        Rapid progression in ultrasound techniques resulted in a wide clinical applications for assessment of cervical vascular system. Duplex scanning of the carotid arteries combines B-mode ultrasound and Doppler ultrasound within a single instrument, providing anatomical imaging of the vessel and flow velocity information. The clinical applications, practical techniques, and basic principles of Doppler scanning in cervical vascular system are discussed.

        Purpose: This study examines deficits in upper-extremity function and trunk control ability on the osmotic demyelination syndrome of a patient with Hyponatremia and Hypokalemia. Using a proprioceptive neuromuscular facilitation program, this case report aims to describe the effects of trunk stability exercises on trunk control ability, hand function, and daily living activities as well as its effects on the osmotic demyelination syndrome of a patient with Hyponatremia and Hypokalemia. Methods: The patient is a 47-year-old woman with osmotic demyelination syndrome as well as trunk and upper extremity impairment. She participated in this training intervention for four weeks. Results: The patient demonstrated improvements in trunk control ability, hand function, and performance of ADL. The following outcomes were measured before and after the training program: trunk impairment scale, hand power, Jebsen-Taylor hand function test, the Chedoke arm and hand inventory, and the modified Barthel index. Conclusion: The results suggest that trunk stability exercises in the form of a proprioceptive neuromuscular facilitation program in Hyponatremia and Hypokalemia patients may increase trunk control ability, increase hand function, and improve ADL.

        PNF을 이용한 엉덩관절 강화운동이 피부 결손을 동반한 개방성 정강뼈 골절 환자의 균형과 앉았다 일어서기, 보행에 미치는 영향 -단일사례연구

        Purpose: Patients with tibial fractures can have functional problems with balance and gait, as well as lower extremity muscle weakness. This case report aimed to describe the effect of hip joint strengthening exercises using proprioceptive neuromuscular facilitation (PNF) on balance and gait and lower limb function in a patient with tibia fracture. Methods: One patient diagnosed with tibial fracture was treated for seven weeks with the basic procedure, pattern, and technique of PNF for a hip joint strengthening exercise. Results: The results of pre- and post-intervention treatment showed improvements in physical function and structure in the clinical tests, including the manual strength test; the modified Ashworth scale; sensory evaluation; balance, sit-to-stand, and gait performance; and evaluation of lower limb function. Conclusion: Based on the results of this study, it is suggested that the use of theory-based proprioceptive neuromuscular stimulation for hip joint strengthening exercises positively affects patients' functional improvement in tibial fracture patients, and this may be used as a therapeutic exercise method for those with orthopedic problems in the lower extremities. One limitation of this study was that it was performed on only one tibia fracture patient, which makes it difficult to extend the treatment effects to all patients with this condition.

        The effect of hip joint strengthening exercise using proprioceptive neuromuscular facilitation on balance, sit to stand and walking ability in a person with traumatic brain injury: a case report

        Objective: The purpose of this study was to investigate the effect of the hip joint strengthening exercises using proprioceptive neuromuscular facilitation (PNF) on the clinical symptoms and the treatment effects in balance, sit to stand, and gait abilities in patients with TBI. Design: A single case study. Methods: A 13-year-old adolescent with quadriplegia and hip joint control impairment participated in this four-week training intervention. The patient, diagnosed with TBI, wastreated with hip joint strengthening exercises using PNF. In the first week, we focused on strengthening the body, relaxing the hip flexors and activating the hip extensor muscles in order to solve the patient’s physical function and body structure. From the 2nd and 4th week, we improved the motivation through the task-oriented method, and then weight-bearing training of the right lower extremity was proceeded by kicking a soccor ball with the left lower extremity. The exercises were performed for 4 weeks, 5 days a week, for 60 minutes with the exercise intensity gradually increased according to the subject’s physical abilities. Results: As a result of the study, the patient demonstrated improvements in the physical examination, which were evaluated before and after intervention and included the manual muscle test, modified Ashworth scale, sensory assessment, coordination assessment, Berg balance scale, 5-time sit to stand test, and the 10 meters walk test. Conclusions: The results of this case suggest that a hip joint strengthening exercise program using PNF may improve hip control ability, balance, sit to stand and gait ability in a patient with TBI.

        Purpose: Deficits in upper-extremity function in individuals with tetraplegia are primarily due to the loss of motor pathways. The purpose of this case report is to describe the effect of closed kinetic chain exercise using PNF on hand function and ADL of patient with incomplete spinal cord injure. Methods: Patient was a 53-year-old man with C4 tetraplegia and hand and upper extremity impairment who participated in this training intervention for 10 weeks. Results: Patient demonstrated improvements in upper extremity strength, hand function and performance of ADL. Outcome measures(ASIA motor scale, Hand power, Jebsen-Taylor Hand Function test, SCIM) were measured before and after the training program. Conclusion: The results of this case suggests that an closed kinetic chain exercise program in incomplete spinal cord injury patient may induce Hand function and ADL.

        Purpose: Deficits in lower-extremity function and trunk control ability have a negative impact on individuals with hemiplegia. This case report aimed to describe the effect of trunk stability exercises using proprioceptive neuromuscular facilitation (PNF) on trunk control ability, balance, and gait in a patient with hemiplegia. Methods: A 77-year-old man with hemiplegia and trunk and lower extremity impairment participated in this four-week training intervention. Results: The patient demonstrated improvements in trunk control ability, balance, and gait performance. Outcome measures (Fugl-Meyer Assessment Lower Extremity (FMA-LE), Trunk Control Test, Berg Balance Scale, Timed Up and Go test, 10 Meter Walk test) were measured before and after the training program. Conclusion: The results of this case suggest that a trunk stability exercise using a PNF program may improve trunk control ability, balance, and gait in a patient with hemiplegia. .

        Purpose: Peripheral neuropathy accompanied by sensory disturbance, such as limb paralysis and hemiplegia, is mainly caused by acute disseminated encephalomyelitis (ADEM). This case study aimed to determine the effect of ankle strengthening exercises that use proprioceptive neuromuscular facilitation (PNF) on the gait, balance, ankle-control ability, and sit-to-stand ability on a patient with ADEM. Methods: A 10-year-old male with quadriplegia and ankle-control impairment participated in this 4-week training intervention. The patient, diagnosed with ADEM, was treated with ankle strengthening exercises that used PNF. Results: The patient demonstrated improvements in balance, ankle-control ability, sit-to-stand ability, and gait performance. Outcome measures (manual muscle test, modified Ashworth scale, sensory assessment, coordination assessment, Berg balance scale, 5 time sit-to-stand test, and 10 m walk test) were taken before and after the training program. Conclusion: The results of this case suggest that an ankle strengthening exercise that uses PNF can improve the gait, balance, ankle-control ability, and sit-to-stand ability in patients with ADEM. In ADEM, the initial treatment is important, and the use of ankle strengthening exercises with PNF could lead to meaningful results. However, there is limited research due to an insufficient number of cases. In the future, more patients will need to be studied.

        Department of Neurology, KyungPook National University Hospital,KyungPook National University School of MedicineOral anticoagulants (OAC)OAC is an antagonist of vitamin K, which is a cofactor for a microsomal enzyme that activates coagulation factors II, VII, IX, and X by carboxylation of their glutamic residues. Administration of OAC in therapeutic doses decreases the synthesis of these coagulation factors in the liver by approximately 40 percent, resulting in hypocoagulability of the blood and reducing the propensity towards thrombus formation. As a result, thrombin generation slows, and clot formation becomesimpaired due to decreased biologic activity of the prothrombin complex proteins. The dose of OAC should be closely monitored because anticoagulant response differs from patient to patient. The dose-response is influenced by both pharmacokinetic factors (differences in absorption) and pharmacodynamic factors (differences in the hemostatic response to several concentrations). The effect of vitamin K antagonists is gradual and reversible. It takes several days before an adequate level of anticoagulation is achieved, and for the anticoagulant effect to disappear after drug discontinuation.The most common agents associated with enhanced anticoagulant effect are allopurinol, common analgesics, antiarrhythmics, antidepressants, antidiabetics, antiplatelets, anxiolytics, disulfiram, levothyroxine, lipid regulatingagents, testosterone, and alcohol. Oral contraceptives, retinoids, and vitamin K have the opposite, reducing effect on anticoagulation. The most common contraindications for OAC include evidence of any active bleeding, uncontrolled severe hypertension, recent brain, eye or spinal cord surgery or injury, propensity for recurrent falling, inability for INR monitoring, and patient non-compliance.
