We have experienced a case of primary intracranial malignant melanoma. A 15-year-old female was admitted to our hospital with chief complaints of headache, vomiting, and left ophthalmoplegia. Darkly brown colored, fragile tumor was found on the medial third of left sphenoid ridge. The tumor enclosed left supraclinoid portion of the internal carotid artery, optic chiasm, and eroded left anterior and posterior clinoid processes. Nearly total removal of the tumor mass was done and pathological diagnosis was malignant melanoma.

저자는 1980년 1월부터 1984년 12월까지 경북대학병원 신경외과에 입원된 출혈을 일으킨 남·녀 86명의 전교통 동맥류 환자를 대상으로 하여 연령 및 성별 분포, 동맥류의 크기, 출혈의 빈도, 신경학적 장애 및 수술 치료의 결과 등을 분석, 관찰하였다. 남·녀의 발생비는 1.6 대 1로서 남자에서 높은 빈도는 나타내었으며 연령별 분포는 41∼50세와 51∼60세층에서 전체의 69.7%를 차지하였다. 출혈을 일으킨 동맥류의 크기는 최장경이 6∼10㎜에 속하는 것이 53.5%로서 가장 많았다. 주위 혈관의 기형은 57%에서 나타났으며 A_1의 Hypoplasia(50%)가 가장 많았다. 2회 이상의 출혈은 31.4%에서 있었으며 재출혈의 발생이 1주 미만 및 1∼2주가 각각 37% 및 22.2%였다. 술전 또는 출혈 후 1주를 기준으로 하였을 때 신경학적 장애 정도는 55.9%가 Grade Ⅰ, Ⅱ였고, 44.1%가 Grade Ⅲ, Ⅳ 및 Ⅴ에 속하였다. 40명의 수술군에서 수술의 시기는 마지막 출혈 후 1∼2주와 2∼3주에 시도한 것이 각각 35%였다. 수술치료의 결과는 생존자가 92.5%였으며 72.5%가 Good 상태로 회복되었다. 그리고 사망자는 3명으로서 7.5%였으며 그 중 2명은 Grade Ⅴ에 속하는 환자였다. 술전 신경학적 장애, 연령 및 혈압은 수술 치료의 결과와 밀접한 관계가 있었다. The authors made analyses and observatrions, January 1980-December 1984, on age and sex distribution, the size of aneurysmal sac, bleeding frequency, neurologic defect and the outcome of 40 surgical cases among 86 patient with anterior communicating artery aneurysm at the department of neurosurgery, Kyungpook National University Hospital. The incidence ratio of male and female was found to be higher in males by 1.6 to 1. The age brackets of 41-50 and 51-60 accounted for 69.7 percent of whole subjects. The size of ruptured aneurysmal sac, ranging from 6㎜ to 10㎜, took up the majority of the cases. Fifty-seven percent of the total showed vascular anomalies with A_1 hypoplasia most outstandingly, as high as 50 percent. The breakdown of bledding times following the initial is; 37 percent-less than one week, 22.2 percent-between the first week and the second week. The incidence of bleeding more than twice is 31.4 percent. Breakdowning of clinical grades, just before operation or after a week from bleeding in nonsurgical group is; Grade Ⅰ and Ⅱ : 55.9 percent, and Grade Ⅲ, Ⅳ, and Ⅴ : 44.1 percent. Thiry-five percent of the 40 patients underwent the surgery between the first-second week and second-third week of the last bleeding. The result of the operations are followings; 92.5 percent of survival, with 72.5 percent benefitting good state; 7.5 percent of death including patients of Grade Ⅴ. It was revealed that the age, blood pressure and preoperative status of the subjects are most responsible for operative results.
흰쥐 척수좌상에서 선택적 Cyclooxygenase-2 억제제인 NS-398 전처치의 효과

Background : Secondary spinal cord injury (SCI) that follows an initial mechanical insult can exacerbate the overall damage, limit the restorative processes and eventually lead to an increased neurological deficit. We hypothesized that selective inhibition of cyclooxygenase-2 (COX-2) may decrease the delayed cell death, and so this will contribute to decreased level of the secondary injury. Methods : The dorsal surface of the cord at the T9 level was subjected to weight drop impact using a 10 g rod. To block COX-2 activation, a selective COX-2 inhibitor (NS-398) was administered (5 mg/kg, i.p.) 15 min prior to SCI. The COX-1, COX-2, Caspase-3 and PGE2 expressions were measured by real time quantitative RT-PCR and fluorescence immunostaining. Results : Many activated caspase-3 positive cells were observed at 6 h and they increased until 72 h after SCI. The expression of COX-2 peaked at 6 h after SCI, while the COX-1 expression was unaffected. The principal cells that showed a COX-2 expression were the neurons and microglia. Pretreatment with NS-398 caused a significant decrease in the expression of prostaglandin E2 and activated caspase-3 positive cells after SCI. Conclusion : These data suggest that COX-2 is one of the main factors related with the pathologic deficits from secondary SCI.
