DNA chip technology in healthcare industry: Ethical and social implications

DNA chip technology, in spite of the numerous benefits, has not been widely accessible. Increasingly, DNA chips have received much research attention. In particular, with recent miniaturization trend of LOC (logic on chip) and the wide publicity of genome map, their commercialization prospect for disease diagnosis and treatment is more promising than before. However, wide acceptance of DNA chips requires overcoming various ethical, privacy and security issues. This paper aims to present effective utilization paths of DNA chip technology in healthcare industry. We discuss the CNSS (Committee on National Security Systems) model and apply it to DNA chips in regard to privacy issues, and further examine ethical and social dimensions.

Healthcare Supply Chain Intelligence:Aligning Organizational Processes and Performance Outcomes

Purpose: In comparison to classic warfare that target for winning outcomes, healthcare operations, by nature, deal with decisions involving ultimate quality of human life. Healthcare information management, different from other industry contexts(e.g., manufacturing and services operations), requires high quality of intelligence which is characterized as deeply personal, urgent, cross-disciplinary expertise and collaborative decision making. However, very little is known about its scope and business process. This paper aims to define healthcare supply chain intelligence in open network age and examines ways to align organizational processes and performance outcomes. Design/methodology/approach: Using literature survey methods, critical factors of healthcare supply chain intelligence are defined and interrelationships between organizational process practices and performance outcomes are explained. Findings: The findings demonstrate need for research path in this area. The core supply chain intelligence factors are the lack of conceptual framework of healthcare supply chain intelligence that defines the scope, processes and performance measures. Accordingly, research in this area is in the early stage of development. Thus, there is serious call for a useful guideline for future research in the form of case study and empirical study. Research limitations/implications: The results from the qualitative review provides a stepping stone for defining and implementing research agenda. Further examination requires theoretical extension and field-based studies that provide managerial insight. Practical implications: The conceptual framework and the findings provide timely respective on healthcare supply chain intelligence for healthcare executives and managers and suggest practical strategic initiatives and managerial action plans. Originality/value: In this article, we argue for the constructive role of healthcare supply chain intelligence that is transparent but private, widely shared but safely guarded, and readily available but highly controlled. We present the key success factors of effective healthcare supply chain intelligence and its evolving patterns.

허혈뇌졸중 환자에서 콩팥기능 감소와 뇌경색 부피와의상관관계

Background: The decreased estimated glomerular filtration rate (eGFR) levels at admission in patients with stroke have been associated with more severe clinical deficits and with worse outcomes; however, the relationship between eGFR levels and volumetric measurement of cerebral infarct size on neuroimaging has not been studied, to our knowledge. Methods: Consecutive patients who presented within 48h of ischemic stroke and underwent brain diffusion-weighted imaging (DWI) over a 55-month period were studied. Patients with ischemic stroke of large artery atherosclerosis (LAA), small vessel occlusion (SVO), or cardioembolism (CE) etiologies were included. Those who were treated with thrombolysis and disabled before index stroke were excluded. Infarction volumes were log transformed to approximate normality. Chronic kidney disease (CKD) was defined as an eGFR < 60 mL/min per 1.73 m2. Subjects were categorized into two groups: CKD or no CKD. The relationship between CKD and DWI infarct volumes was examined using an analysis of covariance. Results: Of the included 405 patients (mean age, 68 years; female, 60.2%; LAA, 60.0%; SVO, 16.5%; CE, 23.5%), 108 patients had CKD. Infarct volumes (mL, median [interquartile range]) were not significantly different between stroke patients with CKD and those without CKD in any stroke subtype (LAA, 0.77 [0.01-2.97] vs. 0.96 [0.01-5.22]; SVO, 0.18 [0.02-0.21] vs. 0.27 [0.01-0.52]; CE, 8.91 [0.04-28.54] vs. 5.22 [0.05-42.39]). Adjustment for possible confounders did not change the associations. Conclusion: Renal impairment is not associated with cerebral infarct volume among acute ischemic stroke patients.

The motivation and aim of Tongguk Yeoji Sungnam is to make an efficient management in politics, economy and military by publishing a book which rages from the topography of our country to products, personages, natural scenary and customs. In the meantime, the reputed officials who were representative of the contemporary intellectuals took part in the edition of this book. Since it was published for the first time in the 12th year of King Sung-jong(A.D.1481), it had been newly edited several times and then it was published as Shinjung Tongguk Yeoji Sungnam in the 26th year of King Jung-jong(A.D.1531). This book was not only a unique geography but it also established a steady position as a source for general information.

A Case of Cutaneous Purpureocillium lilacinum Infection

Purpureocillium lilacinum (formerly Paecilomyces lilacinus) is a saprophytic fungus found in the soil and decaying vegetation and is rarely pathogenic to humans. To our knowledge, only six cases of cutaneous infection caused by P. lilacinum have been reported in journals published by the Korean Dermatological Association and the Korean Society for Medical Mycology. Here, we report the case of a patient with localized cutaneous infection caused by P. lilacinum. An 84-year-old woman presented with a 2-month history of multiple plaques with surrounding erythematous patches on her left forearm and dorsum of the hand. Histopathological examination showed suppurative inflammation accompanied by fungal elements in the dermis. Furthermore, periodic acid-Schiff and methenamine silver staining showed revealed fungal elements. The sub-cultured fungus of the isolate revealed velvety pink colonies that were yellowish-tan on the reverse side, and lactophenol cotton blue staining showed flask-shaped phialides. The DNA sequence from the colony was identical to that of P. lilacinum. The patient was treated with oral itraconazole (200 mg/d) for 6 weeks that achieved significant improvement in the patient's condition.

완전 이식형 인공중이를 위한 체외 및 체내 제어시스템 구현

An infrared remote control-type transcutaneous control device using a μ-processor is design for the totally implantable middle ear system. An infrared light transmission model for the tissue of skin was introduced and then a radiant intensity and the required current of the infrared light emitting diode(IR LED) driving circuit at transmission part were calculated for the external control device. And the transmission part generates IR signal by the system's own data protocol which prevents interferences from other infrared remote controls of the household appliances. The control part of the implanted device was designed to analyze functions of the received infrared(IR) signal that indicate the power ON/OFF and volume UP/DOWN. After the system is implemented, a data transmission experiments using 4 mm thickness of porcine skin were carried out. From the experiment, it was verified that the infrared control signal was transmitted to receiving module of the implemented system without any error. 체내에 이식되는 완전 이식형 인공중이 시스템의 제어를 위해 저전력 마이크로프로세서인 MSP430을 이용한 적외선 리모컨형 외부제어기를 설계하였고, 수신된 신호 해석을 위한 제어 알고리즘을 설계하여 전원 ON/OFF와 소리크기 UP/DOWM 동작을 수행하도록 하였다. 먼저 피부 투과시 발생하는 적외선 신호의 반사 및 투과모델을 이용하여 시스템에 필요한 적외선 LED 방사강도를 계산하고, 일반 적외선 리모컨과의 오작동을 방지하기 위해 고유의 데이터 프로토콜을 설정하였다. 이식되는 수신부의 적외선 수신모듈은 자체 소비전류를 줄이기 위해 마이크로프로세서의 내부 타이머에 의한 25% 듀티비를 갖는 출력신호를 전원으로 사용하였다. 4㎜ 두께의 돼지피부를 이용한 피부투과 전송실험을 실시하여 구현된 송수신 장치로 신호가 전달됨을 확인하였다.

신증동국여지승람의 항목체재와 '제영'에 대한 고찰

신증동국여지승람의 항목체재는 모두 26개로 구성되어 있으며 그 중심 주제는 행정 군사 지리이다. 이는 본서가 범상한 지리서라기보다는 국가의 행정지침서나 군사요무를 위한 편람으로서의 역할을 담당한 포괄적인 정보자료였음을 시사해 주는 것이다. 특히 본서는 여타 지리지가 구비치 못한 '제영' 항목을 설정하여 인문적인 면을 보강하므로서 지리지의 격조를 한층 드높인 자료로 평가된다. 제영의 詩文은 충청 경상 전라도에 많이 등재되어 있으며 이 삼남지방에 게재된 것이 전체의 과반수를 넘고 있다. 이들 시문의 영탄대상은 산 강 성 릉묘 등이며 주로 자연지리적 현상이나 기념비적인 사적들이다. The content systems of Shinjung Tongguk Yoji Sungnam(신증동국여지승람, A Korean Geography Published in Chosen Dynasty) consist of 26 items and the main subject comprises administration, military and geography, This book does not suggest a simple geography but a comprehensive bibliography as an administrative guide book of a nation or a military operation manual. In particular, this book is made much of as a more valuable material in elevating the quality of the geographic book since it contained a 'Cheyong(제영)' item and reinforced the fields of humanities which could not be found out in other geography. The verses of Cheyong are recorded in the documents in the Chungchong, Kyungsang and Chulla Provinces, and these recorded verses In the tree provinces are more than half of the whole country's. The objects of recitation in the these verses are related to such natural phenomena and historic relics as the mountains, the livers, the wails and the royal tombs.
