Chronic eosinophilic pneumonia is a rare diasease characterized by chronic infiltration of the lung witheosinophils, usually associated with peripheral eosinophilia. In 65% of cases, the chest radiograph shows typicalnonsegmental air-space consolodation confined to the outer third of the lung, and in 25% of cases, the"photographic negative of pulmonary edema". Typical lung manifestations with peripheral eosinophilia arecharacteristic of chronic eosinophilic pneumonia. In the remaining cases, radiographic findings are nonspecificand require lung biopsy for confirmation. We repot a case of chronic eosinophilic pneumonia in which chestradiograph and CT scans revealed bilateral patchy or diffuse opacity with nodules scattered throughout the lungs.the lungs.

대망에 발생한 STUMP : 1례 보고 report of 1 case

평활근에서 기원하는 종양은 크게 양성의 평활근종(leiomyoma)과 악성의 평활근육종(leiomyosarcoma)으로 대별된다. 그 중 악성은 병리학적으로 10개의 고배율시야당 5개 이상의 유사분열(Smitoses/10HPF)을 보이는 경우로서, 임상적으로 간과 폐에 전이를 보인다. 그러나 평활근육종의 40%가량에서 유사분열 수는 양성 (fewer than 5 mitoses/10 HPF)을 보이는 경우가 보고되어 , 동결절편 검사에서 그 악성도의 판명이 어려워 그 치료방침을 정하기 힘든 경우가 되고 있다. 이런 경우를 "악성도를 결정하지 못한는 평활근 종양(smooth muscle tumors of undeterminate malignant potential[STUMP])"이라고 칭한다. 이들 종양은 악성에 준하는 치료를 하는 것을 원칙으로 하며 조직괴사, 높은 세포 밀도 및 이형성을 보인다. 저자들은 최근 대망에서 발생한 STUMP 1례를 경험하여 문헌 고찰과 함께 그 방사선학적 소견을 보고하는 바이다. Tumors arising from the smooth muscle contains benign leiomyomas & malignant leiomyosarcomas. One of the most important criterion, differentiating the malignant one from the benign leiomyoma, is mitotic count. If the mitotic count is more than 5 mitoses per 10 high power fields, the mass is malignant. And tumor cell necrosis, pleomorphism and high cellularity are supplementary findings. Forty percent of leiomyosarcomas show mitotic count less than 5 mitoses per 10 HPF, and in these cases it is difficult to determine the malignancy of tumors. The smooth muscle tumors that show malignant findings, such as high cellularity, pleomorphism and tumor cell necrosis, but fewer than 5 mitoses/10HPF, are called as "smooth muscle tumors of undeterminate malignant potential[STUMP] ". We report one case of STUMP arising from greater omentum, in which ultrasonography and CT scans show mainly cystic mass with septation & mural solid mass. Index Words : smooth muscle, tumors, STUMP

신장의 비외상성 자연 파열 : 원인 질환 및 CT 소견

Purpose: To evaluate the usefulness of CT scanning in determining the etiology of spontaneous rupture of the kidneyMaterials and Method: We retrospectively analyzed the CT findings of spontaneous rupture of the kidney in eleven patients, Four were male and seven were female, and they were aged between 20 and 71 (mean, 46.6) years. Both pre- and post-contrast enhanced CT scanning was performed in all patients.Results: Spontaneous renal rupture was induced in seven cases by neoplasms (three angiomyolipomas, three renal cell carcinomas, and one metastatic choriocarcinoma), in three cases by infection or inflammation (acute and chronic pyelonephritis, and renal abscess), in three cases by infection or inflammation (acute and chronic pyelonephritis, and renal abscess), and in one, by renal cyst. Common CT findings of rupture of the kidney were the accumulation of high density fluid in the perirenal and anterior pararenal space, and inhomogeneous irregular low density of renal parenchyma and the rupture site. Angiomyolipoma showed fat and an angiomatous component in the lesion, while acute and chronic pyelonephrities revealed thinning of the renal parenchyma and an irregular renal outline. Renal cell carcinoma showed a dense soft tissue mass in the parenchyma. Well-defined, round low-density lesions were noted in the case of renal cyst and renal abscess.Conclusion: CT is very useful in diagnosing and determining the etiology of non-traumatic spontaneous rupture of the kidney and plays an important role in the evaluation of emergency cases.

A 50-year-old man who had suffered from chronic indigestion complained of upper abdominal pain and vomiting which had persisted for 4 days. Consequently, he had eaten two persimmons before sleeping detected each day for five days. An irregular multinodular villous filling defect was in the duodenal bulb on, the UGI series, which had its location and changed its position. It was difficult to differentiate a papillary gastric polyp or villous tumor from the duodenal bezoar, but the patient's history of having eaten persimmons was helpful. The endoscopic and pathologic study confirmed a phytobezoar which was located in the normal duodenal bulb. Subsequently we are reporting this case having reviewed the related literature of the phytobezoar which was found in the normal duodenal bulb.

Sclerosing Hemangioma of the Lung -a case report- Sclerosing hemangioma of the lung is a rare benign neoplasm of uncertain histogenesis, with characteristic histologic and radiologic appearance.. We experienced a case of histologically proven sclerosing hemangioma of the lung in a 30 year old woman who complained dyspnea on exertion and chest discomfort. Chest radiograph showed a 2x2.5 cm sized, well circumscribed, ovoid soft tissue nodule without calcification on the pericardiac area of the left lower lung zone. CT scan revealed a well defined and smooth marginated, avoid mass with marked enhancement on the lingular division of the left upper lobe. An enucleation fo the nodule for diagnosis and treatment was performed, then histologically confirmed as sclerosing hemangioma of the lung.
