        LABGEO PT Hepatic Test 9의 수행능 평가

        Background: The Samsung LABGEO PT Hepatic Test 9 (Samsung electronics, Korea) was developed as a point-of-care (POC) testing device. The levels of 9 analytes, namely, albumin, AST, ALT, alkaline phosphatase, gamma-glutamyl transferase, glucose, total bilirubin, direct bilirubin, and total protein, could be evaluated simultaneously by using 70 μL of whole blood, plasma, or serum samples. In this study, we assessed the performance of the Samsung LABGEO PT Hepatic Test 9. Methods: The precision and linearity of the test were evaluated according to the CLSI EP5-A2 and CLSI EP6-A guidelines, respectively. Correlational analyses between Samsung LABGEO PT Hepatic Test 9 and Cobas 8000 modular analyzer (Roche, Switzerland) were carried out as per the CLSI EP9-A2 guidelines. Additionally, the results between 3 different specimen types, whole blood, plasma, and serum samples obtained from the same individual were compared to evaluate the matrix effect. Results: The total imprecision at both low and high levels of the 9 analytes was within 10% and in the clinically important concentration range for all test items, all obtained results were linear. We compared the above results with those obtained using Cobas 8000 and a good correlation was observed with a correlation coefficient of more than 0.975 for all 9 analytes. Simple linear regression analyses between the 3 different specimen types indicated that there was no statistically significant difference (P <0.001). Conclusions: The Samsung LABGEO PT Hepatic Test 9 showed good precision and linearity when compared to established assays for 9 clinical test items and could be useful in cases where the POC testing is required. 배경: Samsung LABGEO PT Hepatic Test 9 (Samsung electronics,Korea)은 현장검사용으로 개발되어 혈청, 혈장 검체 뿐 아니라 전혈에서 알부민, AST, ALT, ALP, GGT, 혈당, 총빌리루빈, 직접빌리루빈, 총단백 등 9가지 항목을 동시에 측정할 수 있다. 본 연구는Samsung LABGEO PT Hepatic Test 9의 수행능을 평가하고자 하였다. 방법: CLSI 지침에 따라 9가지 일반화학 검사 항목에 대해 정밀도,직선성, 기존 장비와의 상관성을 평가하였다. 또한 검체 종류에 따른 기질효과를 평가하기 위해 동일 환자의 전혈, 혈장, 혈청 검체로9가지 항목을 측정하여 비교하였다. 기존 장비와의 평가에는 Cobas8000 modular analyzer (Roche, Switzerland)를 사용하였다. 결과: 9가지 검사 항목의 저농도와 고농도에서 총 정밀도는 모두10% 이내였고, 모든 검사 항목은 임상적으로 중요한 범위에서 직선성을 나타내었다. 기존 장비와의 상관성 평가에서 9가지 항목 모두 상관계수 0.975 이상으로 우수한 상관성을 나타내었다. 전혈, 혈장, 혈청 검체 간 단순선형회귀분석에서 9가지 항목 모두 통계적으로 유의한 상관성이 있었다. 결론: Samsung LABGEO PT Hepatic Test 9은 9가지 일반화학 검사 항목 측정에 있어 정밀도, 직선성 및 기존 장비와의 상관성이우수하여 현장검사가 필요한 곳에서 유용하게 사용될 수 있을 것으로 사료된다.

        Accuracy of Creatinine Assay According to Expanded Proficiency Testing in Participants

        Background: In 2020, the accuracy-based creatinine assay (ABCr) proficiency testing (PT) survey was transitioned from an optional to a mandatory program of the Korean Association of External Quality Assessment Service (KEQAS). This study was conducted to analyze the creatinine assay bias according to the time of ABCr PT participation. Methods: Raw data from the ABCr PT survey conducted from 2020 (1st and 2nd round) to 2021 (1st round) by the KEQAS were used. The participating laboratories were divided into two groups (before 2019 vs. after 2020). The after 2020 group refers to the laboratory participating in ABCr PT for the first time since 2020. The creatinine assay bias between the two groups was compared for each PT round. The acceptable rate between the two groups was compared. Results: The mean absolute percentage bias of the before 2019 group was significantly smaller than that in the after 2020 group as follows: 1st round in 2020, 3.8% vs. 5.2%, P <0.001; 2nd round in 2020, 3.3% vs. 4.8%, P <0.001; 1st round in 2021, 4.1% vs. 7.4%, P <0.001. The acceptable rates in the before and after 2019 groups were 91.9%–98.7% and 64.9%–94.3%, respectively. Conclusions: The laboratory that participated in the ABCr PT before 2019 showed smaller bias in the creatinine assay compared to that of laboratories participating in the program for the first time after 2020. Efforts should be made to improve the quality of the creatinine assay through continuous participation in the ABCr PT survey.

        객담 검체에서 다제내성 결핵균의 검출을 위한 GenoType® MTBDRplus의 평가

        Background: GenoType^® MTBDRplus assay (Hain Lifescience, Germany) enables detection of the mutations prevalent in rpoB, katG, and inhA genes and identification of Mycobacterium tuberculosis complex (MTB). We evaluated the performance of the MTBDRplus assay in detecting multidrug resistant M. tuberculosis in sputum specimens by directly comparing it to the performance of conventional drug susceptibility testing (DST) with M. tuberculosis culture isolates. Methods: From December 2007 to July 2008, 40 patients with acid-fast bacilli (AFB) smear-positive and AFB culture-positive sputa, including 19patients with rifampin (RIF)- or isoniazid (INH)-resistant MTB isolates, were enrolled. The MTBDRplus assay was performed using DNA extracted from respiratory specimens. DST of the culture isolates was performed using an absolute concentration method. Results: The result of the AFB smear test was ±1 for 7 specimens, +1 for 8 specimens, +2 for 9 specimens, +3 for 9 specimens, and +4 for 7 specimens. The MTBDRplus assay revealed that 37 of the 40 specimens were positive for an MTB-specific band, 12 specimens were RIF-resistant,and 16 specimens were INH-resistant. The rpoB S531L mutation was detected in 58.3% of the RIF-resistant specimens, and the katG S315T1and inhA C15T mutations were detected in 56.3% and 31.3% of the INH-resistant specimens, respectively. Compared to the sensitivity and specificity of DST, both sensitivity and specificity of MTBDRplus assay for RIF resistance were 100%, and the corresponding values for INH resistance were 82.4% and 90.0%. Discrepant MTBDRplus assay and DST results were obtained in 3 INH-resistant isolates without mutation and 2 INHsusceptible isolates with katG S315T1 and inhA C15T mutations. Conclusions: The MTBDRplus assay can be applied for AFB smear-positive specimens with positivity ± to 4+. The assay was reliable for predicting the RIF resistance of culture isolates, but DST was required for confirming INH resistance.

        표고 균사에 의한 탈지 대두박 생물전환 발효물의 이소플라본, 베타글루칸 함량 및 항산화활성

        This study investigated the isoflavone content, total phenol content, antioxidant activities (DPPH radical scavenging and oxygen radical absorbance capacity) and β-glucan content of defatted soybean extracts by bioconversion. Soybean was fermented with Lentinula edodes using submerged liquid fermentation system. Defatted soybean powder prepared by hexane (HDS; hexane defatted soybean) and ethanol (EDS; ethanol defatted soybean). The major components of non-fermented HDS (NFHDS) and EDS (NFEDS) were glucoside, such as daidzin, glycitin and genistin. During the bioconversion processing, isoflavone glucoside converted into aglycone such as daidzein, glycitein and genistein. The highest total isoflavone contents of fermented HDS (FHDS) were 2577.96 μg/mL, and the lowest total isoflavone contents of NFEDS were 428.27 μg/mL. The highest total phenol contents of fermented EDS (FEDS) was 42.34 mg GAE/g. DPPH radical scavenging and ORAC value were 31.30 to 59.92 % and 247.48 to 786.36 μM TE/g in non-fermented defatted soybean and fermented soybean, respectively. β-Glucan contents were 0.09 to 0.11 % in non-fermented defatted soybean and fermented soybean, respectively. These results indicate that fermented soybean could be used as natural antioxidants for the development of functional foods.

        다람쥐(Tamias sibiricus)의 정자변태

        한국산 다람쥐(Tamias sibiricus)의 정자변태 과정을 투과전자현미경으로 조사하였다. 정자변태는 첨체변화와 핵의 형태의 특징을 기초로 하여 골지기, 두모기, 첨체기, 성숙기 그리고 이탈기로 구분하였고, 골지 두모 첨체기는 각각 전 중 후기, 성숙기는 전 후기, 이탈기는 1기로 세분하였다. 따라서 다람쥐(Tamias sibiricus)의 정자변태는 12기(phases)로 구분되어졌다. 골지기(steps 1-3)의 경우, 잘 발달된 골지복합체는 첨체소포가까이에 위치하고, 첨체소포는 3단계에서 핵막과 융합하여 함입되어있다. 두모기(steps 4-6)에서는, 첨체소포가 핵의 표면 위에 넓게 퍼지며 핵의 1/3을 덮고, 첨체과립은 아직 분산되지 않았다. 첨체기(steps 7-9)동안에 있어서, 핵과 첨체는 신장되었으나 핵질은 농축되지 않았다. 성숙기(steps 10-11)에서는 핵질이 더욱 농축되어 졌으며, 미토콘드리아들은 축사의 중심에 완전하게 배열되어졌다. 이탈기(step 12)에서 정자머리는 완전하게 주걱형태의 모양을 갖추고 있었다. Spermiogenesis in the Korean squirrel, Tamias sibiricus, was investigated by transmission electron microscopy. Spermiogenesis was divided into Golgi, cap, acrosome, maturation and spermiation phases based on the characteristics of acrosomal changes and nuclear shape. Beside, the Golgi, cap and acrosomal steps were subdivided into three phases of early, middle and late phase respectively, the maturation step was divided into two phases of early and late phase, and spermiation step has only one phase. Thus, the spermiognesis of T. sibiricus was divided into a total of twelve phases. In Golgi phase (steps 1-3), a well developed Golgi complex was located close to the vesicles, the acrosomal vesicle fixd to a recess of nuclear membrane at step 3. During cap phase (steps 4-6), the acrosomal vesicle spred over the nuclear surface to cover a third of the nucleus, and the acrosomal granule was not yet flattened. At acrosomal phase (steps 7-9), the nucleus and acrosome were elongated but nucleoplasm was not condensed. During maturation phase (steps 10-11), the nucleoplasm was more condensed, and the mitochondria completely arranged the center of axoneme. The spatulate-sperm head was completely formed at spermiation phase (step 12).

        Prognostic relevance of the Ki-67 proliferation index in patients with mantle cell lymphoma

        BackgroundA high Ki-67 proliferation index (PI) in neoplastic cells is associated with poor survival in mantle cell lymphoma (MCL). We aimed to determine the cut-off values for the Ki-67 PI as a prognostic factor in MCL according to bone marrow findings.MethodsImmunohistochemical (IHC) staining for Ki-67 was performed on formalin-fixed paraf-fin-embedded biopsy tissues from 56 patients with MCL. Patients were grouped based on their Ki-67 PI values. Survival analyses were carried out and the cut-off value for the Ki-67 PI was determined. ResultsOf the 56 patients, 39 (69.6%) showed bone marrow involvement of MCL; 21 of these patients had leukemic manifestations at the time of diagnosis. The results of the Ki-67 IHC staining were as follows: ≤10% in 22 patients, 11‒20% in 14 patients, 21‒30% in 3 patients, 31‒40% in 4 patients, 41‒50% in 4 patients, and >50% in 9 patients. A cut-off value of 20% revealed significantly different survival rates with mean survival times of 69.8 months (Ki-67 PI≤20%) and 47.9 months (Ki-67 PI>20%), irrespective of bone mar-row findings (P=0.034). Clinical outcomes did not differ, regardless of bone marrow findings. However, in cases with bone marrow involvement, the Ki-67 cut-off value of 30% for overall survival was required to yield statistical significance (P=0.033). ConclusionThe 20% cut-off value for the Ki-67 PI was clinically meaningful, regardless of bone marrow involvement of MCL. For patients with bone marrow involvement, the statistically sig-nificant cut-off value increased to 30%.
