Background: Accurate assessment of the severity of psoriasis is important in daily practice and clinical studies. However, the assessment of psoriasis area and severity index (PASI) reflects the physician’s experience, and thus evaluations by physicians are inherently subjective, with intra-rater and inter- rater variability. Objective: To elucidate the effectiveness of PASI educational lectures and the use of reference photographs on the improvement of accuracy and reliability in PASI assessments and to develop effective educational programs for PASI assessments. Methods: We performed a before- and-after comparison study during nation-wide PASI educational workshops. The participants were asked to assess the severity components of PASI (erythema, thickness, scale, and affected area) three times: in the test administered before an educational lecture, the test immediately after the lecture, and lastly the test with the use of reference photographs. The improvement of accuracy and reliability was analyzed by comparing the results of three tests. Results: Ninety-six board-certified dermatologists and residents participated and 72 participants completed all three tests. The accuracy and reliability of the assessment of severity components of PASI increased significantly after the educational lecture and the use of reference photographs. Use of reference photographs resulted in limited improvements when the recognition of three-dimensional structures was required, such as in the assessment of thickness or scale. Conclusion: Our study confirmed that the combination of standardized educational training and reference photographs can improve the accuracy and reliability of PASI assessments. Understanding how to evaluate three-dimensional psoriatic lesions can help with proper assessment of the severity of psoriasis. (Ann Dermatol 30(3) 284∼289, 2018)

Several aniline mustard analogues were obtained by introducing N,N-bis(2-chloroethyl)amino moiety to phenyl ring of A10 analogues in order to increase reactivity of A10 analogs and selectivity into DNA. The in vitro antitumor activity of synthesized compounds was evaluated using five different solid tumor cell lines by SRB method. Aniline mustard analogues exhibited more potent antitumor activity than A10 analogs. Especially, m-aniline mustard of benzoyl analogue displayed remarkable antitumor activity.

Endothelium, particularly pulmonary endothelium, is predisposed to injury by reactive oxygen species (ROS) and their derivatives. Heme oxygenase (HO) has been demonstrated to provide cytoprotective effects in models of oxidant-induced cellular and tissue injuries. In the present study, we investigated the effects of YS 49 against oxidant [tert-butylhydroperoxide (TBH)]-induced injury using cultured sheep pulmonary artery endothelial cells (SPAECs). The viability of SPAECs was determined by quantifying reduction of a fluorogenic indicator Alamar blue. We found that TBH decreased cell viability in a time- and concentration-dependent manner. YS 49 concentration- and time-dependently increased HO-1 induction on SPAECs. As expected, YS 49 significantly decreased the TBH-induced cellular injury. In the presence of zinc protophorphyrin, HO-1 inhibitor, effect of YS 49 was significantly inhibited, indicating that HO-1 plays a protective role for YS 49. Furthermore, YS 49 showed free radical scavenging activity as evidenced by 1,1-diphenyl-2-picrylhydrazyl (DPPH) and inhibition of lipid peroxidation. However, YS 49 did not inhibit apoptosis induced by lipopolysaccharide (LPS) in SPAECs. Taken together, HO-1 induction along with strong antioxidant action of YS 49 may be responsible for inhibition of TBH-induced injury in SPAECs.

Background: Hyperpigmentation on the face and neck can be a devastating psychological burden in patients with Riehl’s melanosis. However, successful treatment of the disease is challenging for clinicians. Objective: To evaluate the effectiveness and safety of low-fluence neodymium-doped yttrium aluminum garnet (Nd:YAG) laser in the treatment of Riehl’s melanosis and to identify prognostic factors determining the response to laser treatment. Methods: We enrolled 10 Korean patients with Riehl’s melanosis. The patients received 10∼28 treatment sessions at 3-week intervals with low-fluence Nd:YAG laser. Results: Among all the patients, seven reached near total improvement, and two and one patient reached marked improvement and minimal improvement, respectively, after low-fluence Nd:YAG laser treatment. The mean number of needed laser treatment sessions to reach marked improvement and near total improvement was 12.1±4.0 (ranged from 6 to 17) and 14.6±4.4 (ranged from 9 to 20), respectively. A further analysis revealed the proportion of patients who reached near total improvement was higher, and the mean number of necessary laser treatment sessions to reach minimal improvement was less in patients with dark brown pigmentation compared to those with light brown pigmentation. Among all patients, three complained of guttate hypopigmentation. However, the hypopigmented lesions spontaneously improved after the interruption of the treatment. Conclusion: We found that low-fluence Nd:YAG laser is an effective and safe treatment modality for Riehl’s melanosis. (Ann Dermatol 31(2) 127∼ 132, 2019)
China`s Social Market Economy: Comparisons To Europe

21세기의 중국경제는 비약적 발전을 도모하고 있다. 2008년에는 미국과 일본에 이어 세계 3위의 경제규모로 성장할 것이고 2020년에는 전 세계 생산량의 27%를 중국이 차지함으로써 미국과 대결구도를 갖게 될 것으로 예측된다. 중국은 경제적으로 이미 자본주의를 받아들여 체화시키고 있다. 금년에는 물권법을 도입하는 등 상당한 수준의 자본주의가 중국시장에 적용되고 있음을 볼 수 있다. 그럼에도 불구하고, 중국은 정치적으로 여전히 사회주의를 신봉하고 있다. 본 논문에서는 이 같은 21세기 중국의 모습을 사회주의시장경제(Social Market Economy)라고 정의하며, 유럽 대륙의 공동체자본주의(Stakeholder Capitalism)과 비교·연계의 측면에서 분석하고 있다. 확실히 중국경제가 성장하면 할수록 도농간 지역간 격차가 커져가고 유럽은 공동체자본주의(Stakeholder Capitalism)의 전개로 말미암아 전통적 사회보장제도가 붕괴되어 계층간 양극화 현상이 깊어가고 있다. 본 논문은 중국의 사회주의시장경제(Social Market Economy)와 유럽의 공동체자본주의(Stakeholder Capitalism)의 비교를 통해 미국의 주주 자본주의(Shareholder Capitalism)의 시각보다는 유럽의 공동체 자본주의 시각에서 중국을 분석할 것을 제안한다. 중국은 세계화 과정에서 영미식 주주자본주의과 유럽의 공동체자본주의의 요소들을 모두 수용하였으나 문제는 중국의 공동체 자본주의적 비즈니스 시스템상에서 법률적 조치의 수준을 높이는 법적 절차의 효율성 여부에 있다. In the early 21st century, China became the world`s largest recipient of foreign direct investment of $65 billion dollars in 2003. In terms of economy size, China is expected to be the world`s 3rd largest economy after the U.S. and Japan by 2008, becoming bigger than the German economy (Economist, 2003). China`s capitalism is described as a, "social market economy". The purpose of this paper is to analyse the similarities between China`s social market economy and Continental Europe`s "stakeholder" capitalism (Albert, 1991). We believe that these similarities have the potential for close collaboration between the societies in Europe and China.

Background: The previous studies on the comorbidity in the patients of vitiligo showed significant associations with autoimmune disorders. However, large scale epidemiologic study focused on the comorbid rheumatic disorders was not performed. Objectives: To clarify the association of vitiligo with rheumatic disorders in a nationwide population study. Methods: We performed cross-sectional study using data from national health insurance system in South Korea. We enrolled all patients with vitiligo and collected age and gender matched controls. Multivariate logistic regression analysis was performed to clarify the association between vitiligo and rheumatic disorders. Results: From 2009 to 2013, a total of 86,210 vitiligo patients and 172,420 controls were enrolled for the study. Vitiligo patients were at increased risks of systemic lupus erythematosus, systemic sclerosis, Sjogren’s syndrome, and rheumatoid arthritis. Subgroup analysis showed an increased risk of dermatomyositis/polymyositis in male, while ankylosing spondylitis in female vitiligo patient. In young vitiligo patients, increased risks of dermatomyositis/ polymyositis and ankylosing spondylitis were found. Conclusion: Our study confirmed the significant association between vitiligo and rheumatic disorders. In addition, the differences in the comorbid rheumatic disorders according to the age or gender suggest patient specific approaches. Careful considerations for rheumatic disorders are needed for proper management of comorbidity.
