiologies of dilated esophagus, sigmoid esophagus, and ED are considered multifactorial and different. Early diagnosis and optimal treatment of AEMDs are necessary to prevent these conditions.

        Cationic Comb-Type Copolymer Promotes DNA Assembly on Gold Nanoparticles While Enhancing Particle Dispersibility

        We previously reported that graft copolymers comprised of a cationic backbone and abundant grafts of hydrophilic dextran formed soluble interpolyelectrolyte complexes with anionic biopolymers and facilitated self-assembly and folding of the biopolymers, such as duplex formation of DNA and helical folding of a peptide. In this study, effects of the cationic graft copolymers on assembly of gold nanoparticles (AuNPs) and that of DNA on AuNPs were explored. While polylysine homopolymer caused aggregation of AuNP, the graft copolymer did not induce the aggregation as monitored by adsorption spectra. The highly-grafted copolymer at nano molar concentration was capable of suppressing AuNP aggregation induced by 3 M NaCl. Moreover, the copolymer did not cause aggregation of AuNPs whose surface were modified with oligonucleotides (ODN) having highly negative charges. In the presence of copolymer, melting temperature of DNA duplex formed between AuNP-ODN and its complementary ODN was increased about 10 °C, indicating that the copolymer enhanced stability of DNA duplex on the surface of AuNPs. It was concluded that the copolymer selectively promoted assembly of negatively charged DNA but inhibited aggregation of negatively charged AuNPs.

        Endoscopic Findings and Treatment of Gastric Neoplasms in Familial Adenomatous Polyposis

        Purpose Gastric neoplasia is a common manifestation of familial adenomatous polyposis (FAP). This study aimed to elucidate the clinical characteristics, endoscopic features including fundic gland polyposis (FGPsis), and treatment outcomes of gastric neoplasms (GNs) in patients with FAP. Materials and Methods A total of 35 patients diagnosed with FAP, including nine patients from four pedigrees who underwent esophagogastroduodenoscopy (EGD), were investigated regarding patient characteristics, GN morphology, and treatment outcomes. Results Twenty-one patients (60.0%) had 38 GNs; 33 (86.8%) and 5 (13.2%) were histologically diagnosed with adenocarcinoma and adenoma, respectively. There were no specific patient characteristics related to GNs. Nodule-type GNs were more prevalent in patients with FGP than without (52.2% vs. 0.0%, P=0.002) in the upper body of the stomach. Conversely, depressed-type GNs were fewer in patients with FGPsis than in those without (13.0% vs. 73.3%, P<0.001). Slightly elevated-type GNs were observed in both groups (34.8% vs. 20.0%, P=0.538). Even within pedigrees, the background gastric mucosa and types of GNs varied. In total, 24 GNs were treated with endoscopic submucosal dissection (ESD) and eight with endoscopic mucosal resection (EMR). EMR was selected for GNs with FGPsis because of the technical difficulty of ESD, resulting in a lower en bloc resection rate (62.5% vs. 100%, P=0.014). Conclusions Our study indicates the necessity of routine EGD surveillance in patients diagnosed with FAP. Notably, the morphology and location of GNs differed between patients with and without FGPsis. Endoscopic treatment and outcomes require more attention in cases of FGPsis.         Effect of a dynamic absorber on friction-induced vibration of a rectangular plate

        When a plate-like object is rubbed by rubber, friction-induced vibration is generated. To reduce the friction-induced vibration, we experimentally investigate the characteristics of the vibration of a rectangular glass plate. The results show that the frequency of the frictioninduced vibration is almost the same as the natural frequency of a glass plate. The vibration is generated when the natural frequency of a rubbing system is close to that of a glass plate. We then examine the effect of a dynamic absorber mounted on the glass plate. The results demonstrate that the damping of a dynamic absorber is effective for suppressing the friction-induced vibration. Numerical simulation is also performed using a simplified analytical model. The calculated results agree qualitatively with the experimental ones.

        Parent Artery Complex Coil Protection for Side-Branched Wide-Neck Aneurysms

        This study aimed to validate the usefulness of parent artery complex coil protection for the treatment of wide-neck, side-branched, and ruptured aneurysms. A microcatheter was first introduced into the aneurysmal sac, and another microcatheter was introduced into the parent artery or near the orifice of the branch artery. A framing coil was deployed partially from the first microcatheter, and a protection coil was deployed from the second microcatheter to prevent protrusion of the first framing coil to the parent artery and side branches. After the first framing coil insertion, the protection coil was withdrawn to confirm the stability of the framing coil and blood flow. The procedures with this technique were successful for 3 patients. Parent artery complex coil protection can be an effective and safe coil embolization technique for the preservation of parent and side branch arteries and an alternative method for emergent ruptured cases.

        Tester Structure Expression Language and Its Application to the Environment for VLSI Tester Program Development

        VLSI chips have been tested using various automatic test equipment (ATE). Although each ATE has a similar structure, the language for ATE is proprietary and it is not easy to convert a test program for use among different ATE vendors. To address this difficulty we propose a tester structure expression language, a tester language with a novel format. The developed language is called the general tester language (GTL). Developing an interpreter for each tester, the GTL program can be directly applied to the ATE without conversion. It is also possible to select a cost-effective ATE from the test program, because the program expresses the required ATE resources, such as pin counts, measurement accuracy, and memory capacity. We describe the prototype environment for the GTL and the tester selection tool. The software size of the prototype is approximately 27,800 steps and 15 manmonths were required. Using the tester selection tool, the number of man-hours required in order to select an ATE could be reduced to 1/10. A GTL program was successfully executed on actual ATE.

      • Evaluation of flick-motion based representation used for presenting Japanese kana characters by tactile information display device

        This study introduces a potential technique for Japanese kana characters on tactile information display device (tactile display) that allows the user effectively perceived. Currently tactile display has not been developed for the purpose of character information transmission due to poor performance of recognition rates in terms of characters. The purpose of this study is to evaluate a novel technique, flick motion based display, for presenting Japanese kana characters on the tactile display (FMR tactile display). Flick motion is normally used as an input scheme for mobile devices, as one of touch screen gestures. In Japan, a set of flick motions on 10 isolated areas on the touch screen is especially assigned as Japanese kana character entry. Many Japanese mobile phone users have its mental model, which was applied to present character information as an output scheme into tactile display. Experiments were conducted to evaluate how the implemented presentation system can perform well to let the user perceive kana characters. A total of 10 subjects, including users unfamiliar to FMR, participated in the study. The subjects located their right hand on the tactile display and a kana character by using FMR was given on the display. They responded the characters as soon as they perceived. Number of correct recognition of the characters were obtained throughout the set of trials consisted of 20 character presentations. The results showed that the average correct recognition rates were 41.5 percent and the average reaction time was 6.8 seconds, which were faster than conventional trace-based character representations. The results suggested that fine adjustment of the palm to the tactile display and changing stimulus duration would improve the performance of recognition rates.
