        관동맥 우회술 환자에서의 경동맥 협착을 진단하기 위한 Duplex sono 선별검사의 의의

        In an effort to reduce the stroke rate of patients after coronary artery bypass, many authors have studied the prevalence of the extracranial carotid disease and its role in determining neurologic morbidity and mortality rates. From April 1992 to August 1993, Seventy-five patients undergoing coronary artery bypass were preoperatively evaluated for presence of carotid and femoral stenosis by Duplex sono. Among them, fourteen patients was positive by Duplex sono and overall prevalence of carotid or femoral stenosis was 18.7%. And significant carotid stenosis [ > 60% ] had proved to be in 3 patients [ 4.0% ].Prophylatic bilateral carotid endarterectomy was performed in one patient, at 5 months prior to and , 1 week prior to coronary artery bypass respectively. Their mean age was 57.6 years [ ranged from 40 to 70 years] and were composed of 10 males and 4 females. There was no postoperative morbidiry and mortality related to neurologic complications. Our data, although small, suggest that preoperative carotid screening is helpful to determine patients at high risk of stroke, and significant simultaneous carotid and coronary atherosclerosis should be corrected in selected patients by staged operations when feasible.

        국내 치과의사의 근골격계질환 실태 조사

        Dentists are known to be highly exposed to the musculoskeletal disorders (MSD). The present study investigated the prevalence of MSD among Korean dentists and association between their MSD and physical workload. In addition, work-related causes of high physical workload were identified and needs of dentists were collected for improvement of dental instrument and environment. Four types of questionnaires including the Nordic questionnaire were used to investigate body troubles, physical workload, causes of physical workload, and improvement priority of dental components. A total of 104 dentists in Seoul were participated in the survey. Neck trouble (82%) was the most prevalent in the participated dentists and shoulder (68%) and low back trouble (56%) followed, while low back trouble was reported to be most common in previous western studies. The body troubles were related to the physical workload of the corresponding body parts, although they were not associated with personal characteristics. Most dentists selected 'to keep direct view inside patient's mouth' and 'no support of the hand with dental instruments' as causes of awkward and strenuous work postures. They wanted design improvement for some components in their operating room such as operating light and arrangement of workplace.

      • 고령 및 당뇨환자에서 양측 내흉 동맥을 이용한 관상동맥 우회술

        As Life expectancy increase, more elderly patients are being referred for coronary artery bypass grafting(CABG). The current conventional and most commonly used operative procedure for myocardial revascularization in patients of all ages involves one internal thoracic artery together with one or more saphenous vein grafts. However, Recently complete arterial grafting with bilateral internal thoracic arteries(BITAs) is the preferred method of myocardial revascularization for all ages, in some centers. The extensive use of internal thoracic artery(ITA) grafting, including EITA grafting, sequential anastomosis, and free grafts, is based on the superior graft patency of this conduit and on the fact that the ITA improves long term survival, decreases the rate of reoperation, and improves reoperation-free survival and cardiac morbidity in multiple coronary artery disease, but most published series failed to show additional survival benefit with BITA grafting. This review based on the two frequent asked questions in coronary artery bypass surgery using bilateral internal thoracic arteries: Does BITA grafting increase surgical morbidity & mortality? Does BITA grafting increases the likelihood of perioperative myocardial infarction and sternal infection , especially in elderly and diabetic patients?
