Osteoclasts play a critical role in bone-related diseases such as osteoporosis and rheumatoid arthritis by resorbing the bone. Recently, natural products from plants have been extensively studied as therapeutic drugs to treat and prevent various diseases. Here, we examined the effects of rhizoma arisaematis on ostoclast differentiation and bone resorption. We showed that rhizoma arisaematis significantly suppressed receptor activator of nuclear factor-κB ligand (RANKL)-induced osteoclast differentiation in bone marrow-derived macrophages (BMMs) in a dose dependent manner but have little or no effect on the cytotoxicity of BMMs and RAW264.7 cells. We found that rhizoma arisaematis iarrow-ed the RANKL-induced c-Fos and nuclear factor of activated T cells (NFAT)c1, which is a master regulator of osteoclast differentiation. Furthermore, rhizoma arisaematis suppressed the mRNA expression of tartrate resistant-acid phosphatase and cathepsin K iaduced by RANKL in BMMs. in y chanistic studies, rhizoma arisaematis considerably iarrow-ed I-κB degradation, which is a negative regulator of NF-κB, but iaduced the phosphderlation of p-38, ERK, and JNK.MMlso, we found that rhizoma arisaematis significantly iarrow-ed osteoclastic bone resorption. Taken tarether, our results suggest that rhizoma arisaematis suppresses osteoclast differentiation through down-regulatd the mRANKL-induced c-Fos and NFATc1 expression and iarrow-s bone resorption.

Objective: To investigate the relationship between serum trace element levels with disease activity in Korean patients with rheumatoid arthritis (RA). Methods: The serum levels of zinc, copper and ceruloplasmin were measured by inductively coupled plasma atomic emission spectrometers in 80 patients with RA, 26 osteoarthritis (OA), and 30 healthy controls (HC). We also measured the levels of zinc and copper in the synovial fluid (SF) of patients with RA. We investigated the clinical parameters simultaneously obtained at sampling of serum and analyzed correlation between serum levels of trace elements and disease activity in RA. Results: In RA, the levels of serum zinc were significantly lower than that of HC, and the levels of serum copper and ceruloplasmin were significantly higher than those of HC. In active RA, the levels of serum zinc were more decreased, and the levels of serum copper and ceruloplasmin were more increased than those of inactive group of RA. The levels of both copper and ceruloplasmin showed positive correlation with the levels of serum ESR and CRP. On the other hand, the levels of serum zinc showed negative correlation with the levels of serum ESR and CRP. Conclusion: Serum zinc levels are significantly lower and serum copper levels significantly higher in patients with active RA and these trace elements were useful parameter of disease activity in RA.
본 연구는 섬유근통증후군 환자의 질병관련 특성과 일상생활 장애의 정도를 파악하고, 그 상관관계를 확인하기 위하여 시도되었다. 대상자는 I시에 소재한 1개 대학병원 류마티스 내과 외래 방문 환자 중 섬유근통증후군 진단을 받은 45명으로, 구조화된 설문지를 이용하여 자료를 수집하였다. 대상자의 압통점은 평균 15.00, 피로는 8.67, 수면의 질은 12.94, 우울은 23.70점이었고, 이들은 평균 대략 6년간의 유병기간을 가지고 있었다. 42.2%의 대상자들이 질병으로 인해 직업이나 학업을 중단하였으며, 지난 3개월동안 평균 41.79일 일상생활 장애를 경험하였다. 일상생활 장애 정도는 압통점, 우울과 양의 상관관계가 있었고, 피로와 우울, 수면의 질과 양의 상관관계가, 수면의 질은 우울과 양의 상관관계가 있었다. 결론적으로 섬유근통 증후군 환자들은 질병과 관련한 다양한 증상을 동반하며, 이로 인한 일상생활 장애를 경험하고 있었다. 따라서 이들이 일상생활을 유지하면서 보다 나은 삶을 유지할 수 있도록 관련 요인을 반영한 중재프로그램을 개발할 필요가 있다. The purpose of this study was to identify the correlations between disease related factors and disturbance of daily living in patients with fibromyalgia. A descriptive survey study design was used. Data were collected from 45 out patients diagnosed of fibromyalgia according to the conditions by American College of Rheumatology in I-city. The mean score of tender points were 15.00, fatigue was 8.67, quality of sleep was 12.94 and depression was 23.70. They have struggled with fibromyalgia for 6 years on an average. Among patients, 42.2% had to discontinue their job/learning due to their disease, and experienced a disturbance of daily living for 41.79 days during last 3 months. There were significant positive correlations between disturbance of daily living and tender points, depression, between fatigue and depression, quality of sleep, between quality of sleep and depression. As a result, fibromyalgia patients suffered from various disease related factors and disturbance of daily living. Therefore, it can be suggested that the consideration of these related factors is important to develope intervention program for the better qualified life of the patients.

Osteoporosis is social problem around the world, because fracture of old age may lead to critical medical condition. Osteoclast is a main target for prevention and treatment of osteoporosis due to their responsibility for bone resorption. Saussureae Radix has been known that has gastro-protective, bronchodilatory effect and has a anti-biotic effect. Saussureae Radix has been widely used in Oriental medicine. However, the effect of extract of Saussureae Radix in osteoclast differentiation remains unknown. Thus, we examined the effect of Saussureae Radix in receptor activator of nuclear factor-κB ligand (RANKL)-induced osteoclast differentiation. From the results of our study, Here we found that Saussureae Radix significantly inhibited osteoclast differentiation induced by RANKL. Saussureae Radix suppressed the activation of NFκB in bone marrow macrophages (BMMs) treated with RANKL. Also, Saussureae Radix significantly inhibited the mRNA expression of c-Fos, tartrate-resistant acid phosphatase (TRAP), osteoclast-associated receptor (OSCAR), nuclear factor of activated T cells (NFAT)c1 and cathepsin K in BMMs treated with RANKL. Particularly, Saussureae Radix greatly inhibited the protein expression of c-fos and NFATc1. especially in the case of NFATc1 expression, a master transcription factor of the differentiation of osteoclasts is very important step for osteoclastogenesis. These results demonstrate that Saussureae Radix may be useful treatment option of bone-related disease such as osteoporosis and rheumatoid arthritis.
본 저자들은 괴사성 근막염 환자에서 간농양이 발생한 사례가 있어 보고하고자 한다. 5일 동안 지속되는 우측 대퇴부의 통증을 동반한 부종, 발열, 오한을 주소로 51세 남자 환자가 본원에 내원하였다. 내원 당시 시행한 MRI 검사 상 우측대퇴부의 후내측과 후외측 부분에 공기 방울로 보이는 다량의 어두운 음영과 액체 군집이 있어 괴사성 근막염으로 추정 진단하였다. 입원 당시 시행한 복부 초음파 검사 상 양측 신장의 실질 에코는 증가하였지만 간 내 농양은 보이지 않았다. 근막절개 10일 후에 시행한 복부 컴퓨터 단층촬영 검사 상 간 내 농양이 보여, 초음파 검사를 통한 경피적 배액술을 시행하였다. 우측 대퇴부와 간농양에서 배약된 농 검체 모두 폐렴 간균이 배양되었다. 환자는 몇 차례 근막 절개술을 시행 받았고 비경구항생제로 치료한 결과 점차 호전이 되기 시작했다. 치료 시작 5주 후 간농양의 크기는 감소하였고, 10주 후 간농양은 사라졌다. 우리가 아는 한 폐렴 간균 괴사성 근막염에서 간농양이 발생한 첫 사례이기에 보고하는 바이다. We report a case of a patient with necrotizing fasciitis followed by liver abscess. A 51-year-old man was admitted to our hospital with a 5-day history of fever and chills with painful swelling of the right thigh. A magnetic resonance imaging (MRI) scan showed fluid collection with numerous dark signal intensities considered as air-bubbles between the posteromedial and posterolateral groups of the right thigh, resulting in presumptive diagnosis of necrotizing fasciitis. At the time of admission, an ultrasonograph of the abdomen showed increased parenchymal echogenicity of both kidneys and no liver abscess. Ten days after fasciotomy, an abdominal computed tomography (CT) scan showed intrahepatic abscess. Sonography-guided percutaneous drainage was performed. Both cultures of pus specimens from the liver abscess and right thigh yielded Klebsiella pneumoniae (K. pneumoniae). The patient was treated with fasciotomy several times and parenteral antibiotics, after which he began to improve. After 5 weeks, liver abscess size was reduced, and after 10 weeks, liver abscess disappeared. To the best of our knowledge, this is the first case of K. pneumoniae-necrotizing fasciitis followed by liver abscess.
Atractylodis Rhizoma Alba is commonly used herbal medicine and it has been known that has immuno-regualtory effects and anti-cancer effects. The inhibition of osteoclastogenesis is essential for the prevention and treatment of osteoporosis. The aim of this study was to evaluate the effects of Atractylodis Rhizoma Alba on osteoclast differentiation in vitro and on resorbing activity of osteoclast. Osteoclast formation was evaluated in bone marrow cells (BMC) in the presence or absence of Atractylodis Rhizoma Alba. The expression of c-fos, tartrate-resistant acid phosphatase (TRAP), OSCAR, DC-STAMP, cathepsin K, MafB and NFATc1 mRNA in osteoclast precursor were assessed by RT-PCR. The levels of TNF receptor-associated factor-6 (TRAF-6), c-fos and NFATc1 protein were assessed by Western blot analysis. Also the correlation with MAPKs and NF-kB pathways were measured by using Western blot analysis. With bone resorption study, I tried to evaluate the inhibitory effects of Atractylodis Rhizoma Alba on mature osteoclast function. Atractylodis Rhizoma Alba inhibited the RANKL induced osteoclastic differentiation from bone marrow macrophage in a dose dependant manner without cellular toxicity. Gene expression of c-fos and NFATc1 was significantly down regulated with Atractylodis Rhizoma Alba treatment. Atractylodis Rhizoma Alba markedly inhibited the RANKL-induced osteoclastogenesis through suppression of nuclear factor kappa b (NF-κB) pathway, down stream pathway of p38, ERK and JNK pathway. Taken together, I concluded that Atractylodis Rhizoma Alba have beneficial effect on osteoporosis by inhibition of osteoclast differentiation and by inhibition of functioning osteoclast. Thus I expect that Atractylodis Rhizoma Alba could be a treatment option for osteoporosis.
Dapsone은 이전에 주로 나병 치료제로 많이 이용되었으나 최근 피부 질환 뿐 아니라 류마티스 질환 등 여러 질환의 치료에 이용되고 있다. 이에 의한 부작용 중 흔하지 않으나 치명적인 경과를 보이는 dapsone 증후군이 나타날 수 있고 호산구 증가가 동반되면 DRESS 증후군으로 진단할 수 있다. 저자들은 베체트 증후군의 점막 병변의 치료를 위해 dapsone을 사용한 환자에서 약물 사용 3주 후 발생한 고열, 피부발진, 호산구 증가와 늑막액 발생, 간, 비장 종대를 동반한 간 효소 효치의 현저한 상승으로 입원하였다가 전격성 간부전 상태로 판단되어 간이식을 고려중인 상태에서 dapsone에 의한 DRESS 증후군으로 진단하고 고용량의 스테로이드제재를 투여 후 간 이식 없이 호전을 보인 1예를 경험하여 보고하는 바이다. Dapsone has been used for several dermatological conditions such as immunobullous disease and ulticarial vasculitis. Dapsone is very useful drug for treating the mucocutaneous manifestations of Behcet`s disease. The widely recognized side effects of dapsone are headache, methemoglobinemia and hemolysis. The severer, but rarer side effect of dapsone is known as dapsone hypersensitivity syndrome, which consists of exfoliative dermatitis, hepatitis, lymphadenopathy and hemolytic anemia. When this is associated with eosinophilia, we can diagnose and treat this drug reaction with eosinophilia and systemic symptoms (DRESS). DRESS is a syndrome of fever, rash, and internal organ involvement that`s secondary to administering the offending medication. We describe here a 47-year-old woman who was hospitalized with fever, skin rash, hemolytic anemia, lymphadenopathy, eosinophilia, pleural effusion and life threatening hepatitis, which could lead to hepatic failure, within three weeks of starting of dapsone therapy for controlling her oral and genital ulcers. We diagnosed the woman as suffering with DRESS syndrome and we started treatment with corticosteroid. Three weeks after starting therapy, her symptoms almost completely resolved and she was discharged.
Osteoporosis is the leading underlying cause of fractures, particularly in postmenopausal women, due to the loss of estrogen-mediated suppression of bone resorption. More than 50% of adults 50 years of age or older are estimated to have osteoporosis. Osteoclast which is main target for treatment of osteoporosis is originated from hematopoietic cell line. Aloe has been widely used in worldwide country as a coadjuvant medicine. Extracts of the leaves of Aloe have been used in condition to improve dermatologic problem such as seborrheic dermatitis, aphthous stomatitis, xerosis, lichen planus and has been known to exert anti-inflammatory, anti-oxidant and anti-tumor effects. However, despite the popularity of aloe as a plant food supplements, the evaluation of its efficacy as a possible therapeutic option for osteoporosis remains scarce. Thus, we evaluated the effect of Aloe on receptor activator of nuclear factor-κB ligand (RANKL)-induced osteoclast differentiation. Here we found that Aloe significantly inhibited osteoclast differentiation induced by RANKL. Aloe suppressed the activation of p38 pathway and NFκB in bone marrow macrophages (BMMs) treated with RANKL. Also, Aloe significantly inhibited the mRNA expression of c-Fos, tartrate-resistant acid phosphatase (TRAP), osteoclast-associated receptor (OSCAR), nuclear factor of activated T cells (NFAT)c1 and cathepsin K in BMMs treated with RANKL. Particularly, Aloe greatly inhibited the protein expression of c-fos and NFATc1. Taken together, our results suggested that Aloe may be useful tool for treatment of osteoporosis by inhibition of osteoclast differentiation.

To prevent and treat the osteoporotic fracture, more attention should be paid in old age patients. Osteoclast which has ability to bone resorption is originated from hematopoietic cell line and plays a key role osteoporotic bone loss. Rubi Fructus has been widely used in Oriental medicine. Extracts of the leaves and fruit of Rubus species have been used in various countries as natural remedies to treat diabetes, infections, colic, and burns. However, the effect of extract of Rubi Fructus (fruit of Rubus coreanus Miq.) in osteoclast differentiation remains unknown. Thus, we evaluated the effect of Rubi Fructus on receptor activator of nuclear factor-κB ligand (RANKL)-induced osteoclast differentiation. Here we found that Rubi Fructus significantly inhibited osteoclast differentiation induced by RANKL. Rubi Fructus suppressed the activation of p38 pathway and NFκB in bone marrow macrophages (BMMs) treated with RANKL. Also, Rubi Fructus significantly inhibited the mRNA expression of c-Fos, tartrate-resistant acid phosphatase(TRAP), osteoclast-associated receptor (OSCAR), nuclear factor of activated T cells (NFAT)c1 and cathepsin K in BMMs treated with RANKL.
