        예방치의학의 미래와 4차 산업혁명

        올해 10월에 개최된 대한예방치과·구강보건학회 종합학술대회는 ‘예방치의학 분야의 새로운 비전과 전략’이라는 주제로 예방치의학의 과거를 돌아보고 현재를 기반으로 미래를 꿈꾸어 보는 기회를 제공하였다. 미래의 예방치의학과 관련하여 최근에 많은 관심과 논의의 대상이 되고 있는 것 가운데 하나로 4차 산업혁명과 관련된 발전을 생각해 볼 수 있다. 4차 산업혁명이란 핵심기술이 융합되어 초연결사회를 만들어 가는 것으로, 사물인터넷을 통해 생산 기기와 생산품 간 상호소통체계를 구축하고 전체 생산과정의 최적화를 구축하고 물리학, 생물학, 디지털 등 영역 간 경계가 완전히 허물어진 기술융합과 사람과 사물, 기계와 기계의 연결로 대량의 정보 공유가 가능한 네트워크세상을 만들어가는 것이다. 이러한 4차 산업혁명의 대표 미래 기술인 3D 프린팅을 포함한 인공지능, 빅데이터, 가상현실과 같은 핵심기술들은 치과계에 큰 영향을 미치고 있고 앞으로 더 많은 영향을 미치게 될 것이다. 최근에 급속도로 발전하고 있는 디지털 덴티스트리(Digital Dentistry)가 하나의 예시가 될 수 있는데 현재는 수술과 수복 등 치료를 목적으로 개발이 이루어지고 있으나 앞으로 대용량 데이터를 활용한 빅데이터 연구를 통한 구강보건관련 연구, 가상현실을 이용한 구강보건교육 등 예방치의학의 발전을 위해 4차산업혁명의 핵심기술들이 유용하게 활용될 것으로 생각한다. 따라서 구강보건을 연구하는 연구자들이 이러한 4차산업혁명에 보다 더 깊은 관심을 가지고 현재 연구하고 있는 내용들을 새로운 기술들과 융합시켜 나감으로써 보다 더 효과적이고 효율적이며 획기적인 발전을 이룰 수 있기를 바라고, 이러한 새로운 시도와 발전이 학회지를 통해 소개되고 공유되어 시너지 효과를 얻을 수 있는 기회가 될 수 있기를 기대한다. The General Conference of Korean Academy of Preventive Dentistry and Oral Health was held in October of this year with the theme of “Promising Vision and Strategy for Preventive Dentistry.” It provided an opportunity to reflect on the history of preventive dentistry and dream about the future based on the present. Advances associated with the Fourth Industrial Revolution can be considered one of the subjects that has drawn much recent interest and discussion with respect to the future of preventive dentistry. The Fourth Industrial Revolution refers to creating a hyper-connected society through the convergence of core technologies. It entails establishing a mutual communication system between production equipment and products through Internet of Things and optimizing the entire production process. It also entails creating a world of networks in which large quantities of information could be shared through convergence of technologies that blur the boundaries between different fields such as physics, biology, and digital technology, and through human-to-things and machine-to-machine connections. Core technologies such as 3D printing, artificial intelligence, big data, and virtual reality, which are the future technologies that represent the Fourth Industrial Revolution, are having a major impact on dentistry and will have even greater impact moving forward. Digital dentistry, which has undergone rapid advances in recent years, can be an example, in which current developments are for treatment purposes such as surgery and restoration. However, in the future, I believe that core technologies of the Fourth Industrial Revolution will be useful for the development of preventive dentistry, including big data-based research on oral health using large quantities of data and oral health education using virtual reality. Therefore, I hope that researchers studying oral health will take an even greater interest in the Fourth Industrial Revolution and achieve more effective, efficient, and groundbreaking advancements by integrating the contents of current research with new technologies. Moreover, I am also hopeful that introduction and sharing of such new efforts and advancements through academic journals could provide the opportunities to gain synergistic effects.

        죽염을 이용한 우식예방

        Bamboo salt is a special processed salt by Korean traditional recipe. Recent study results showed that bamboo salt or bamboo salt with some other materials like herbal extracts have the anti-microbial activity, inhibition effects of dental plaque and gingival inflammation. Bamboo salt also showed anti-cariogenic effects; remineralization and acid resistance. Compare to fluoride toothpaste, bomboo salt toothpaste with fluoride showed the more effective remineralization on inner part of the early dental caries lesion. It increased the surface hardness and decreased lesion depth of early dental caries lesion. Thus, it is suggested that bamboo salt could be used as a anti-microbial, anti-plaque, anti-inflammatory and anti-cariogenic material for oral disease prevention. Especially, bamboo salt dentifrice with fluoride can be recommanded as a useful remineralizing agent.

        하수처리장 방류수의 총인 제거를 위한 P-CAP 시스템에서 PDA 기법의 활용가능성에 대한 연구

        Recently, to using chemical coagulation process for T-P removal in STP effluent as tertiary treatment process is generalized in the country. The importance of analysis technique to save the treatment & maintenance cost during coagulation process is becoming more increased each day. Thus, it is necessary for the analysis technique during coagulation process to be presented well the characteristic of coagulation in field apply. There are a few analysis techniques such as Jar Test, zeta potential analysis and streaming current detecting techniques. But there are difficult to apply in field immediately due to long test time and difficult analysis techniques. And using PDA technique, it is reviewed applicability of the techniques as field index on pilot plant of P-CAP system The P-CAP system is composed of an in-line static mixer, a Flocculation Tank and the CAP reactor with 2 stage weir for effluent. Pre-test is performed to fix the mixing velocity in the Flocculation Tank using the PDA equipment and it fixed with 30RPM. Also, Jar Test is performed to select optimum dose of each coagulant for each T-P concentration level of influent. Result of continuous test on pilot plant of P-CAP system, the FSI in the Flocculation Tank is increased consistently by increasing each dosing concentration of coagulant such as LAS and PAC in the low level influent T-P concentration comparatively. It is considered that formed Al-hydroxide complexes for dosed coagulant are caused FSI variation. Furthermore, it seems that FSI value in the high level influent T-P concentration appeared lower than the opposite influent condition relatively because it is formed simultaneously Al-hydroxide complexes as solid type and Al-phosphorus complexes as soluble type. Thus, relation of FSI by PDA technique and T-P removal of final effluent on pilot plant of P-CAP system are very limited for the kind of coagulant and the characteristics of influent. And it though that FSI value by PDA technique with analyzing of turbidity in Flocculation Tank will be used restrictedly on field as the relative field-index.

        구강보건과 치과의료산업

        2016년 10월 29일과 30일에 서울에서 개최되는 대한예방치과·구강보건학회 종합학술대회의 주제는 ‘구강보건과 치과의료 산업’이다. 그동안 구강보건의 발전은 치면세균막을 제거하기 위한 칫솔과 치약을 비롯한 다양한 구강위생용품의 개발, 초기치 아우식증을 재광화시킬 수 있는 불소와 같은 재광화 물질의 개발, 치아 표면의 열구를 미리 차단하여 치아우식증을 예방할 수 있도록 하는 재료의 개발, 초기우식증 진단장비의 개발 등 치과용품과 치과재료의 개발과 발전에 큰 영향을 받아왔다. 최근 국가적으로 의료의 산업화에 대한 관심이 커지고 있는 상황에서 이러한 치과용품 및 치과재료의 개발과 산업화에 대한 관심도 함께 증가하고 있다. 이번 학술대회는 이러한 상황에서 구강보건과 관련된 치과의료산업의 동향을 살펴볼 수 있는 좋은 기회를 제공할 것이다. 구강보건과 관련한 치과의료산업의 발전을 위해서는 개발되는 치과용품 및 치과재료에 대한 품질, 효과 및 안전성 등에 대한 다양한 기초 및 임상적 연구가 필요하다. 이러한 연구를 담당하는 연구자들의 역할은 치과의료산업에 있어 중요한 한 부분을 담당하고 있다고 볼 수 있 으며 이러한 연구자들은 치과용품 및 치과재료와 관련된 많은 연구결과들을 학회지를 통해 보고하고 있는 상황이다. 앞으로 국내에서 치과의료산업의 비중은 점점 더 확대될 것이고 더불어 이와 관련한 다양한 연구도 함께 활발하게 이루어질 필요가 있다. 이번 학술대회를 통해 많은 연구자들이 치과의료산업에 대해 더 많은 관심을 갖게 되기를 바라고 치과의료산업과 관련된 다양한 연구가 더 활성화되어 좋은 연구결과들이 본 학회지를 통해 많이 보고될 수 있기를 기대한다. The topic of the Korean Academy of Preventive Dentistry and Oral Health Symposium 2016, held in Seoul on October 29 and 30, is “Oral Health and Dental Industry”. Until now, advances in oral health have largely been influenced by the development of dental products and devices. Examples of such products and devices include oral hygiene products such as toothbrushes and toothpastes for the removal of the Oral biofilm on the dental surface, remineralizing agents such as fluoride that can induce remineralization of incipient dental caries, pit and fissure sealants, and diagnostic devices for the early detection of dental caries. Interest in the industrialization of healthcare has been increasing nationwide, and the development of dental products and devices is attracting increasing attention as well. This upcoming symposium will provide an excellent opportunity to examine the direction of the dental health industry. The advancement of the oral and dental health industry requires diverse basic and clinical research on the quality, efficacy, and safety of the dental care materials and products being developed. Naturally, researchers play a crucial role in the dental health industry by reporting their research results on dental products and materials in peer-reviewed journals. Considering the continuing growth of the dental health industry, the importance and necessity of dental research is evident. It is our hope that this symposium will stimulate interest in research pertaining to the dental health industry and promote active publication of quality data.

        죽염과 불소 혼합용액의 우치 초기인공우식법랑질 재광화 효과

        Objectives: The purpose of this investigation was to evaluate the remineralization effect of CPP-ACP, NaF, bamboo salt and NaF+bamboo salt solutions on bovine incipient artificial enamel caries by microcomputed tomography (micro CT). Methods: Specimens were prepared from extracted bovine teeth and immersed in a demineralizing solution for 72 h. Specimens were divided into five groups of five specimens each by randomized blocks according to surface microhardness (VHN) and mineral density. The specimens were each treated by one of the following agents for 24 h: distilled water, 2% sodium fluoride solution (2% NaF group), 3% bamboo salt solution (3% BS group), and a solution of 2% sodium fluoride solution and 3% bamboo salt solution mixed together (2% NaF+3% BS group) and 1% CPP-ACP solution. After this, the specimens were immersed in artificial saliva for 15 and 30 days. After 15 days and 30 days treatment, VHN and mineral density values (g/cm3) were measured and analysed. Mineral density values were obtained using micro CT. Results: Mineral density and VHN of the five groups according to the immersion period in artificial saliva showed statistically significant differences (P<0.05). In mineral density and VHN, the values of the 2% NaF+3% BS group were significantly higher than those of the other groups, and there were no significant differences among the 2% NaF, 3% BS, CPP-ACP groups after 30 days. Conclusions: Bamboo salt showed a similar remineralization pattern to CPP-ACP on an incipient artificial enamel caries lesion. Thus, bamboo salt (like CPP-ACP) can act as a remineralization agent for subsurface lesions of incipient enamel caries.
