        피질 및 피질하청각중추 활성화 검출을 위한 기능적 자기공명영상 연구: 1.5-T MRI 스캐너를 위한 표준 프로토콜 개발

        Purpose: We wanted to develop a standard protocol for auditory functional magnetic resonance imaging (fMRI) for detecting blood oxygenation level-dependent (BOLD) responses at the cortical and subcortical auditory centers with using a 1.5-T MRI scanner. Materials and Methods: Fourteen normal volunteers were enrolled in the study. The subjects were stimulated by four repetitions of 32 sec each with broadband white noise and silent period blocks as a run (34 echo planar images [EPIs]). Multiple regression analysis for the individual analysis and one-sample t-tests for the group analysis were applied (FDR, p <0.05). Results: The auditory cortex was activated in most of the volunteers (left 100% and right 92.9% at an uncorrected p value <0.05, and left 92.9% and right 92.9% at an uncorreced p value <0.01). The cochlear nuclei (100%, 85.7%), inferior colliculi (71.4%, 64.3%), medial geniculate bodies (64.3%, 35.7%) and superior olivary complexes (35.7%, 35.7%) showed significant BOLD responses at uncorrected p values of <0.05 and p <0.01, respectively. On the group analysis, the cortical and subcortical auditory centers showed significant BOLD responses (FDR, p <0.05), except for the superior olivary complex. The signal intensity time courses of the auditory centers showed biphasic wave forms. Conclusion: We successfully visualized BOLD responses at the cortical and subcortical auditory centers using appropriate sound stimuli and an image acquisition method with a 1.5-T MRI scanner. 목적: 1.5-T MRI 장비를 사용하여 피질 및 피질하청각중추의 신경활동을 효과적으로 측정할 수 있는 기능적자기공명영상의 표준 프로토콜을 제시하고자 한다. 대상과 방법: 정상 성인 14명을 대상으로 광대역 백색잡음의 음자극기와 휴식기를 각각 32초 동안 4회 반복하는 블록디자인을 사용하였다. 통계는 개인별분석은 다중회귀분석을, 그룹분석은 단일표본 t-test를 적용하여 분석하였다(false discovery rate, p < 0.05). 결과: 개인별분석 결과는 p < 0.05와 p < 0.01(uncorrected) 유의수준에서 청각피질(100%, 85.7), 와우핵(100%, 85.7%), 하구(71.4%, 64.3%), 내측슬상체(64.3%, 35.7%), 상올리브핵(35.7%, 35.7%)의 빈도로 반응이 검출되었다. 그룹분석에서 양쪽 와우핵, 하구, 내측슬상체, 청각피질에서 유의한 BOLD 신호반응이 검출되었으나 상올리브핵은 유의하지 않았다(FDR, p < 0.05). 청각중추의 신호변화는 자극음의 시작과 종료에 대해 이상파 형태(biphasic)의 time-course를 보였다. 결론: 1.5-T MRI 장비를 사용하여 청각중추의 활성화에 적합한 음자극과 영상획득 및 자료분석법을 적용함으로써 청각중추의 활성화 영상을 성공적으로 획득할 수 있었다.

        Effects of Various Intracranial Volume Measurements on Hippocampal Volumetry and Modulated Voxel-based Morphometry

        Purpose : To investigate the effects of various intracranial volume (ICV) measurement methods on the sensitivity of hippocampal volumetry and modulated voxel-based morphometry (mVBM) in female patients with major depressive disorder (MDD). Materials and Methods : T1 magnetic resonance imaging (MRI) data for 41 female subjects (21 MDD patients, 20 normal subjects) were analyzed. Hippocampal volumes were measured manually, and ICV was measured manually and automatically using the FreeSurfer package. Gray and white matter volumes were measured separately. Results : Manual ICV normalization provided the greatest sensitivity in hippocampal volumetry and mVBM, followed by FreeSurfer ICV, GWMV, and GMV. Manual and FreeSurfer ICVs were similar in normal subjects (p = 0.696), but distinct in MDD patients (p = 0.000002). Manual ICV-corrected total gray matter volume (p = 0.0015) and Manual ICV-corrected bilateral hippocampal volumes (right, p = 0.014; left, p = 0.004) were decreased significantly in MDD patients, but the differences of hippocampal volumes corrected by FreeSurfer ICV, GWMV, or GMV were not significant between two groups (p > 0.05). Only manual ICV-corrected mVBM analysis was significant after correction for multiple comparisons. Conclusion : The method of ICV measurement greatly affects the sensitivity of hippocampal volumetry and mVBM. Manual ICV normalization showed the ability to detect differences between women with and without MDD for both methods.

        Regional Gray Matter Volume Reduction Associated with Major Depressive Disorder: A Voxel-Based Morphometry

        Background and Purpose: The association between the low emotional regulation and the structural change of major depressive disorder (MDD)’s brain has been proposed, and the VBM study on female MDD is still rare. The purposes of the present study are to show the regional volume changes of gray matter (GM) in the cortical and sub cortical structures of the female patients with MDD by optimized voxel-based morphometry (VBM). Methods: Twenty female MDD patients and age, sex matched 21 normal controls were included for the VBM analysis. To identify the change of regional gray matter volume (GMV), the optimized VBM was performed with T1 MRIs. The amounts of gray/white matter and intracranial cavity volumes (ICV) were measured. The analysis of covariance (ANCOVA) and partial correlation analyses covariate with age and ICV were applied for VBM. Results: The age and ICV distributions were similar between the two groups. In the ANCOVA, the total GMV of MDD was smaller than that of normal controls. In the VBM, regional GMV was relatively decreased in the limbic system (amygdalae, ambient gyri, hippocampi heads, subiculum, posterior parahippocampal gyri, pulvinar nuclei, dorsal posterior cingulate gyri, and left pregenual cingulate gyrus. The lingual gyri, short insular gyri, right fusiform gyrus, and right inferior frontal gyrus were also showed decreased regional GMV. Conclusion: The results of this study indicate that the female MDD is mainly associated with the structural deficits of the limbic system and limbic system related cortices, which were known to the center of emotions.

        Reduced Gray Matter Volume of Auditory Cortical and ubcortical Areas in Congenitally Deaf Adolescents: A Voxel-Based Morphometric Study

        Purpose: Several morphometric studies have been performed to investigate brain abnormalities in congenitally deaf people. But no report exists concerning structural brain abnormalities in congenitally deaf adolescents. We evaluated the regional volume changes in gray matter (GM) using voxel-based morphometry (VBM) in congenitally deaf adolescents. Materials and Methods: A VBM8 methodology was applied to the T1-weighted magnetic resonance imaging (MRI) scans of eight congenitally deaf adolescents (mean age, 15.6 years) and nine adolescents with normal hearing. All MRI scans were normalized to a template and then segmented, modulated, and smoothed. Smoothed GM data were tested statistically using analysis of covariance (controlled for age, gender, and intracranial cavity volume). Results: The mean values of age, gender, total volumes of GM, and total intracranial volume did not differ between the two groups. In the auditory centers, the left anterior Heschl’s gyrus and both inferior colliculi showed decreased regional GM volume in the congenitally deaf adolescents. The GM volumes of the lingual gyri, nuclei accumbens, and left posterior thalamic reticular nucleus in the midbrain were also decreased. Conclusions: The results of the present study suggest that early deprivation of auditory stimulation in congenitally deaf adolescents might have caused significant underdevelopment of the auditory cortex (left Heschl’s gyrus), subcortical auditory structures (inferior colliculi), auditory gain controllers (nucleus accumbens and thalamic reticular nucleus), and multisensory integration areas (inferior colliculi and lingual gyri). These defects might be related to the absence of general auditory perception, the auditory gating system of thalamocortical transmission, and failure in the maturation of the auditory-to-limbic connection and the auditorysomatosensory- visual interconnection.

        Gray, White Matter Concentration Changes and Their Correlation with Heterotopic Neurons in Temporal Lobe Epilepsy

        Objective: To identify changes in gray and white matter concentrations (GMC, WMC), and their relation to heterotopic neuron numbers in mesial temporal lobe epilepsy (mTLE). Materials and Methods: The gray matter or white matter concentrations of 16 left and 15 right mTLE patients who achieved an excellent surgical outcome were compared with those of 24 healthy volunteers for the left group and with 23 healthy volunteers for the right group, by optimized voxel-based morphometry using unmodulated and modulated images. A histologic count of heterotopic neurons was obtained in the white matter of the anterior temporal lobe originating from the patients’ surgical specimens. In addition, the number of heterotopic neurons were tested to determine if there was a correlation with the GMC or WMC. Results: The GMCs of the left and right mTLE groups were reduced in the ipsilateral hippocampi, bilateral thalami, precentral gyri, and in the cerebellum. The WMCs were reduced in the ipsilateral white matter of the anterior temporal lobe, bilateral parahippocampal gyri, and internal capsules, but increased in the pons and bilateral precentral gyri. The heterotopic neuron counts in the left mTLE group showed a positive correlation (r = 0.819, p < 0.0001) with GMCs and a negative correlation (r = - 0.839, p < 0.0001) with WMCs in the white matter of the anterior temporal lobe. Conclusion: The present study shows the abnormalities of the cortico-thalamo-hippocampal network including a gray matter volume reduction in the anterior frontal lobes and an abnormality of brain tissue concentration in the pontine area. Furthermore, heterotopic neuron numbers were significantly correlated with GMC or WMC in the left white matter of anterior temporal lobe.         Shape Analysis of the Subcortical Nuclei in Amyotrophic Lateral Sclerosis without Cognitive Impairment

        Background and Purpose Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease that mainly affects the pyramidal motor system. However, recent studies have suggested that degeneration of the extramotor system plays a role in the disability experienced by patients with ALS. We investigated the local shape changes and mean volumes of the subcortical nuclei in sporadic ALS patients with preserved cognition. Methods The participants comprised 32 patients with ALS and 43 age- and sex-matched healthy controls. Three-dimensional T1-weighted structural images were acquired. Surfacebased vertex analysis was performed with fully automated segmentation of both amygdalae, hippocampi, caudate nuclei, nuclei accumbens, putamina, pallida, and thalami, and the brainstem. The scalar distances from the mean surfaces of the individual subcortical nuclei were compared between groups, and correlations of the local shape distances with initial Amyotrophic Lateral Sclerosis Functional Rating Scale Revised (ALS-FRS-R) scores and the delta FRS-R and with the disease duration were analyzed. Results ALS patients showed regional shape contractions on the lateral surfaces of both pallida, the lateroposterior surface of the right putamen, and the anterior basal surface of the right accumbens. Delta FRS-R scores were negatively correlated with local shape distances in the right hippocampus and the putamina. However, the initial ALS-FRS-R score and disease duration were not correlated with local shape distances. Conclusions Subcortical gray-matter structures are involved in the neurodegenerative process of ALS before cognitive impairment becomes evident.
