텍스트마이닝 기법을 이용한 『상한론』 내의 증상-본초 조합의 탐색적 분석

Shanghanlun (Treatise on Cold Damage Diseases) is the oldest document in the literature on clinical records of Traditional Asian medicine (TAM), on which TAM theories about symptoms-herbs relationships are based. In this study, we aim to quantitatively explore the relationships between symptoms and herbs in Shanghanlun. The text in Shanghanlun was converted into structured data. Using the structured data, Term Frequency - Inverse Document Frequency (TF-IDF) scores of symptoms and herbs were calculated from each chapter to derive the major symptoms and herbs in each chapter. To understand the structure of the entire document, principal component analysis (PCA) was performed for the 6-dimensional chapter space. Bipartite network analysis was conducted focusing on Jaccard scores between symptoms and herbs and eigenvector centralities of nodes. TF-IDF scores showed the characteristics of each chapter through major symptoms and herbs. Principal components drawn by PCA suggested the entire structure of Shanghanlun. The network analysis revealed a ‘multi herbs – multi symptoms’ relationship. Common symptoms and herbs were drawn from high eigenvector centralities of their nodes, while specific symptoms and herbs were drawn from low centralities. Symptoms expected to be treated by herbs were derived, respectively. Using measurable metrics, we conducted a computational study on patterns of Shanghanlun. Quantitative researches on TAM theories will contribute to improving the clarity of TAM theories.
Foreign Body Inclusion Cyst of the Nasal Radix after Augmentation Rhinoplasty

Development of a cystic mass on the nasal dorsum is a very rare complication of aesthetic rhinoplasty. Most reported cases are of mucous cyst and entrapment of the nasal mucosa in the subcutaneous space due to traumatic surgical technique has been suggested as a presumptive pathogenesis. Here, we report a case of dorsal nasal cyst that had a different pathogenesis for cyst formation. A 58-yr-old woman developed a large cystic mass on the nasal radix 30 yr after augmentation rhinoplasty with silicone material. The mass was removed via a direct open approach and the pathology findings revealed a foreign body inclusion cyst associated with silicone. Successful nasal reconstruction was performed with autologous cartilages. Discussion and a brief review of the literature will be focused on the pathophysiology of and treatment options for a postrhinoplasty dorsal cyst.

Angiolipoma is a benign adipose tissue tumor with marked vascular proliferation, which differentiates angiolipoma from common lipoma. Angiolipoma rarely occurs in head and neck regions. This is the first report on infiltrating angiolipoma arising from the pterygopalatine fossa accompanied by invasive aspergillosis. A-61-year-old male presented with diplopia which occurred after head trauma. An ill-defined mass originated from right pterygopalatine fossa was incidentally found by magnetic resonance imaging. The mass was removed via endonasal endoscopic approach combined with Caldwell-Luc approach. It was partially removed because the mass was firmly adhered to surrounding normal tissues and was adjacent to the right cavernous sinus. Pathologic examination confirmed the diagnosis of angiolipoma. Additionally, it unexpectedly revealed the presence of fungal hyphae inside the tissue obtained from right lamina papyracea. Following an antifungal medication for 6 months after surgery, the patient currently shows no evidence of invasive fungal infection at postoperative 9 months.
달동네에 거주하는 저소득층 노인의 건강과 삶에 대한 현상학적 연구 - 광주광역시 발산마을 거주 노인을 중심으로

Objectives : The elderly in South Korea are the poorest among OECD countries in 2015. The aim of this study was to explore the health and life of the low-income elderly living in vulnerable areas in a metropolitan city. Methods : Data were collected through in-depth individual interviews with 7 participants from October to November 2015 and analyzed through Colaizzi’s phenomenological methodology. The participants were interviewed for over 60 minutes in each person. Results : 7 categories were identified from 17 subcategories: “My life history: sick body,” “Living with a sick body,” “My poor but precious life,” “A sense of distance from the hospital,” “Narrowed area of my life,” “Thankful for help,” and “The village where I have lived my destiny.” There is a lack of medical accessibility, mobility, and economic independence for low-income seniors. In addition, full-fledged redevelopment comes to them as violence. Conclusions : The health and life of the low-income elderly in vulnerable areas are products of many social factors, reaffirming the importance of social health.

Objectives With the development of the Internet of things and big data, it becomes possible to analyze large medical records. However, a lack of data and a risk of private information disclosure act as a barrier to this possibility, and generating synthetic patient records can be used as an alternative to circumvent this limitation. In this paper, we reviewed the researches on the rapidly developing synthetic patient records. Methods We searched for articles that studied the synthetic patient records and sorted them according to data types, generation methods and evaluation methods. Results The types of synthetic patient data could be largely divided into binary, ordinal, image, and sequential data. But the studies of models that could handle a dataset of several mixed types were relatively scarce, and more researches are needed. In terms of generation methods, researches could be divided into researches about rule-based models and data- driven model, and the data-driven models could be divided into classical machine learning and deep learning models. The criteria for evaluating the model were largely the reproducibility and risk of privacy disclosure. Both quantitative evaluation and qualitative evaluation were used to evaluate the reproducibility, on the other hand, most the studies lacked privacy assessments and it needs to be supplemented in future studies. Conclusions Applying methods for generating synthetic patient records to Korean traditional medical data will facilitate the data science researches of Korean traditional medicine and this can be an opportunity to understand the structure of Korean traditional medical data.

Juvenile hyaline fibromatosis (JHF) is a rare, progressive autosomal recessive disease that’s characterized by papulonodular skin lesions, soft tissue masses, joint contractures, gingival hypertrophy and osteolytic bone lesions. We present here the case of a 2-yr-old boy with JHF along with a review of the relevant literature. This case demonstrates that JHF should be considered in the differential diagnosis when multiple subcutaneous nodules are observed in the face, head and neck.
