Branchial anomalies are the most common cause of congenital lateral neck masses. Second branchial anomalies account for 90% of all branchial anomalies, and cysts are more common than sinuses and fistulas. Branchial cysts frequently occur close to the anterior border of sternocleidomastoid muscle. However, their development in oropharynx is relatively rare. We experienced a case of branchial cyst in palatine tonsil presenting with throat discomfort and hemorrhage after vomiting. Histological examination revealed that the mass is filled with lymphoid aggregation, lined with squamous epithelium, which was a distinctive feature of branchial cleft cyst. The cyst was assumed to arise from a second branchial arch based on anatomic location. We made a report of a branchial cleft cyst within a palatine tonsil with a review of literature.

Due to space and geographical constraints, the power source may be located outside the island area, resulting in the considerable length of transmission line. In these cases, when an active power is transmitted, unexpected reactive power is generated at a point of common coupling (PCC). Unlike the power transmitted from the power generation source, the reactive power adversely affects the system. This study proposes a new algorithm that controls reactive power at PCC. Causes of reactive power errors are separated into parallel and series components, which allows the algorithm to compensate the reactive current of the inverter output and control reactive power at the PCC through calculations from the impedance, voltage, and current. The proposed algorithm has economic advantages by controlling the reactive power with the inverter of the power source itself, and can flexibly control power against voltage and output variations. Through the simulation, the algorithm was verified by implementing a power source of 3 [kVA] capacity connected to the low voltage system and of 5 [MVA] capacity connected to the extra-high voltage system. Furthermore, a power source of 3 [kVA] capacity inverter is configured and connected to a mock grid, then confirmed through experiments.

Herpes zoster oticus is a common inflammatory disease caused by reactivation of varicella zoster virus in the dorsal root ganglia. Its symtpoms include unilateral distribution of the vesicular eruptions, neuralgia and various type of cranial nerve palsy depending on the involving site of head and neck region. Most commonly affected cranial nerves are facial nerve (VII) and vestibulocochelar nerve (VIII), but on rare occasions cranial nerves V, IX, X, XI are affected as well. Although neurologic symptom of herpes zoster was commonly reported, zoster associated hiccups is very rare. We experienced one case of herpes zoster oticus involving multiple cranial nerve palsy with intractable hiccups. We report our case with a review of literature.

In order to increase the power generation efficiency of the photovoltaic system, a new algorithm that can follow the maximum power point of the photovoltaic power generation system having nonlinear output characteristics is proposed. Conventional maximum power point tracking (MPPT) algorithms such as Perturbation and Observation (P&O) and InCond (Increment and Conductance) schemes can not find the global maximum power point at a plurality of pole points in the unmatched state of unbalanced PV modules. However, even if the global maximum power point is found at a plurality of pole points, the global maximum power that can not be the real maximum power by the photovoltaic generation system. In order to solve this problem, a few PV companies propose installing several small PV inverters instead of if big one. However, since this will require additional costs, we herein propose a Multi-MPPT system using individual 3-point MPPT to track true MPPT at a plurality of pole points in the unmatched state of unbalanced PV modules.

Esophageal diverticulum is a rare entity, the true incidence of which is unknown, mainly because there are usually no clinical symptoms. Most esophageal diverticulum are found incidentally during an endoscopic or radiologic examination. Their classification is based on anatomical location; namely, upper third(Zenker or pharyngoesophageal), middle third(thoracic), or lower third(epiphrenic). Here we report a 52-years-old female presenting with dysphagia and regurgitation. Esophagogram showed esophageal diveticulum at lower cervical esophagus. Its positional aspect, it is different from Zenker's diveticulum. Treatment is surgical via an endoscopic or external approach. In view of the patient's age and anatomical location, various surgical approaches were considered as a therapeutic option for the management. This paper presents our experience in the management of esophageal diverticulum which was unusual location and stapled open resection without sternotomy.

When Photovoltaic inverter is connected to grid and used as PVPCS (Photovoltaic Power Conditioning System), 120 Hz AC ripple occurs at the dc-link capacitor voltage. This AC ripple reduces the efficiency of PVPCS and shortens the lifetime of the capacitor. In this paper, we design a notch filter to remove AC ripple. As a result, the AC voltage ripple was removed from the dc link and the THD of the PVPCS output current with the notch filter was lowered. This notch filter is determined by the damping coefficient, the bandwidth coefficient, and the switching frequency. Among these, the switching frequency determines the switching loss and the size of the LC filter, and the PVPCS with the high switching frequency has a greater efficiency loss due to the switching loss than the efficiency improvement by the notch filter. Therefore, it is important to set the optimum switching frequency in the PVPCS with the notch filter applied. In this paper, THD and switching loss of PVPCS output current with notch filter are calculated through simulation, and cost function to calculate optimum switching frequency through data is proposed.

Background and Objectives Recently, The number of patients who have been complaining of the vertigo or dizziness has been increasing due to rapid growth of the elderly population and senile change. Aging is associated with decreased balance abilities, resulting in an increased risk of fall. The purpose of this study is to analyze the result of elderly organized vestibular rehabilitation therapy for presbystasis. Materials and Methods A prospective study was performed on elderly population over sixty-five years. 148 dizzy patients who were admitted to Hallym University Medical Center. We recruited 64 consecutive patients with a diagnosis of presbystasis and treatment of rehabilitation. They were asked to complete the Korean vestibular disorders activities of daily living scale (K-VADL) for the functional aspect before and two to twelve weeks after elderly organized vestibular rehabilitation therapy. The result was analyzed by statistical methods. Results The K-VADL scores were decreased in patient of presbystasis compared with first questionnaire. There were statistically significant differences in the K-VADL scores between before and after rehabilitation therapy. Conclusion Most patients can effectively and safely utilize the modified vestibular rehabilitation therapy. Our results suggest that the vestibular rehabilitation therapy could provide a recovery for dizzy symptoms in presbystasis.
Cimetidine 마취전 투약이 위액 산도에 미치는 영향

The effect of preanesthetic administration of glycopyrrolate and cimetidine on gastric fluid pH and volume were studied. Forty patients scheduled for elective surgery were fandomly placed into two groups. Patients in group 1 were given glycopyrrolate intramuscularly (0.003 mg/kg) 30-60 minutes before induction of anesthesia, while group 2 patients received glycopyrrolate intramuscularly and 200mg of cimetidine intravenouly 2 hours before induction of anesthesia. Nasogastric tubes were placed and gastric pH and volume were measured. The results showed that patients in group 1 hade a medn gastric pH of 2.428±1.378. Twenty five percent of these patients had gastric contents with a pH of 2.428±1.378. Twenty five percent of these patients had gastric contents with a pH above 2.5 and a volume greater than 25ml. In contrast, group 2 patients had a mean gastric pH of 5.566±1.969, 85 percent had a ph above 2.5 and 15 percent had a volume greater than 25ml. These findings demonstrate that cimetidine markedly increasgastric pH. The authors recoment that patients reciving general anesthesia be given cimetidine preoperatively.
