Dieulafoy 유사 병변의 임상양상 및 내시경적 고무밴드 결찰술의 효과

Background/Aims: The Dieulafoy lesion is an important cause of gastrointestinal bleeding. The bleedings from caliber- persistent vessel or abnormally large and tortuous submucosal artery are usually located on proximal stomach. Endoscopic band ligation (EBL) is currently regarded as the treatment of choice of Dieulafoy lesions. This study reports on the clinical features, the efficacy and safety of EBL. Methods: 10 patients with Dieulafoy-like lesion (median age: 47 yr range: 22∼71, M:F 8:2 ) were treated using EBL. EBL was performed as the primary hemostatic tratment as follows in 8 cases of urgency and 2 cases of emergency. Results: The location of all lesions (n=10) were in stomach, 8 in body (4 in posterior wall), 2 in fundus. The bleeding focuses were identified during the first endoscopy. Five lesions were bleeding, 5 had adherent clots or protruding vessels without active bleeding. Hemostasis were achieved with first session of EBL in 9 patients successfully, one case in the fundus was failed due to early band release. Conclusion: EBL is a simple, effective, and safe endoscopic method of Dieulafoy-like lesions and, it should be proposed as a primary option. (Korean J Gastrointest Endosc 2002;24:267-272) 목적: Dieulafoy 병변은 상부소화관 대량 출혈의 한 원인이다. 소동맥의 직경이 가늘어지지 않으며 비정상적으로 크고 구불구불하게 점막하까지 도달하여 혈관노출로 인한 대량 출혈을 유발하는 것으로, 위식도 문합부에서 6~8 cm의 원위부 내에 주로 분포한다. 최근 치료 내시경의 발달로 90% 이상 내시경 지혈술로 지혈이 가능해졌다. 특히 Dieulafoy 유사병변에서 hemoclip과 함께 고무밴드 결찰술이 효과적인 것으로 알려져 있다. 이에 본원에서 경험한 내시경적 진단의 Dieulfoy 유사병변에 대한 임상적인 특성과 1차적인 치료법으로서의 고무밴드 결찰술의 유용성에 대하여 알아보았다. 대상 및 방법: 1997년 1월부터 2001년 7월까지 Dieulafoy 유사병변으로 진단되어 일차적인 지혈 치료술로 고무밴드 결찰술을 시행한 10명을 대상으로 하였다. 결과: 병변의 위치는 모두 위 내였으며 각각 체부 8명(후벽에 4명), 기저부 2명이었고, 첫 상부내시경검사에서 출혈 병소를 확인하였으며, 발견 당시 출혈 중인 경우가 5명, 혈관 노출과 혈괴가 부착된 경우가 5명이었고, 일차적인 고무밴드 결찰술로 성공한 경우가 9명이고, 기저부 출혈 1명에서 조기밴드 이완으로 재출혈이 있어 경화요법과 hemoclip 복합요법으로 치료하였다. 결론: Dieulafoy 유사병변의 일차적인 치료법으로 고무밴드 결찰술이 90%의 성공률을 보였다. 고무밴드 결찰술은 간단하고 높은 성공률을 보이는 안전한 지혈 치료법으로 Dieulafoy 유사병변의 일차적인 치료방법으로 보고한다.

Helicobacter pylori 양성인 소화성 궤양 환자에서 Pantoprazole의 궤양 치유효과 및 Pantoprazole 포함 삼제요법의 제균효과

목적 : H. pylori는 소화성 궤양의 중요한 원인인자로서 H. pylori의 박멸은 소화성 궤양의 치료에 있어서 가장 중요한 요소로서 추천을 하고 있다. H. pylori 양성인 소화성 궤양 환자를 대상으로 pantoprazole의 소화성 궤양에 대한 치유 효과와, pantoprazole, amoxicillin, clarithromycin 삼제요법의 H. pylori에 대한 제균효과 및 이에 영향을 주는 인자를 알아보고자 하였다. 대상 및 방법 : H. pylori 양성인 소화성 궤양 환자 24예를 대상으로 pantoprazole 하루 40 ㎎, 투여 및 pantoprazole 40 ㎎, amoxicillin 1000 ㎎, clarithromycin 500 ㎎ 하루 2회 1주간 투여 후, 궤양의 치유 및 H. pylori 제균율을 전향적으로 분석하였다. 결과 : 1. 소화성 궤양의 치유율은 연구를 완료한 활동성 궤양 환자 17예에서 5주 치료 후에 76.5%이었으며, 8주 치료 후에 94.1%이었다. 2. H. pylori 제균율은 ITT분석에서는 66.67%이었고, PP 분석에서는 76.19%이었다. 궤양의 위치에 따른 제균율은 십이지장 또는 십이지장 및 위궤양군에서 83.33% (ITT), 100% (PP)로 위궤양군의 50% (ITT), 54.55% (PP)보다 우수하였다(각각 p=0.0164(ITT), p=0.001(PP)). 흡연, 삼제요법·선행, 궤양의 재발, 출혈 동반 여부에 따른 제균율에는 유의한 차이가 없었다. 3. pantoprazole 단독 투여 중에는 부작용이 없었고 삼제용법 중에 약 16.7%에서 경한 일과성 부작용을 호소하였으나 이로인하여 약물투여를 중단한 예는 없었다. 결론 : Pantoprazole은 소화성 궤양의 치료에 있어서 우수한 효과를 나타내었다. pantoprazole, amoxicillin, clarithromycin 삼제용법의 H. pylori 제균율은 76.19%이었으며, 위궤양에 비하여 십이지장 궤양에서 제균효과가 매우 우수하였다. 제균효과에 영향을 주는 다른 변수는 관찰할 수 없었다. Background : proton pump inhibitor-based triple therapies are effective for Helicobacter pylori(H. pylori) eradication. We studied the ulcer healing effect of pantoprazole and the effect of pantoprazole-based triple therapy according to clinical variables in eradicating H. pylori. Methods : 24 patients with H. pylori-positive peptic ulcer received pantoprazole 40 ㎎, amoxicillin 1000 ㎎, and clarithromycin 500 ㎎, twice daily for a week and took pantoprazole 40㎎/day till complete healing of ulcer confimed by gastroduodenoscopy. The status of H.pylori infection was assessed at least 4 weeks after completion of medication by rapid urease test, histology and ^(13)C urea breath test. Result : The ulcer healing rates were 76.5% at 5th week and 94.1% at 8th week. H. pylori eradication rates were 66.67% on intention to treat (ITT) analysis and 76.19% on per protocol (PP) analysis. The H. pylori eradication rates of pantoprazole-based triple therapy were 83.33% on ITT and 100% on PP analysis in patients with duodenal or duodenal/gastric ulcers, and 50% on ITT and 54.55% on PP in those with gastric ulcers (p=0.0164 (ITT), p=0.001 (PP)). However, there was no significant difference in eradication rates according to smoking, protocol, recurrence of ulcer, and hemorrhage. There was no serious side effect despite mild adverse events in 16.7% of the patients during triple therapy. Conclusions : Pantoprazole was effective for healing of peptic ulcer. Pantoprazole-based triple therapy resulted in 76.2% of H. pylori-eradication rate, and was more effective for duodenal ulcers than gastric ulcers. There was no other clinical factor affecting eradication rate.

Gastric antral vascular ectasia (GAVE) is a rare but important cause of chronic gastrointestinal bleeding. Endoscopically, it has characteristic thickened red vascular folds radiating from the pylorus to the antrum. Diagnosis is made primarily by endoscopy. Histologic examination of the endoscopic mucosal biopsies may confirm the endoscopic diagnosis. Many treatment modalities of the gastric antral vascular ectasia exist. One of them, the argon plasma coagulation (APC) is an excellent therapeutic tool. Inactive argon gas is converted to ionized form by means of electrical energy. Ionized argon plasma conducts high frequency electrical energy to tissues and leads coagulation necrosis of tissues. We experienced a case of gastric antral vascular ectasia presenting melena for about one month in a 72-year-old man treated endoscopically in four sessions with argon plasma coagulation.
상부위장관질환의 조직염증 정도와 Helicobacter pylori 감염에 따른 혈청 가스트린치의 변화

Background/Aims : It is well known that serum gastrin titers in patients infected with Helicobacter pylori (H. pylori) are much higher than those in uninfected asymptomatic controls. The aim of this study was to investigate the relationship between serum gastrin titers and histologic scorings according to disease entities in Koreans. Methods: Ninty-nine patients infected with H. pylori and 35 uninfected patients were enrolled in this study and their serum gastrin titers were measured by 125I radioim- munoassay. The H. pylori infection was determined by rapid urease test and histology. Results: No significant differences in serum gastrin titers were noted between the 99 patients infected with H. pylori and the 35 uninfected patients. Serum gastrin titers in the all patients infected with H. pylori were significantly higher in the fundus of high chronic lymphocyte infiltration score than in the fundus of low chronic lymphocyte infiltration score (55.1±45.8 pg/mL vs 22.9±15.0 pg/mL, p<0.05). In addition, serum gastrin titers in the uninfected patients were significantly higher in the fundus of high chronic lymphocyte infiltration score than in the fundus of low chronic lymphocyte infiltration score. Conclusions: Serum gastrin titers were significantly high in high histologic score group. However, they were not significantly increased by H. pylori infection or gastroduodenal disease entities. Therefore, there is no direct correlation between H. pylori infection and hypergastrinemia. (Kor J Gastroenterol 2000;36:326 - 335)
