Improving Quality of Care in Nursing Homes

Objective: Ensuring the quality of care in nursing homes is a persistent concern in many countries. This study aimed (a) to clarify the most important quality-of-care problems and priorities to improve the quality of nursing home care, and (b) to identify some of the best measures of quality-of-care and quality-of-life. Methods: This explanatory descriptive study entailed conducting a nursing home director questionnaire; 43 directors participated. Results: Directors answered that quality of care means providing “timely care” for patients with symptoms or diseases. Pressure ulcer rate (74%) and activities-of-daily-living (ADL) function (67%), direct-care hours (67%), and the number of nursing staff (56%) were the best measures chosen as quality-of-care indicators. The directors defined the quality of life as helping residents be healthy and dignified, and the best set of quality-of-life indicators, included by more than 70% of participants, were physical comfort (84%), ADL level (81%), respect for individual dignity (79%), and satisfaction in level of protection and safety (74%). Conclusion: These results provide clear direction for policy-makers in changing policies and regulations for the best care of nursing home residents. More research is needed to improve the quality of care and quality of life in nursing homes using the best measures identified in this study.
초등교사의 정서코칭 경험과 교육적 의미에 관한 내러티브 탐구

본 연구의 목적은 초등학교에서 체육 시간 및 학교생활 전반에 걸쳐 발생하는 갈등상황에서 부정적인 정서 해결에 어려움을 겪는 학생의 정서 코칭 경험을 기술하고 그 과정에 내포된 교육적 의미를 탐색하는 데 있다. 연구 목적 달성을 위해 연구자는 ‘마로’라고 하는 초등학교 5학년 남자 어린이를 연구 참여자로 선정하여 1년 6개월간의 정서 코칭을 실시하였다. 연구자는 연구 참여자의 담임을 맡으며 정서 코칭을 통해 체육 시간과 학급에서 ’마로‘가 변화 하는 과정을 다루고 있다. 학생 연구 참여자는 초기 갈등상황을 주로 짜증, 화, 욕설, 폭력으로 해결하는 학생이었 다. 연구자는 정서 코칭을 하는 과정을 학생과의 심층면담, 정서코칭 일지, 연구자의 현장일기를 통해 자료를 수집 하였다. 연구결과 정서코칭은 학생 스스로의 정서 조절로 학교생활을 보다 안정적으로 영위할 수 있도록 하는 데 많은 도움을 주었다. The purpose of this study was to describe emotional coaching experiences of a student who had difficulties in dealing with negative emotions in the conflict situations that occurred throughout physical education classes and daily life in elementary school, and to explore the educational implications involved in the process. In order to achieve the purpose of the research, the researchers selected a 5th grade male child named 'Maro' as a research participant and conducted emotional coaching for 1 year and 6 months. One of the researchers has been involved in the process of changing "Maro" in physical education as well as other subject classes through emotional coaching by taking the role of homeroom teacher. The participant in the study was a student who tried to solve conflicts by irritation, anger, profanity, and violence. The researchers gathered data on the process of emotional coaching through in-depth interview with the student, emotional coaching journal, and researcher's field note. The results of the study showed that emotional coaching helped the research participant maintain his school life more stable by adjusting his own emotions.
