        약제 내성을 지닌 페니실리움 안내염에서 유리체강내 보리코나졸 주입술을 이용한 치료 1예

        Purpose: To report a case of a drug-resistant Penicillium endophthalmitis that was treated with intravitreal voriconazole injection. Case summary: A 73-year-old male who developed endophthalmitis after cataract surgery was treated with intravitreal vancomycin and ceftazidime injection after aqueous and vitreous cultures were done. The condition were not improve after intravitreal injection, and the culture showed Penicillium species. Fungal endophthalmitis was diagnosed and pars plana vitrectomy, intraocular lens removal, and intravitreal amphotericin B injection were performed. However, a progressive worsening of the ocular condition was observed, and the patient was transferred to our hospital. At initial visit, the best corrected visual activity was hand movement. The hypopyon and thick exudative membrane filled the anterior chamber. Pars plana vitrectomy and intravitreal voriconazole injection were performed under the diagnosis of drug-resistant Penicillium endophthalmitis. The condition improved after the operation, and there was no recurrence of endophthalmitis until four months after vitrectomy. Conclusions: Voriconazole is useful in the treatment of drug-resistant fungal endophthalmitis.

      • 도재치은 보철법에 의한 심미적 임프란트 상부구조의 제작

        This article described a procedure for fabricating an esthetic gingival porcelain restoration as an implant-supported fixed prosthesis for edentulous maxilla. Alternative treatments for fully edentulous patients include an implant-supported overdenture or a fixed implant-supported prosthesis with bilateral distal cantilevers. But, from a functional and biomechanical point of view, the fixed implant-supported prosthesis with posterior cantilevers or implant-supported tissue-borne overdenture do not significantly improve masticatory effectiveness compared with a distributed implant restoration as a fixed implant-supported prosthesis. The fact that the prosthesis is supported by distributed implants over eight for edentulous maxilla in general, provides increased masticatory efficiency as a fixed restoration and similar gingival appearance with esthetic gingival porcelain. It is also detachable by dentist to allow easier after-care of soft tissue and the prosthesis.

        단백질 S 결핍 환자에서 발생한 망막중심동맥폐쇄

        Purpose: To report a case of central retinal artery obstruction (CRAO) caused by protein S deficiency. Case summary: A 36-year-old man presented with acutely reduced visual acuity in the right eye. The patient had no other diseases such as diabetes mellitus or hypertension. The visual acuity was 20/20 in the right eye and light perception in the left eye. Afferent pupillary defect was observed in theleft eye. Fundus examination showed diffuse retinal opacification and a cherry-red spot at the macula. Thus the patient was diagnosed with CRAO in the left eye. There were no abnormal findings on routine hematologic tests including blood coagulation test. Protein C activity, antithrombin III activation, anticardiolipin antibody were within normal limits but protein S activity was decreased. Thus, the patient was diagnosed with CRAO with protein S deficiency. Conclusions: Coagulation factors such as protein S need to be measuredin cases of CRAO in young patients without medical problems.

        습관적(習慣的) 저작(咀嚼)과 저작근(咀嚼筋)의 동통유발(疼痛誘發)과의 관계에 대한 근전도학적(筋電圖學的) 연구(硏究)

        The purpose of this study was to investigate electromyographically the relationship between preferred chewing side and side of initial muscle pains. In this study, 20 normal healthy subjects were selected , and each subject chewed randomly chewing gum for 20 minutes to establish preferred chewing side. To induce initial muscle pains, biting force of 10Kg on the gnathodynamometer was maintained by the subjects. And the Bioelectric processor EM2(Myo-Ironies Research, Inc. U.S.A.) with the surface electrodes was used to record the EMG activity during all experimental procedures. The results were as follows; 1. A majority of the present subjects (60%) had a preferred chewing side, but with few exceptions, subjects were unable to explain why a given side was preferred; explanations were only 'comfort' and 'habit' 2. The chewing, or working side was determined largely by the mean voltage of the surface electromyogram (EMG); in comparison with EMG from the non-wlring (contralateral) side, the working (ipsilateral) side showed a higher amplitude. 3. After the effort, the right masseter muscle is the most frequent site of pains, followed by the left masseter muscle, the anterior part of the right temporalis muscle and tile anterior part of the left temporalis muscle. 4. After the effort, mean voltages of masseter muscles were slightly increased, but mean voltages of temporalis anterior were slightly decreased at physiologic rest position. 5. No relationships could be established between preferred chewing side and side of initial muscle pains.

        부순모래를 사용한 콘크리트의 현장 품질 특성에 관한 실험 연구

        This study is to investigate the properties of concrete with crushed sand on site and to propose a quality guideline for its use as artificial sand and concrete. From our experimental result in laboratory and site, we found that demand water of concrete with crushed sand for target slump increased by 18kg/m3 compared to mixed sand and l8kg/m3 compared to sea sand respectively. The compressive strength increased by around 3∼6% when compared to concrete with sea sand. Accordingly, our study showed that the combined sand mixed with sea sand would be desirable to obtain workability and strength of concrete including dry shrinkage and bleeding test. Furthermore, the optimal replacement percentage of crushed sand was 50% with sea sand. As such, crushed sand would be sufficient as fine aggregate for concrete in terms of economic efficiency and quality. Crushed sand, on the other hand can only be used as fine aggregate when VFS(Very Fine Sand) is below 3.5 percentage of weight of sand and particle shape is above 55 percentage. Also, the particle shape and microsand passing NO.200 sieve should continually be improved to increase workability of concrete on site.

        한국 운동선수의 스포츠 치의학에 대한 인지도 및 치아교모상태에 관한 연구

        This study was accompolished to analyze and compare the number and area of the occlusal contact points in healthy volunteers and athletes with normal occlusion. Before the study, 89 athletes(sports career:8.6 years, average age 20) at Kyung Hee University were selected, and survey was accomplished for athlete's recognition about sports dentistry. For this study, 15 athletes(13 amles and 2 females with average age 20) and 21 healthy volunteers(14 mles and 7 females with average age 20.09) at Kyung Hee University were selected. The visual display acquired by scanning of occlusal record in maximal intercuspation was converted into 16 gray value image. Then, using computer program(J & Lee Occlusal Analyser), the pixel which was in definite range of the gray value was recognized, and the numbers of recognized pixel were calculated to area. The results were as follows : (1) On the survey about sports dentistry, 28% of 89 athletes didn't agree that human occlusion may be important, and 30% didn't have any idea of the influence of human occlusion during their sports activities. (2) The average numbers of total occlusal contact points were 31.05 in control group, and 34.67 in athlete group. The average area of total occlusal contacts was $100.25mm^2$ in control group, and $127.78mm^2$ in athlete group. (3) In control group, the average numbers of occlusal contact points were revealed in order as follows; the first molar(8.48), the second molar(8.24), the second premolar(4.71), the lateral incisor(2.90), the first premor(2.43), the central incisor(2.19), and the canine(2.1). The least average in canine(2.1) was similar to the average(2.19) in central incisor and (2.09) in lateral incisor. In athlete group, the average numbers of occlusal contact points were revealed in order as follows; the first molar(8.87), the second molar(8.47), the second premolar(5.60), the canine(3.80), the lateral incisor(3.33), the first premolar(2.67), and the central incisor(1.93). (4) In control group, the average areas of occlusal contact surface were revealed in order as follows; the first molar($39.47mm^3$), the second molar($37.54mm^3$), the second premolar($9.54mm^3$) the first premolar($6.18mm^3$), canine($3.49mm^3$), the central incisor($2.76mm^3$), and the lateral incisor($1.28mm^3$). In athlete group, the average areas of occlusal contact surface were revealed in order as follows; the first molar($44.11mm^3$), the second molar($40.69mm^3$), the second premolar($16.50mm^3$), the first premolar($9.39mm^3$), the canine($5.08mm^3$), the lateral incisor($3.7mm^3$), and the central incisor($2.25mm^3$). (5). With aging in both control and athlete group, there was a decreasing tendancy in average number of occlusal contact point, and was an increasing tendancy in average area of occlusal contact surface. In comparison at each age, both the numbers and area of occlusal contact were greater in athlete group than in control group. It was not significant in the numbers of occlusal contact points beween athlete and control group(p>0.1), but significant in the area of occlusal contact surface(p<0.05). (6) In comparision as to the kind of sports(Gymnastics:2, Rugby:3, Soccor:5, Ice hocky:5), the numbers of occlusal contact points were the most in ice hocky, and the area of occlusal contact surface was the greatest in gymnastics. With increasing a career in athlete group, there was a decreasing tendancy in average numbers of occlusal contact points, and was an increasing tendancy in average area of occlusal contact surface.

        안와 외상 후 발생한 하사근 기능항진 환자 1예

        Purpose: To report a case of inferior oblique muscle overaction after orbital trauma. Case summary: A 19-year-old male presented with a blowout fracture of the right inferior orbital wall. The patient had no history of facial asymmetry, head tilt, strabismus or diplopia. The day after the reduction operation, the patient complained of diplopia on the down-gaze. The patient had orhthophoria in the primary position. However, the right eye showed a limitation on infraduction. Six weeks later, the limitation of infraduction resolved, however the diplopia worsened. The right eye showed excessive elevation in adduction and hypertropia in the primary position. The hypertropia increased on left gaze, and decreased on right gaze. Bielschowsky’s head test revealed a negative result. The fundus photographs showed a mild excyclotorsion of the right eye. Five months later, the symptoms and signs were stable and surgery was performed. Under general anesthesia, the exaggerated forced duction test and traction with strabismus hook on the right inferior oblique muscle revealed tightness. The inferior oblique muscle was found to be recessed. After the operation, the diplopia, hypertropia and the excessive elevation on adduction of the right eye resolved. Conclusions: A contracture and resultant overaction of inferior oblique muscle could develop after trauma on the inferior orbital wall. Trauma on the inferior orbital wall should be considered as one of the primary causes of acquired inferior oblique overaction. J Korean Ophthalmol Soc 2011;52(5):639-643

        BES 프로그램을 이용한 국내 대표적 대형온실의 에너지 부하 예측

        Reliable estimation of energy load inside the greenhouse and the selection of cooling and heating facilities are very important preceding factors to save energy as well as initial and maintenance costs of operating a greenhouse. Recently, building energy simulation (BES) technique to simulate a model similar to the actual conditions through a variety of dynamic simulation methods, and predict and analyze the flow of energy is being actively introduced and developed. As a fundamental research to apply the BES technique which is mainly used for analysis of general buildings, to greenhouse, this research designed four types of naturally-ventilated greenhouses using one of commercial programs, TRNSYS, and then compared and analyzed their energy load properties, by applying meteorological data collected from six regions in Korea. When comparing the greenhouse load of each region depending on latitude and topographical characteristics through simulation, Chuncheon had nearly 9~49 % higher heating load per year than other regions, but its annual cooling load was the reverse to it. Except for Jeju, 1-2W type greenhouses in five regions showed about 17 % higher heating load than a widespan type greenhouse, and 1-2W type greenhouses in Chuncheon, Suwon, Cheongju, Daegu, Cheonju and Jeju had 23 %, 20 %, 17 %, 16 %, 18 % and 20 % higher cooling load respectively than a wide span-type one. Glasshouse and vinyl greenhouse showed 8~11 % and 10~12 % differences respectively in heating load, while 2~10 % and 7~10 % differences in cooling load respectively.

        악관절의 자기공명영상과 시상단층 방사선촬영상에 관한 비교연구

        For understanding of anatomy, physiology, and diseases of human TMJ, it is required to evaluate quantitatively the movement of the disc and condyle head of mandible. The histologic section of cadaver TMJ were examined, and the magnification of the MR image and its details of anatomy were evaluated. And then a quantitative analytic method, by comparing the Sectograph and the MR image of vital human TMJ, was proposed. For this study, 15 subjects(Male, 24~35years) were selected from a prosthodontic examination randomly, and each subject’s five interocclusal rubber registration records were made on the ICP, and 5, 10, 15, and 20mmjaw opening positions. All subjects were radiographed with a Denar Quint Sectograph Image System(Denar Corp., USA), and imaged with a MRP-20EX MR Image System(0.2T, Permanent Magnet Type, Hitachi Medical Corp., Japan) using an 100mm diameter bilateral type surface coil. These images were traced on the acetate tracing paper, and analyzed In this study, the findings led to the following conclusions. 1. In comparison of the histologic section of autopsy specimen with the MR image at the same section, the size(dimension) of MR image was 70% of the real one. It was possible to recognize the shape of articular disc, anterior and posterior attachments, and adjacent soft tissues, because of the excellent reproducibility of anatomical structure. 2. When we compared the amount of joint space on MR image with that of joint space on sectograph, the amount of joint space on sectograph was significantly greater than that of joint space on MR image, except at the top of condylar head. 3. The position of minimum joint space on sectograph at intercuspal position didn't coincide with the middle position of articular disc on MR image, and was approximately in the anterior third of posterior band of articular disc. 4. The amount of condylar movement on MR image at opening movement was greater than that of articular disc movement. From Intercuspal position to 5mm jaw-opening movement, the condylar movement showed hinge one, and over the range 5mm jaw-opening it suggested hinge & translatory one. 5. In terms of area variation of articular disc measured on MR image in sagittal plane, the area of posterior band increased with increasing the amount of Jaw opening, but the area of anterior band decreased conversely.

        터슨 증후군에서 발생한 전층황반원공

        Purpose: To report a case of a full-thickness macular hole caused by Terson syndrome. Case summary: A 49-year-old man presented to our clinic with decreased visual acuity in the right eye after a subarachnoid hemorrhage due to spontaneous rupture a middle cerebral artery aneurysm. On the first examination, the corrected visual acuity was hand movement in the right eye and 1.0 in the left eye. There was massive vitreous hemorrhage in the right eye. In the left eye, there was no abnormal ocular finding. He was diagnosed with vitreous hemorrhage due to Terson syndrome and waited for the spontaneous absorption of the vitreous hemorrhage. Six months later, the vitreous hemorrhage was considerably absorbed, but a macular hole was observed. OCT scans showed a full-thickness macular hole with an epiretinal membrane.
