        α<sub>2</sub>-Adrenoceptor Agonists의 흰쥐 대동맥 이완 작용

        The vasorelaxant actions and blood pressure lowering of the ${\alpha}_2$-adrenoceptor agonists (${\alpha}_2$-AAs) clonidine and xylazine were investigated in rat isolated aortic rings and anesthesized rats. Both clonidine and xylazine produced a concentration-dependent inhibition of the sustained contraction induced by norepinephrine (NE), but not by KCl. NE-induced contractions were attenuated partly by nifedipine or verapamil, voltage dependent $Ca^{2+}$ channel blockers. These $Ca^{2+}$ channel blockers-resistant contractions were abolished by clonidine or xylazine. Inhibitory effects of a ${\alpha}_2$-AAs on contractions could be reversed by ryanodine, an intracellular $Ca^{2+}$, transport blocker, and tetrabutylammonium (TBA), a $Ca^{2+}$ activated $K^+$ channel blocker, but not by nifedipine, glibenclamide or removal of extracellular $Ca^{2+}$ and endothelium. Moreover, ${\alpha}_2$-AAs produced relaxation in NE-precontracted isolated intact aortic rings in a concentration-dependent manner, but not in KCl-precontracted rings. The relaxant effects of ${\alpha}_2$-AAs were inhibited by ryanodine and TBA, but not by nifedipine, glibenclamide, N (G)-nitro-L-arginine (L-NNA), N(omega)-nitro-L-arginine methyl ester (L-NAME), aminoguanidine (AG), 2-nitro-4-carboxyphenyl N,N-diphenylcarhurnte (NCDC), lithium sulfate, staurosporine or removal of extracellular $Ca^{2+}$ and endothelium. In vivo, infusion of xylazine elicited significant decrease in anerial blood pressure. This xylazinelowered blood pressure was completely inhibited by the intravenous injection of TBA, but not by the intravenous injection of glibenclamide, L-NNA, L-NAME, AG, nifedipine, lithium sulfate or saponin.. These findings showed that the receptor-mediated and ${\alpha}_2$-adrenoceptor A-stimulated endothelium-independent vasorelaxant effect may be explained by decreasing intracellular $Ca^{2+}$ release and activation of $Ca^{2+}$-activated $K^+$ channels, which may contribute to the hypotensive effects of ${\alpha}_2$-AAs in rats.

      • S-101 알코올성 간경화 환자에서 발생한 모자용종증 1예

        배경: 모자용종증은 복통, 설사 등의 임상적 증상과 무경성 염증성 용종의 내시경적 소견을 보이며 조직학적으로 섬유소화농성 삼출물과 염증성 육아조직으로 씌워진 신장되고 팽창된 사행성의 과형성 음와를 가진 무경성 용종의 형태를 특징으로 한다. 발병기전, 임상경과, 치료방법 등이 확립되지 않은 상태이다. 저자들은 복수를 동반한 알코올성 간경화 환자에서 진단된 모자용종증에서 보존적 치료를 통해 호전된 경과를 경험하였기에 보고하는 바이다. 증례: 49세 남자 환자가 10일 전부터 복부팽만, 전신 부종을 주소로 입원하여 복수를 동반한 알코올성 간경화 악화에 대한 보존적 치료 중, 하루 10회 이상 점액성 설사와 후중감으로 전신무력감 발생하였다. 환자는 5년 전부터 고혈압, 당뇨로 약물 복용하고 있었고 1년 전부터 알코올성 간경화로 본원 소화기내과 외래 추적 관찰 중이었으며 염증성 장질환 혹은 대장암의 가족력은 없었다. 입원 당시 양측 하지 부종 및 복수로 인한 이동탁음 있었으나 직장수지검사에서 종괴 및 혈변은 없었다. 대변 검사에서 잠혈 반응과 급성설사 원인균 및 바이러스 선별검사 모두 음성이었다. 복부 단순 촬영은 정상 소견이었으나, 복부 전산화단층촬영에서 복수를 동반한 간경화와 대장벽의 부종이 관찰되었다. 대장내시경에서 전 대장 점막의 부종성 변화 및 직장-S상 결장 연결부에 황백색의 삼출물로 덮힌 다발성 무경성 용종들과 산재된 충혈된 점막이 관찰되었다. 조직 검사에서 만성 염증 세포의 침윤, 비정형의 선조직, 선와의 왜곡, 표면층에 부분적으로 육아조직이 덮힌 표재성의 활동성 미란이 관찰되었고 악성 세포는 관찰되지 않았다. 환자는 임상 증상, 내시경 및 조직검사 소견을 종합하여 모자용종증으로 진단되었다. 알코올성 간경화에 대하여 간장용제, 복수 조절을 위해 복수천자 및 이뇨제, 알부민 등의 치료 후 임상 증상이 호전되었다. 추적 대장내시경에서 이전과 비교하여 전 대장점막의 부종 및 직장-S상 결장 연결부에 보였던 다발성 무경성 용종들과 산재된 충혈된 점막은 호전된 소견을 보였다. 조직 검사에서도 이전과 비교하여 호전된 소견을 보였다. 현재 퇴원 후 특별한 증상 없이 본원 소화기내과 외래 추적 관찰 중이다. 고찰: 현재까지 모자용종증은 매우 드문 질환이고 발병기전이 확립되어 있지 않은 상태로 점막탈 증후군과의 연관성이 보고되고 있다. 복수를 동반한 알코올성 간경화 환자에서 대장벽의 부종 소견이 점막탈 증후군에서와 같이 모자용종증의 발생에 영향을 주었고 복수 조절 등 알코올성 간경화에 대한 보존적 치료 후 임상증상, 내시경 및 조직검사 소견의 호전을 보였기에 보고하는 바이다.

        Feasibility of the Use of RapiGraft and Skin Grafting in Reconstructive Surgery

        Background Skin grafting is a relatively simple and thus widely used procedure. However, the elastic and structural quality of grafted skin is poor. Recently, various dermal substitutes have been developed to overcome this disadvantage of split-thickness skin grafts. The present study aims to determine the feasibility of RapiGraft as a new dermal substitute. Methods This prospective study included 20 patients with partial- or full-thickness skin defects; the patients were enrolled between January 2013 and March 2014. After skin defect debridement, the wound was divided into two parts by an imaginary line. Split-thickness skin grafting alone was performed on one side (group A), and RapiGraft and split-thickness skin grafting were used on the other side (group B). All patients were evaluated using photographs and self-questionnaires. The Manchester scar scale (MSS), a chromameter, and a durometer were used for the scar evaluation. The average follow-up period was 6 months. Results The skin graft take rates were 93% in group A and 89% in group B, a non-significant difference (P=0.082). Statistically, group B had significantly lower MSS, vascularity, and pigmentation results than group A (P<0.05 for all). However, the groups did not differ significantly in pliability (P=0.155). Conclusions The present study indicates that a simultaneous application of RapiGraft and a split-thickness skin graft is safe and yields improved results. Therefore, we conclude that the use of RapiGraft along with skin grafting will be beneficial for patients requiring reconstructive surgery.

        Changes of Blood Mg^(2+) and K^+ after Starvation during Molting in Laying Hens

        Either the fasting during natural molting or the starvation in induced molting would be a severe metabolic stress to laying hens. The metabolic stress during starvation and subsequent refeeding syndrome could lead to unbalance of mineral homeostasis, including Mg^(2+), K^+ and P required by ATP synthesis. Since Mg^(2+) is a fundamental ion for normal metabolic processes and stress may not only increase in demands of Mg^(2+) but also produce consequence of Mg^(2+) deficiency, we investigated the changes of blood ionized and total ions related to starvation during molting in laying hens. We founded the significant decrease in blood Mg^(2+) and K^+ accompanied by the changes of biochemical parameters relating to increased metabolic stress after molting. These results suggested that appropriate Mg^(2+) and K^+ supplements to laying hens could have beneficial effects during molting and subsequent refeeding that could produce a severe hypomagnesemia and hypokalcemia.

        Acute Adrenal Insufficiency Associated with Acute Coronary Syndrome

        Acute adrenal insufficiency (AAI) in acute coronary syndrome (ACS) patients is rare and may be frequently underestimated as simple ACS, since symptoms of AAI are nonspecific. Physicians should be fully aware of the possibility of occult AAI combined with ACS, if clinical suspicion is high. Herein, we report a rare case of a 67-year-old female patient with concomitant AAI and drug eluting stent fracture-induced ACS. To our knowledge, there have been no case reports of AAI associated with ACS in Korea.

        흰쥐 대동맥에서 melatonin의 내피 의존적 혈관 이완 작용에 대한 lithium의 영향

        Melatonin, the principal hormone of the vertebral pineal gland, participates in the regulation of cardiovascular system in vitro and in vivo. Lithium inhibits both inositol polyphosphate phosphatase (IPPase) and inositol monophosphatase (IMPase), which are involved in a wide range of signal transduction pathways. The aim of the present study was to assess the effect of lithium on endothelial-dependent relaxation to melatonin and on the melatonin-induced inhibition of contraction by phenylephrine (PE) in isolated rat aorta. Melatonin induced a concentration-dependent relaxation in PE-precontracted in endothelium-intact (+E) aortic rings. Melatonin inhibited a PE-induced sustained contraction in +E aortic rings. These effects of melatonin on relaxation and contractile responses were inhibited by pretreatment with lithium. In PE-precontracted +E aortic rings, the melatonin-induced vasorelaxations and the inhibitory effects of melatonin on maximal contractions were inhibited by endothelium removal or by pretreatment with L-$N^G$-nitro-arginine (L-NNA), 1H-[1,2,4] oxadiazolo-[4,3-a] quinoxalin-1-one (ODQ) and nifedipine and verapamil, but not by tetrabutylammonium, clotrimazole and glibenclamide, However, in endothelium-denuded (-E) aortic rings and in the presence of L-NNA and ODQ in +E aortic rings, the melatonin-induced residual relaxations and the melatonin-induced residual contractile responses to PE were not affected by lithium. It is concluded that the inositol phosphate pathway may be involved in endothelial-dependent relaxation induced by melatonin.
