        Streptozotocin으로 유발된 당뇨쥐에 대한 백강잠(白?蠶)의 혈당 및 당대사 효소활성에 관한 효과

        Objective : This study was undertaken to investigate how Bombycis corpus (BC) effects the development and progress of complications occurring in Diabetes Mellitus (DM). Methods : Laboratory rats were seperated into three groups; normal, rats with DM and treated with BC, and rats with DM and not treated. In this study DM was experimentally induced through injection of streptozotocin. The BC treated group was given BC extract p.o. for 15 days. Then, The activities of glucose phosphatic enzymes and polyol pathway channels were observed. Results : The blood glucose level greatly increased in the DM groups after injection of streptoztocin, but it significantly decreased in the BC treated group. Significantly enhanced levels of serum insulin levels were seen in the BC treated group, while supressed levels were seen in the untreated DM group. Weight was recovered by the BC treated group, matching the normal group. Decreased enzyme activity of aldose reductase, sorbitol dehydrogenase and glucose-6-phosphatase were seen in BC treated diabetic rats. Increased enzyme activity, of the glucokinase and hexokinase were seen in BC treated diabetic rats. Conclusions : This study suggests that BC normalized the blood glucose and serum insulin levels destablized by DM. Because increased activity of glucose phosphatic enzymes, glucokinase and hexokinase, and decreased glucose-6-phosphatase activity, and suppression of polyol pathway enzymes, aldose reductase and sorbitol dehydrogenase, were all seen, these observations suggest that BC suppresses blood glucose levels and prevents complications due to DM.

        현훈과 돌발성 난청을 주소로 하는 뇌교경색

        Anterior inferior cerebellar artery(AICA) is a branch of the basilar artery which supplies the bloods for ventrolateral cerebellum and the lateral tegmentum of the lower two-thirds of pons. AICA occlusion can cause such symptoms as acute-onset unilateral deafness, vertigo, facial weakness and ataxia. A case of sudden hearing loss, facial palsy and vertigo caused by AICA territory infarction recently presented itself. Korean Oriental and Western medical therapy were applied in combination. Facial palsy and vertigo improved, but hearing loss did not improve. This case is reported with a brief overview of related literatures.

        Concurrent use of Korean Medicine and Western Medicine: findings from the 2008 National Survey

        Objectives : The concurrent use of Korean Medicine (KM) and Western Medicine (WM) for the management of disease are increasing. In this study, the factors determining the choice of concurrent use of KM and WM was investigated based on national survey data for the public. Methods : Survey data from 1239 people with experience treating diseases with KM were utilized for this study. The national survey data were gathered using e-mail and face-to-face interviews between December 2007 and January 2008. Results : Among people who had been treated with KM, 819 (66%) used both KM and WM to the treat same disease. Multivariable analysis revealed that concurrent users of KM and WM were significantly associated with age 40–69 (OR=1.43, 95% CI=1.12-1.82), university education or higher (OR=1.34; 95% CI=1.05-1.71), enhanced health status (OR = 0.47, 95% CI=0.35-0.62) and unsatisfied with WM (OR = 1.71, 95% CI=1.02- 2.87). The respondents that reported KM was more effective than WM among concurrent users of KM and WM were significantly associated with being an office worker (OR=1.78, 95% CI=1.25-2.53), age 40–69 (OR = 0.62, 95% CI=0.44-0.86), unsatisfied with WM (OR=2.61, 95% CI=1.51-4.50) and overall satisfied with KM (OR=8.38, 95% CI=5.65-12.44) Conclusions : This study showed that two-thirds of KM user were concurrent user of KM and WM, and some influential factors determined the choice of a concurrent use of KM and WM. Therefore, Korean Medicine Doctors should consider the possibility of concurrent use, need to know the information on the benefits and risks of concurrent use.

        Association between cold-heat symptoms and sleep disturbances according to the Sasang constitution: a cross-sectional community study

        Objectives : Evidence supporting the cold-heat symptom and sasang constitution type, which are diagnostic items of traditional Korean medicine, is needed to manage sleep disturbances, which is a typical symptom of mibyeong (subhealth). This study examined the association between each cold-heat symptom and sleep disturbances according to each sasang constitution type. Methods : This research was a cross-sectional study of 5,793 subjects from the Korean Medicine Data Center (KDC) community cohort survey. The association between each cold-heat symptom and sleep disturbances was analyzed by logistic regression analysis adjusted for several demographic variables. Subgroup analysis was then performed for each type of sasang constitution. Results : The soeum and soyang types were 1.53 and 1.26 times more likely to have sleep disturbances than the taeum type. Sleep disturbances were associated with ‘coldness of the abdomen’, ‘watery mouth’ in the cold domain items, and ‘body feverishness’, ‘flushed face and eye’, ‘thirst’, and ‘scanty dark urine’ in the heat domain items. The soeum and soyang types were 1.55 and 1.39 times more likely to sleep less than five hours per night than the taeeum type. In addition, the associations of those showed a different pattern for each sasang constitution type. Conclusions : Sleep disturbances are associated with specific cold-heat symptoms, and the associated coldheat symptoms differ according to the sasang constitution type. These results may help traditional medicine specialists select customized interventions for patients with sleep disturbances.

        노인성 폐렴환자 1례에 대한 증례보고

        Pneumonia is an inflammation that forms at the mesopulmonum and alveolus that is also an acute infection with fever and cough. It is difficult for doctors to diagnose pneumonia in elderly patients, due to the variety of symptoms. For this reason, it, with its complications, is a major cause of death for the elderly. In this case, improvement was seen in an elderly patient diagnosed with pneumonia by X-ray, and by using Si-Kyung-Ban-Ha-Tang-Ka-Mi-Bang as a way of Kong-Bo-Kyum-Si, because in this case pneumonia was presumed Dam-Yel-Ong-Pye and Ki-Eum-Yang-Huh due to the weakness associated with old age. This thesis proposes that the elderly are better treated differently from other pneumonia patients due to the weakness that comes with old age.

Methods : The subjects were divided into 4 groups : control, acupuncture , pharmacopuncture PA-J1(11.43 ㎎ / 250 g / 40 ㎕) and pharmacopuncture PA-J2(2.29 ㎎ / 250 g / 40 ㎕). The focal brain ischemia was induced by intraluminal filament insertion into the middle cerebral artery. After 3 days of MCAO, Jodeungsan pharmacopuncture treatment was performed on the GB20, and the day after being treated with pharmacopuncture, the Morris water maze test was carried out on the assigned group. The series of processes were administered 6 times. Thereafter mGluR5, density of neuronal cell and ChAT were measured. Results : The results were as follows. 1. The distance to target significantly decreased in the 2nd trial of the Acu group on the water maze test for short-term memory. 2. The distance to target significantly decreased in the 4th trial of the PA-J2 group on the water maze test for long-term memory. 3. The intensity of mGluR5 significantly increased in the PA-J1 group compared with the control group. 4. The neuroprotective effect on the hippocampal CA1 significantly increased in the PA-J1 and PA-J2 groups compared with the control group. 5. The density of ChAT in the hippocampal CA1 significantly increased in the PA-J1 and PAJ2 groups compared with the control group. Conclusion : These results suggest that Jodeungsan pharmacopuncture may improve memory and cognitive impairment and also have neuroprotective effects on focal brain ischemia.

        Effect of Work on Medical Expenditures by Elderly: Findings From the Korean Health Panel 2008–2013

        Background: This study was conducted to investigate the effects of work on medical expenditures by the elderly. Methods: Data pertaining to individuals aged 65 or older collected by the Korean Health Panel 2008 -2013 were used. The effects of work on medical expenditures were analyzed in a panel tobit model adjusted for several variables of demographic factors, socioeconomic factors, and health factors for health care. Data were also analyzed based on age groups (65-74, 75 ), type of work (waged or self-employed), and working time (daytime work or night time work). Results: Among the elderly older than 65 years, 34-37% were workers. Work among the elderly reduced medical expenditures relative to nonworking elderly. Specifically, medical expenditures were lower in individuals older than 75 years, as well as among those who were self-employed insured and had medical aid insurance and those who exercised. However, medical expenditures were higher among females, married individuals, those with a higher household income, and those with a chronic disease. Elderlywageworkers showed reduced medical expenditures than nonworking elderly and elderly daytime workers did. Conclusion: The elderly population’s work, especially wage work and daytime work, reduced medical expenditures relative to no work. These results provide valuable information for policymakers by indicating that work was associated with lower medical expenditures than no work. If elderly work is to be encouraged, it is necessary to provide a variety of high-quality wage work.
