In this study, we proposed an image quality control for an automatic exposure control (AEC) of digital radiographic imaging system and tried to analyze the performance of the AEC by various manufacturer. The subjects of the experiment were analyzed for the AEC image quality evaluation using digital radiation generators from four manufacturer such as PHILIPS, GE Healthcare, SAMSUNG Healthcare, DK Medical Solution. We used as materials for the implementation of the image quality evaluation by coins (500 won, KOMSCO, Korea). This study evaluated the performance evaluation of the AEC as image quality and exposure dose (Milliampere-seconds; mAs). The image quality evaluation was tried visual assessment by two radiologic technologists and contrast to noise (CNR) by ImageJ. The exposure dose investigated mAs on digital radiation generators. The radiographic coin images acquired 360 images based on change in the control factors of the AEC, which were kVp, the consistency of field configuration and dominant zone, sensitivity and density. As a result, there was a significant difference in the AEC performance between manufacturer. The CNR by the AEC for each manufacturer showed a difference of up to about 1.9 times. The exposed tube current by the AEC for each manufacturer showed a difference of up to about 5.8 times. It is expected that our proposed evaluation method using coins could be applied as the AEC performance evaluation method in the future.

The purpose of this study was to analyze the effect of automatic exposure control (AEC) control factors in digital radiography systems based on the fine contrast images using coin phantoms. The AEC control factors were targeted at the range of dominent zone, sensitivity, and density. The dominent zone was divided into cases where a single coin was used to cover the field configuration, and cases where seven coins were used to cover the field configuration. The sensitivity was classified into three stages (200, 400, 800) and the density was classified into three stages (2.5, 0, 2.5). Image quality was evaluated by signal to noise ratio (SNR) and contrast to noise ratio (CNR). Then, the automatically exposed tube current was measured. As a result, the X-ray image of seven coins obtained a result value of about 1.2 times higher for SNR and 1.9 times higher for CNR than the X-ray image for one coin. The tube current was also about 1.6 times higher. In conclusion, In AEC, the higher the field configuration and dominent zone are matched and the higher the density, the lower the sensitivity, which increases the tube current and CNR, which increases the image quality. Therefore, it is judged that the appropriate setting of the range of dominent zone, sensitivity, and density of the control, which is the AEC control factor, could improve the fine contrast of images.

Tremor is a rhythmic, involuntary and oscillatory movement of body parts, and it is the most common movement disorder. Spasticity is also one of the movement disorders that is commonly accompanied with Complex Regional Pain Syndrome; however, the basic nature of spasticity has not yet been proved. A 25-year-old male patient had two operations and he was being treated because of a back injury that occurred 4 years ago. He suffered from pain, tremor and spasticity on both his lower legs, and his symptoms were diagnosed as failed back surgery syndrome. The tremor and spasticity were aggravated despite of continuous treatments. We then treated him with spinal cord stimulation. His pain, tremor and spasticity disappeared after spinal cord stimulation.

This study examined the anti-diabetic effects of aqueous extracts of Dendropanax morbifera leaves (DMWEs) in streptozotocin-induced diabetic Sprague-Dawley (SD) rats. Thirty male SD rats (body weight [BW], 250.4 ± 19.7 g) were divided into the following six groups: normal control rats (NC), diabetic control rats (DC), diabetic rats treated with metformin HCl 100 mg/kg BW (DT), diabetic rats treated with DMWEs 50 mg/kg BW (DM-50), diabetic rats treated with DMWEs 100 mg/kg BW (DM-100), and diabetic rats treated with DMWEs 200 mg/kg BW (DM-200). From two weeks of administration of DMWEs, the BW of all groups treated with DMWEs increased significantly compared to DC (p < 0.05). At four weeks after treatment, the blood glucose levels in DT, DM-100, and DM-200 decreased below 200 mg/dL, while the glycated hemoglobin concentrations in all groups administered DMWEs were similar to those of NC and DT. Regarding the blood biochemical parameters, the levels of aspartate transaminase, alanine transaminase, blood urea nitrogen, and creatinine in DM-100 and DM-200 were similar to those in NC and DT. Overall, these results highlight the effectiveness of DM-100 in the treatment of diabetes.

Background: We evaluated the correlation between the difference of estimated lean body mass (LBM(E)) with actual lean body mass (LBM(A)) and actual blood concentration of propofol, hemodynamic variables and Bispectral index (BIS) during target controlled infusion (TCI) of propofol in Korean female patients who would take total intravenous anesthesia (TIVA) using propofol-remifentanil combination. Methods: Eighty Korean women participated in this trial. Demographic data were collected and LBM(A) was measured using body mass analyzer. Target concentration of propofol was set at 6 μg/ml during induction and 3μg/ml during maintenance. Hemodynamic variables and BIS were measured at 60 minutes after anesthetic induction. Peripheral blood sample was collected from a large forearm vein on the contralateral side of infusion for measurement of whole blood concentration of propofol at 60 minutes after anesthetic induction. Delta lean body mass (ΔLBM) was obtained by subtracting LBM(A) from LBM(E). Correlation analyses were done between ΔLBM and plasma propofol concentration, blood pressure, and BIS, respectively. Results: Blood concentrations of propofol were 1.9-6.9μg/ml. Mean plasma concentration of propofol was higher than target concentration. ΔLBM had a positive correlation with actual plasma propofol concentration. ΔLBM was not correlated with hemodynamic variables and BIS. Conclusions: TCI of propofol resulted in large variation in measured plasma concentration. We thought ΔLBM was one of interpatient variation factors of propofol plasma concentration during TCI. We found that TCI of propofol using Schnider model would result in higher blood concentration than target concentration in Korean women. (Korean J Anesthesiol 2008; 55: 666~9)

Background: Remifentanil combined with propofol is usually used to induce anesthesia. However, remifentanil and propofol depress the cardiovascular system. This study investigated the effects of a continuous infusion of remifentanil on the propofol dose and hemodynamics using the bispectral index (BIS) during anesthetic induction. Methods: Sixty female ASA physical status class I or II patients, who were scheduled to undergo gynecologic surgery were randomly assigned to one of three groups (n = 20). Normal saline 20 ml/hr (Group S), remifentanil 0.25μg/kg/min (Group 0.25), or remifentanil 0.5μg/kg/min (Group 0.5) was infused intravenously. Propofol was administered slowly two minutes after administering remifentanil or normal saline. The heart rate, mean arterial pressure (MAP) and BIS were measured at baseline, preintubation and postintubation. Result: There were no significant differences in the changes in the BIS among the groups. The MAP and heart rate decreased at preintubation compared with baseline (P < 0.05). The MAP of Group 0.5 at postintubation was lower than that in the other groups (P < 0.05). The heart rate in all groups increased at postintubation compared with baseline (P < 0.05). The heart rate of Group 0.5 at postintubation was lower than that of Group S (P < 0.05). The propofol requirement for unconsciousness was lower in Groups 0.25 and 0.5 than in Group S. The propofol requirement in Groups S, 0.25 and 0.5 was 1.56 ± 0.2 mg/kg, 1.07 ± 0.2 mg/kg and 0.9 ± 0.1 mg/kg, respectively. Conclusions: A combined injection of 0.5μg/kg/min remifantanil with 0.9 mg/kg of propofol decreases the heart rate and MAP at preintubation without adverse effects and appropriately prevents the cardiovascular responses to tracheal intubation, and reduces the propofol dose needed for a loss of consciousness.
