        재가노인 돌봄제공자를 위한 모바일 애플리케이션 콘텐츠 분석: 국내 이용 앱을 중심으로

        본 연구는 재가노인 돌봄제공자를 위한 모바일 앱의 주요 특징과 기능을 파악하고 주요 콘텐츠를 분석하는 것을 목적으로 한다. 앱 분석을 위해 Android와 iOS에서 관련 키워드를 검색하여 820개의 앱이 검색되었고, 선정기준 및 제외기준에 따라 40개의 앱을 선정하였다. 앱 플랫폼은 Android 27개(67.5%), Android/iOS 13개(32.5%)였고, 앱 콘텐츠 분석 결과 주요 콘텐츠는 일정관리, 건강상태 모니터링, 재가노인 홈케어 교육, 돌봄제공자를 위한 교육, 정보제공(일방향), 의사소통(양방향), 위치확인, 응급상황 알람, 지불관리, 환경 모니터링이었다. mHealth 환경에서 많은 돌봄제공자들이 여러 앱을 사용하고 있으나 돌봄제공자의 필요에 맞는 기능을 유기적으로 통합한 앱은 존재하지 않았다. 돌봄제공자의 다양한 요구를 반영하여 편의성과 효율성을 증진할 수 있는 앱을 마련하고, 돌봄제공자의 노인 돌봄 역량강화를 위한 교육적 콘텐츠 및 돌봄제공자 간 원활한 의사소통을 위한 앱을 구축할 필요가 있다. The purpose of this study is to identify the main characteristics and functions of mobile apps for caregivers of community-dwelling older adults and to analyze major contents. For app analysis, 820 apps were searched by searching for related keywords on Android and iOS, and 40 apps were selected according to the selection criteria and exclusion criteria. The main contents were calendaring, health condition monitoring, education for home care, education for caregiver, information sharing/alarm (one way), communication (two way), GPS location tracking, emergency alarm, payment management, environment monitoring. In the mHealth environment, many caregivers use apps, but there was no application that organically integrated functions to meet the needs of caregivers. It is required to prepare an app that can enhance convenience and efficiency by reflecting various needs of care providers. In particular, it is necessary to prepare an app for communication between caregivers and educational contents to strengthen older adults care capabilities of caregivers.

        학습성과기반 단계적 통합간호실습교육 매트릭스 개발

        Purpose: The purpose of this study was to develop an integrated nursing practicum education matrix based on the learning outcomes of each stage. Methods: In this Delphi technique study, 10 experts, consisting of six professors and four nurses, participated in the development of the matrix. The first step was an in-depth review for the composition of the questionnaire and the second step was the Delphi technique. The Delphi survey was conducted two times in order to complete the components of the matrix. The survey data was analyzed for statistical averages and standard deviations to decide the order of priority. Results: According to each stage (i.e. fundamental stage, competent stage, and proficient stage), the matrix was composed of education contents, methods, evaluation methods, and curriculum outcomes. Conclusion: The integrated nursing practicum education matrix of Y University was completed. The developed matrix will result in a reduction in the gap between nursing education and clinical practice and an improvement in nursing competency.

        장기재원환자의 특성 및 전원 인지도와 전원 의향과의 관계 - 장기재원환자의 효율적 전원을 위한 전략 제시 -

        Background : It is very common in Korea to take care of non-acute patients in an acute setting, due to the lack of long-term facilities. Long term hospitalization increase medical expenses and decreases the bed utilization, which can affect the urgent and emergent admissions, and eventually jeopardize the hospital financially. In this study, strategies for effective transfers to the lower levels of care, and to decrease the length of stay were presented by surveying and analyzing the patient's knowledge of the transfer needs, and the willingness to transfer those whose hospital length of stay was more than 30days. Method : The survey is subject to a group of 251 patients who have been hospitalized over 30 days in a general hospital in Seoul. Excluding those that were in the Intensive Care Unit and psychiatric ward, 214 in-patients were used as participants. They were surveyed from April 9, 2002 to April 17, 2002. One hundred and thirty seven out of 214 were responded which made the response rate 64%. Data were analyzed by SAS and SPSS. Result : Multi-variable Logistic Regression Analysis showed a significant effect in medical expenses, knowledge of referral system and the information of the receiving hospital. The financial burden in medical expenses made the patient 10.7 times more willing to be transferred, knowledge of the referral system made them 5 times more willing to be transferred, and the information of receiving hospital makes 6.5 times more willing to be transferred. Reasons for willing to be transferred to a lower level of care were the phase of physical therapy, the distance from home, the attending physician's advice and being unable to be treated as an out patient. Reasons for refusing to be transferred were the following. The attending physician's competency, not being ready to be discharged, not trusting the receiving hospital's competency due to the lack of information, or never hearing about the referring system by the attending physician. Conclusion : Based on this, strategies for the effective transfer to the lower levels of care were suggested. It is desirable for the attending physician to be actively involved by making an effort to explain the transfer need, and referring to the Healthcare Coordinating Center, which can help the patient make the right decision. Nationwide networking for the referral system is the another key factor that may need to be suggested as an alternative to decrease the medical expenses. Collaborating with the Home Health Agency for the early discharge planning and the Social Service Department for financial aid are also needed. It is recommended that the hospital should expedite the transfer process by prioritizing the cost and the information as medical expenses, knowledge of referring system and the information of the receiving hospital, are the most important factors to the willingness to transfer to a lower level of care.

        국내 병원간호사의 재직 영향요인

        Purpose: To systematically review research articles that examined factors affecting Korean clinical nurses’ intention to stay in their jobs. Methods: Articles related to Korean clinical nurses and published both in Korean and English languages were retrieved from computerized databases using a manual search. Data extraction, quality assessment, and analysis including meta-analysis were completed. Results: The review included 15 studies. There was strong evidence for the effects of 19 factors on Korean clinical nurses’ intention to stay. Factors were categorized into three major categories; demographic, extrinsic, and intrinsic factors. In the meta-analysis of 10 articles, career experience of more than 6 years was the main factor that led to high intention to stay (OR=-0.5, p<.001). Job performance related factors (28.1%) and nursing work environment or organizational climate for caring (21.9%) were studied as the main extrinsic factors. Job related attitude (28.1%) including job satisfaction and organizational commitment and nursing value (15.6%) were studied as the main intrinsic factors. Conclusion: Expert nurses are more likely to retain their current positions as well as quality workplace environments or psychological capitals. Healthcare organizations must be responsible for improving professional and psychological capital of beginner nurses and affecting change for a warm organizational environment.

        육아휴직 후 복귀한 간호사의 양육스트레스와 동료지지가 일과 삶의 균형에 미치는 영향

        Purpose: This was a descriptive study aimed at providing basic data on readaptation of hospital nurses after parental leave by exploring levels of parenting stress, co-worker support, and work-life balance and identifying factors affecting work-life balance. Methods: A survey using a convenience sampling was conducted with 86 nurses who were within their first year reinstatement after parental leave. Structured questionnaires were used, including the above named main variables and other general characteristics. Data were collected from February to May, 2019 from three tertiary hospitals in Seoul. Data were statistically analyzed using independent t-test, one-way ANOVA, Pearson correlation coefficient, and multiple regression analysis. Results: Work-life balance showed a statistical difference depending on the level of education; it showed negative correlations with commuting time and fatigue and positive correlations with the attitude to job rotation, health status, and quality of sleep. There was a negative correlation between parenting stress and work-life balance and a positive correlation between co-worker support and work-life balance. Work-life balance was influenced by fatigue, parenting stress, health status, and co-worker support in the regression model. Conclusion: Parenting stress, co-worker support, health status and fatigue of nurses reinstated from parental leave should be improved to enhance their work-life balance.

        전원 관련 안전간호수행에 영향을 미치는 요인

        Purpose: The purpose of this study was to investigate predictors of nurses’ patient safety behavior during inter-hospital transfer. The study was based on the Theory of Planned Behavior (TPB). Methods: A descriptive survey design was used. Data were collected with a self-administrated 39-item questionnaire completed by 111 nurses from a university hospital in Seoul, South Korea. The questionnaire was developed based on the Theory of Planned Behavior (TPB) guideline and included measure of self- reported past patient safety behaviors, intentions, attitudes, subjective norms and perceived behavioral controls. Ethical approval was granted by the hospital review board. Hierarchical regression analyses were carried out. Results: The average score of patient safety behavior was 4.21±0.63 (5 point scale). The TPB variables explained 49.9%of the variance in patient safety behavior. Intention and subjective norm were the most significant predictors of nurses’ patient safety behavior. Attitude was related to nurses’ patient safety behavior. Conclusion: TPB variables predicted the nurses’ patient safety behavior during inter-hospital transfer of patients except for perceived behavioral controls. The results of this study suggest that better strategies for subjective norms and intentions related to patient safety behavior will be helpful in safety culture reform.

        간호대학생의 사회적 책임 인식수준 및 영향요인

        Purpose: The purpose of this study was to identify the perception and factors affecting social responsibility in nursing students. Methods: The participants for this study were 301 nursing students in three universities and three colleges located in Seoul, Gyeonggi or Gyeongbuk in South Korea. Data were collected in Dec, 2015 using structured questionnaires and analyzed using independent t-test, one way ANOVA, Pearson correlation coefficient, multiple regression analysis with the SPSS/WIN 18.0 program. Results: The mean score for social responsibility of nursing students had a significant positive correlation with satisfaction with nursing major, nursing professionalism and sociality. Sociality, nursing professionalism and nursing major satisfaction were the major predictors of social responsibility and explained 51.0%(F=142.67, p<.001) of the total variance in the study. Conclusion: The results suggest that it is necessary to develop strategies to promote the various nursing curricular programs that reinforce social responsibility in nursing college students.

        간호간병통합서비스병동과 일반병동 간호사가 지각하는 타 직종 근무자에 대한 신뢰, 협력 및 팀워크 비교연구

        Purpose: This study was performed to compare the perceived level of trust, collaboration, and teamwork about other healthcare personnel, professionals and assistants between nurses who work on integrated nursing care service wards and general wards. Methods: Participants were 216 nurses working on integrated nursing care service wards and general wards of 5 general hospitals located in Kyeongido and Kyeongsangbuk-do (integrated nursing wards: 109, general wards: 107). Data were collected using structured questionnaires and analyzed using x² test, two sample independent t-test and Multivariate Analysis of Covariance (MANCOVA) with SPSS/WIN 23.0 programs. In particular, MANCOVA was conducted after controlling two independent variable covariants which are marital status (p=.045) and work department (p=.022) which had significant differences for both groups and three dependent variables which is trust, collaboration, and teamwork that highly correlated. Results: There were significant differences in the dependent variables of trust (p=.001), collaboration (p=.014), and teamwork (p<.001) between the two groups of nurses. The mean scores for trust, collaboration, and teamwork with other healthcare personnel as perceived by nurses working on integrated nursing care service wards were significantly lower than that of nurses on general wards. Conclusion: These findings show that strategies are needed to strengthen the trust, collaboration, and teamwork among nurses working on integrated nursing care service wards and other healthcare personnel.

        간호사가 인식하는 경력관리수준과 조직공정성이 직무순환태도에 미치는 영향

        Purpose: The purpose of this study was to identify the relationship between career management, organizational justice, and job rotation attitude among tertiary hospital nurses. Methods: A cross-sectional descriptive design was used. The participants were 195 nurses with more than one year of experience working at tertiary care hospitals. Data was collected with self-reported questionnaires from April to May 2018. Data was analyzed using SPSS Statistics version 25.0 for windows including descriptive statistics, bivariate analysis, and multiple regression analysis. Results: The mean score of career management of participants was 3.14±0.49 (range 1~5), organizational justice was 2.95±0.53 (range 1~5), and job rotation attitude was 4.02±0.97 (range 1~7). There were significant positive correlations among the participants" career management, organizational justice, and job rotation attitude. 37% of job rotation attitude was explained by position, organizational justice, career management, and the frequency of job rotation experience in the regression model. Conclusion: This study provided the empirical evidence that it is necessary to improve the perception of organizational justice, and to establish a systematic job rotation in order for nurses to positively recognize job rotation.
