The Windblown Hand의 치험례 -2례보고-

This syndrome is very rare and firstly reported by Freeman and sheldon in 1938. Features of this syndrome include a characteristic appearance with multiple skeletal anomalies, probably due to abnormal muscle tone. The diagnosis is based on the characteristic appearance with prominent supraorbital ridge, sunken eyes, telecanthus, hypoplastic alae nasi, high narrow palate, long philtrum and marked microstomia with puckered lips. The hand showed symmetrically clenched fingers and flexion contracture of the MP joint with ulnar deviation and palm-cluthed thumb without bony abnormality. The feet demonstrated mild bilateral talipes equinovarus. We reported one case of the windblown hand who were admitted after failure of correction by spliting which were corrected surgically, another case to be treated by only splinting.

Spontaneous rupture of tendon has been described in association with colles' fracture, rheumatoid arthritis, infection, occupational trauma and tumors. We experienced three cases of spontaneous rupture of tendon, two cases of rupture of the extensor pollicis longus tendon were developed after Cellos' fracture and wrist sprain. Another one case was rupture of the flexor digitorum profundus tendon of the little finger caused by occupational overuse. These patients sustained abrupt onset of lack of extension or flexion with dull pain. Authors treated spontaneous rupture of the extensor pollicis longus tendon by tendon transfer using the extensor indicis proprius tendon and rupture of the flexor digitorum profundus by direct repair. We achieved satisfactory results.

Glomus tumor is rare benign tumor of the normal glomus, which is a neuroarterial canalsystem called the Sucquet-Hoyer canal. The function of the glomus appears to be to regulate local blood pressure and local heat by blood flow. The etiology of its development is unknown, but some patients have history of trauma. Glomus tumor has been described in the trunk as well as in upper and lower extremits hand (the most common location). The triad of pain, tenderness and sensitivity to cold is characteristics of glomus tumor of the hand. Treatment consists of surgical excision of the lesion under the tourniquet control and magnification for complete removal of the lesion. Glomangiosarcoma or malignant form of glomus tumor is exceptionally rare. Only a few cases have been reported. We experienced five cases of glomus tumor and a cases of glomangiosarcoma treated by complete removal of tumors through partial elevatioin of nail. Typical pain of the lesion was disappeared after operation, also nail deformity did not occurred.

Syndactyly is one of the most common malformations occurring in the hand. the child born with a syndactylized hand may be handicapped for life if successful surgery is not carried out. Syndactyly is frequently associated with other anomalies, of which congenital constriction ring, heart anomalies, orodigital anomalies, and hemangioma. The aim of surgery is to construct a web or commissure and cover any defects on the sides of the fingers. Unsatisfactory results may be due to subsequent web contractures of the divided fingers. Fifty-three cases of syndactyly in twenty-nine patients were corrected by the authors' method. Authors used a large dorsal rectangular flap with a somewhat wider proximal base than distal end, and small volar triangular flap with lateral relaxing incisions for interdigitation of flaps and reconstruction of web space, and performed zig-zag incision for division of syndactylyzed fingers.

In this study, the effect of novel BMP-2-loaded photo-cured hydrogels on in vitro and in vivo osteogenesis were explored. The well-defined HA-heparin hydrogel was prepared by a photopolymerization of aminoethyl methacrylate (AEMA) grafted hyaluronic acid (HA) and AEMA-grafted heparin. Subsequently, the resulting hydrogels contained BMP-2 at different concentrations. First, a series of BMP-2-loaded hydrogels were characterized to determine their mechanical properties and release profiles. The results from rheology, swelling ratio, and release kinetics suggest that BMP-2 contained HA-g-Hhep gel (B/HA-g-Hhep gel) had sustained release during 4 weeks with minimal initial burst. Furthermore, the released BMP-2 maintained its biological activity. MTT and live/dead assays showed that the B/HA-g-Hhep gel had good biocompatibility to be used as a scaffold for bone regeneration. In vivo studies of B/HA-g-Hhep gel exhibited significantly enhanced osteogenesis, compared with other types of HA-heparin hydrogels. These excellent results indicated that B/HA-g-Hhep gel has a great potential for a controlled release of growth factor, leading to a good bone regeneration.
