        유사(보완)의료에 대한 보건의료정책학적 고찰

        Nowadays, it is surely the quack which stands as one of the most controversial, problematic. the quack has been a consistent target of contested public protection strategies in the past few centuries in many countries. Recently, complementary and alternative medicine (CAM) is increasingly utilized and accepted by patients and providers throughout the health care system in the world, most accounts attribute this growing acceptability to the shortcomings of conventional medicine, the appeal of CAM’s core beliefs, and the growing body of research indicating that CAM actually works. However, the governments of western countries have called for measures to ensure that the public are protected from incompetent and dangerous practitioners. Common to these controversies has been a suggestion to ban, exclude or limit the medical practice of those deemed to be damaging rather than improving the health of individuals as a measure of public protection. This article describes the experiences of western counties' health care system which is moving in a more pluralistic direction. By examining the ways in which regulatory efforts in the countries have come to address what is invariably described as a growing interest in CAM, this study show how the problem of CAM/quackery today is increasingly located in an ethical field of practitioner competency, qualifications, conduct, responsibility and personal professional development, regardless of the form of therapy in question. Many countries developed a series of measures and strategies to contain the acceptance of CAM groups, such as insisting on scientific evidence of safety and efficacy, resisting integration of CAM with conventional medicine and opposing government support for research and education. In a sense, those countries' movements serve to protect not only patients, but the dominant position of medicine and its allied professions, and to maintain existing jurisdictional boundaries within the healthcare system. The popular support for CAM will require that health professional stakeholders continue to address the challenges this poses, and at the same time protect their position at healthcare system. To cope with the quack, professional body, public sector and health authorities should consider the safety of consumers of healthcare and responding to the demands of the community for CAM therapies as well as the claims of the established healthcare professions. Finally, some implications for future health care were suggested.

        우리나라 양.한방 보건의료 부문간의 현황과 과제 : 새로운 의료체계로의 전환을 위한 공공정책의 선택

        Internationally, many countries are facing the demand for reshaping health care systems to cope with rapid changing circumstances in health care sector. The recent growth of oriental medicine and complementary and alternative medicine (CAM) in the many countries is, to a large extent, due to the growth of the number of oriental medical doctors and physicians who have taken up alternative therapies alongside conventional medicine. To cope with the changing environments, many countries consider to develop integrative health care which is now used widely in health care sector. In both biomedical and CAM sectors(including oriental medicine), attention appears to have shifted away from separating therapeutic modalities into categories such as biomedical or CAM, towards a focus on merging diverse modalities into a 'new' integrative health system. In Korea, one of peculiar characteristics of health care system is that as health care provider, Hanbang medicine (traditional Korean medicine) and (western) medicine coexist since 19 century. Recently, the government of Korea has given many efforts to enhance the role and function of traditional Korean medicine in health care sector. However, the strategies and measures for integrative health care settings combining traditional Korean medicine and western medicine on health sector have not been developed yet. The research question of this study is In Korea, what are the trends and problems in interface of traditional Korean medical sector and Western medical sector; what are the causes of or associated factors to the problems; how to cope with the problems and how to resolve the causes?; what are the health policy directions and its strategies that the government should take to cope with the future demand and the burden on health care sector? In order to do this, this study explores the current situations and issues on the interface between traditional Korean medicine and (western) medicine in various ways using contents analysis of existing data and documents related to traditional Korean medicine and health policy. Finally, we discussed stakeholders' views on the interface in the health care sector. Then, health policy options to have shifted away from separating therapeutic modalities into categories such as 'traditional Korean medicine' or 'western medicine', towards a focus on merging diverse modalities into a 'new' integrative health system.

        울진 원전 1$\cdot$2호기가 88년과 89년에 각각 상업 운전을 개시한 이래, 지난 3월 18일로 총 1천억kWh의 발전량을 달성했다. 울진 1$\cdot$2호기는 이 기간 동안 최장 382일을 포함해 네 차례의 OCTF를 달성하는 등 설비 운영과 관리에서 탁월한 실적을 나타냈다. 원자력발전량 1천억kWh 달성의 경과와 의의를 들어본다.

        선진국의 일차의료부문의 통합의료 - 일차보건의료체계에서의 한의학에 주는 함의 -

        The objective of this study is to discuss the role of integrative medicine (IM) in contemporary health care settings, and how and which factors affect and facilitate the success of IM in terms of the integration of complementary and alternative medicine (CAM) and conventional medicine in primary health care (PHC). IM is meant to provide the best possible health care, for both patient and physician. The way of IM use in the developed countries presents various ways that IM can be provided, and it appears that strategies have been successfully developed to facilitate integration. Although few of the barriers to the integration of CAM and conventional medicine have been resolved, concerns over the legitimacy of CAM in health care (e.g., safety, biomedical evidence, and efficacy) are being overcome by the use of evidence-based practice in IM delivery. There are dominant models of IM that have been developed. The model types signify different levels of equity between CAM and conventional medicine in regard to the power, autonomy, and control held by each. However, the factors common to all IM models, whether describing CAM as supplementary or complementary to conventional medicine, is the concept of a health care model that aspires to be client-centred and holistic, with focus on health rather than disease as well as mutual respect among peer practitioners. Finally, this study concluded that the growth and viability of traditional Korean medicine(TKM) depend on evidence-based practices and identifying the successful influences on the integration of TKM and conventional medicine for recognition of its inherent value in PHC. Some recommendations for the integration of TKM and conventional medicine were suggested.

        국내 소에서 Kazba에 대한 혈청 역학적 조사

        This study was carried out to understand the recent prevalence of Kazba virus using 8,023 sera collected during the year 1989~2000. Seropositive rates to Kazba virus showed the highest antibody rate(86.7%) in 1994, decreased up to 22.8% in 1997, and then largely fluctuated its levels including 60.8% in 1998 and 5.8% in 1999. Mean seropositive rates to Kasba virus from 1999 to 2000 year recorded 9.9% respectively. Therefore, positive rates of neutralizing antibody against Kazba viruses showed continuous low level during the past 3~4 years. Seropositive rates of neutralizng antibodies against Kasba virus in cows by age showed high level as they grew old. The Kasba virus showed a peak of 16.3% at their 5~6 ages.

        Current Issues in Evaluating Health Promotion Programme Using Traditional Korean Medicine in Korea

        Purpose:The aim of this study is to identify current situation and issues of outcome measures to evaluate the public health programs using traditional medicine by public health centers in Korea. Method:This study analyse and review existing data and documents related to traditional Korean medicine and health policy using contents analysis method. To collect the information on outcome measures evaluating the programmes, this study reviewed annual reports for health promotion programmes using traditional Korean medicine(HP-TKM) of Hub public health centers, as pilot public health centers, which have implemented the health promotion programmes collectively. Additionally, the review included research articles, government documents and book chapters on the topics related assessments in health promotion. Results:HP-TKM are stroke prevention education, smoke free program, health promotion according to Four Constitutional Medicine, home visiting treatment, etc. However, there are only a few studies of traditional medicine focused health promotion evaluation strategies. The benefits of health promotion programs using TKM can be categorized as non-health benefit, physiological, psychological and physical effects. To manage and monitor the intervention programmes efficiently, attention should be given to developing relevant and valid outcome measures for evaluating the programmes by government and public health center. Conclusion:Up to now, considering number of researchers, research projects undertaken or published articles and reports, within traditional Korean medicine there is a lack of capacity in research. Thus, government should pay more attention to developing relevant and valid outcome measures for evaluating the programs.         우리나라 한방의료의 현황과 과제 : 미래사회를 위한 정책적 선택

        Since 1990's, the Korean society, experiencing the low fertility and aging society, has been confronting with the threats in health care sector. The threats are the increases in the demand for health care, health care financial burden, and so on. In particular, the change of disease pattern and aging population result in the increases patients' demand for not only western medical services but also oriental medical services and complimentary medicine. Recently, the increases in availability of oriental medical services and the health care resources related to oriental medicine are raising some issues and conflicts in the Korean health sector. Theses circumstance required policy makers, central and local government, and public health sector to develop health policies related to oriental medicine and interface or integrate of traditional Korean medicine and Western medicine. For the near future, these issues will probably remain the focus of integration of traditional Korean medicine and Western medicine in public health sector. To cope with the threats in health care sector, one of the opportunities is to scale-up e public role of traditional Korean medical services. The main purpose of this study was to develop strategies to scale-up the Public role of traditional Korean medical services for the future society. The research questions are: what are the trends and problems in traditional Koran medical sector; what are the causes of or associated factors to the problems; how to cope with the problems and how to resolve the cause?; what are the health policy directions and its strategies that the government should take to cope with the future demand and the burden on health care sector? The results of this study are as follows. In order ta scale-up the public role of traditional medicine, this research offered health policy directions for traditional Korean medicine in response to a change environment of health care sector. There are four directions to be addressed: 1) the development of and investment in public oriental medicine infra-structure; 2) the development of public policy on oriental medicine; 3) modernization and globalisation of traditional Korean medicine; 4) the expansion of academic exchange between Western medicine and traditional Korean medicine. Finally, we discussed stakenholders' on traditional Korean medicine in the health care market. Then, public policy options for future society was suggested.

        한우 송아지의 후지에 발생한 과다사지

        Congenital anomalies occur at low levels in Korean antive cattle. PCR and Serum neutrlizing test against BVD, Akabane virus, Ibaraki virus, Kasba virus were examined. The results were all negative. Gross anatomical analysis was carried aout. Congenital abnormal newborn calf had 2 atropic left hindlimbs, which was nor moved autonomosly. The length of 2 atropic hindlimbs was 72 em and 48cm respectively. Both of them were curved severely at joints. Affected limbs were developed in connective tissue but lack of muscular tissue. In the abdominal region right kidney was normal. But there were two left kidneys which was half size compared with normal kidney. One of them had polycysts. In skeletal region, affected left limb is defective of head of femur and tibia.
