        제주지역 오존 및 미세먼지 고농도일의 기류패턴에 따른 농도변화와 기상조건 분석

        The classification of airflow patterns during high ozone (O<sub>3</sub>) and PM10 episodes on Jeju Island in recent years (2009-2015), as well as their correlation with meteorological conditions according to classified airflow patterns were investigated in this study. The airflow patterns for O<sub>3</sub> and PM10 were classified into four types (Types A-D) and three types (Types E-G), respectively, using the HYbrid Single-Particle Lagrangian Integrated Trajectory (HYSPLIT) model and synoptic weather charts. Type A was the most dominant airflow pattern for O<sub>3</sub> episodes, being characterized by the transport of airflows from urban and industrial areas in China with the highest frequency (about 69%, with a mean of 67 ppb). With regard to the PM<sub>10</sub> episodes, Type E was the most dominant airflow pattern, and was mostly associated with long distance transport from Asian dust source regions along northwesterly winds, having the highest frequency (about 92%, with a mean of 136 μg/m<sup>3</sup>). The variations in the concentration of O<sub>3</sub> and PM10 during the study period were clarified in correlation with two pollutant and meteorological variables; for example, the high (low) O<sub>3</sub> and PM10 concentrations with high (low) air temperature and/or wind speed and vice versa for precipitation. The contribution of long-range transport to the observed PM10 levels in urban sites for different airflow patterns (Types E-F), if estimated in comparison to the data from the Gosan background site, was found to account for approximately 87-93% (on average) of its input. The overall results of the present study suggest that the variations in O<sub>3</sub> and PM<sub>10</sub> concentrations on Jeju Island are mainly influenced by the transport effect, as well as the contribution of local emissions. emissions.

      • 요측 전완부 감각 피판을 이용한 감각 재건

        Purpose : The sensory radial forearm flap is used to recover the soft tissue defect of hand and foot. The aim of this study is to evaluate the results after sensory reconstruction using sensate radial forearm flaps. Materials and Methods : There were 9 cases of 8 patients (6 males and 2 females). The 7 cases of 6 patients were donated to foot, the 2 cases of 2 patients to hand. At operation, the ages of them ranged from 18 to 63 years (mean: 37.9 years). Sensory evaluation was performed using percentage(%) of responding area to the pinprick test and the static and moving two-point discrimination test (s-2PD and m-2PD). Results : The mean percentage of responding area to the pin prick test was 48.5%. The mean s-2PD and m-2PD of recipient hands were 37.2 mm and 28.8 mm. The mean s-2PD and m-2PD of recipient feet were 46.7 mm and 45.0 mm. The mean s-2PD and m-2PD of controls were 23.5 mm and 19.7 mm. Conclusion : All the recipients were less sensitive than the control, but the recipient hands more sensitive than the recipient feet. Although the small numbers were studied, the results suggested that the recovery of sensation in the innervated flaps was influenced by the recipient nerve. So the donor nerve seems to be selected as more sensitive and more distal nerve of donor site for better outcome.

      • 유리 생비골 및 생비골 피부편 이식 후 공여부의 평가

        Purpose : We have evaluated the morbidity of donor site after transfer of free fibular osseous and osteocutaneous flap to defect site of bone and soft tissue due to chronic osteomyelitis of long tubular bone, open fracture with bony defect, bone or soft tissue tumor and congenital anomaly. Materials and methods : The 54 cases of 79 cases to be carried out from May, 1982 to May, 2001 which could be followed up were reviewed. There were forty nine in male and five in female. The mean age was 35(4 to 66)years old and mean follow up period is 21.3 month(12 to 72). We have retrospectively analyzed the various postoperative complications such as compartment syndrome, donor site infection, skin defect, hypesthesia, hammer toes, ankle instability and activity of daily living by help of questionnaire, telephone, physical examination, follow up x-ray study and chart. Results : In the total 54 cases the medication period for pain control after operation were classified into three groups under 2 weeks(49 cases), from 2 weeks to 6 weeks(3 cases) and over 6 weeks(2 cases). The postoperative morbidity were occurred in total 12 cases(compartment syndrome: 0, infection : 2, skin defect: 1, hypesthesia: 5, hammer toe: 2 ankle pain: 2 discomfort in activity of daily living: 0), and also the morbidity rates of donor site were 23.5% in osseous flap and 21.6% in osteocutaneous flap were occurred. There was no statistical significonce in morbidity between osseous and osteocutaneous free fibular flap transfer(P>0.05). Discussion : In general the morbidity of free fibular flap transfer was relatively high but it did not have any effect on daily activity of living. We think that the meticulous operation technique, detailed wound care and early range of motion exercise will reduce the morbidity of donor site of flap.

        대퇴골 전자간부 골절의 내고정 실패에 대한 원인분석 및 치료

        목적: 압박 고 나사를 이용하여 수술한 대퇴골 전자간부 골절 중 고정에 실패한 15예에 대하여 내고정 실패의 양상과 원인, 치료방법의 결과를 분석하고자 하였다. 대상 및 방법: 1999년 1월부터 2005년 8월까지 활강 압박 고 나사를 이용한 내고정술 시행 후 내고정 실패가 발생하여 치료받은 환자들 중 1년 이상 추시가 가능하였던 15예에 대해 골절의 형태, 정복의 정확도 및 실패의 양상, 추정되는 원인, 그리고 술 후 임상적 결과와 방사선학적 결과에 대하여 분석하였다. 결과: 실패 양상은 골두 내 천공 7예, 압박 고 나사의 파손 1예, 압박 고 나사의 금속판으로부터의 이탈 1예, 압박 고 나사의 과도한 활강 2예, 금속판의 파손 2예, 금속판을 고정한 나사못 파손에 의한 고정소실 2예였다. 추정되는 원인은 부적절한 내고정물의 선택이 6예, 기술적인 실패가 9예였다. 인공관절 치환술을 시행한 10예에서 술 후 Harris Hip score는 평균 67.8점(46~97점)으로 3예에서만 만족스러운 결과를 보였고, 95도 칼날 금속판을 이용하여 내고정술을 시행한 4예에서는 술 후 Harris Hip score는 평균 88.4점(80~95점)이었으며 전예에서 5개월 이내에 골 유합 소견을 보였다. 결론: 대퇴골 전자간 골절의 성공적인 내고정을 위해서는 적절한 내고정물의 선택과 정확한 술기가 필요하며, 내고정 실패시 대퇴골두와 비구부가 건전할 때에는 환자의 연령과 전신상태를 파악하여 인공관절 치환술과 함께 95도 칼날 금속판등을 이용한 재내고정술도 고려하여야 할 것으로 판단된다. Purpose: We wanted to analyze the causes of fixation failure after performing compression hip screw fixation for intertrochanteric fracture and we wanted to evaluate the methods to mange this type of failure. Materials and Methods: From January 1999 to August 2005, 15 patients who underwent operation due to fixation failure after compression hip screw ixation for intertrochanteric fracture were evaluated, with at least 1 year follow up, for the fracture type, the status of the reduction, the aspects of failure, the causes of the failure and the clinical and radiologic results after operation. Results: There were 7 cases of cut out by the hip screw, 1 case of hip screw breakage, 1 case of dislodging the hip screw, 2 cases of excessive sliding of the hip screw, 2 cases of plate breakage and 2 cases of fixation loss of the plate. The failure of cases was due to incorrect choice of the implants, and that of 9 cases was due to technical error. The Harris Hip score was an average of 67.8 for 10 cases of failed arthroplasty at the last follow up and 88.4 for 4 cases of re-fixation with a 95 degree angled blade plate, and bone union was achieved at a minimum of 5 months. Conclusion: It is important to choose an adequate implant and a precise technique to achieve a successful result for fixation of unstable intertrochanteric fracture, and refixation with a 95 degree angled blade plate could be considered for the management of failed intertrochanteric fracture with arthroplasty.

        연구논문 : 최근(2010∼2012년) 제주지역 대기환경 변화에 관한 기상특성 분석

        The characteristics of meteorological conditions related to changes in atmospheric environment on Jeju Island were investigated during recent years (2010-2012). This analysis was performed using the hourly observed data of meteorological variables (air temperature, wind speed and direction) and air pollutants (O3, PM10, SO2, NO2, and CO). Out of 5 pollutants, O3 and PM10 concentrations have frequently exceeded national environmental standards in the study area during the study period, with relatively higher concentrations than the others. The concentrations of O3 and PM10 in 2010 and 2011 were somewhat higher than those in 2012, and their highest concentrations were mostly observed in spring followed by fall. Nighttime O3 concentrations (with relatively high concentration levels) were almost similar to its daytime concentrations, due to less O3 titration by very low NO concentrations in the target area and in part to O3 increase resulting from atmospheric transport processes. The transport effect related to the concentration variations of O3 and PM10 was also clarified in correlation between these pollutants and meteorological variables, e.g. the high exceedance frequency of concentration criteria with strong wind speed and the high concentrations with the westerly/northwesterly winds (e.g., transport from the polluted regions of China). The overall results of this study suggest that the changes in atmospheric environment in the study area were likely to be caused by the transport effect (horizontal and vertical) due to the meteorological conditions rather than the contribution of local emission sources.

        우레탄 고압 주입에 의한 수부의 재발성 염증

        다른 고압 주입 수부 손상과는 경과 및 치료가 비전형적이었던 폴리 우레탄 고압 주입 손상 환자 증례를 보고한다. 수상 15일 이후에도 지속되는 좌측 2수지 화농성 분비물로 전원되었다. 환자는 이물감을 호소하지 않았으며, 단순 방사선 사진상에서도 보이지 않았다. 보존적 치료에도 분비물의 증상이 호전되지 않아 수술적 치료를 시행하였으며, 고형화된 폴리우레탄을 발견하고 이를 제거하였다. 이후 증상은 호전되었고 수부 기능을 회복할 수 있었다. We describe a case of a high-pressure polyurethane injection injury to the hand, and discuss how its management differs from other injection injuries. A male patient was transferred to our hospital due to persistent purulent discharge from the 2nd finger, which began 15 days after the injury. The injected foreign body, polyurethane, could not be recognized by the patient and was not indicated radiographically. During surgery, the polyurethane foam was removed. The patient had no functional impairment 24 months after surgery.

        쇄골 간부 분쇄 골절에서 최소 침습 금속판 고정술의 유용성

        목적: 본 연구의 목적은 전위된 쇄골 간부 분쇄 골절에 있어서 고식적 수술법(conventional open plating, COP)과 최소 침습적 금속판 고정술(minimally invasive plate osteosynthesis, MIPO) 사이의 임상적, 영상의학적 결과를 비교하고 MIPO의 유용성을 평가하고자 하였다. 대상 및 방법: 쇄골 간부 골절로 수술을 시행한 39예 중 COP 20예, MIPO 19예를 대상으로 후향적으로 조사하였다. 영상의학적 평가는 골유합 기간과 쇄골 길이를 건 측과 비교하여 길이의 변화를 비교하였고, 임상적 평가는 simple shoulder test score, American Shoulder and Elbow Surgeons score, quick disability of the arm, shoulder and hand score를 이용하여 비교하였다. 또한 두 군 간의 평균 수술 시간과 방사선 조사 시간, 조사량을 비교하였다. 결과: 모든 환자에서 골유합을 얻었으며 골유합까지 걸린 시간은 MIPO에서 12.1주로 COP의 14.6주보다 짧았으나 유의한 차이는 보이지 않았다(p=0.587). 2년 추시 시 임상적, 영상의학적으로 만족스러운 결과를 보였고 두 군 간에 통계적으로 유의한 차이를 보이지 않았다. MIPO의 경우 COP보다 통계적으로 유의하게 수술 시간이 적게 걸렸으며(75.8분 vs. 106.9분, p=0.002) 방사선 조사 시간과 조사량이 많았다(52.8초 vs. 37.1초, p=0.002; 209.4 mGy vs. 43.1 mGy, p=0.005). 결론: 쇄골 간부 분쇄 골절에서 MIPO는 임상적, 영상의학적으로 모두 우수한 결과를 보였고 수술 시간이 짧으며 골이식술이 불필요한 효과적인 치료법으로 판단된다. 하지만 방사선 피해를 줄이는 노력을 간과하지 말아야 한다. Purpose: The purpose of this study was to compare the clinical and radiological outcomes of minimally invasive plate osteosynthesis (MIPO) with those of conventional open plating (COP) for treating comminuted mid-clavicular fractures and to evaluate the usefulness of MIPO. Materials and Methods: Thirty-nine patients who underwent surgical treatment for mid-clavicular comminuted fractures were analyzed retrospectively. Nineteen patients were treated with MIPO and twenty patients with COP. Radiological evaluation included time to union, fracture healing, and clavicular length difference measured as the proportional length difference with the unaffected side. Clinical assessment was performed using the simple shoulder test score, American Shoulder and Elbow Surgeons score, and quick disability of the arm, shoulder and hand score. Moreover, the mean operation and radiation times, as well as exposure were compared. Results: All clavicles achieved bone union. The mean time to union was 12.1 weeks in the MIPO group, and 14.6 weeks in the COP group (p=0.587). There was no significant difference between the two groups regarding the functional and radiological outcomes at 2-year follow-up. A significantly shorter operation time was observed in the MIPO group than in the COP group (75.8 min vs. 106.9 min, p=0.002). More radiation time and exposure were identified in the MIPO group (52.8 s vs. 37.1 s, p=0.002; 209.4 mGy vs. 43.1 mGy, p=0.005). Conclusion: Both COP and MIPO were shown to be effective treatment options for mid-clavicular comminuted fractures. MIPO may be a better alternative to COP due to shorter operation time and no need for a bone graft, although the functional and radiological outcomes were not significantly different. However, all surgeons should pay close attention to minimize radiation hazard.
