The standard therapy for acute pulmonary embolism is initiated with heparin. Massive pulmonary embolism (PE) is defined by the presence of cardiogenic shock, persistent arterial hypotension, or both. It is associated with a high risk of in-hospital death, particularly during the first hours after admission. Thrombolysis is recommended in massive PE. Results from a randomized trial suggested that selected patients with evidence of right ventricular dysfunction and a low risk of bleeding may benefit from thrombolysis. In this issue of the Journal, Kim Yang-Ki and colleagues conducted retrospective review of thrombolytic therapy in PE last 6.5 years in a single center. The mortality rate of massive PE (44%) was higher than submassive PE (8%) in patients with thrombolysis. Major bleeding occurred in 3/21 (14%) patients. This signal reminds us real-world hazards of thrombolysis is higher than randomized controlled trial. Although massive PE patients need more intensifying therapy to reduce mortality, we should careful assessment of bleeding risk factors before starting thrombolysis. With regard to submassive PE, this study has limitation to assessing the efficacy of thrombolysis. (Korean J Med 2013;84:46-48)

Background: Interleukin-13 (IL-13) is essential for the synthesis of Immunoglobulin E (IgE) which is a critical risk factor for the development of asthma. The high-affinity IgE receptor β (FCER1B), which is expressed on mast cells and basophils, plays an important role in the IgE-mediated allergic response. Objective: To analyze the association between IL-13 (+2044G/A) and FCER1B (E237G) polymorphisms, and IgE production in asthmatic children. Method: Seven hundred sixty asthmatic children and 235 healthy controls were enrolled, and the levels of serum total IgE and blood eosinophil counts were evaluated. Pulmonary function test and methacholine challenge test were also performed. The genotypes of IL-13 (+2044G/A) and FCER1B (E237G) polymorphisms were analyzed by using the restriction fragment length polymorphism method. Result: IL-13 (+2044G/A) and FCER1B (E237G) polymorphisms were not associated with the development and phenotypes of asthma. However, in homozygote of variant genotypes, gene-gene interaction of IL-13 (+2044G/A) and FCER1B (E237G) polymorphisms was associated with an increase of serum total IgE, whereas in homozygote of wild genotypes, this interaction was not associated with an increase of serum total IgE (P=0.030). Conclusion: Gene-gene interaction between IL-13 (+2044G/A) and FCER1B (E237G) polymorphisms may be associated with increased levels of serum total IgE in children with asthma. 흡입용 베타 - 2 교감신경작용제가 심전도 QTc 간격 및 혈청칼륨에 미치는 영향

N/A Objectives: Despite major advances in understanding of pathogenesis and treatment, the mortality associated with bronchial asthma has risen progressively since 1977. Analysis of contemporary medical history indicates that at least some excess deaths in bronchial asthma is related to the use of inhaled beta-adrenergic agonists. So we undertook a study to examine the effects of inhaled beta-2 agonist in obstructive airway disease patients group whether it can produce abnormal cardiac function even in usual dosage. Methods: Heart rate, QTc interval, and serum potassium changes after inhalation of salbutamol were studied in 20 normal controls, 11 asthmatic patients, and 9 chronic obstructive pulmonary disease (COPD) patients. Base-line serum potassium level and electrocardiogram were taken just before and 60 minutes after 2 puffs(200㎍) inhalation of salbutamol. Three days later, repeated base-line serum potassium and electrocardiogram were taken just before and 60 minutes after 4 puffs(400 ㎍) inhalation of salbutamol. Results: The serum potassium concentraton was changed in normal control and COPD subjects, but statistical singificance was not found. Prolongation of the QTc interval was significant in asthma and COPD subjects after 4 puffs inhalation of salbutamol (p<0.05). Changes in QTc interval following 2 puffs inhalation of salbutamol was significant only in COPD subjects (p=0.05). Conclusion: Salbutamol causes QTc prolongation in bronchial asthma and COPD patients group in usual dosage. Futher studies are recommanded to evlauate the clinical significancy of QTc prolongation after inhaled beta-2 agonist use.

Rapid growth of management Information system (MIS) and evolution of software system enlarge maintenance necessity and it need a process to implement information system service in business. This paper proposes service-oriented maintenance process of component software based on service design and implementation provided by existing processes for maintenance, MaRMI-RE and ITIL. The Proposed process derives activities and tasks of the process by identifying services according to service guidelines. Service-oriented maintenance process is produced by analyzing correlation between the derived activities and tasks and MaRMI-RE's and adding the tasks to MaRMI-RE. The verify availability of the proposed service-oriented maintenance process, correlation between artifacts is proved.
백내장 수술 후 0.1% 히알우론산과 0.05% 사이클로스포린 A가 눈물막에 미치는 효과

Purpose: To evaluate the efficacy of topical 0.1% sodium hyaluronate (HA) and 0.05% cyclosporine A (CsA) on tear film parameters after cataract surgery. Methods: A total of 109 eyes of 86 patients who underwent cataract surgery were divided into four groups: 74 eyes without dry eye syndrome (groups 1 and 2) and 35 eyes with dry eye syndrome (groups 3 and 4). Groups 1 (37 eyes) and 3 (17 eyes) used 0.1% HA only, while Groups 2 (37 eyes) and 4 (18 eyes) used both 0.1% HA and 0.05% CsA. Tear film break-up time (BUT), Schirmer’s test, tear clearance rate (TCR) and keratoepitheliopathy were evaluated before surgery and 1 week, 1 month, 2, 3 and 6 months after surgery. Results: BUT, Schirmer’s test and TCR recovered at postoperative 3 months in groups 1 and 3, after 2 months in group 2 and after 1 month in group 4. Keratoepitheliopathy recovered at postoperative 1 month in groups 1, 2 and 4, after 2 months in group 3. Conclusions: The combined use of topical 0.1% HA with 0.05% CsA is helpful in restoring tear film parameters after cataract surgery especially in patients with dry eye syndrome. 건성안 환자의 눈물 내 케모카인 리간드 5의 농도 분석

Purpose: To investigate the expression of CCL5/RANTES (regulated upon activation, normal T cell expressed and secreted) in the tears of dry eye patients. Methods: Forty patients with dry eye (15 Sjögren’s and 25 non-Sjögren’s syndrome patients) and ten control subjects were recruited for the present study. The concentration of RANTES in tears was measured using an enzyme-linked immunosorbent assay. The correlations between RANTES level, tear film and ocular surface parameters, including tear film break-up time, basal tear secretion, tear clearance rate, corneal sensation, keratoepitheliopathy, and conjunctival goblet cell density, were analyzed in patients with dry eye syndrome. Results: The concentrations of RANTES were 435.46 ± 104.45 pg/ml in Sjögren’s syndrome patients, 257.42 ± 46.72 pg/ml in non-Sjögren’s syndrome patients, and 97.53 ± 29.15 pg/ml in the control patients (p < 0.01). The levels correlated significantly with basal tear secretion, tear clearance rate, keratoepitheliopathy, and goblet cell density (p < 0.05). Conclusions: CCL5/RANTES level increases in the tears of dry eye patients and correlates with various tear film and ocular surface parameters. J Korean Ophthalmol Soc 2011;52(6):658-664
