        Craniospinal Neurenteric Cysts: Various MR Imaging Features

        Purpose : Craniospinal neurenteric (NE) cysts are rare developmental non-neoplastic cysts of the central nervous system with diverse MR imaging findings. The purpose of this study was to evaluate various MR imaging findings of intracranial and intraspinal NE cysts. Materials and Methods : We retrospectively reviewed the MR imaging findings of six NE cysts that were confirmed by pathology. We evaluated anatomic location, signal intensity, size and enhancement pattern of NE cysts. Results : Two intracranial lesions were located extra-axially in the cerebellopontine angle and quadrigeminal cisterns. Three spinal lesions were intraduralextramedullary cysts, located ventral to the spinal cord, but one thoracic lesion was an intramedullary cyst. The signal intensity of the cysts was hyperintense on T1-weighted images as compared with the cerebrospinal fluid (CSF) for two intracranial lesions and one cervical lesion. In addition, all intracranial lesions showed diffusion restriction. For the remaining three spinal lesions, the signal intensity was nearly the same as the signal intensity of the CSF as seen on both T1- and T2-weighted images. On contrast-enhanced studies, two intracranial cysts showed a small nodular enhancement and one thoracic spinal lesion showed rim enhancement. Conclusion : NE cysts have various locations, signal intensities, and possible focal nodular or rim enhancement. Therefore, NE cysts can be included in the differential diagnosis of various craniospinal cystic or tumorous cystic lesions.

        The main purpose of this study was to investigated the relationship between test anxiety and several variables (sex, grade of school, parent's school year, academic performance, parent's occupation, religions, blood type, personality trend, etc) in middle and high school students in Kwangju City (total numbers = 1060). Test Anxiety was assessed by the TAI-K(Kim Moon Joo, 1990). The data were processed by student t-test and ANOVA . The results were as follows , 1. Female students showed significantly higher level of test anxiety scores than males (especially worry factors were more prominent than emotionality factors) 2. In the area of grade difference in test anxiety, 2nd grade students were the highest group, 3rd grade were the lowest group in high school, In middle school, 1st grade were the highest and 3rd grade were appeared the lowest score of test anxiety. 3. In the area of mothers school graduate, female student with high school graduated mother group were appeared significantly higher test anxiety scores than other groups. 4. School performance and test anxiety was correlated negatively and significantly. 5. If student dislikes Korean, Mathematics and English subjects, they experienced higher test anxiety significantly than other groups. 6. Test anxiety scores were significantly higher in groups of live with their brothers or sisters than have no brothers and sisters group. 7. In male students, one of parent was not employed group had higher test anxiety scores than employed all. 8. In female students with religion, test anxiety scores were higher than had no religion group.

        수종의 항진균제 치료에도 호전을 보이지 않는 만성 전신성 체부백선 1예

        Chronic dermatophyte infection rarely fails to respond to topical or systemic antifungal therapy. Such refractory condition relates to many factors and one of them is the decreased response of delayed type hypersensitivity. A plausible mechanism by which the delayed hypersensitivity response may cause dermatophyte inhibition has been proposed already. Our patient had skin rashes for 6 years. It was diagnosed as tinea corporis and treated with various systemic antifungal agents, such as griseofulvin, itraconazole, fluconazole, terbinafine and topical forms of econazole and terbinafine. But the skin lesions did not resolve completely and reaggravated frequently. Numerous verrucae planae were found on face, neck and both extremities. Trichophyton rubrum was identified by fungus culture study. Laboratory examination showed no response against multi-CMI test, DPCP sensitization and prick test for trichophytons. We challenged the therapy with the combined antifungal agents and immune stimulatory drugs. This case is thought to be a chronic dermatophyte infection due to the defects in the both cell mediated immunity and immediate type hypersensitivity which is crucial for the host defence mechanisms against fungal infection.         고해상도 초음파현미경을 위한 미세기포초음파조영제개발

        Purpose: To develop optimal microbubble contrast agents (MBCAs) for performing ultrasound microscopy when examining small animals. Materials and Methods: We prepared three types of MBCAs. First, a mixture of three parts of 40% dextran and one part of 5% human serum albumin were sonicated with perfluorocarbon (PFC) (MB1-D40A5P). Second, three parts of 40% dextran and one part of 1% human serum albumin were sonicated with PFC (MB2-D40A1P). Third, all parts of 1% bovine serum albumin were sonicated with PFC (MB3-A1P). We measured the microbubbles' sizes and concentrations with using image analysis software. The acoustic properties of the microbubbles were assessed both in vitro and in vivo. Results: The majority of the MB1-D40A5Ps had a diameter of 2-5 μm, the mean diameter of the MB2-D40A1Ps was 2.5 μm, and the mean diameter of the MB3-A1Ps was less than 2.0 μm. Among the microbubbles, the MB1-D40A5Ps and MB2-D40A1Ps showed increased echogenicity in the abdominal vessels, but the duration of their contrast effect was less than 30 sec. On the contrary, the MB3-A1Ps exhibited strong enhancement in the vessels and their duration was greater than 120 sec. Conclusion: A microbubble contrast agent consisting of all parts of 1% serum albumin sonicated with PFC is an effective contrast agent for ultrasound microscopy. 목적: 작은 동물의 초음파현미경영상검사에 이용할 수 있는 적절한 미세기포초음파조영제를 개발하고자 하였다. 대상과 방법: 세 가지 종류의 미세기포초음파조영제를 개발하여 실험하였다. 첫 번째는 40%의 덱스트란과 5% 혈청알부민을 perfluorocarbon(PFC)에 혼합한 제제(MB1-D40A5P). 두 번째는 40%의 덱스트란과 1% 혈청알부민을 PFC에 혼합한 제제(MB2-D40A5P). 세 번째는 1%의 혈청알부민에 PFC 에 혼합한 제제(MB3-A1P)였다. 이들 세 가지 형태의 미세기포의 크기와 농도를 영상분석소프트웨어로 분석하였고, 초음파현미경을 이용하여 시험관 및 생체영상검사를 시행하였다. 결과: 미세기포조영제의 크기는 90%이상에서 MB1-D40A5P는 2-5 μm, MB2-D40A1P는 2.5 μm, MB3-A1P 2.0μm 이하였다. 이들 조영제중 MB3-A1P 이 다른 조영제보다 강한 조영증강효과를 보였다. MB1-D40A5P과 MB2-D40A1P는 주요복부혈관에서 증가한 조영효과를 보였으나. 조영증가효과가 30초 이내였다. 이에 비해 MB3-A1P는 같은 혈관에서 강한 조영증강효과를 보였으며, 120초 이상의 조영증가효과를 보였다. 결론: 초음파현미경용 미세기포조영제로서 1%의 혈청알부민에 PFC에 혼합한 제제(MB3-A1P)가 효과적으로 이용할 수 있을 가능성이 있는 것으로 보인다.

        Diagnostic Benefit of Thyroglobulin Measurement in Fine-Needle Aspiration for Diagnosing Metastatic Cervical Lymph Nodes from Papillary Thyroid Cancer: Correlations with US Features

        Objective: Our goals were to determine the added value of fine-needle aspiration biopsy (FNAB)-thyroglobulin (Tg) measurements over FNAB-cytology alone for diagnosing metastatic nodes, and to determine whether the ultrasound features of lymph nodes can be used to identify lymph nodes that may benefit from FNAB-Tg measurement in patients with papillary thyroid cancer. Materials and Methods: We retrospectively evaluated 76 surgically proven cervical lymph nodes. Twenty-nine patients were awaiting surgery and 18 patients had undergone thyroid surgery for papillary thyroid cancer. Ultrasoundguided FNAB and Tg measurements were performed and the ultrasound features were evaluated. Results: The accuracies, sensitivities, and specificities of FNAB-cytology, FNAB-Tg, and combined FNAB-Tg/cytology were 90%, 80%, and 100%; 92%, 95%, and 90%; and 93%, 96%, and 90%, respectively. The diagnostic sensitivity of FNAB-Tg for metastatic nodes was significantly higher than that of FNAB-cytology (p = 0.011). Furthermore, combined FNAB-Tg/cytology significantly increased sensitivity (p = 0.002) and accuracy (p = 0.03) as compared with FNAB-cytology. Conclusion: Combined FNAB-Tg/cytology is significantly more sensitive and accurate at detecting metastatic nodes than FNAB-cytology alone. FNAB-Tg was better at diagnosing metastases in small lymph nodes.
