        Upper Arm Contouring with Brachioplasty after Massive Weight Loss

        Background: As the obese population increases in Korea, the number of patients who are trying to lose weight has been increasing steadily. In these patients, skin laxity and deformation of the body contour occurs, which could possibly be corrected by various body contouring surgeries. Here, we introduce the brachioplasty method and our experience of various body contouring surgeries performed in our center. Methods: From November 2009 to August 2011, five cases of brachioplasty were performed. When the patient presented with sagging of the lateral inframammary crease and bat wing deformity in the axilla, extended brachioplasty was performed; in this case, the deformation of the axilla and lateral chest was corrected at the same time. A traditional brachioplasty was performed when contouring was needed only for skin laxity in the upper arm. Results: Complications, such as hematomas or nerve injuries, were not evident. Some patients experienced partial wound dehiscence due to tension or hypertrophic scars found during the follow-up. In general, all of the patients were satisfied with the improvement in their upper arm contour. Conclusions: Given the demands for body contouring surgery, the number of brachioplasty surgical procedures is expected to increase significantly, with abdominoplasty comprising a large portion of these surgeries. For the brachioplasty procedure, preparation and preoperative consultation regarding design of the surgery by experienced surgeons was important to prevent complications such as nerve damage or hematoma formation.

        Combined, Minimally Invasive, Thread-based Facelift

        Background As society changes, patients have high expectations of plastic surgeryand rapid recovery after surgery. A thread-based facelift meets these requirementsand is growing in popularity. Methods Former thread lifts were either floating or fixed types. The authors used bothtypes. Using a vertical line from the lateral orbital rim as the center, the anterior facewas lifted with floating-type threads and the lateral face was lifted with fixed-typethreads. The Blue RoseTM thread was used for the combined-type facelift, as it is strongerthan other threads. Improvements were measured using the Global AestheticImprovement Scale (GAIS). Results Eighteen patients out of 28 were followed over 3 months. Five patients (28%)reported a GAIS score of 2, 10 patients (55%) had a GAIS score of 3, 2 patients(11%) had a GAIS score of 4, and one patient was unsatisfied with the surgicalresults. Results showed improvements in 83% of the patients. Conclusions The combined, minimally invasive, thread-based facelift has somebenefits. First, the functional anatomy of the face is considered. Second, the pulling force ofthe threads is stronger than formerly used threads. Third, especially in Asian patients,the postoperative broadening of the malar area can be minimized. Thus, a combinedthread lift using Blue Rose threads can provide a natural-looking and strong facelift.

        청소년들의 학교따돌림 피해 위험과 인터넷게임장애 위험의 연관성 : 성별차이 중심으로

        Purpose: The purpose of this study was to investigate the relationship between the risk of school bullying victimizationand the risk of Internet gaming disorder according to gender in adolescents. Methods: The data of 1,920middle school students collected at the baseline of the Internet user Cohort for Unbiased Recognition of gamingdisorder in Early Adolescence (iCURE) study were analyzed. For statistical analysis, x2 test, t-test and stratifiedmultiple logistic regression analysis were conducted using SAS 9.4. Results: The prevalence rate of Internet gamingdisorder of middle school boys was greater than that of girls (Boys: 9.9%, Girls: 6.2%). The greater the riskof school bullying victimization, the greater both the risk of Internet gaming disorder and the average daily timespent on Internet gaming. In girls, the relationship between the experience of being bullied in school and Internetgaming disorder was not statistically significant. However, the boys who had been bullied in school were 3.2 timesmore vulnerable to the risk of Internet gaming disorder than those without such experience (95% CI: 1.135-8.779). Conclusions: When considering interventions for Internet gaming disorder for adolescents, bullying victimizationshould be taken into account as well. Particularly, relieving stress related to bullying victimization can be importantfor boys with Internet gaming disorder.

      • 단일 영상 초해상도를 위한 질감-공간 분리 기반의 특징 분류 네트워크

        본 논문은 단일 영상을 이용하여 초해상도 방법을 수행하기 위해 질감-공간 영역을 분리한 뒤 세부정보를 중심으로 특징을 분류하는 방법을 제안한다. CNN(Convolutional Neural Network) 기반의 초해상도는 세부정보를 개선하기 위한 특징 추정 과정에서의 복잡한 절차와 중복된 특징 정보의 생성으로 인해 초해상도에서 가장 중요한 기준인 품질 저하가 발생할 수 있다. 제안하는 방법은 절차적 복잡성을 줄이고 중복 특징 정보의 생성을 최소화하여 초해상도 결과의 품질을 개선하기 위해 입력 영상을 질감과 공간의 두 채널로 분리하였다. 질감 채널에서는 세부정보 복원을 위해 다중스케일로 변환한 영상에 단계별 skip-connection을 적용한 잔차 블록 구조를 적용하여 특징 정제 과정을 수행함으로써 특징 추출을 개선하였고, 공간 채널에서는 평활화된 형태의 특징을 활용하여 잡음을 제거하고 구조적 특징을 유지하도록 하였다. 제안하는 방법을 이용해 실험한 결과 기존 초해상도 방법대비 PSNR 및 SSIM 성능 평가에서 향상된 결과를 보여 품질이 개선됨을 확인할 수 있었다. In this paper, I proposes a method for performing single image super resolution by separating texture-spatial domains and then classifying features based on detailed information. In CNN (Convolutional Neural Network) based super resolution, the complex procedures and generation of redundant feature information in feature estimation process for enhancing details can lead to quality degradation in super resolution. The proposed method reduced procedural complexity and minimizes generation of redundant feature information by splitting input image into two channels: texture and spatial. In texture channel, a feature refinement process with step-wise skip connections is applied for detail restoration, while in spatial channel, a method is introduced to preserve the structural features of the image. Experimental results using proposed method demonstrate improved performance in terms of PSNR and SSIM evaluations compared to existing super resolution methods, confirmed the enhancement in quality.

        단일이미지에서의 초해상도 영상 생성을 위한 패치 정보 기반의 선형 보간 연구

        본 논문은 단일 이미지에서 초해상도 영상 생성을 위해 저해상도 이미지에서 생성한 패치정보를 기반으로 선형 보간하는 방법을 제안하였다. 기존의 초해상도 생성 방법인 전역 공간의 회귀 모델을 사용하면 특정 영역에 대해 참조할 정보가 부족하여 일반적으로 품질이 떨어지는 결과가 나타난다. 이러한 결과를 보완하기 위해 제안하는 방법은 초해상도 이미지 생성 과정에서 영상을 패치 단위로 지역을 분할하여 의미있는 정보를 수집하고, 수집된 정보를 기반으로 초해상도 이미지 생성을 위해 확장시킨 이미지 매트릭스 영역의 구성정보를 분석하여 선형 보간 과정을 거치고 패치정보를 대응시켜 탐색한 최적의 패치 정보를 기준으로 선형 보간하는 방법을 제안하였다. 실험을 위해 원본 이미지를 복원된 영상과 PSNR, SSIM으로 비교 평가하였다. In this paper, we propose a linear interpolation method based on patch information generated from a low - resolution image for generating a super resolution image in a single image. Using the regression model of the global space, which is a conventional super resolution generation method, results in poor quality in general because of lack of information to be referred to a specific region. In order to compensate for these results, we propose a method to extract meaningful information by dividing the region into patches in the process of super resolution image generation, analyze the constituents of the image matrix region extended for super resolution image generation, We propose a method of linear interpolation based on optimal patch information that is searched by correlating patch information based on the information gathered before the interpolation process. For the experiment, the original image was compared with the reconstructed image with PSNR and SSIM.

        Retroperitoneal Giant Liposarcoma

        Retroperitoneal liposarcoma is an infrequent, locally aggressive malignancy. We report two cases of huge retroperitoneal liposarcomas. The presence of a palpable abdominal mass was a common symptom of the two patients. Preoperative imaging study showed huge retroperitoneal tumors. Both patients underwent complete surgical resections, and a negative microscopic margin was achieved in both cases. The histopathologic diagnosis was a well-differentiated retroperitoneal liposarcoma. Neither of the two patients developed a recurring tumor during the 1.5 years of follow-up.

        Clinical Assessment of Follow-Up Cystatin C-Based eGFR in Live Kidney Donors

        Purpose: We aimed to compare the cystatin C-based estimated glomerular filtration rate (eGFR) and the serum creatinine-based eGFR and to investigate the clinical roles of the cystatin C-based eGFR in assessing the follow-up renal function of kidney donors. Materials and Methods: We enrolled 121 healthy kidney donors who underwent live donor nephrectomy between October 2009 and December 2010 in a prospective manner. Serum creatinine and cystatin C were measured preoperatively and were followed after the surgery (1st, 4th, and 7th postoperative day and 1st, 3rd, 6th, and 12th postoperative month). We also compared the sensitivity and specificity of each eGFR method for predicting the development of chronic kidney disease (CKD) after donor nephrectomy. Results: For those who had a Modification of Diet in Renal Disease postoperative day 4 eGFR of less than 60 ml/min/1.73 m2, the probability of developing CKD was 89.0% (Chronic Kidney Disease Epidemiology Collaboration eGFR, 66.0%; Cockcroft-Gault eGFR, 74.0%; cystatin C eGFR, 57.1%). A cystatin C eGFR of below 60 ml/min/1.73 m2 at postoperative day 4 predicted CKD at 6 months with a specificity of 90.3%, which was the highest among the estimation methods used. Cystatin C eGFRs were generally higher than the creatinine-based eGFRs. Conclusions: We conclude that cystatin C-based estimations of the GFR are helpful for predicting the recovery of renal function in kidney donors and could be added to the follow-up protocol of kidney donors who may develop CKD, especially patients whose immediate postoperative renal function is marginal.
