        슬관절 전치환술에서 척추경막외 병용마취

        Spinal anesthesia has a rapid onset and requires small doses of local anesthe?? to provide reliable surgical anesthesia and good muscular reiaxation but the disadvantages are the unpredictability of upper level of block, precipitous hypotension, inability to extend the block, and the risk of postdural puncture headache. A combined spinal-epidural (CSE) technique can be used to reduce or eliminate some of the disadvantages of spinal and epidural anesthesia while preserving their activity. A combined spinalepidural block may combine the reliability of spinal block and the flexibility of epidural block while minimizing their drawbacks. CSE anesthesia was performed in the 19 patients scheduled for elective total knee arthroplasty. At first 17 G Tuohy needle was inserted L2-3 interspace, the epidual space would be identified, and then a long 22 G spinal needle was introduced through the Tuohy needle until the tip of the spinal needle would penetrate the dura. The correct placement of the spinal needle was confirmed by the appearence of cerebrospinal fluid at the head of needle, then 0.5% hyperbaric tetracaine 1.6 - 2 ml was injected into subarachnoid space. The spinal needle was withdrawn and a 18 G epidural catheter was introduced into the epidural space. If the patients complained pain during operation, 5 ml of 2% lidocaine was injected through epidural catheter, 2.5 mg Morphine was injected into epidural space for postoperative pain control after operation. Operations were well performed under CSE anesthesia and postoperative pain controls were well managed, too. The responses of the patients who has experienced CSE anesthesia were mostly good.         외경정맥의 정맥류 1예

        A case of lateral neck mass caused by thrombosis of the right external jugular vein is presented. Macroscopic examination of the excised thrombosed vein showed a pin-point opening of the jugular vein connecting to the hemorrhagic mass and microscopic examination revealed the vein to have an organized thrombus with somewhat degenerated vessel walls. Venipuncture would seem the most likely cause of thrombosis of the venous aneurysm in this 6 year old female patient.

        흰쥐에서 척수강내 NMDA 와 non-NMDA 수용체 길항제 투여가 열손상에 의해 발생한 통각과민에 미치는 영향

        Background: Previous reports have described NMDA antagonist reduced the nerve injury induced or inflammatory thermal hyperalgesia. This study evaluated the effects of spinally administered excitatory amino acid antagonists on the thermal hyperalgesia state induced by mild burn. Methods : The measured response was the latency to paw withdrawal of each hindpaw after application of a focused heat lamp on the plantar surface of the paw through a glass plate upon which the animal stood. In this work, MK801, non-competitive NMDA receptor antagonist, AP5, competitive NMDA receptor antagonist, CNQX, non-NMDA receptor antagonist were injected through chronically implanted lumbar intrathecal catheters in rats with mild burn injury on the right hindpaw. Results : In the normal left hindpaw, MK801, AP5 and CNQX had little effect upon paw withdrawal latency(PWL) at intrathecal doses which do not produce readily detectable motor weakness. In the right hyperalgesic hindpaw, AP5 significantly reduced PWL at a dose-dependent fashion, MK801 reduced PWL to some extent, and CNQX did not reduced PWL Conclusions : These results suggested that spinal NMDA receptors play an important role in the hyperalgesia induced by mild burn injury.         유도 저혈압 마취에서 Sodium nitroprusside 의 지속 주입 용량에 관한 연구

        Sodium nitroprusside(SNP) is used to induce hypotension for a wide variety of indica- tions. Ordinarily, blood pressure responds sensitively to infusion of SNP in low doses, but occasioally resistance is seen, and actual tachyphylaxis during SNP infusion has been reported. To investigste the continuous infusion rates of SNP, we retrospectively reviewed 144 cases of spinal fusion operations which had been performed under deliberate hypotensive anesthesia (mean arterial pressure at 50-60 mmHg). To produce deliberate hypotension, The mean dose of SNP was 17.16 mg, the mean infusion time 283.85 minutes, and the average infusion rates 1.05 ㎍/kg/min. Patients who received csptopril required less SNP than untreated patients(0.95 vs 1.23 ㎍/kg/min., p$lt;0.05). Isovolemic hemodilution also reduced aversge infusion rates of SNP (0. 87 vs 1.22㎍/kg/min., p$lt;0.05). There were, however, no significant differences in preoperative hypertention vs normotension, mild hypothermia vs. normothermia during the operation, and mavs. female. In addition, the average infusion rates of SNP were significantly correlated with body mass index(r=0.3329, p$lt;0.01). But those were not correlated with age, infusion time of SNP, weight, volume of transfusion, height/age, and height.

        Subcutaneous emphysema of the face, neck and thorax is an unusual complication following tonsillectomy. It occurs under a wide variety of conditions. It appears in all age group, both sexes and under local and general anesthesia. In most instances serious complication is absent, but a few cases were reported where respiratory embarassment from mediastnal emphysema and subsequent pneumothorax presented a threat to life. We present two cases of subcutaneous emphysema following tonsillectomy. Subcutaneous emphysema were developed after operation, the emphysema subsided spontaneously without any complications.

        흰쥐에서 척수강내로 투여한 AMPA 수용체 길항제, ACEA 2085 의 항통각과민 효과

        N/A Background: To study the role of spinal alpha-amino-3-hydroxy-5-methylisoxazole-4-propionic acid (AMPA) receptors in pain behaviors caused by mild bum, we examined the effect of intrathecal administered ACEA 2085, which has been recently characterized as a high potency competitive AMPA receptor antagonist, on the thermal hyperalgesia state induced by mild burn. Methods: A thermal injury was induced by applying the left hind paw to a thermal surface (52.5C) for 45 sec, Thermal escape latency of the hind paw was determined using an underglass thennal stimulus. Thirty min after thermal injury, the paw withdrawal latency (PWL) in injured paw of all groups fell from 10-12 sec to 5-7 sec. At that time, ACEA 2085 (0.01-0.1 mcg) and 6-cyano-7-nitroquin- oxalinedione (CNQX, 1-30 mcg) were injected through intrathecal catheters in rats with mild burn injury on the right hindpaw, And then, PWL were measured in the both hindpaw every 30 minutes for about three hours. Results: The intrathecal injection of ACEA 2085 produced a dose dependent reversal of the hyperalgesia in the right hindpaw and more potent than CNQX, but had no effect upon the response latency of the normal left hind paw even at the largest doses. All effects were observed at doses that had no significant effect upon motor function. Conclusions: Intrathecal ACEA 2085, highly selective AMPA receptor antagonist produce a dose- dependent reversal of the thermal hyperalgesia evoked mild burn injury. These results suggested that spinal AMPA receptor play an important role in the hyperalgesia induced by mild burn injury.

      • 가토에서 급성 신동정백 차단이 Vecuronium과 Pancuronium의 근이완 작용에 미치는 영향

        The neuomuscular blocking effect of vecuronium and pancuroium was studied when the renal artery and vein of rabbits were ligated. Forty rabbits were divided into four groups:the first control group of ten rabbits with normal renal function which received vecuronium 0.1mg/kg and the second study group of ten with acute renal occlusion given the same drug; the third control group of ten with normal renal function which received pancuronium 0.1mg/kg and the fourth study group of ten with acute renal occlusion given the same drug. It was undertaken to obtain more information on spontaneous reversal from vecuronium and pancuronium induced neuromuscular block for monitoring the train-of-four(TOF) ration to full recovery with TOF stimulation. In control and study groups with vecuronium, mean recovery times to 25% TOF ratio were 1,009 and 1,635.2 seconds, those to 50% were 1,160.9 and 1,790.9 seconds, those to 75% were 1,271.2 and 1,972.6 seconds, and recovery indices were 262.2 and 337.4 seconds, respectively. In control and study groups with pancuronum, mean recovery times to 25% were 1,333.9 and 1,678.3 seconds, those to 50% were 1,525.4 and 2,005.5 seconds, those to 75% were 1,679.9 and 2,446.8 seconds, and recovery indices were 346 and 768.5 seconds, respectively. The prolongation of action in each study group becomes significant. It is concluded that the duration of neuromuscular blockade induced by either vecuronium or pancuronium was significantly prolonged when renal artery and vein of rabbits were ligated.
