        라섹 수술 후 발생한 세균성 각막염

        Purpose: To report two cases of bacterial keratitis occurring after laser epithelial keratomileusis (LASEK). Case summary: Two patients were referred to our hospital because of ocular pain, conjunctival injection, and decreased visual acuity occurring 3 days after LASEK surgery. At that time, visual acuity was hand movement in both cases and they presented with corneal epithelial defects, stromal infiltration, and inflammation in the anterior chamber of their eyes. Patients were treated aggressively with hourly topical broad-spectrum fortified antibiotics after corneal scrapings were obtained. Gram stain was positive for gram negative bacilli in case 1 and culture was positive for Staphylococcus aureus in case 2. Both eyes responded well to treatment, resulting in residual corneal scarring with best corrected visual acuity of 20/25 in case 1 and 20/40 in case 2 after 12 and 6 months respectively. Conclusions: Infectious keratitis can occur after LASEK surgery usually during early postoperative period and early diagnosis and aggressive therapy should be followed through the close follow-up.

        임신부에서 발생한 포도막염녹내장의 수술적 치료 1예

        Purpose: To report a surgical treatment for a case of uveitic glaucoma during pregnancy. Case summary: A glaucoma female to be pregnant with 26 weeks visited for our clinic, who had used anti glaucoma medication. At the initial examination, intraocular pressures (IOP) of right and left eye were 38, 30 mmHg, respectively. We performed selective laser trabeculoplasty of both eyes with no complication, and then IOPs of right and left eyes were decreased to 26, 23 mmHg, respectively. 1 week later, she came to our emergency department due to acute visual impairment. Her IOP was 45 mmHg in right eye with uveitis. We regard that she had uveitic component related to the stress of fetal cardiac abnormality and recent her husband’s car-accident. Afterall she underwent trabeculectomy because of sustained high IOP. After surgery, IOP was decreased to 5 mmHg in right eye with elevated bleb but anterior chamber was shallow. We waited for 3 days because overfilteration would be resolved. After all, we did bleb revision with air injection in anterior chamber. After surgery, that IOP was persisted 10-16 mmHg with cystic bleb. Conclusions: This study showed the course of glaucoma during pregnancy was various, and emotional stress can lead uveitic glaucoma in contrast to the studies showed an association with pregnancy and lower IOP. Also, gentle manipulation is needed for pregnant women, because of thinner and softer sclera in high myopia and pregnancy itself.

      • 백반증 환자의 정서상태와 성격특성

        연구목적 : 백반증 환자의 치료에서 간과되어지고 있는 심리적인 요인이 치료에서 중요하게 다루어져야 한다는 것을 제시하기 위하여 첫째 백반증 환자에서의 우울과 불안의 정도 및 성격유형이 어떠한가를 알아보고, 둘째 이 결과를 정상 대조군과 비교함으로써 환자들의 정서상태 및 성격특성을 고려하여 이들의 스트레스나 정신과적인 문제에 어떻게 접근하는 것이 도움이 될 수 있는가를 알아보고자 하였으며, 마지막으로 환자들의 특성에 따른 차이를 알아봄으로써 이를 고려한 치료방법을 모색하고자 하였다. 방법 : 21명의 백반증 환자와 정상인 20명을 대상으로 Beck의 우울 검사, Spielberger의 상태-특성 불안검사 및 염태호와 김정규가 한국판 표준화한 Cattell의 16 성격요인검사를 시행하였다. 결과 : 백반증 환자군과 정상 대조군 간에 우울과 불안에서 차이를 보이지 않았고, 성격특성에서는 백반증 환자군이 정상 대조군보다 소심하지만 고집이 센 것으로 나타났으며 다른 성격특성에서는 차이가 없었다. 그리고 백반증 환자들의 특성에 따른 비교에서는 여성 환자들이 더욱 민감한 것으로 나타났으며 미용 상 문제가 되는 병변 유무와 진행유무는 특별한 영향을 미치지 않는 것으로 나타났지만, 30세 이상의 환자들이 지배적, 도덕적이며 의심이 많고 사변적이며 자가 충족적이고 통제적이고 독립적이며 강한 초자아를 가지고 있으며 창조적인 것으로 나타났다. 그러나 유병기간은 유의한 상관관계를 보이지 않았다. 결론 : 백반증 환자들은 정상인에 비해 대인관계에서 소심하면서 고집이 샌 것으로 나타나 나이에 맞는 성격 발달을 하고 있지만 대인관계에서는 문제점이 있는 것으로 나타났다. 백반증 환자들의 치료에 있어서 대인관계에서의 문제 해결에 도움을 줄 수 있는 정신과적인 접근을 피부과적인 치료법과 병용함이 유익하리라는 것을 제시한다. Objectives : This study was aimed to investigate the emotional state and personality factors of patients with vitiligo, and the impacts of clinical characteristics. Methods : Twenty one patients with vitiligo and 20 normal controls were studied. Beck Depression Inventory(BDI), Spielberg's State and Trait Anxiety Inventory(STAI) and Cattell's 16 Personality Factors Questionnaire(16-PF) were used for assessment. Statistically, t-test, Oneway ANOVA, Mann-Whitney U test and Pearson's correlation test were used. Results : The Results were as follows. 1) The comparison of total score of BDI, state anxiety and trait anxiety between the patients with vitiligo and normal controls were not significantly different. 2) In first-stratum source traits of 16-PF, normal control group was significantly higher than the patients with vitiligo in H-factor(Boldness) and I-factor(Tender-mindedness). In second-stratum source traits of 16-PF, there was no significant difference between the patients with vitiligo and normal control in all factors. 3) According to sex, the comparison of total scores of BDI, STAI between male and female patients with vitiligo were not significantly different. In the comparison of 16-PF, female patients were higher than male patients in I-factor only. 4) As according to age(under and over 30 years of age), the comparison of total scores of BDI, STAI between patients over 30 years of age and under 30 years of age were not significantly different. In the comparison of 16-PF, patients over 30 years of age were higher than patients under 30 years of age in E-, G-, L-, M-, Q2-, Q3-, IND-, SUP-, and CRE-factor. 5) As according to the presence of progression of vitiligo and the presence of visible lesion, the comparison of total scores of BDI, STAI and 16-PF were not significantly different. 6) There were no significant correlation between the duration of the vitiligo and BDI, STAI-S, STAI-T, and 16-PF. Conclusions : There were no significant difference between patients with vitiligo and normal controls on the emotional state and personality characteristics. But patients with vitiligo were more bolder and tender-minded than normal controls on 16-PF. The author propose that methods of treatment to resolve problems of the interpersonal relations will be helpful.

        졸-겔법에 의한 $(Pb_{0.9}Ca_{0.1})TiO_3$ 박막의 강유전 특성

        The $(Pb_{0.9}Ca_{0.1})TiO_3$[PCT] thin films have been deposited by sol-gel processing on Si-wafer and ITO glass substrates. The creak-free films have been obtained by rapid thermal annealing at $700^{\circ}C$ for 10 minute and characterized by XRD, SEM and electrical measurements. Their tetragonality c/a was 1.041 and grain size was $0.15{\sim}0.2{\mu}m$. When the electrode system of sample was Au/PCT/ITO(MFM) and film thickness was $0.8{\mu}m$, dielectric constant, dielectric loss and Curie temperature were about 149, 0.085 and $449^{\circ}C$ at 10kHz, respectively. Spontaneous polarization $P_s$, remnant polarization $P_r$ and coercive field $E_c$ were about $5.29{\mu}C/cm^2$, $4.15{\mu}C/cm^2$ and 82kV/cm calculated by hysteresis loop.

        생강(Zingiber officinale Roscoe) 부위별 이화학적 특성 및 항산화 활성

        생강을 기능성 소재로 이용하기 위하여 생강 부위별 각종 영양성분 분석 및 용매 추출을 달리하여 항산화능 비교 분석, 유효성분으로 gingerol류, shogaol류 및 catechin류를 분석하였다. 일반성분은 생강 잎, 줄기, 뿌리 모두 탄수화물과 회분이 주된 구성성분이었으며, 생강 뿌리에 비해 잎, 줄기에서 조섬유의 함량이 4∼5배 유의적으로 높은 값을 보였다(P<0.05). 그리고 환원당 함량은 뿌리에 비해 줄기에서 약 3배 정도 높았다. 조사포닌 함량은 줄기< 뿌리< 잎 순으로 함량이 높았으며, 특히 뿌리보다 잎에서 1.7배 높았다(P<0.05). 무기질 조성 및 함량은 생강 부위에 따라 큰 차이를 나타내었다. 줄기의 Sr, Ca, K 함량은 뿌리보다 4배 이상 높은 값을 보였으며, 잎의 Ca 함량은 뿌리보다 5배 유의적으로 높은 값을 보였다(P<0.05). Gingerols, shogaols 및 capsaicin 함량은 생강 뿌리에서 7종 성분 모두 확인되었으며, 잎과 줄기에서도 6-gingerol, 10-gingerol, 8-shogaol을 확인할 수 있었다. 특히 8-shoaol은 뿌리보다 잎에서 5배 높은 함량을 나타내었다. 지방산 함량 및 조성분석에서는 지방산 중 capric acid는 생강 뿌리에서만 검출되었으며, arachidic acid는 잎, 뿌리에서만 검출되었다. 잎은 줄기, 뿌리에 비해 linoleic acid, linolenic acid의 필수지방산 함량이 높았고, 뿌리보다 약 5배 높은 함량을 보였다. 총 페놀함량 측정 결과, 메탄올 추출물은 뿌리< 잎< 줄기 순으로 함량이 높았으며, 분획에서 잎은 물층에서 43.9 g/kg, 줄기는 에틸아세테이트에서 65.5 g/kg, 뿌리는 헥산에서 187.2 g/kg으로 높은 함량을 나타내었다. 그리고 총 플라보노이드 함량은 잎, 줄기의 모든 분획물은 뿌리의 분획물 함량과 비슷하거나 높은 함량을 보였다. Catechin류 함량은 메탄올 추출물에서 뿌리< 잎< 줄기 순으로 높았으며, 특히 줄기는 뿌리에 비해 약 30배 높은 함량을 보였다. 항산화 활성은 생강의 잎, 줄기에서도 높은 폴리페놀 함량을 보인 분획물에서 높은 항산화 활성을 보였다. 이 결과로 보아 생강은 뿌리 단독보다는 뿌리와 잎, 줄기를 모두 활용하는 것이 바람직하나 잎의 경우 식용으로 인정되어 있지 않아 이를 활용하기 위한 허가 추진 및 제도 개선이 필요하다. The physicochemical characteristics and antioxidant activities of leaf, stem, and root of ginger (Zingiber officinale R.) were determined. Nutrient composition, reducing sugar, saponin, mineral, heavy metal, total phenolic and total flavonoid contents, and antioxidant activities based on DPPH radical scavenging and FRAP assay were measured. Catechins, gingerols, shogaols, and capsaicin compositions were also determined by HPLC. The contents of water, proteins, fats, carbohydrates, fiber, and ash from ginger root were 6.4, 6.8, 3.2, 65.4, 7.3, and 18.2%, respectively. Crude fiber contents of leaf and stem were 4~5 times higher than those of root (P<0.05), and reducing sugar content of stem was about 3 times higher than those of root. Crude saponin contents were in the order of stem< root< leaf, and saponin content of leaf was 1.7 times higher than that of root (P<0.05). Mineral contents and composition varied among different parts, and Ca contents of leaf and stem were 5 times higher than that of root. The 6-gingerol, 10-gingerol, and 8-shogaol contents of leaf and stem were identified by HPLC, and 8-shogaol content of leaf was 5 times higher than that of root (P<0.05). The major fatty acids in ginger were palmitic acid, oleic acid, linoleic acid, and linolenic acid. Lauric acid was only detected in root, arachidic acid was not detected in stem, and the linoleic acid content of leaf was 5 times higher than that of root. The total catechin content of methanol extract was in the order of root< leaf< stem, and stem was about 30 times higher than that of root. Antioxidant activity was highly correlated with total phenolic and total flavonoid contents. Based on these experiments, it can be concluded that leaf, stem, as well as root of ginger can be used as natural preservatives, and they can be developed as healthy foods due to their antioxidant activities and abundant polyphenols. This study suggests that the whole ginger plant, including root, leaf, and stem, might be new functional vegetables.
