16세된 남자환자는 고환염전을 주소로 입원하여 고환적출술을 시행하고자 하였다. 과거력과 신체검사상에 별다른 이상이 없었다. 좌측와위로 제 3∼4요추사이를 천자하여 tetracaine 12 mg을 주입하여 제 4흉추부위까지 감각소실이 되도록 하였다. 약간의 혈압감소와 서맥이 있었으나 정상범위에 있어 혈압상승제를 사용하지는 않았다. 이때 환자가 천장이 빙글빙글 돌아간다고 호소하였는데 상하, 좌우안진이 관찰되었다. 안진은 점차 감소하였지만 1시간 후까지 지속되었다. 회복실에서 안진이 완전히 소실되었으며, 좌위를 하여도 안진은 촉발되지 않았다. 그후 별다른 이상없이 3일후 퇴원하고, 3개월 뒤에 다시 검진하였으나 안과 및 신경학적 증상은 없었다. The latest hacking techniques through a wireless network in smart car information security has emerged in the security market. In especial, many attacks are conducted on smart car by using IP address scanning. This paper analyzes the problem of IP address scanning technology and proposes solutions. The proposed technique filters the MAC address and intrusion detection technology is used by blocking IP address that is not allowed.

N/A Background: AFP is known as one of the most sensitive tumor markers for hepatocellular carinoma. But in many cases of hepatocellular carcinoma, low or negative levels of AFP have been observed. And because AFP levels may be increased in cases of other liver diseases such as liver cirrhosis, its specificity and sensitivity are problems. PIVKA-II has been studied as a new tumor marker for hepatocellualr carcinoma with AFP. It is a precursor protein of prothrombin and is converted to active form of prothrombin by the action of Vitamin K dependent carboxylase in hepatocyte microsomes. As this process can be suppressed by Vitamin K deficiency or Vitamin K antogonist such as warfarin, PIVKA-II levels might be increased due to functional derangement of Vitamin K-dependent carboxylation in hepatocellular carcinoma. We measured the levels of PIVKA-II in patients with various liver diseases including hepatocellular carcinoma and evaluated the meaning of increased PIVKA-II levels. Methods: We measured the levels of PIVKA-II in the plasma of 30 patients with hepatocellular carcinoma and 41 patients with other liver diseases by EIA method using monoclonal antibody specific to PIVKA-II. AFP was checked by RIA method. Results: The levels of PIVKA-II were increased above 0.1 AU/mL in the plasma of 25 (83%) patients with hepatocellular carcinoma and 1 patient with liver cirrhosis and 1 chronic hepatitis, 1 receiving warfarin, 2 toxic hepatitis, 2 cholangiocarcinoma and 1 liver metastatis from stomach cancer. In this study, with diagnostic cut-off value of 0.1 AU/ mL, the sensitivity and specificity of PIVKA-II to detect hepatocellular carcinoma were 83.3% and 78% each. With cut-off value of 8.0 AU/ml, the sensitivity and specificity were 66.7% and 100% each. By the combination assay with the cut-off values of AFP above 100ng/mL & PIVKA-II above 0.1 AU/mL the sensitivity for hepatocellular carcinoma could be elevated to 86.7%. Conclusion: We can conclude that PIVKA-II is more sensitive and specific than AFP and is a useful tumor marker which can elevate the sensitivity and specificity to detect hepatocellular carcinoma by combination assay with AFP. And PIVKA-II can be a useful screening method to detect hepatocellular carcinoma arising from liver cirrhosis.
발치후 즉시 임플란트의 8년 누적 생존율에 대한 후향적 연구

Purpose : Schulte introduced immediate implant cases in 1970s, since then many patients have been treated by immediate implants. The aim of this study was to evaluate the cumulative survival rate of immediate implant and to analyze various factors associated with survival of immediate implant in a 8-year period retrospectively. Materials and methods: From May, 2000 to October, 2007, 77 implants were placed in 63 patients immediately after tooth extraction at Department of Periodontology, Yonsei University Hospital. The implant survival rate was analyzed using a life-table analysis. After delivery of prosthesis, along I year of observation period, all implants were evaluated clinically and radiographically. Results: Among 77 implants, 59 were placed in maxilla and 18 in mandible. The most common site of the implantation was Mx anterior (65%). The overall survival rate was 94.8% over a mean follow-up period of 44.2±21 months and the 8-year cumulative survival rate was 94.6%. Failure occurred in 4 implants, all of which were due to early failures. None of the following factors were significantly associated with the survival of immediate implants: advanced surgical technique including bone graft and guided bone regeneration, the types of implants, causes of tooth loss, and types of prosthesis. Conclusion: On the basis of 8-year life time analysis, immediate implant placement can be considered a safe and predictable treatment mordality.

저자들은 비타민의 공급 없이 총정맥영양법을 시행 중에 발생한 젖산산증에서 티아민 공급 후 수시간 이내로 산증이 교정된 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이며, 티아민을 공급하지 않는 총정맥영양법을 장기적으로 시행 중 발생한 젖산산증에는 티아민 결핍에 의한 젖산산증의 가능성을 반드시 염두에 두어야 할 것으로 사료된다. Lactic acidosis commonly occurs in association with shock. We encountered lactic acidosis in a patient with normal blood pressure. The patient was a 17 year-old man with acute lymphocytic leukemia. He was admitted for bone marrow transplantation. During hospitalization, he relied on total parenteral nutrition due to his poor oral intake. On the 37th day after admission, he developed lactic acidosis without an episode of hypotension or any causative medication. Because vitamins were not included in the parenteral nutrition, we prescribed thiamine replacement, and this corrected the acidosis within a few hours. Thiamine (in its active derivative) is a coenzyme for pyruvate dehydrogenase; thus, its deficiency causes accumulation of pyruvate and lactate. This case suggests that thiamine deficiency should be included in a differential diagnosis of lactic acidosis in patients who are on total parenteral nutrition without vitamin supplementation.(Korean J Med 73:443-447, 2007)
