복강경 복막외 접근 탈장교정술과 Perfix<SUP>®</SUP>를 이용한 무긴장 탈장교정술의 비교

Purpose: Tension-free open repair for inguinal hernia is a safe and popular operation with a high success rate, but laparoscopic techniques are fairly recent. Comparing the tension-free herniorrhaphy using mesh and plug (Perfix<SUP>®</SUP>) with laparoscopic totally extraperitoneal (TEP) hernia repair is the aim of this study. Methods: We compared two groups of patients in which 39 patients with inguinal hernias were treated by tension-free repair using Perfix<SUP>®</SUP> (group P) and 39 patients were treated by laparoscopic TEP repair (group T). Information about operation time, length of hospital stay, use of analgesics (pain), recurrence, complications, cosmetic satisfaction, medical costs, and time until return to work were evaluated retrospectively. Results: Mean operation time, postoperative hospital stay, and the time until return to work were 101±33 min, 1.9±1.0 days, 11±8 days in group T, 86±28 min, 2.0±0.8 days, 12±10 days in group P. Postoperative complications, medical costs and the cosmetic satisfaction were 20.5%, 821,048 won, 94% (32/24) in group T, 7.7%, 692,149 won, 79% (19/24) in group P. There was difference between the two groups but not significant statistically. Group P used more analgesics than group T, significantly (P<0.05). There was only 1 recurrence in group T. Conclusion: Although we need more experience in patients with inguinal hernias, laparoscopic TEP repair may be a secure and feasible procedure compared to tension-free repair using Perfix<SUP>®</SUP>.

The microstructural changes of three pieces from an ancient iron pot were studied in order to identify present the material degradation due to repeated heating for one-thousand years. The microstructures of the pieces were divided into the areas of ferrite/graphite, ferrite/pearlite, and corroded oxidation. The area of ferrite/graphite was undergone by severe Galvanic corrosion, but that of ferrite/pearlite was not even during a thousand years` using . The shape of the graphites was coexisted with types of A, B, and C of as modern graphite classification. In the ferrite/pearlite area, abnormal acicula precipitates with a high aspect ratio of 0.2㎛thickness and several hundreds ㎛length were presented. They might be a kind of carbide in the ferrite matrix with its special precipitate plane.

풍납토성은 한성백제시기에 해당하는 주거지와 토기 등이 다량으로 확인되고, 고고학과 자연과학적 연구가 다양하게 진행되면서 학계의 주목을 받고 있는 유적이다. 특히 근래에 발굴 조사된 풍납토성 라-19호 주거지 바닥에서 주거지 토양과는 특성이 상이한 점토성분의 토양이 확인되었는데, 이 토양은 토기를 제작하기에 적합한 정도의 점성을 가지며, 혼입물이 포함되어 있는 것으로 볼 때 고고학적 입장에서는 토기제작을 위한 태토로 추정하고 있다. 이를 동일 유적지 경질무문토기 2점과 광물학적, 지구화학적 분석결과 구성광물이 유사하게 나타났으며, 주성분원소와 미량원소, 희토류원소의 거동패턴 또한 매우 유사하여 토기제작에 사용된 토양일 가능성을 가진다. 또한 온도별 소성시편을 제작하였으며, 광물학적 특성을 이용하여 소성온도를 추정하였다. 토기의 소성온도 추정결과 붉은색토기는 $550{\sim}600^{\circ}C$, 갈색토기는 $900{\sim}1,000^{\circ}C$로 추정되고 있다. Pung-nap mudcastle has brought wide attention to archaeologists since scientific studies were carried out on the various types of building sites and excavated potteries related to A period of Baekje Kindom when it sat Hansung as the capital. It is identified that clay from Ra-No.19 site of Pung-Nap mudcastle has different from compositions that of residential soil. From archaeological point of view, it is believed that the the soil was a source material for potteries due to the appropriate viscosity and inclusions. Mineralogical and geochemical studies show that the compositions between the source material and randomly chosen two potteries from the same site are similar. Behavior pattern classification of major, trace and rare elements provides also very similar result. Artificial specimen has been made at different temperature in order to estimate firing temperature of the potteries by means of various kinds of instrumental application. As a result of the study, it is believed that the firing temperature of the red and brown pottery was $550{\sim}600^{\circ}C$ and $900{\sim}1,000^{\circ}C$ respectively.
