2개의 독립 결절로 발현한 흉선암종: 증례 보고

Thymic carcinoma is a rare malignant neoplasm of the thymus arising in the thymic epithelium and has a higher frequency of local invasion and metastasis than other subtypes of thymic epithelial tumors. Thymic carcinoma is usually demonstrated as a large, irregular mass located in the anterior mediastinum and commonly contain a necrotic or cystic component. We report atypical CT findings and multicentricity in a case of thymic carcinoma presenting two small nodules in the anterior mediastinum. 흉선암종(Thymic carcinoma)은 흉선의 상피세포에서 기원하는 드문 악성 종양으로 흉선 상피종양(thymic epithelial tumor)의 다른 아형에 비해 주변 조직으로의 침습이나 전이를 잘하는 것으로 알려져 있다. 대부분 전종격동에 위치하는 크기가 크고 불규칙한 윤곽을 가지는 종괴로 나타나며 내부에 괴사성 혹은 낭성 성분을 흔히 포함한다. 본 증례는 전종격동에 크기가 작은 결절로 나타난 흉선암종으로 비전형적인 흉부 전산화단층촬영 소견을 보이며 흉선 내에 2개의 병변이 서로 떨어져 다발성으로 생겼다는 점에서 문헌고찰과 함께 보고한다.

갱년기 여성의 안면홍조 증상에 대한 국내 한의학 연구 동향 분석

Objectives: The purpose of this study is to analyse the domestic trends of Traditional Korean Medicine on Hot flush in menopausal women. Methods: We searched six databases and selected relevant papers according to the criteria. Results: 12 papers included case report, literature review, randomized controlled clinical trials (RCTs), Non-RCTs, and retrospective chart analysis study design. 1. The daily diary, Kupperman's Index, Menopause Rating Scale, Menopause-specific Quality of Life Questionnaire, Digital Infrared Thermographic Imaging were used as a measure of the symptoms of hot flush. 2. There were significant correlations between Thermographic images, Pulse energy, Lifestyle Habits, Low frequency power/High frequency power (LF/HF) ratio and certain items of Menopausal Symptoms Measurement. 3. The case report using Er-Xian decoction and Hwangryunhaedoktang-Gamibang showed significant effect. 4. Domestic RCT studies using herbal or acupuncture treatments showed a significant value of 3.4 on average in the quality assessment conducted in the study. However, the number of domestic studies included was five, which is not enough compared to foreign countries. Conclusions: This study examined domestic research trends on hot flushes of menopausal women and found that various evaluation tools and diagnostic methods were applied. It is necessary to conduct various RCT studies in Korea, where the number of studies is insufficient compared to overseas.

Association of a Methanol Extract of Rheum undulatum L. Mediated Cell Death in AGS Cells with an Intrinsic Apoptotic Pathway

Objectives: Rheum undulatum L. has traditionally been used for the treatment of many diseases in Asia. However, its anti-proliferative activity in cancer has still not been studied. In the present study, we investigated the anti-cancer effects of methanol extract of Rheum undulatum L. (MERL) on human adenocarcinoma gastric cell lines (AGS). Methods: To investigate the anti-cancer effect of MERL on AGS cells, we treated the AGS cells with varying concentrations of MERL and performed 3-(4, 5-dimethylthiazol- 2-yl)-2, 5-diphenyltetrazolium bromide (MTT) assays. Cell cycle analyses, measurements of the mitochondrial membrane potential (MMP), caspase activity assays and Western blots were conducted to determine whether AGS cell death occurred by apoptosis. Results: Treatment with MERL significantly inhibited growth of AGS cells in a concentration dependent manner. MERL treatment in AGS cells leaded to increased accumulation of apoptotic sub G1 phase cells in a concentration dependent manner. In control cultures, 5.38% of the cells were in the sub G1 phase. In MERL treated cells, however, this percentage was significantly increased (9.95% at 70 μg/mL, 15.94% at 140 μg/ mL, 26.56% at 210 μg/mL and 38.08% at 280 μg/mL). MERL treatment induced the decreased expression of pro-caspase-8 and -9 in a concentration dependent manner, whereas the expression of the active form of caspase-3 was increased. A subsequent Western blot analysis revealed increased cleaved levels of poly (ADP-ribose) polymerase (PARP) protein. Also, treatment with MERL increased the activities of caspase-3 and -9 compared with the control. MERL treatment increased the levels of the pro-apoptotic truncated Bid (tBid) and Bcl2 Antagonist X (Bax) proteins and decreased the levels of the anti-apoptotic B-cell lymphoma 2 (Bcl-2) protein, whose is the stabilization of mitochondria. However, inhibitions of p38, extracellular signal regulated kinases (ERKs) and C-Jun N-terminal kinases (JNK) by MERL treatment did not affect cell death. Conclusion: These results suggest that MERL mediated cell death is associated with an intrinsic apoptotic pathway in AGS cells.

내측 반월상 연골판 손상에 대한 신바로 약침을 포함한 한의학적 치료효과 증례보고 5례

연구 목적 이 연구는 내측 반월상 연골판 손상으로 인한 무릎 통증을 호소하는 5명의 환자에서 한 의학적 치료, 특히 신바로 약침으로 인한 통증 개선이 있었기에 보고함이다. 연구 방법 무릎 통증으로 본원 입원치료를 받은 환자 중, MRI 영상 검사상 내측 반월상 연골 파열 진단을 받은 환자들의 차트를 분석하였다. 슬관절 통증 호전 및 기능 개선은 Numeric Rating Scale (NRS), Western Ontario and McMaster Universities Arthritis Index (WOMAC Index), 슬관절 Range Of Motion(ROM) 및 special tests 측정으로 판단하였다. 연구 결과 5증례에서 NRS는 평균 3.4의 호전, WOMAC Index는 평균 64.4%의 호전을 보였다. 슬관절 ROM 측정에서 환자 모두 입원시보다 증가되었으나, 무릎에 손상에 사용되는 보편적 검사인 Stress valgus, Stress varus, Drawer test, Apley compression test, Mcmurray s test 상 전후비교에서 4명의 환자는 호전되었고, 1명 환자에서는 변화가 없었다. 결과 이 연구는 내측 반월상 연골판 손상에 대한 신바로 약침을 포함한 한의학적 치료 후 단기간 내 통증 및 기능 향상을 보여주었다.

영유아기 자녀를 둔 북한이주민가정 어머니와 일반 저소득가정 어머니의 양육태도와 사회적 지지

본 연구는 영유아기 자녀를 둔 북한이주민가정 어머니와 일반 저소득가정 어머니의 양육태도와 사회적 지지 현황을 비교하고, 북한이주민가정 어머니의 양육태도와 사회적 지지를 예측하는 요소가 무엇인지 파악하는데 그 목적이 있다. 연구대상은 서울시와 경기도에 거주하면서 유사한 수준의 소득이있고, 만 2세에서 만 5세까지의 자녀를 둔 북한이주민가정 어머니와 일반 저소득가정 어머니 각 59명이었다. 북한이주민가정 어머니들은 일반 저소득가정 어머니들보다 더 권위적인 양육태도를 가지고 있었고, 사회적 지지의 모든 영역(즉, 물질, 정서, 정보, 평가)에서 사회적 지지를 적게 받고 있었다. 탈북에서 남한입국까지 소요된 기간, 수입, 연령, 이주형태가 북한이주민가정 어머니의 권위적 양육태도와 빈약한 사회적 지지를 예측하는 것으로 나타났다. 본 연구결과는 영유아기 자녀를 둔 북한이주민가정 어머니의 양육태도와 사회적 지지의 현황을 파악하고 이를 강화할 수 있는 부모 및 가족지원프로그램에 관한 실천적 시사점을 갖는다. The purposes of this study were (1) to compare the child-rearing attitudes and maternal social supports of North Korean immigrant mothers to those of Korean low-income mothers and (2) to examine factors affecting the child-rearing attitudes and maternal social supports of North Korean immigrant mothers. Fifty-nine North Korean immigrant mothers with young children and fifty-nine South Korean low-income mothers with similar characteristics in terms of the ages of their children and income participated in the study. The results showed that North Korean immigrant mothers tended to have a higher level of authoritative child-rearing attitudes and a lower level of maternal social support(material, emotioanl, informational, evaluative) compared to their South Korean counterparts. The number of years between the escape from North Korea to settlement in Korea, income, age, and the presence of other family members when entering South Korea affected the child-rearing attitudes and maternal social supports of North Korean immigrant mothers. The results of this study suggest some implications for the providing educational and social support to the mothers.

서비스 디자인 프로세스 모델 연구

제조업 중심의 산업사회에서 지식산업 기반의 서비스사회로 세계 산업의 패러다임이 변화함에 따라 서비스 디자인의 중요성이 대두되고 있다. 제조업과 서비스업의 경계가 무너짐에 따라 모든 산업 분야에서 서비스 디자인 프로세스와 방법론에 대한 필요성과 요구가 증가했다. 그러나 서비스 디자인의 특성상 그 방법론이 방대하고 전문적이며, 서비스 디자인 프로세스의 실행에 있어 협업에 따른 시간과 비용의 문제 때문에 대부분의 기업 이 서비스 디자인 프로세스를 수행하기 어렵고, 아직 명문화 된 서비스 디자인 프로세스를 보유하고 못한 실정이다. 때문에 보다 용이하게 전체 산업에서 누구나 활용 할 수 있는 서비스 디자인 프로세스 모델을 개발할 필요성이 있다. 이에 따라 본 연구에서는 기존에 존재하는 서비스 디자인 프로세스에 비해 그 과정이 보다 단순하여 시간과 비용의 낭비를 줄이고, 창의적 문제해결을 적용하여 발산과 수렴의 사고를 통해 양질의 아이디어를 얻음으로써 전문가의 도움 없이도 진행할 수 있는 새로운 서비스 디자인 프로세스 모델을 제안하였다. 본 연구에서 개발된 모델은 ‘4-3-2 서비스 디자인 프로세스 모델’이라 명명하였고, 대학원생 그룹을 대상으로 예시 프로젝트를 적용하였다. 본 연구의 모델은 효율적으로 전체 서비스 디자인 산업 분야에서 활용될 수 있으며, 특히 기존의 서비스 디자인 프로세스가 너무 복잡하거나 추상적이라고 느끼는 기업에서 널리 쓰일 수 있다. As the industrial paradigm around the world changes from manufacturing to service industry, the importance of service design increases. And as the boundaries between manufacturing and service are collapsing, demands for the service design process and methodology also increase in all industries. However, most companies don"t have a service design process because the process is too complex due to the nature of the Service Design and there is the problem of expense and time. So, there is a need for developing a utilitarian service design process model which can be used for all industries. This study focused on proposing a new service design process model, based on the divergent and convergent thinking in Creative Problem Solving to get quality ideas while avoiding the expense and time of professional assistance. The "4-3-2 Service Design Process Model" was tested by means of an exemplary group project. The service design process model is proposed for the entire service design industry especially in companies which find the existing process too abstract and complex.

For quantitative analysis of MIT/CMIT in household chemical products Standard material development research

CMIT/MIT(Chloromethylisothiazolinone/Methylisothiazolinone) methyl has been widely used as a powerful preservative in household chemicals. However, regulations are being enforced across households due to strong inhalation toxicity. Related manufacturers need standard materials to check the contents and contents of CMIT/MIT in raw materials and finished products. A reference material must have the same or similar medium as a sample to compensate for the effect of the medium. Surfactants and spices are used as the main ingredients of cleaning agents, and various raw materials such as surfactants, fatty acids, waxes, diluents, preservatives, and spices are used for cosmetics. CMIT/MIT is developed by pro-nuclear attacks and is greatly affected by pH. In this study, the key goal is to create a chemical environment so that CMIT/MIT can exist reliably in cleaning agents and cosmetics media. Currently, CMIT/MIT standard substances in the form of living chemicals are not supplied at home and abroad. KTR is developing cosmetics cream preparation and cleaning liquid CMIT/MIT standard materials as a standard substance production agency and is planning to supply them to industries after 2023.

자음강화탕(보험제제)으로 호전된 갱년기 장애 3례에 대한 증례 보고 : Kupperman’s Index, Menopause Rating Scale을 중심으로

Objectives: This case report describes the management of three patients with menopausal disorder treated with Jaeumganghwa-tang, an herbal extract in national health insurance. Methods: Three patients suffering from Menopause disorder were hospitalized and treated with herbal extract Jaeumganghwa-tang for 4 weeks. This study was measured by Kupperman’s Index, Menopause Rating Scale and EuroQol-Five Dimensions. Results: The symptoms of menopausal disorder have improved after treating with Jaeumganghwa-tang. Also the Kupperman’s Index and Menopause Rating Scale have decreased, while the EuroQol-Five Dimensions has increased. Conclusions: As seen in these three cases, Jaeumganghwa-tang in herbal extract form has effects on the management of menopause disorder.
