        Molsidomine, Nicorandil, Trimetazidine의 안전성 관련 체계적 고찰

        Background: Ischemic heart disease is the most common type of heart disease and an important cause of death in Korea. Among marketed anti-anginal medications, molsidomine, nicorandil, and trimetazidine are approved in Korea with unique mechanism of actions. As these drugs are not approved by the US Food and Drug Administration, the access to the up-to-dated and comprehensive safety-related information has been less than optimal from drug information resources used by Korean pharmacists. Methods: A systematic review was conducted using Embase and Korean manuscripts to compile safety updates for these medications. Out of 418 articles from keyword searches, 52 studies were reviewed in full to compare adverse effects (AEs) with the approved package inserts (PI). Results: Molsidomine related adverse effects were mostly mild or moderate, but anxiety, palpitation, epigastric pain, and sexual potency reduction were additional AEs found from the review not listed in PI. Although PI has included ulceration in oral cavity and gastrointestinal tracts including anus by nicorandil, the Korea FDA recently recommended adding corneal, genital, and skin ulcers to the approved PI. Trimetazidine induced Parkinsonism, worsening of the symptoms for patients diagnosed with Parkinson's disease, gastrointestinal burning, and muscle cramps were additionally identified AEs not listed in PI for trimetazidine. Conclusion: Continuous evaluations of the safety profile of these agents are needed to balance the risks and benefits to provide evidence-based safety counseling to the patients. In addition, more focused efforts on spontaneous reporting are warranted by healthcare professionals to safeguard patients against AEs.

        Fabrication of carbon nanofiber electrodes using poly(acrylonitrile‑co‑vinylimidazole) and their energy storage performance

        For electrodes in electrochemical double-layer capacitors, carbon nanofibers (CNFs) were prepared by thermal treatment of precursor polymer nanofibers, fabricated by electrospinning. Poly(acrylonitrile-co-vinylimidazole) (PAV) was employed as a precursor polymer of carbon nanofibers due to the effective cyclization of PAV polymer chains during thermal treatment compared to a typical precursor, polyacrylonitrile (PAN). PAV solutions with different comonomer compositions were prepared and electrospun to produce precursor nanofibers. Surface images obtained from scanning electron microscopy showed that their nanofibrous structure was well preserved after carbonization. It was also confirmed that electrospun PAV nanofibers were successfully converted to carbon nanofibers after the carbonization step by Raman spectroscopy. Carbon nanofiber electrodes derived from PAV showed higher specific capacitances and energy/power densities than those from PAN, which was tested by coin-type cells. It was also shown that PAV with an acrylonitrile/vinylimidazole composition of 83:17 is most promising for the carbon nanofiber precursor exhibiting a specific capacitance of 114 F/g. Their energy and power density are 70.1 Wh/kg at 1 A/g and 9.5 W/kg at 6 A/g, respectively. In addition, pouch cells were assembled to load the higher amount of electrode materials in the cells, and a box-like cyclic voltammetry was obtained with high capacitances.

        Metformin에 추가로 병용되는 SGLT2 inhibitors의 효능과 안전성에 관한 고찰

        Background: The new type of diabetes treatment, SGLT2 inhibitors, has been approved for monotherapy and combination therapy, but medical insurance is only allowed in combination therapy with metformin, which is the first choice for type 2 diabetes treatment. Methods: The SGLT2 inhibitors prescribed in Korea are dapagliflozin, empagliflozin and ipragliflozin. A review was conducted using Pubmed to evaluate efficacy and safety for these medications with metformin combination therapy. 10 studies were selected by searching for keywords and related references and were reviewed in full. The mechanism of action, pharmacokinetics, and the economics of treatment with SGLT2 inhibitors were examined. Results: SGLT2 inhibitors had moderate glycemic control when added to the treatment of patients with type 2 diabetes who were not being regulated by metformin monotherapy. They also showed positive effects such as weight loss, as well as the lowering of blood pressure. Hypotension and serious side effects were relatively low. However, the risk of genital infection was increased. Conclusion: The SGLT2 inhibitors are a new class of drugs that promote glucose excretion in the urine. They are a good choice for combination therapy with metformin for the treatment of type 2 diabetes, with weight loss and very low risk of serious side effects.

        Clinical Implications of Using Post-Challenge Plasma Glucose Levels for Early Diagnosis of Type 2 Diabetes Mellitus in Older Individuals

        Background: The aim of this study was to explore the differences in the clinical characteristics and diagnostic rates of diabetes mellitus (DM) according to various criteria in different age groups and to evaluate the efficacy of each criterion for screening older patients. Methods: We studied 515 patients and measured the fasting plasma glucose level (FPG), 2-hour plasma glucose level after the 75 g oral glucose tolerance test (2-hour postload glucose [2-h PG]), and glycosylated hemoglobin (HbA1c) for re-evaluation of hyperglycemia without a history of diabetes. Patients with newly diagnosed DM were grouped by age as younger (<65 years) or older (≥65 years). Results: Older patients had significantly lower HbA1c, FPG, and 2-h PG levels and a higher homeostatic level of pancreatic β-cell function compared with younger patients (P<0.001). The older group had the lowest diagnostic rate when using the FPG level (45.5%) and the highest diagnostic rate when using the 2-h PG level (84.6%). These results were mostly due to the higher frequency of isolated post-challenge hyperglycemia in the older patients than in the younger group (28.8% vs. 9.2%). The use of both the FPG and HbA1c levels significantly enhanced the low diagnostic power when employing only the FPG levels in the older group (71.2% vs. 45.5%). Conclusion: In the older patients, the 2-h PG level was the most accurate diagnostic criterion. When we consider the costs and convenience, a combination of the FPG and HbA1c criteria may be recommended as a screening test for DM in older people.

        고주파시술과 운동병행요법이 20대 복부비만에 미치는 영향

        It is purposed to understand the influence of high-frequency treatment on the abdominal obesity program of 20s women and men who suffer obesity and to provide guidelines for the quantitative use of the equipment and for the balanced physical and abdominal obesity management programs. In the change of body composition before and after the management, in the waist girth and hips girth, according to the each experimental group, the HG(high-frequency and applied Gel)group reduced, in the WHR, HH(high-frequency and applied Hot gel) group received, in the weight, BMI, body fat mass and percent bady mass, HW(high-frequency and Walking) group HF diathermy and applied Gel group reduced most. Except in the skeletal fat mass, HG and HH group and HW group reduced meaningfully. Regarding the change of blood lipid concentration before and after the management according to the each experimental group, almost group reduced in order of appearance, as well as in TG, LDLC, TC and incresed in HDLC. Meaningfully in the TG and TC, HW group reduced more and in HDLC, LDLC HH group effected most. 본 연구는 고주파시술과 핫젤, 걷기운동을 각각 병행했을 때 신체조성 및 혈청지질에 미치는 영향을 검증하고자 20대 학생 및 직장인 100명 중 BMI가 23kg/㎡, WHR가 0.8 이상인 비만대상자를 24명 선정하여 실험을 진행하였다. 실험은 4주 동안에 주 2회씩 총 8회, 매 회에 40분간 관리를 시행 하였고 고주파젤군, 고주파핫젤군, 고주파운동군 각각 8명으로 나누어 실험을 하였다. 실험 전‧후 BMI, 체중, 체지방량, 체지방률, WHR, 허리둘레, 엉덩이둘레와 TG, HDLC, LDLC, TC를 측정하였다. 실험 결과 고주파젤군에서 허리둘레는 3.1cm, 엉덩이둘레는 1.7cm로 사이즈 감소 차이가 크게 나타냈고, 고주파핫젤군에서WHR은 0.018%로 큰 차이를 보였으며 고주파운동군에서 체중은 2.01kg, BMI는 0.81kg/m2, 체지방량은 2.12kg, 체지방률은 2.01%로 감소의 차이가 가장 크게 나타냈다. 고주파운동군에서 TG는 39.13mg/dl, TC는 37.58mg/dl로 크게 감소 되었고, 고주파핫젤군에서 HDLC는 6.82mg/dl 증가하였으며, LDLC는 25.5mg/dl 감소하였다. 본 연구를 통하여 고주파시술 병행요법이 20대 복부둘레 감소와 체중감량에 효과적임을 입증하였다.

      • 에너지 저장 장치용 탄소 나노 섬유의 새로운 전구체 고분자

        슈퍼커패시터는 에너지 저장 장치의 하나로 높은 파워 밀도와 짧은 충전 시간, 긴 수명을 가지지만, 에너지 밀도가 낮다는 단점이 있다. 탄소 나노 섬유를 슈퍼커패시터의 전극으로 사용하여 에너지 밀도를 높일 수 있다. 탄소 나노 섬유는 주로 전구체 고분자를 전기 방사하여 제작한 나노 섬유를 탄소화라는 열처리를 통해 만든다. 본 연구는 기존의 전형적인 전구체 고분자인 polyacrylonitrile (PAN)를 대체할 수 있는 전구체로 poly(acrylonitrile-co-vinylimidazole)을 소개한다. 이 고분자는 성공적으로 전기방사가 가능하고 1000℃의 탄소화 단계 후에도 나노 섬유 구조를 유지하였다. 또한 기존의 PAN으로 제작한 전극에 비해 높은 전기용량, 에너지 밀도, 파워 밀도를 보였다. 그리고 acrylonitrile과 vinylimidazole의 비율을 다양화한 결과 75:25에서 가장 훌륭한 에너지 저장 특성을 보였다. Supercapacitors fill a unique niche in terms of energy storage because they have high power density and long cyclability but low energy density. Carbon nanofibers are good candidate for supercapacitor electrode materials to improve their energy density. Typically, carbon nanofiber electrodes are prepared by thermal treatments of precursor polymer nanofibers, fabricated by electro spinning. This study introduces poly(acrylonitrile-co-vinylimidazole) as a noble precursor polymer of carbon nanofiber electrode. The polymer solutions were successfully electrospun, and their nanofibrous structure was well preserved after the carbonization step at 1000°C. The carbon nanofibers derived from poly(acrylonitrile-co-vinylimidazole) showed superior specific capacitances, and energy and power densities compared to those from polyacrylonitrile. It is also shown that the copolymer with a feed rate of 75:25 (acrylonitrile:vinylimidazole) is most promising for the carbon nanofiber precursor exhibiting high energy storage performance.

        클로로프로파미드와 싸이클로덱스트린과의 포접 화합물에 관한 연구

        Inclusion complexation of alpha-cyclodextrin(alpha-CD), beta-cyclodextrin(beta-CD) and tri-0-methyl-beta-cyclodextrin(tri-0-methyl-beta-CD) with chlorpamide(CPA) in aqueous solution and in the solid state were studied by the solubility method, spectroscopy (UV, IR), differential scanning calorimetry (DSC) and powder X-ray diffractometry, and all their molar ratios were found to be 1:1. The solid complexes of CPA with three kinds of cyclodextrins were prepared by a freeze-drying method, and their dissolution behaviors were examined. As a result, the release of CPA from the inclusion complexes was significantly improved. The intrinsic dissolution rate of CPA in cyclodextrin inclusion complexes was about 51 times (alpha-CD inclusion complex) and 12 times (beta-CD inclusion complex) larger than that of intact CPA.
