        심실 부수축기 조율환자의 마취 경험 1예 보고

        Parasystole is of special interest among the disturbances of ectopic impulse formatioo. First, a parasystolic arrhythmia is of clinical importance because the ventricular origin ordinarily indicates the presence of organic heart disease. Second, parasystole, although not rare, is frequently overlooked because it is difficult to be differentiated from extrasystole. In this paper a ventricular parasystole is presented. A 48 year-old woman was seen with an irregular pulse rate not accompanying cardiac symptoms or signs. The chest radiograph failed to show cardiomegaly. E.K.G. revealed the dominant rhythm of sinus origin with intermittent ventricular parasystolic beats. The basic rhythm was sinus with a rate of 83 per minute. The coupling interval varied between 0.52 & 0.7 second. The shortest, calculated interectopic interval ranged from 45.2 to 49.7 (These numbers represent hundreds of a second). Occasional ventricular fusion beats appeared. She was uneventfully nephrectomized under general anesthesia with halothane-N2O-O₂for left renal empyema of tuberculosis. She dispensed well with any antiarrhythmic agent during admission

        대학 인력양성프로그램을 통한 한국 중소기업의 경쟁력 강화 방안 연구

        The objective of this study is to suggest a way of the competitive improvement of Korea's SMEs through industrial cluster and human resources training program at universities model. This model is that Two or More leading SMEs cooperatively establish industrial agglomeration for improving and promoting their R&D and innovation of management with human resources training program at universities. This study focuses on the SMEs of Korea which have a low R&D level and a low competition in Korean market as well as in international market. This paper presents an industrial cluster and human resources training program at universities model for Korean SMEs in order to increase themselves competition such as R&D level and managements. In this model, an industrial cluster is cooperatively organized by the top SMEs and many other affinitive SMEs located in industrial area of Korea. Also, The industrial cluster will be supported by Korean government and universities with financial, Taxes, workforce training, technical upgrading, and etc. In this model, the function of universities will provides SMEs in the industrial cluster innovation activities and a workforce training, and facilitate R&D with affiliated local government. The industrial cluster model and human resources training program at universities makes Korean SMEs's increase R&D level and promote innovation of management that Korean SMEs will become a strong and small enterprises. Therefore, this model is a good way that Korean SMEs have themselves competitive improvement for Korean market as well as International market. 본 연구는 우리나라의 중소기업의 현황 및 경쟁력과 문제점 등을 검토하여 산업클러스터와 대학 간 인력 협력 네트워크 구축을 통한 중소기업의 경쟁력 강화 방안을 제시한다. 위를 위해 산업 클러스터의 이론적 배경 및 선행연구와 일본의 중소기업 클러스터와 대학 간 협력 네트워크 구축 사례를 검토 한 후 한국 중소기업의 경쟁력 강화 방안으로 중소기업 클러스터 및 대학 인력양성프로그램 모형 구축을 제시한다. 한국 중소기업 클러스터 및 대학 인력양성프로그램 모형 구축에서 클러스터 운영주체인 클러스터 센터의 핵심기업들과 이들을 연계한 대학의 인력양성프로그램을 실시하고, 정부가 클러스터센터와 대학을 후방 지원함으로써 한국 중소기업들은 자생적 경쟁력이 강화되게 된다. 또한 이모형 구축에서 중소기업들 끼리 서로간의 상생에 의한 협력문화를 토착화시켜 클러스터센터를 형성하고 클러스터 센터에 대학과 정부가 통합적으로 지원한다면 한국 중소기업의 경쟁력은 크게 강화 될 것이다. 그 결과 한국의 중소기업은 오랜 역사를 가진 독일 및 일본의 중소기업처럼 강소기업이 되어 대기업과 수평적 거래가 가능한 역량이 되고 수출증가와 함께 청년실업 문제, 대기업과 중소기업간 상생문제, 기타 경제사회 양극화 문제 등이 점진적으로 향상 될 것이라는 것을 본 연구는 소개한다.

        뇌동맥류가 파열된 산모에 제왕절개술과 뇌동맥류 클리핑을 동시에 시행한 마취관리

        Aneurysmal subarachnoid hemorrhage in a pregnant woman is a rare catastrophic situation that places both the mother and fetus at high risk. When this situation arises, numerous relevant issues must be individualized in the effort to reduce hazards threatening both the mother and fetus. A 29 year old woman who was 39 weeks pregnant presented with a subarachnoid hemorrhage, secondary to a ruptured left ophthalmic artery aneurysm. Following initial full recovery, the simultaneous sequence of cesarean section followed by aneurysmal clipping was electively underwent. Successful maternal and perinatal outcome was achieved. The anesthetic management of the case is described and discussed. (Korean J Anesthesiol 1997; 32: 463∼466)

        신경근차단제와 약동학-약역학모형

        A major goal in pharmacokinetic-pharmacodynamic (PK/PD) modeling of neuromuscular blockade (NMB) is to quantitatively estimate the dose-response relationship. Our PK/PD model consists of three submodels: PK, link kinetics, and PD. A virtual effect compartment in which the drug concentration is in equilibrium with the observed concentration is used to extract the kinetic component (keo) from the pharmacodynamic data alone. Parameters of this model are keo, Ce(50), and γ. The underlying structural pharmacokinetics and pharmacodynamics for NMB have been well understood, and new novel PK/PD models have been substituted for the gold standard PK/PD model for NMB. The purpose of this review was to describe progress in the field of PK/PD modeling of NMB from the first model, a simultaneous PK/PD model developed by Sheiner et al in the 1970s, to some of the more complicated models. Specific PK/PD models, which accurately described the behaviors of rocuronium, mivacurium, atracurium, and cisatracurium, include the recirculatory model, the peripheral link model, the peripheral elimination model, and a nonparametric model for link kinetics.

        방송사업의 소유겸영규제 개선

        This paper uses the horizontal regulation system as the base analysis framework. The study clearly defines the regulatory goals of the followings: the horizontal cross-ownership regulations on program provider (PP) and platform provider, the vertical regulation on cross-ownership between PP and platform operator, the regulation on cross-ownership of program provider by terrestrial broadcasting company, and the regulation on cross-ownership between terrestrial broadcasting company and platform provider. Then, by analyzing the conformity between goals and criteria of regulations and the adequacy of the regulation level according to regulatory purposes, this paper examines the justifiability of each regulation and extracts improvement measures that suite regulatory purposes. This analysis finds following appropriate measures: replacing the horizontal cross-ownership regulation on PP with conduct regulations, such as designating major broadcasting programs or replacing the current criterion of cross-ownership regulation from sales to the audience market share; reshaping the horizontal cross-ownership regulation on platform provider so that system operator (SO), satellite broadcaster and Internet protocol television (IPTV) operator would be applied by the same regulation based on the number of subscribers of pay television services; and discontinuing other cross-ownership regulation. In this way, the study shows that with appropriate regulations on cross-ownership of PP, there would be no need for additional regulation on vertical integration between PP and platform operator. On the other hand, given that the regulation on terrestrial broadcasting cross-ownership of PP could be justified only by regulatory purpose of the protection of the diversity of public opinions, it would be desirable to replace the current criteria of the number of PPs with the criteria of the audience market share. Lastly, the study shows that when platform operator is targeted by the cross-ownership regulation based on the number of subscribers of pay television services, the regulation on cross-ownership between terrestrial broadcasting company and platform provider should be replaced with conduct regulations, such as designating must-offer channels and major broadcasting programs. 본 논문은 수평적 규제체계를 분석의 준거 틀로 하여 방송채널사용사업자(PP)에 대한 수평적 소유겸영규제, 플랫폼사업자에 대한 수평적 소유겸영규제, PP와 플랫폼사업자 간 수직적 소유겸영규제, 지상파방송사의 PP 겸영규제, 지상파방송사와 플랫폼사업자 간 소유겸영규제 각각에 대하여 규제목적을 명확히 설정하고, 규제 목적과 규제기준의 정합성, 규제목적에 따른 규제수준의 적정성을 분석함으로써, 각 규제의 타당성을 검토하고 규제목적에 부합하는 개선방안을 도출하였다. 분석 결과, PP에 대한 수평적 소유겸영규제는 주요 방송프로그램의 지정 등 행위규제로 대체하거나 소유겸영규제의 기준을 현행 매출액에서 시청점유율로 변경하고, 플랫폼사업자에 대한 수평적 소유겸영규제는 종합유선방송사업자(SO), 위성방송사업자, IPTV사업자에게 유료방송 가입자 수를 기준으로 하는 동일한 소유겸영규제를 적용하고, 여타의 소유겸영규제는 폐지하는 것이 타당함을 보였다. 이와 같이 PP와 플랫폼사업자에 대해 적정한 소유겸영규제가 설계된 상태에서는 별도로 PP와 플랫폼사업자 간 수직결합을 규제하는 것이 불필요함도 보였다. 한편, 지상파방송사의 PP 겸영규제는 여론의 다양성 보호라는 규제목적에서만 정당화될 수 있으므로, 현행 PP 사업자 수의 기준은 시청점유율 기준으로 변경하는 것이 합리적이다. 마지막으로 플랫폼사업자에게 유료방송 가입자 수를 기준으로 한 소유겸영규제가 설계되는 경우, 지상파방송사와 플랫폼사업자 간 소유겸영규제는 의무제공(must offer)채널 지정, 주요 방송프로그램 지정 등 행위규제로 대체되어야 함을 보였다.

        유체를 이송하는 직선관의 진동 해석을 위한 새로운 비선형 모델링

        A new non-linear modelling of a straight pipe conveying fluid is presented for vibration analysis when the pipe is fixed at both ends. Using the Euler-Bernoulli beam theory and the non-linear Lagrange strain theory, from the extended Hamilton's principle are derived the coupled non-linear equations of motion for the longitudinal and transverse displacements. These equations of motion are discretized by using the Galerkin method. After the discretized equations are linearized in the neighbourhood of the equilibrium position, the natural frequencies are computed from the linearized equations. On the other hand, the time histories for the displacements are also obtained by applying the generalized-$\alpha$ time integration method to the non-linear discretized equations. The validity of the new modelling is provided by comparing results from the proposed non-linear equations with those from the equations proposed by Paidoussis.
