        심한 갑상샘연관안와병증과 연관된 위험인자

        Purpose: To determine risk factors for severe Thyroid-Associated Orbitopathy (TAO). Methods: We retrospectively studied the clinical data of 385 patients of the New York City area diagnosed with TAO from January 1990 to December 2000. We reviewed clinical characteristics and tried to identify several risk factors. The statistical analysis was applied to the collected data. Results: Men more frequently had proptosis, restrictive myopathy, and optic neuropathy than women. Patients older than 50 years presented more frequently with proptosis, restrictive myopathy, and optic neuropathy than younger than 50 years. Patients with a family history of thyroid disease had similar clinical characteristics compare to those without family history. Smokers more often had proptosis, restrictive myopathy, and compressive optic neuropathy than nonsmokers. Patients who had received radioactive iodine (RAI) treatment were older, and were more frequently smokers. If patients were smokers and treated with radioactive iodine treatment, they were more likely to have proptosis, restrictive myopathy, and compressive optic neuropathy than if not treated with radioactive iodine. However, the RAI treatment did not seem to be associated with severe characteristics of TAO in non-smokers. Conclusions: The risk factors for severe TAO are: male, age older than 50, smoking, treated with RAI in smokers.

        이차원 전기영동법을 이용한 굴절교정레이저각막절제술 후 눈물 내 단백질의 비교 분석

        Purpose: To investigate the change in the tear protein composition of patients who underwent refractive surgery. Methods: Tear samples were collected before photorefrative keratectomy (PRK), on the first, the second, and the third postoperative day, and then a month after the operation from 40 eyes of 20 patients. These tear samples were analyzed using two-dimensional gel electrophoresis (2-DE). Matrix-associated laser desorption/ionization time of flight (MALDI-TOF) was employed for the identification of expressed proteins. Control tear samples were collected from 40 eyes of 20 healthy volunteers who had no history of ocular surgery or pathology. Results: On the first postoperative day, lipocalin-1 precursor, lipocalin-1, and lysozyme were up-regulated. On the second postoperative day, serum albumin precursor and serum albumin were up-regulated. The tears collected on the third postoperative day and after 1 month had similar protein expression levels to the control group. Lipocalin 1 precursor and lysozyme were up-regulated and down-regulated after reftactive surgery, respectively. However, each protein had a different molecular weight and isopotential point. Conclusions: The tear protein composition changed uniquely in the early postoperative period, and proteins with different isopotential points were detected after PRK. We hypothesized that the healing process might influence the expression of the tear proteins. 목적: 굴절교정수술 후 눈물 내 단백질의 구성이 어떻게 변화하는지에 대해 알아보고자 하였다. 대상과 방법: 40안의 굴절교정각막절제술 환자에서 수술 전, 수술 후 첫째 날, 둘째 날, 셋째 날, 그리고 한 달 후에 눈물을 채취하였고, 40안의 건강한 지원자를 대조군으로 하여 눈물을 채취하였다. 채취된 눈물은 이차원 전기영동법과 MALDI-TOF 질량분석기를 이용하여 분석하였다. 결과: 수술 후 첫째 날에는 lipocalin-1 precursor, lipocalin-1, 그리고 lysozyme이 대조군에 비해 더 많이 발현되었고, 수술 후 둘째 날에는 serum albumin precursor와 serum albumin이 더 많이 발현되었다. 수술 후 셋째 날과 한 달 후는 대조군 및 수술 전과 같았다. Lipocalin-1 precursor와 lysozyme은 대조군에 비해 증가된 단백질과 감소된 단백질에서 모두 나타났지만, 각각의 단백질은 분자량과 등전위점값이 달랐다. 결론: 눈물 내 단백질의 구성은 수술 후 초기에 많은 변화를 보였으며, 등전위점값이 다른 단백질의 발현도 발견되었다. 각막의 치유과 정이 단백질의 발현에 영향을 주는 것으로 생각된다.

        백내장 수술 후 인공수정체의 종류에 따른 앞방깊이와 굴절력의 변화

        Purpose: To evaluate the changes of refraction and anterior chamber depth in pseudophakia with the single-piece intraocular lenses (IOLs) and the three-piece IOLs after cataract surgery. Methods: This single-center prospective study comprised of 34 eyes in 27 cataract patients who underwent cataract surgery. Eighteen eyes of seventeen patients received the single-piece IOLs, and sixteen eyes of fifteen patients received the three-piece IOLs. At one day, one week, and six months after cataract surgery, the refraction, corneal thickness, and anterior chamber depth of both groups were measured and analyzed. Results: The refraction after cataract surgery was not significantly different from the preoperative predictive refraction in both IOL groups. The corneal thickness increased significantly as compared with preoperative value at operative day in both groups. However, after 1 week postoperatively, there was no statistically significant difference between the preoperative and the postoperative corneal thickness. The anterior chamber depth of the single-piece IOL group decreased constantly, and the decrease was statistically significant at 6 months postoperatively. In the three-piece group the anterior chamber depth was stationary and there was no statistically significant change from 1 day postoperatively. Conclusions: There was remarkable difference of the changes of anterior chamber depth after cataract surgery between the single-piece and the three-piece IOL groups. In particular, the relative decrease of anterior chamber depth of the single-piece IOL group would affect the postoperative refractive errors.

        눈물점 마개 사용 후 발생한 합병증의 치험 증례

        Purpose: To report cases of complications after using punctual plugs with a literature review. Methods: We studied 3 complicated cases after the use of punctual plugs: a 46-year-old female with recurrent mucopurulent discharge, a 42-year-old male with a pinkish mass over the punctum, and a 29-year-old female with recurrent epiphora. Results: The 46-year-old female was diagnosed with canaliculitis caused by actinomyces, the 42-year-old male with chronic pyogenic granuloma, and the 29-year-old female with canalicular obstruction. All 3 cases improved after proper treatments. Conclusions: Complications after the use of punctal plugs are not common but they can be significant. They have to be used carefully after through evaluation and obtained proper informed consents from the patient.

        새로운 Sulfonamide 유도체의 합성과 Acetolactate Synthase(ALS) 저해

        Triazolopyrimidine sulfonanilide (TP) derivative is one of excellent herbicide compounds. We have synthesized three classes of a new sulfonamide derivative (TPP) as Acetolactate synthase (ALS) inhibitors, in which the benzene ring in TP skeleton was converted to substituted pyrimidyl ring and examined their inhibitory activities on barley for ALS. I_(50) values of the inhibitors ranged from 0.005 to 2 mM. Comparing the I_(50) value of each class of TPP derivatives, the substituents in pyrimidine and triazolopyrimidine ring were found to affect the degree of ALS inhibition. TPP with substituted methyl group in pyrimidine ring showed higher inhibitory activity than that with methoxy group, while the substitution of the cyclopentano group in triazolopyrimidine ring gave very large inhibitory activity than that of methyl group. The present study established that variation of the electron density by substitution at heterocyclic ring is a very important factor for ALS inhibition, but showed no dependence on steric effect by substituents.

        유기산의 해리평행에 미치는 치환기효과와 그의 온도 및 압력의 영향. (6) 수용액중에서 몇가지 아미노산의 해리상수

        세가지 아미노산 즉 글리신, 알라닌 및 트립토판의 두가지 해리상수를 전도도법을 이용하여 온도(15 ~ 35$^{\circ}$C)와 압력(1~2,500bar)을 변화시키면서 측정하였다. 두가지 해리상수는 온도가 높아지면 모두 증가했으나 압력이 높아지면 첫째 해리상수는 증가하고 둘째 해리상수는 트립토판을 제외하면 감소하였다. 이와같은 현상은 반응의 열역학적 성질을 이용하여 설명하였으며 나아가서 반응의 특성을 알아보기 위해서 Hammett의 법칙을 적용하고 반응의 치환기 상수와 압력에 따른 반응상수를 계산하였다. 아미노산의 해리상수의 특성을 치환 효과로 설명하였고 반응의 유형을 반응상수로서 알아보았다. The two dissociation constants of three amino acid, glycine, alanine, and tryptophan were measured by the conductometric method in the temperature from 15 to 35$^{\circ}$C and pressure up to 2,500bar in aqueous solution. The both dissociation constants were increased as the temperature increased but the pressure effect were not same as the temperature effect. The first constants were increased as the pressure increased but second constants were decreased except tryptophan. The characteristic properties of these amino acids were discussed from the thermodynamic properties of the dissociation reaction. The substituted effects of the reaction were deduced from Hammett reaction and substituted constants which were calculated from the dissociation constants.
