      • 정맥용 면역글로불린 투여 후 발생한 무균성 수막염 3례

        IVIG 투여에 의해 무균성 수막염은 그 기전이 밝혀지지 않았으나 IVIG 투여와 관련된 신경학적 부작용 중 가장 중한 합병증의 하나이다. 저자들은 서울대학교 어린이병원에서 2003년부터 2004년까지 3명의 환아에서 확인된 IVIG 투여와 관련된 무균성 수막염 4례를 경험하여 보고하였다. 3명의 환아는 ITP, 가와사키병, 중증 근무력증으로 기저질환은 각각 상이하였으나 기저질환에 대한 치료를 위해 모두 고용량의 IVIG를 투여받았으며, IVIG를 투여받은 후 1~2일 이내에 심한 두통과 뇌막자극 징후를 보였다. 척수액 소견상 백혈구 수는 $100/{\mu}L$ 미만에서부터 $1,000/{\mu}L$ 이상까지 다양하였으나 주로 다핵구로 구성된 척수액 백혈구 분포를 보였고(66~98%), 세균 배양검사와 장바이러스 배양과 PCR 검사에서 음성 결과를 보였다. 3명의 환아 모두 수막염 발생 2일 후까지는 증상이 호전되었으며, 후유증 및 장애가 없었다. Intravenous immune globulin(IVIG) is widely used for immune thrombocytopenic purpura (ITP), Kawasaki disease and other autoimmune neuromuscular disease. Aseptic meningitis was one of the most serious neurologic complications reported following the use of IVIG. We experienced 4 episodes of aseptic meningitis associated with IVIG usage in 3 patients from 2003 to 2004. Underlying disease of each patients was ITP, Kawasaki disease and myathenia gravis and all of them received high dose IVIG treatment for their underlying disease. Within a days, they started to complain severe headache and diagnosed meningitis by cerebrospinal fluid analysis. Cerebrospinal fluid leukocyte counts varied from 92 to over a thound per microliter with dominance of polymorphonuclear leukocytes. Microbiologic studies revealed no organisms. All of them were free from headache within 2 days and did not suffer any neurological sequelae.

        2001년 경상북도에서 발생한 콜레라 유행에 관한 역학조사

        Objectives : This study was carried out to investigate the cause, magnitude and transmission route of the cholera outbreak in 2001 Methods : The study population were those persons who ingested foods at the restaurant, were confirmed as cholera patients, had symptoms of diarrhea and sewed as workers at the restaurant. A questionnaire survey and microbiological examinations on the microbes isolated from rectal swabs were conducted. Of the cases, 316 food histories were surveyed by an analysis of the restaurant menu. Results : There were 139 confirmed cases of cholera reported in Korea in 2001. Of these, 104 were related to the restaurant. By region, Gyeongsangbuk-do had the highest incidence with 91 cases. Of these 91 cases, 74 had ingested foods at the restaurant, 2 were employees and 3 were secondary infection cases within the families. The results of the odds ratio analysis of the 316 persons having ingested foods at the restaurant were as follows: sandwiches 5.07 (95% CI, 1.85-14.59), soybean curd 2.45 (95% CI, 1.09-5.56), noodles 2.34 (95% CI, 1.24-4.42), steamed squid 2.01 (95% CI, 1.17-3.47) and vinegared rice 1.82 (95%, CI, 1.08-3.09). It was certain the restaurant in question was the cause of the 2001 outbreak. Conclusions : We suspected that more than one restaurant employee contaminated foods served at the restaurant. In addition, eating raw fishes purchased at the Pohang Fisheries infected the employees of the restaurant. There is a possibility that these raw fishes were themselves contaminated by cholera bacilli in the sea.

      • Experimental Evaluation of Q-Parameterization Control for the Imbalance Compensation of Magnetic Bearing Syatem

        This paper utilizes the method of Q-parameterization control to design a controller which solves the problem of imbalance in magnetic bearing systems. There are two methods to solve this problem using feedback controal. The first method is to compensate for the imbalance forces by generating opposing forces on the bearing surface (imbalance compensation). The second method is to make the rotor rotate around its axis of inertia (automatic balancing);in this case no imbalance forces will be generated. In this paper we deal with only imbalance compensation. The free parameter of the Q-parameterization controller is chosen such that these goals are achieved. After the introduction of a model of the magnetic bearing system, we explain the Q-parameterization controller design of the magnetic bearing system with emphasis on the rejection of sinusoidal disturbance for imbalance compensation design. The design objectives are formulated as a linear equations in the controller free paramete Q. Finally, simulation and experimental results are presented and showed the robustness and effectiveness of the proposed controllers.

        A 37Ch Visible Adaptive Optics System for Wavefront Compensation

        A visible adaptive system was developed to compensate for phase distortions of a visible (He-Ne) laser. The visible adaptive optics (AO) system consists of 5 subsystems: a Shack-Hartmann wavefront sensor, a deformable membrane mirror of 15 mm with 37 actuators, a high-speed tip/tilt mirror of 20 mm, a position sensor and a control & display unit. The lab-developed Shack-Hartmann sensor was experimentally shown to have a measurement accuracy of better than RMS /50 at 0.6328 μm over the maximum phase distortion that the deformable mirror could generate, i.e., approximately /4. The adaptive optics system operated the deformable mirror at 10 Hz and the tip/tilt mirror at 30 Hz, which were, respectively, closed by the Shack-Hartmann sensor and the position sensor. The closed adaptive optics system was demonstrated to compensate for time-varying wavefront aberrations of PV (Peak-to-Valley) 1.2 μm with a time frequency of 1.4 Hz. The residual RMS wavefront error was shown to be smaller than 0.014 μm, i.e., /45 at 0.6328 μm.

        Purpose: In the UICC staging system, stage IV contains not only those patients with distant metastasis but also patients with far advanced T and N status but without distant metastasis. We investigated the prognostic factors of stage IV gastric carcinoma patients without distant metastasis after curative resection. Materials and Methods: 190 stage IV gastric carcinoma patients without distant metastasis were reviewed after curative resection. Results: Male sex, distal third location, Borrmann type III, IV and histologically undifferentiated type were common. 5 year survival rate of the 190 patients was $22.2\%$. Depth of invasion and lymph node metastasis did not influence survival. The lymph node ratio (positive lymph node / retrieved lymph node) and combined resection affected survival by univariate and multivariate analysis. Conclusion: Combined resection and positive lymph node ratio were the independent prognostic factors in the patients with stage IV gastric carcinoma who underwent curative resection.

        Calcium sulfate제재가 치주인대세포에 미치는 영향

        Calcium sulfate has a long history of medical use as an implant material. The biocompatibiliry of the material has been clearly established. Bone ingrowth concomitant with resorption occurs rapidly with efficient conduction of bone from particle to particle. Calcium sulfate also has a potential for functioning as a good bamer membrane. The purpose of this study was to compare the biocompatibility of different types of calcium sulfate grafting materials including an expelimental calcium sulfate compound on periodontal ligament cells in vitro as a preliminary test towards the development of a more convenient and useful form of grafting material which could promote regeneration of periodontal tissue. Human periodontal ligament cells were collected from the premolar teeth extracted for orthodontic treatment. cells were cultured in a.MEM culture medium containing 20% FBS, at $37^{\circ}C$ and 100% humidity, in a 5% CO2 incubator. Cells were cultured into 96 well culture plate $1{\times}104$ cells per well with $\alpha$-MEM and incubated for 24 hours. After discarding the medium, those cells were cultured in $\alpha$-MEM contained with 10% FBS alone (control group), in medcal-grade calcium sulfate(MGCS group), in plaster(plaster group), experimental calcium sulfate paste(CS paste group) for 1, 2, 3 day respectively. And then each group was characterized by examining of the cell counting, MTI assay, collagen synthesis. The results \vere as follows. 1. In the analysis of cell proliferation by cell counting, both medical-grdde calcium sulfate group and plaster group showed no stastically significant difference at day 1, 2, 3 accept for plaster group at day 1 compared to control group, but there was stastically significant difference between CS paste group and all other groups at day 1, 2, 3(P<0.05). 2. In the analysis of cytotoxicity by MIT assay, both medical-grade calcium sJlfate group and plaster group showed no stastically significant difference compared to control group at day 1, 2, 3 but there was stastically significant difference between CS paste group and all other groups at day 1, 2, 3(P<0.OS). 3. In the analysis of collagen synthesis by immunoblotting assay, high level was detected for medical-grade calcium sulfate group and plaster group at day 1, 2, 3 compared to CS paste group. On the basis of these results, medical-grade calcium sulfate and plaster was shown to possess biocompatibility whereas the CS paste had unfavourable outcome. This observation shows a need for modification of the materials contained in calcium sulfate paste.

        The polycrystalline superconductor $SmBa_2$$Cu_3$$O_{x}$ is fabricated, and intergranular magnetic properties are investigated using the critical state model, from which some useful parameters such as the critical current density and the intergranular volume fraction are obtained. The curve fitting for M-H hysteresis loop shows that the intergranular critical current density of $SmBa_2$$Cu_3$$O_{x}$ / decreases in the form of ($1-T/T_{c}$ )$^{1.5}$ . The intergranular volume fraction is influenced by granular morphology. From SEM image, the grains of $SmBa_2$$Cu_3$$O_{x}$ are found to be randomly shaped. This mean:; that the intergranular volume fraction of $SmBa_2$$Cu_3$$O_{x}$ / should be smaller than those of superconductors, of which grains are plate-shaped such as Tl-based superconductor.

        레드비트 추출물의 항산화 활성 및 레드비트를 첨가한 돈육패티의 냉장저장 중 이화학적 성상 및 미생물의 변화

        본 연구는 인공합성 항산화제를 대신할 천연 추출물을 사용한 항산화제 개발을 목적으로 레드비트를 이용하였다. 먼저 항산화 활성을 알아보기 위하여 레드비트 에탄올 추출물(RBW)과 레드비트 물 추출물(RBW)을 각각(0.05-1.0%) 농도로 희석하여 항산화 활성을 평가하였다. 그 결과 DPPH 라디칼 소거능과 철이온 흡착력에서 레드비트물 추출물(RBW)이 레드비트 에탄올 추출물(RBE) 보다 유의적 차이를 보이며 더 높은 활성을 보였다(p<0.05). 특히 철이온 흡착력에서 RBW는 0.1% 농도부터 87.0%의 높은 활성을 보였고, 1.0% 첨가에서는 100%의 활성을 보였다. 또한 DPPH 라디칼 소거능에서도 0.25%부터 60% 정도의 높은 활성을 보였다. 반면, 환원력에서는 RBE가 유의적으로 더 높은 활성을 보였다(p<0.05). Linoleic acid 과산화 억제능에서는 두 처리구간 유의적인 차이를 보이지 않았지만(p>0.05), RBW는 모든 농도에서 83% 이상의 활성을 보였고, RBE는 0.05%에서 80% 정도의 활성을 보였다. 이를 토대로 0.5%의 RBE와 RBW를 각각 돈육패티에 적용하였고, 참조구로는 BHT 0.01%를 이용하여 저장기간 동안(0-14일) 이화학적 특성 및 미생물 검사를 실시하였다. 그 결과 백색도와 황색도는 처리구별, 그리고 저장기간에 따른 유의적인 차이를 보이지 않았으며(p<0.05), pH와 적색도는 저장기간이 길어짐에 따라 유의적으로 더 높아졌다(p>0.05). 미생물에서는 총균과 대장균군 모두 참조구가 다른 처리구와 유의적으로 낮은 값을 보였고(p>0.05), 저장기간이 경과함에 따라 유의적인 차이를 보이며 증가하였다(p>0.05). 그리고 지방산화 억제 활성을 평가한 TBARS에서는 냉장 저장 중 대조구(CTL)가 유의적으로 가장 높은 값을 보였고, 레드비트 처리구와 유의적인 차이를 보였다(p<0.05). 특별히 레드비틀 첨가한 TRT1과 TRT2는 저장기간 동안 REF와 유의적인 차이를 보이지 않아(p<0.05) 우수한 지방산화 억제능력을 보였다. This study was performed in order to evaluate the antioxidant activities of red beet extracts as well as the physicochemical properties and microbial changes of pork patties containing red beet during refrigerated storage. Red beet was extracted with water and ethanol. Red beet water extracts (RBW) and red beet ethanol extracts (RBE) were diluted with various concentrations (0.05~1.0%). DPPH radical scavenging activity and iron chelation activity of RBW showed a higher level than those of RBE (p<0.05). In particular, the iron chelation activity of RBW was over 53.4% at all levels. In addition, RBW at 1% had nearly 100% activity. On the other hand, the reducing powers of RBE were higher than those of RBW (p<0.05), and the antioxidant activity on linoleic acid emulsion of RBW was over 83% at all levels. Based on these model studies, 0.5% levels of RBW and RBE were added to ground pork patties (GPP), and the physicochemical properties and microbial changes of red beet GPP were evaluated during storage (0~14 d) at $4^{\circ}C$. The pH and microbial counts increased with increased storage time (p<0.05). Pork patties with BHT showed the lowest thiobarbituric acid reactive substances (TBARS) and microbial counts, and those with red beet had lower TBARS than the control (p<0.05). These results indicated that both red beet water and ethanol extracts could be used as natural antioxidants of pork patties during storage.

        Optomechanical Design of a Compact Imaging Spectrometer for a Microsatellite STSAT3

        A compact imaging spectrometer (COMIS) is currently under development for use in the STSAT3 microsatellite. COMIS images the Earth’s surface and atmosphere with ground sampling distances of ~30 m in the 18~62 spectral bands (4.0~1.05 μm) for the nadir looking at an altitude of 700 km. COMIS has an imaging telescope and an imaging spectrometer box into which three electronics PCBs are embedded. These are designed into a single assembly with dimensions of 35(L) × 20(W) × 12(H) cm3 and a mass of 4.3 kg. Optomechanical design efforts are focused on manufacturing ease, alignment, assembly, testing and improved robustness in space environments. Finite element analysis demonstrates that COMIS will survive in launch and space environments and perform the system modulation transfer function (MTF) in excess of 0.29 at the Nyquist frequency of the CCD detector (38.5 lines-per-mm).
