From January 1994 to August 1995 we performed 270 cases of fine needle aspiration cytology for bone and soft tissue lesions. Among them 137 cases were diagnosed histologically. Sixty-seven cases were benign and 70 cases were malignant. We analysed these on the aspect of sensitivity, specifity, positive predictive value, negative predictive value of the cytologic examination. Malignant cases consisted of bone tumor 29 cases; soft tissue tumor 25 cases; and metastatic tumor 16 cases. The sensitivity and specifity of the aspiration cytology for malignant tumor were 67.8% and 97%, respectively. The positive and negative predictive value of aspiration cytology were 97.4% and 72.7%, respectively. Among them 40 cases were primary bony lesions; 14 benign lesions, 26 malignamt bone tumors. Sensitivity of aspiration cytology for these primary bony lesion was 90%(18/20) and its specifity was 100%(13/13). Three cases showed atypical cells in cytology but with biopsy we could find them as one benign and two malignancies. Two cases of manignancy showed negative result in cytology; one was parosteal sarcoma of proximal femur and the other was osteosarcoma of proximal tibia which was biopsied already at other hospital. Although the overall sensitivity and specifity for malignant bone and soft tissue tumors were relatively low and not so predictable, the fine needle aspiration cytology for primary bony lesion was useful in the differential diagnosis of benign or malignancy.
전이성골암에 의한 통증에 대한 Salmon Calcitonin($Miacalcic^{(R)}$) 비강분무의 효능 및 안정성

Metastaic bone tumors are usually accompanied with severe pain. The treatment modalities for this pain are so variable that patients are sometimes afraid of using them. Salmon calcitonin has a function to increase beta-endorphines followed by increasing the blood level of prostaglandin and thromboxan A2, which results in analgesic effect. This drug also has been known to decrease bone resorption. There were a few reports that parenteral use of salmon calcitonin decrease the pain from metastatic bone tumor. We wanted to know the effectiveness and tolerability of nasal spray of salmon calcitonin in relieving bone pain with metastatic tumor. We analyzed the effectiveness in the aspects of pain, sleep, performance status, mobility, supplementary analgesic use. The biologic effect of salmon calcitonin was analysed with CBC, Ca/P, BUN/Cr, uric acid. Simple radiography, alkaline phosphatase, osteocalcin, pyrilink-K were used as parameters for bone change. Eighteen cases of metastatic bone tumors took nasal spray of salmon calcitonin($Miacalcic^{(R)}$, 200IU/day) for 4 weeks, to relieve bone pain. With Wilcoxon Matched-Pairs Signed Ranks Test, we could find pain decreased significantly at 3 week and mobility become improved at 4 week of salmon calcitonin use. Other parameters didn't show any significant changes. We think the analgesic effect is mainly due to effect not on the local bone lesion but on the central nervous system, and that increased dose of salmon calcitonin can induce earlier and stronger analgesic effect.

ICRU 구(球)를 피사체(被射體)로 하여 300 keV 중성자(中性子)의 방사선량(放射線量) 관계량(關係量)을 평가(評價)하였다. 피사체내(被射體內)의 선량당량(線量當量) 분포(分布)를 직접(直接) 산정(算定)하기 위해 중성자(中性子)-광자(光子)-하전입자(荷電粒子) 결합수송(結合輸送)을 다룰 수 있는 몬테칼로 코드 NEDEP을 사용하였다. 계산결과(計算結果) 얻은 방사선량(放射線量) 관계량(關係量)은 다음과 같다. 심부선량당량지수(深部線量當量指數) $H_{I,d}:1.78{\times}10^{11}\;Sv-cm^2$ 표층선량당량지수(表層線量當量指數) $H_{I,s}:2.08{\times}10^{-1}\;Sv-cm^2$ 주위선량당량(周圍線量當量) $H^*(0.07):1.70{\times}10^{-11}\;Sv-cm^2$ 주위선량당량(周圍線量當量) $H^*(10):1.78{\times}10^{-11}\;Sv-cm^2$ 실효선질계수(實效線質係數) $\bar{Q}^*(10):12.4$ Dosimetric quantities for 300 keV neutrons in the ICRU standard tissue sphere were evaluated. The Monte Carlo code NEDEP which performs neutron-photon-charged particles coupled transport was used in the direct estimation of absorbed dose and dose equivalent. Protection against non-ionizing radiation(NIR) is the subject of an increasing interest but the use of very different concepts depending on the type of radiation or application, makes it rather difficult to compile studies and the data obtained in an uniformity in this field. The main object of the present paper is to summarizes and provide an inventory of concepts, quantities, units and terminology currently used for purposes of NIR protection. Furthermore a systematic classification and comparison of these quantities is given, and in particular the concepts used to quantify exposure limitation and radiation protection standards are summarized and discussed.

This paper presents critical buckling loads of laminated composites under cylindrical bending. In-plane displacements are assumed to vary exponentially through plate thickness. The accuracy of this theory is examined for symmetric/antisymmetric cross-ply, angle-ply and unsymmetric laminates under cylindrical bending. Analytical solutions are provided to investigate the effect of transverse shear deformation on critical buckling loads of the laminated plates, and the results are compared with those obtained from the first-order shear deformation plate theory and the classical laminated plate theory.

Purpose : Dermatofibrosarcoma protuberans(DFSP) is a rare tumor of the skin with a strong tendency of infiltration to surrounding tissues. Inadequate surgical intervention brings about frequent recurrence and poor prognosis. We attempted to find a guideline for adequate treatment for DFSP. Materials and Methods : Fourteen cases who had been treated in our department since Mar. 1993 and followed up for more than 12 months postoperatively were reviewed. Including nine cases who were transferred from other hospital after recurrence, thirteen cases underwent wide resection. One case was treated by intralesional resection followed by chemotherapy (CYVADIC) due to neurovascular abutment to the mass in the inguinal area. Results : The nine cases who were transferred due to recurrences experienced recurrence in average 1.3(1-2) times and the average period until first local recurrence from primary operation was 11.8(2-24) months. The thirteen cases with wide surgical margin showed no recurrence at the final follow up. One case treated by intralesional resection and chemotherapy showed multiple recurrence and died of the disease due to lung metastasis. Conclusions : From these data, we could find that primary wide resection can be the way of reducing recurrence and metastasis, and the follow up period for the detection of recurrence should be at least two years.

This is a study on the optimization of machining condition considered cutting forces. In this paper, first of all, generate NC codes by using PowerMill. secondly, optimize machining conditions by using Production Module of AdvantEdge which is the cutting force analysis program, and then we are going to cut the MC NYLON as a material by using a small-scale MC. Finally, By comparing each CAD data with before optimization and after optimization of data by using 3D scanner to confirm the cutted gear contour form, execute necessary cutting gear contour when manufacturing form related the gear, optimize machining condition and present the method which can improve productivity.

Auto transmission is a device which accomplishes upshift and downshift alternatively for converting the output of engine properly according to driving conditions. Many researches about AT has been progressed, but many people focus their researches on upshift, not downshift. In this study, downshift control algorithm is developed. First, mathematical modeling of target system and simulation program is developted and verified by experimental results.<br/> In order to get the improvement of shift quality, control algorithm is designed to increase output torque linearly during inertia phase. To do so, engaged clutch pressure and disengaged clutch pressure is controlled using virtual sensor. Because the target system has clutch-to-clutch shift type transmission, independent control of each clutch is possible. So MISO control which controls engaged clutch and disengaged clutch at the same time is used to control one output element, output torque. The controller is designed using fuzzy logic control, and the performance of the controller is evaluated by simulation.
