증례 : 감염 ; 위절제술 후 Streptococcus mitis에 의한 비장 Botryomycosis 1예

Botryomycosis는 과립상 물질을 형성하는 만성 화농성 육아종 감염 질환이고 세균이 원인이며 조직학적으로 진균종, 방선균균종, 방선균증과 감별된다. Botryomycosis의 발생에 있어 세균의 독성에 대한 숙주의 국소적인 면역체계 이상이 원인이 될 수 있으나 아직 명확하게 밝혀진 것은 없다. 내장형 botryomycosis는 비특이적인 증상을 보이고 특징적인 방사선 소견이 없어 종양으로 오진되기도 하므로 진단에 주의를 요한다. 지금까지 전 세계적으로 사람에서 발생한 botryomycosis는 200예 이상의 보고가 있었으나 비장을 침범한 사례는 2예밖에 없었으며 우리나라에서 보고된 증례는 없었다. 이에 저자들은 면역 기능이 정상인 53세 여자에서 위암진단 후 시행한 위절제술에서 합병된 Streptococcus mitis에 의한 botryomycosis의 비장 침범 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Botryomycosis is an uncommon chronic suppurative bacterial infection of the skin and visceral organs seen primarily in immunocompromised patients. Here, we report a case of splenic botryomycosis caused by Streptococcus mitis in a 53-year-old immunocompetent woman with a history of distal gastrectomy for advanced gastric cancer. 신(腎)과 갑상선(甲狀腺)의 상관성(相關性)에 관(關)한 고찰(考察)

So far we oriental medical doctors have referred to Shin(腎) as endocrine system, especially suprarenal gland, sexual gland and autonomic nervous system, thyroid etc. as well as kidney. But the sight on thyroid is weak and the relationship with Shin(腎) hasn't been suggested clearly. The purpose of this study is to find out the relationship between Shin(腎) with thyroid. The following are the results. 1. Ki Gi(氣機) of Shin(腎) is similar to the fuction of thyroid in that they are the base of human metabolism, influencing all the metabolism of human tissue. 2. Shin Yang(腎陽) and thyroid hormone are almost identical in that they are the base of Yang Gi(陽氣), the former as source of heat energy, the latter as energy hormone. 3. Shin(腎) and thyroid hormone are almost the same in that they activate growth of human body. 4. Ki Gi(氣機) of Shin(腎) such as Ju Gol(主骨), Saeng Su(生髓), Tong uh Noi(通於腦) is similar to the effects of thyroid hormone on bones, central nervous system and hair 5. The symptoms of deficiency of Shin Eum(腎陰虛) are almost identical with those of hyperthyroidism, so the process of Yang Hwa Gi(陽化氣) caused by exuberance of Yang due to deficiency of Shin Eum(腎陰虛陽亢) is similar to excessive metabolism caused by hyperthyroidism. 6. The process of Eum Seong Hyung(陰成形) caused by preponderance of Eum due to deficiency of Shin Yang(腎陽虛陰盛) is similar to the lowering of metabolism caused by hypothyroidism. 7. Deficiency of Shin Eum(腎陰虛) is similar to hyperthyroidism, deficiency of Shin Yang(腎陽虛) to hypothyroidism. But there are major difference in edema and the fuction of intestine. To conclude, Ki Gi(氣機) of Shin is similar to the function of throid in many respects. I hope that there will be further studies on the relationship beteen thyroid malfuction with deficiency of Shin Yang(腎陽虛) or of Shin Eum(腎陰虛) in the future.

Acute pancreatitis is the acute inflammatory disease of the pancreas. and abcomal pain on demanding emergency care is the major symptom of acute pancreatitis. In these two cases, we administrated oriental medical treatment with medical treatment such as fluid injection. After administration of these medication, clinical symtoms and clinicopathological figure improved. Oriental medical treatment showed disirable effect on acute pancreatitis.

Objective : This study was designed to investigate the possibility of AR for chronic renal injury. Methods : The author first investigated the expression levels of DNA by inducing of chronic renal injury. Then, the author investigated the effects of AR on chronic renal injury induced by combination treatment with Adriamycin and cisplatin in terms of changes in body weights and renal tissues, urine volume, BUN and creatinine levels, creatinine clearance and histopathological changes in renal tissues. Total expression levels of 546 genes were elevated or lowered by induction of chronic renal injury. Genes of which whose expression levels were elevated by induction of chronic renal injury were related to the PPAR signaling pathway and fatty acid mechanism, etc. Genes of which expression levels were lowered by induction of chronic renal injury were related to the neuroactive ligand-receptor signaling pathway. Results : Oral administration of AR restored renal mass which was reduced by induction of chronic renal injury. AR also restored creatinine clearance and lowered serum BUN level. In histopathological observation, the AR group has a tendency to prevent tissue damages as shown in the chronic renal injury group. Conclusions : AR can be used to treat patients with chronic renal injury although further study will be needed to elucidate the exact mechanisms in the efficacy of AR on chronic renal injury.
황백(黃柏)이 토끼의 전립선요도압 및 혈압에 미치는 영향

Objective: This study was undertaken to examine the effect of Cortex Phellodendri on prostatic urethral pressure and mean arterial blood pressure of rabbits. Methods: To measure prostatic urethral pressure and mean arterial blood pressure, a Mikro-Tip catheter transducer was inserted and positioned in the prostatic urethra and left carotid artery. After a stabilizing period, phenylephrine ($1{\mu}/kg$) was intravenously administered two or three times to increase the urethral pressure and mean arterial blood pressure. Cortex Phellodendri (2.5 mg/kg and 5 mg/kg doses of Cortex Phellodendri extracted from 80% Ethanol) was administered intravenously, followed by phenylephrine, with no time interval between the doses. The urethral pressure and mean arterial blood pressure were then measured to determine whether they had stabilized. Results and Conclusion: Cortex Phellodendri appeared to inhibit phenylephrine-induced increases in prostatic urethral pressure and mean arterial blood pressure.
황금(黃芩)이 전립선비대증(前立腺肥大症) Rat에 미치는 영향

Objective : In benign prostatic hyperplasia, dihydrotestosterone acts as a potent cellular androgen and promotes prostate growth. Inhibiting enzyme 5${\alpha}$-reductase, which is involved in the conversion of testosterone to the active form dihydrotestosterone, reduces excessive prostate growth. Recently Scutellaria baicalensis has been related reports about the effect of baicalein on anti-proliferation of the prostate gland. In this study, we investigated the effects of Scutellaria baicalensis on cytopathological alterations and expression of 5${\alpha}$-reductase in the rat model of benign prostatic hyperplasia induced by castration and testosterone treatment. Methods : Sprague-Dawley rat were treated with testosterone after castration for induction of experimental benign prostatic hyperplasia, which is similar to human benign prostatic hyperplasia in histopathological profiles. Scutellaria baicalensis as an experimental specimen, and finasteride as a positive control, were administered orally. The prostates were evaluated by histopathological changes, testosterone levels, and the expression of 5${\alpha}$-reductase genes. Results : While prostates of control rats revealed severe acinar gland atrophy and stromal proliferation, the rats treated with Scutellaria baicalensis showed a diminished range of tissue damage. In the reverse transcription-polymerase chain reaction(RT-PCR) of 5${\alpha}$-reductase genes. Scutellaria baicalensis inhibited the expression of 5${\alpha}$-reductase genes. Conclusions : These findings suggest that Scutellaria baicalensis may protect glandular epithelial cells and also inhibit stromal proliferation in association with the suppression of 5${\alpha}$-reductase. From theses results, we suggest that Scutellaria baicalensis could be a useful remedy agent for treating benign prostatic hyperplasia.

Objective : Benign prostatic hyperplasia(BPH) is one of the most common diseases among elderly men. In BPH, dihydrotestosterone (DHT) acts as a potent cellular androgen and promotes prostate growth. Many reports conclude the component melittin in bee venom has the potential to treat various diseases including prostate cancer. In this study, we investigated the therapeutic effects and action mechanism of Bee venom herbal acupuncture with BPH induced by castration and testosterone treatment. Methods : Sprague-Dawley rats were treated with testosterone after castration for induction of experimental BPH. A total of 24 rats were equally divided into four groups: Group 1 was the model group; Group 2 served as control (sham-operated group); Group 3 animals were treated with Bee venom herbal acupuncture as an experimental specimen; Group 4 served as a positive control group and was treated with finasteride at a dose of 1 mg/kg. The drugs were administered orally. The prostates were evaluated by prostatic weight, volume, histopathological changes and testosterone levels. Results : While prostates of control rats revealed severe acinar gland atrophy and stromal proliferation, the rats treated with Bee venom herbal acupuncture showed a diminished range of tissue damage and showed significant decrease in their prostatic weights, volume and histopathological examination. Conclusions : These results suggest that Bee venom herbal acupuncture may protect the glandular epithelial cells and also inhibit stromal proliferation. From theses results, we suggest that Bee venom herbal acupuncture could be a useful remedy agent for treating the benign prostatic hyperplasia.

Alcoholism is a chronic behavior disorder that disturbs the health, social, and economical functions by intaking alcohol repeatedly. Alcoholism includes some habituation, dependency, and addiction. It may be clinically silent or severe enough to lead to the rapid development of hepatic, renal and gastrointestinal failure. Alcoholism can also cause death. In this case, we administrated saenggangunbitang and sungjoocheonggantang to a patient suffering from alcohoism and its withdrawal symptoms. After administration of saenggangunbitang-sungjoocheonggantang medication, clinical symptoms, including liver function with diabetes mellitus and splenomegaly improved. saenggangunbitang-sungjoocheonggantang showed desirable effect on alcoholism symptoms.
