        Reflux Following Esophagectomy for Esophageal Cancer

        Gastroesophageal reflux is a common problem after gastroesophageal resection and re- construction, despite the routine prescription of proton pump inhibitors (PPIs). Resection of the lower esophageal sphincter and excision of the vagus nerve are generally thought to be the main factors that interfere with gastric motor function. However, physiological studies of reflux symptoms after esophagectomy are still lacking. Gastroesophageal reflux occurs frequently after esophagectomy, but there is no known effective method to pre- vent it. Therefore, in order to manage gastroesophageal reflux after esophagectomy, strict lifestyle modifications and gastric acid suppression treatment such as PPIs are needed, and further clinical studies are required.

        Retrospective Molecular Epidemiology Study of PD-L1 Expression in Patients with EGFR-Mutant Non-small Cell Lung Cancer

        Purpose Data are limited on programmed death ligand 1 (PD-L1) expression in epidermal growth factor receptor (EGFR)-mutant non-small cell lung cancer (NSCLC). Materials and Methods We retrospectively evaluated the relationship between PD-L1 expression and recurrencefree survival (RFS) and overall survival in 319 patients with EGFR-mutant NSCLC who were treated at Samsung Medical Center from 2006 to 2014. Membranous PD-L1 expression on tumor cells was measured using the PD-L1 IHC 22C3 pharmDx antibody and reported as tumor proportion score (TPS). Kaplan-Meier methods, log-rank test, and Cox proportional hazards models were used for survival analysis. Results All patients had ! 1 EGFR mutation—54% in exon 19 and 39% in exon 21. Overall, 51% of patients had PD-L1–positive tumors. The prevalence of PD-L1 positivity was higher among patients with stages II-IV versus stage I disease (64% vs. 44%) and among patients with other EGFR mutations (75%) than with L858R mutation (39%) or exon 19 deletion (52%). PD-L1 positivity was associated with shorter RFS, with an adjusted hazard ratio of 1.52 (95% confidence interval [CI], 0.81 to 2.84; median, 18 months) for the PD-L1 TPS ! 50% group, 1.51 (95% CI, 1.02 to 2.21; median, 31 months) for the PD-L1 TPS 1%-49% group, and 1.51 (95% CI, 1.05 to 2.18) for the combined PD-L1–positive groups (TPS ! 1%) compared with the PD-L1–negative group (median, 35 months). Conclusion PD-L1 expression is associated with disease stage and type of EGFR mutation. PD-L1 positivity might be associated with worse RFS among patients with surgically treated EGFRmutant NSCLC.

      • 악성 기관-기관지 종양에 대한 냉동 수술법

        냉동 치료의 장점은 비침습적이며 통증이 없고 재치료가 가능하기 때문에 수술 고위험군, 재발환자 등에서 국소암 치료로서 적용될 수 있다는 점이다. 외부영상과 굴곡내시경을 통하여 위치를 파악하고 내시경으로 표적부위만을 냉동하기 때문에 주변조직의 손상이 거의 없으며 심폐 기능에 큰 위험을 주지 않는다. 처음 치료 후에도 치료를 한 부위나 다른 부위에도 여러 번의 추가적 치료가 가능하므로 치료로 불완전한 부분을 추가적으로 치료를 하여 좋은 결과를 얻을 수 있다. 냉동 수술의 특성을 잘 이용하여 적절한 환자에게 적용한다면, 기관-기관지의 폐색을 일으키는 악성 종양 환자들에게 좋은 치료방법의 하나라고 생각된다. Cryosurgery is a safe method for palliation of endobronchial malignancies causing airway obstruction. Due to its simplicity and effectiveness for controlling bleeding, endobronchial cryosurgery is considered to be a good method that is clinically applicable to a malignant endobronchial tumor. In cases with stenosis caused by an endoluminal tumor, cryo-recanalization with a cryoprobe was immediately effective in most of the patients. A novel technique described by Hetzel in 2004 to achieve rapid recanalization of central airway obstruction with endobronchial cryoprobe. Cryorecanalization technique is feasible and offers many advantages in the interventional therapy of malignant intraluminal tumors of the respiratory tract.

        종격동에 발생한 성장 기형종 증후군 (Growing Teratoma Syndrome)의 치험-1예 보고-

        A 15-year-old male was admitted with right-sided chest pain and cough for one month. On chest computed tomographic scan, a 10×15×16 cm-sized huge mediastinal mass was occupied in the right hemithorax. Radiologically, it seemed that the tumor was severely adhesive on the heart and the superior vena cava. Therefore we decided on chemotherapy and radiotherapy first instead of surgery. The tumor marker was nearly normalized afterwards, but the tumor size was seemed to be bigger on chest tomographic scan. This suggests the growing teratoma syndrome. After the successful resection, he showed symptomatic improvement and is being followed up without any symptoms in an out patient department up to now. 특이 병력 없던 15세 남자가 내원 한 달 전부터 시작된 우측 흉통과 함께 기침을 호소하여 본원 응급실로 내원하였다. 흉부 전산화 단층촬영에서 10 15 16 cm 크기의 전 종격동 종양으로 상대 정맥과 심장을 압박하면서 침범하고 있는 소견을 보였다. 수술 절제가 불가능한 것으로 판단하여 항암 및 방사선 치료를 시행하였다. 이후 종양 표지자는 거의 정상화되었으나 오히려 종양의 크기가 증가하는 소견을 보여, 종격동의 성장 기형종 증후군(Mediastinal Growing Teratoma Syndrome)으로 판단되어 수술을 시도하였다. 종양의 수술적 제거 후 환자는 24개월째 잔존 종양 및 재발 없이 외래 관찰 중이다.

        텅스텐을 치환한 비납계 (Bi0.5Na0.5)(Ti₁-xWx)O₃ 세라믹의 유전 및 압전 특성

        W$^{6+}$-doped lead-free (Bi$_{0.5}$Na$_{0.5}$)(Ti$_{1-x}$W$_x$)O$_3$ ($x$ = 0.0$\sim$0.016; BNTW) ceramics have been prepared by using a solid-state reaction process, and their microstructural and electrical characteristics were investigated. The grain size decreased up to the solubility limit ($x$ $\sim$ 0.8 mol\%) as $x$ was increased. The piezoelectric constant $d_{33}$ and the remnant polarization $P_r$ of the BNTW ceramics had maximum values of 84 pC/N at $x$ = 0.004 and 42 $\mu$C/cm$^2$ at $x$ = 0.002, respectively. At room temperature the mechanical quality factor $Q_m$ and the coercive field $E_c$ decreased with increasing W content while dielectric constant increased. The phase transition temperature $T_m$ and their maximum dielectric constant $\varepsilon_{max}$ decreased slightly, and the phase transition became more diffuse as $x$ was increased. These results are explained by the formation of A-site cation vacancies to maintain charge neutrality when W$^{6+}$ ions substitute for Ti$^{4+}$ ion on B sites of perovskite, similarly to the case of donor doping in the PZT system. W$^{6+}$를 치환한 비납계 (Bi$_{0.5}$Na$_{0.5}$)(Ti$_{1-x}$W$_x$)O$_3$ (BNTW) 세라믹을 고상반응법으로 제조하여 유전 및 압전 특성을 조사하였다. 단일의 능면정 페로브스카이트 구조를 갖는 고용한계 ($x$ $\leq$ 0.8 mol\%)까지 W의 양이 증가함에 따라 낟알의 크기는 작아졌으며, 소결시편의 상대밀도는 증가하였다. BNTW 세라믹 시편의 압전상수 $d_{33}$는 W의 양이 0.4 mol\%에서 최대값 84 pC/N을 나타내었으며, 잔류분극 $P_r$은 W가 0.2 mol\%에서 최대값 42 $\mu$C/cm$^2$을 가졌다. W의 농도가 증가함에 따라 기계적 품질계수 $Q_m$와 항전계 $E_c$의 값은 감소하였고, 실온에서 유전상수의 값은 증가하였다. 또한 $x$가 증가함에 따라 큐리 온도 $T_m$ 및 이 온도에서의 최대 유전상수 $\varepsilon_{max}$ 는 감소하였으며, 보다 완만한 상전이 거동을 보였다. 이는 PZT 계에서 주개를 도핑한 경우와 마찬가지로 (Bi$_{0.5}$Na$_{0.5}$)TiO$_3$ (BNT)에서 $B$ 자리의 Ti$^{4+}$를 W$^{6+}$ 이온이 치환될 때 전하보상을 위하여 $A$ 자리에 양이온 빈자리가 형성되는 것으로 설명할 수 있었다.

        Dielectric and ac Conductivity Properties of Iodine-Doped Poly (2-Ethynyl-N-Hexylpyridinum Bromide)

        The dielectric response and the ac conductivity of heavily iodine-doped poly(2-ethynyl-N-hexylpyridinum bromide) (PHEPB) were studied. The samples were prepared by adding iodine to the conjugated ionic polymer PEHPB, and the dielectric and conductance properties were measured in the frequency range from 50 Hz to 1 MHz. The real part of the dielectric constant showed a stronger dispersion in the low-frequency region, and the imaginary part exhibited a nearly constant slope. The ac conductivity was well described by using the universal dielectric response, and the dc and ac activation energies that represent the conduction and the relaxation mechanisms were 0.507 and 0.596 eV, respectively. The room-temperature conductivity at 1 kHz was found to be enhanced by more than three orders of the magnitude after iodine doping. The impedance Cole-Cole plot suggested that the dielectric relaxation occurred through a non-trivial relaxation mechanism.

        용융법에 의한 수용성 유리의 제조 및 특성 (I) : 용해 특성, 살균 효과 및 세포 독성

        Water-soluble phosphate glasses containing Ag or Cu ion were prepared through melting process. Then the powdered glass samples were dissolved in D.I. water at room temperature with changing the dissolution time. In terms with the glass composition, dissolution characteristics, bactericidal effects and cytotoxicities were investigated. Dissolved amounts increased uniformly with dissolution time, and the dissolution rate was higher for ternary glass than for binary glass and with less metal oxide amount. And the dissolution rate of the glass with Ag ion was higher than that with Cu ion, and the bactericidal effect of the glass with Ag ion was also greater. Solution with more than 25 ppm of Ag was observed to have strong cytotoxicity to L929, and solutions of lower Ag concentration or with Cu seemed to have little cytotoxicity.

        Establishment of Minimally Invasive Thoracic Surgery Program

        The establishment of minimally invasive surgery is a complex and difficult task. Video-as- sisted thoracic surgery (VATS) refers to a minimally invasive surgical technique that rep- resents a less invasive approach to thoracic surgery using thoracoscopy. For lung cancer or esophageal cancer surgery, planning and establishing a team for minimally invasive surgery for the first time is not a simple task. Technical advances in surgical devices and the enhanced skill of surgeons are cornerstones of the development of minimally invasive surgery. Here, we review the meaning of minimally invasive thoracic surgery and discuss how to establish a team approach for VATS procedures.
