        구미공단 불화수소 누출사고 주변 지역 근로자들의 화학물질과민증 유병률

        Objectives: This study was conducted to evaluate the prevalence rate of multiple chemical sensitivity/idiopathic environmental intolerance(MCS/IEI) among workers in the Gumi industrial complex around the region of accidental release of hydrogen fluoride in 2012. Materials: We evaluated MCS/IEI using the Korean version of the Quick Environmental Exposure and Sensitivity Inventory(QEESI). A total of 535 workers at six manufacturing companies in the Gumi industrial complex were investigated using self-administered questionnaires from February to March 2015. After exclusion of incompletely answered questionnaires, 271 were analyzed. Results: The prevalence rate and proved positive rate of MCS/IEI were 5.9%(16 out of 271) and 3.7%(10 out of 271), respectively. The scores of chemical intolerance, other intolerance, symptom severity and life impact were significantly higher(p<0.05) in females than those of males. In terms of masking index scores, males showed significantly higher(p=0.003) than female. The self-reported MCS/IEI prevalence rate, 7.7%, of workers exposed to hydrogen fluoride in 2012 was higher than no-exposure group(5.6%), but not statistically significant(p=0.815). Conclusions: Although the prevalence rate of MCS/IEI symptoms of workers exposed to hydrogen fluoride gas in 2012 was not significantly higher than no-exposure group, it is necessary to conduct follow-up study on the exposure group of hydrogen fluoride.

        한국 성인과 노인을 대상으로 이중표식수법을 이용한 신체활동분류표 타당도 평가

        그동안 국내 연구에서 PAL을 산출하기 위해 사용해온 18단계 신체활동분류표로 신체활동을 평가하는 데 한계가 있었기에 새롭게 한국인을 위한 신체활동분류표가 보고되었다. 이에 본 연구에서 총에너지소비량을 측정하는 gold standard인 이중표식수법을 이용하여 새로 보고된 신체활동분류표의 타당도를 평가한 결과, TEE<SUB>DLW</SUB>와 TEE<SUB>PACT</SUB> 간의 유의한 차이가 없었으며 TEE<SUB>DLW</SUB>와 TEE<SUB>PACT</SUB> 간의 스피어만 상관관계 (r = 0.769)는 유의한 높은 양의 상관성을 보였다. 즉, 본 연구에서 TEE 및 PAL 산출을 위해 새로운 신체활동분류표의 사용이 타당한 것으로 평가되었으며, 앞으로 이와 관련된 연구 결과는 2025 한국인 영양소 섭취기준에서 에너지필요 추정량 산출식에 필요한 신체활동단계별 계수 (PA)를 결정하는데 유용한 자료가 될 것으로 기대된다. 또한 개인별 및 연구 목적으로도 쉽게 사용하기 위해서, 신체활동분류표를 이용한 웹 버전의 소프트웨어 프로그램 및 스마트폰 어플리케이션의 개발이 필요하다. Purpose: This study evaluated the validity of a physical activity classification table (PACT) based on total energy expenditure (TEE) and physical activity level (PAL) measured using the doubly labeled water (DLW) method in Korean adults and the elderly. Methods: A total of 141 (male 70, female 71) adults and elderly were included. The reference standards TEE<SUB>DLW</SUB>, PAL<SUB>DLW</SUB> were measured over a 14-day period using DLW. A 24-hour physical activity diary was kept for three days (two days during the week and one day on the weekend). PAL<SUB>PACT</SUB> was calculated by classifying the activity type and intensity using the PACT. PAL<SUB>PACT</SUB> was multiplied by resting energy expenditure measured by indirect calorimetry to estimate TEE<SUB>PACT</SUB>. Results: The mean age of the study participants was 50.5 ± 18.8 years, and the mean body mass index was 23.4 ± 3.3 kg/m2. A comparison of TEE<SUB>DLW</SUB> and TEE<SUB>PACT</SUB> by sex and age showed no significant differences. The bias, the difference between TEE<SUB>DLW</SUB> and TEE<SUB>PACT</SUB>, was male 17.3 kcal/day and female −4.5 kcal/day. The percentage of accurate predictions (values within ± 10% of the TEE<SUB>DLW</SUB>) of TEE<SUB>PACT</SUB> was 58.6% in males and 54.9% in females, with the highest prediction values in the age group 40–64 years (70.9%) in males and over 65 years (73.9%) in females. The spearman correlation coefficient (r) between TEE<SUB>PACT</SUB> and TEE<SUB>DLW</SUB> was 0.769, indicating a significant positive correlation (p < 0.001). Conclusion: In this study, the use of a new PACT for calculating TEE and PAL was evaluated as valid. A web version of the software program and a smartphone application need to be developed using PACT to make it easier to apply for research purposes.

        다중 영향 및 환경정의를 고려한 지역 환경보건수준 평가 사례연구

        Objectives: Based on the concept of environmental justice, we developed an evaluation model for setting and adopting the direction of environmental health policy using environmental health indicators and statistics from the local governments of Seoul, including environmentally susceptible populations. Methods: We selected a total of 20 variables based on data officially released from national and local governments. After the classification of these variables into the five components of environmental pressure, environmental status, environmental disease, sensitive population, and socioeconomic status, a basic model was constructed to calculate the relative scores of the local governments. Results: The cumulative impact scores for assessment of environmental health status were similar to those of the environmental pressure and status components. The highest five cumulative impact scores fell between 114 and 147. Local government A reported the highest scores and had high environmental pressure, environmental status and socioeconomic status. In addition, the evaluation of the other four local governments indicated that they exhibited above average scores for environmental pressure and environmental status, two did so for environmental disease, and four for sensitive population. Conclusion: We constructed a model to evaluate the environmental health status of the local governments of Seoul based on cumulative impact scores under the concept of environmental justice. As an approach for studying environmentally vulnerable areas through relative ranking, this model was feasible for policy-setting. In addition, this approach would be an analytically useful tool for decision-makers.

        어깨온열치료 효과에 따른 반도체 홀소자 집게형맥진기와 PPG로 측정한 말초혈류속도 특성 연구

        The average life expectancy of modern people is rapidly increasing due to the development of medicine. The shoulder thermotherapy device (STD) used as a home medical device is studied for the purpose of relieving muscle pain in the upper part of the human body. The peripheral blood flow velocity (PBFV) could be measured from the two waveforms obtained by simultaneously measuring the pulse waves obtained by using a Hall element clip-type pulsimeter in the radial artery of the wrist and a photo-plethysmography (PPG) in the finger. Two women in their 20s with normal blood pressure used the STD by twice per a week for 13 weeks and compared before, middle, and after treatment, the PBFV improved by 12% to 17%.
