        만성신부전증을 동반한 Laurence-Moon-Biedl 증후군 1예

        A case of end stage renal failure occuring in association with the Laurence-Moon-Biedl syndrome is reported. 36-year-old woman shows retintis pigmentosa, mild mental retardation, obesity, hexadactyly on right foot, varginal atrophy and end stage renal failure. The light microscopy of renal biopsy reveals thickening of Bow-man's capsule, hyalinization and widening of mesangium with loss of capillaries, blunted glomerular tuft and mild interstitial fibrosis. Abnorrnalities in renal function and morphology are increasingly recognized in Laurence-Moon-Biedl syndrome which uremia is an important cause of morbidity and mortality. Chronic renal failure seems to be the most frequent cause of death.

        만성신부전증 환자에서 β₂-Microglobulin치의 의의

        To evaluate the significance of beta 2-microgulobulin ( g ) in chroinc renal failure, serum and urinary levels were measured in 10 subjects without renal ds (control) and 16 patients with ehronic renal failure without dialysis. The results were as follows: 1) The mean level of serum β₂m, was l. 27± 0. 35 mg/L in control and 18.58±7.46 mg/L in chronic renal failure. 2) The mean amount of urinary B,, was 0.14±0.11 mg/L in control and 14.5±13.7mg/L in chronic renal failure. 3) The levels of the serum creatinine, serum creatinine clearance and clearance were significantly different respectively in cntrol and chronic renal failure. 4) The serum level was correlated with the serum creatinine level (r=0.82, p$lt;0.01), and creatinine clearance (r= 0.74, p$lt;0.01) in studied cases. 5) The urinary B,, level was not correlated with the serum level, the serum creatinine level and the creatinine clearance in studied cases. These results suggest that the determination of the serum β₂m, level is a useful laborataid in patients with chronic renal failure.

        혈액투석중인 만성 신부전 환자에서 혈청 Neopterin 농도에 관한 연구

        N/A Neopterin is a low-molecular-weight product released by human macrophages upon stimulation with interferon gamma. It permits the quantification of the level of cellulalr immune activation in vivo. We measured the level of serum neopterin and creatinine concentrations in 20 patients before and after hemodialysis in order to test whether hemodialysis induces specific activation of the T-lymphocyte macrophage axis. The results were as follows: 1) In normal adults, the serum neopterin level was 6.7±1.3 nmol/L with no sex difference. 2) In 20 hemodialysis patients, the serum neopterin level before hemodialysis was 246.2±97.6 nmol/L and increased significantly compared to normal adults (p<0.001). 3) In 20 hemodialysis patients, the serum neopterin level was 163.0±76.7 nmol/L after hemodialysis and decreased significantly (p<0.001), the neopterin/creatinine ratio was 25.4±14.7, 32.8±23.7 before and after hemodialysis, respectively, and there was no significant difference between both groups. 4) In groups of short-term hemodialysis (less than three months), the serum neopterin level was 248.4±100.6, 181.7±91.2nmol/L before and after hemodialysis, respectively. In groups of long-term hemodialysis (more than one year), the neopterin level was 244.0±99.9, 144.2±57.6 nmol/L before and after hemodialysis, respectively, and decreased significantly after hemodialysis in both groups (p<0.001), and there was no significant difference between both groups. 5) In 12 male hemodialysis patients, the serum neopterin level was 226.4±97.6, 145.5±60.5nmol/L before and in after hemodialysis, respectively, and in eight female hemodialysis patients, the serum neopterin level was 275.9±96.0, 189.1±94.4 nmol/L before and after hemodialysis, respectively, and decreased significantly in both groups (p<0,05, respectively).         만성신부전환자에서 혈청 Adenosine Deaminase 활성도에 관한 연구

        N/A To evaluate the clinical availability of serum adenosine deaminase (ADA) activity as an immunological parameter in chronic renal failure patients undergoing hemodialysis, serum ADA activities and other immunologica1 parameters, such as total lymphocyte count and its subpopulations and concentrations of immunoglobulin G, A and M, were measured simultaneously and analyzed for their correlations in 20 normal adults and 30 chronic renal failure patients. The results were as follows: 1) In normal adults, the mean value of serum ADA activities was 18.22±1.58U/L with no age and sex differences. 2) In two grous of long-term hemodialysis (over 12months) and short-term hemodialysis (under 1 month), mean values of serum ADA activities were 31.4±10.1U/ L and 33.6±12.3U/L respectively. There were significant differences between normal adults and the two hemodialysis groups, but there was no significant difference between the two hemodialysis groups. 3) There was no significant difference among various renal diseases. 4) Mean values of serum ADA activities decreased after hemodialysis in each group. 5) There were no significant correlations between serum ADA activity and BUN, creatinine, total lymphocyte count and subpopulations, and concentrations of immunoglobulin G, A and M. As mentioned above, serum ADA activities in chronic renal failure patients undergoing hemodialysis were higher than in normal adults, and there was a decreasing tendency of serum ADA activity immediately after hemodialysis, but serum ADA activity seems of little use as an immunological parameter in chronic renal failure patients.
