Purpose: Patients with gastroesophageal reflux disease without esophagitis show varying responses to proton pump inhibitors(PPIs). The aim of this study was to objectively evaluate the effect of a new PPI, ilaprazole, on patients with heartburn but withoutreflux esophagitis. Materials and Methods: This prospective study was performed on 20 patients with heartburn but without reflux esophagitis. Allpatients underwent upper endoscopy and 24-hr combined multichannel intraluminal impedance and pH esophageal monitoring(MII-pH). They were then treated with ilaprazole (20 mg) once daily for 4 weeks. The GerdQ questionnaire, histologic findings,and inflammatory biomarkers were used for assessment before and after ilaprazole. Results: Among the 20 patients, 13 (65%) showed GerdQ score ≥8. Based on MII-pH results, patients were classified as true nonerosivereflux disease (n=2), hypersensitive esophagus (n=10), and functional heartburn (n=8). After treatment, patients showeda statistically significant improvement in GerdQ score (p<0.001). Among histopathologic findings, basal cell hyperplasia, papillaryelongation, and infiltration of intraepithelial T lymphocytes improved significantly (p=0.008, p=0.021, and p=0.008; respectively). Expression of TNF-α, IL-8, TRPV1, and MCP-1 decreased marginally after treatment (p=0.049, p=0.046, p=0.045, andp=0.042; respectively). Conclusion: Daily ilaprazole (20 mg) is efficacious in improving symptom scores, histopathologic findings, and inflammatorybiomarkers in patients with heartburn but no reflux esophagitis.

스마트폰 시청 후 나타난 근거리 사위 및 폭주근점의 변화

Purpose: This study was to compare changes in near lateral phoria and near point of convergence after viewing smartphones and monitors during short periods. Methods: 50 subjects with healthy eyes from 20s to 30s (mean age, 21.84±5.13 years) were examined. Viewing time was set at 5 min to 20 min, and recovery time was 10min. Near lateral phoria and near point of convergence (NPC) were examined before and after watching smartphones and monitors, and subjectively symptoms for visual fatigue were surveyed using a questionnaire before and after viewing image. Results: The tendency of phoria in viewing smartpones found more exophoric results than in viewing monitors, and both exophoric shifts were recovered after 10 min. The receded NPC in the smartphones was more remote than in the monitors. The smartpones induced more visual fatigue than the monitors. Conclusions: Sustained smartphone works induced changes in phoria and NPC, and this type of stress could cause visual fatigue. Therefore, an appropriate rest breaks can be helpful in relieving visual fatigue. 목적: 본 연구는 스마트폰과 모니터 영상을 짧은 기간 동안 시청한 이후 근거리 사위와 폭주근점의 변화를 비교하였다. 방법: 20~30대(평균나이 21.84±5.14세)의 안질환이 없고 건강한 눈을 가진 50명을 실험 대상자로 하였다. 시청 시간은 5~20분으로, 회복 시간은 10분으로 구성하였다. 근거리 사위와 폭주근점은 스마트폰과 모니터의 시청전후로 검사하였고, 시각적 피로도에 의한 자각증상은 영상 시청 전후 설문지로 조사하였다. 결과: 스마트폰 영상시청에서 나타난 외사위 경향은 모니터보다 큰 것으로 나타났고, 이러한 외사위의 변화는 10분 후 회복하였다. 스마트폰 시청 이후 나타난 폭주근점은 모니터에서보다 멀어지는 경향을 보였으며, 스마트폰은 모니터보다 더 많은시각적 피로를 유발하였다. 결론: 지속적인 스마트폰의 작업은 사위 및 폭주근점의 변화를 초래하였고, 이러한 유형의 스트레스로 시각적 피로를 초래할 수 있다. 그러므로 적절한 휴식은 시각적 피로의 회복에 도움을 줄 수 있을것으로 사료된다.

Rapidly resolved IgG4-related retroperitoneal fibrosis after steroid pulse therapy

Retroperitoneal fibrosis (RF) is a disorder characterized by the presence of a retroperitoneal mass and concurrent systemic inflammation. Some cases of RF are recognized as belonging to the spectrum of immunoglobulin G4-related disease (IgG4-RD). Glucocorticoids are highly effective for treatment of retroperitoneal fibrosis, although the optimal dose and duration of therapy have not been established. An initial dose of prednisone (40-60 mg) daily is usually administered with a tapering scheme. We report on a 55-year-old man diagnosed with IgG4-related RF and successfully treated with a 3-day course of daily 250 mg (4 mg/kg) intravenous methylprednisolone, which resulted in the prompt resolution of urinary obstruction and systemic symptoms.

‘Miho’ (Milyang300) was developed as a processing rice by means of pedigree selection. ‘Milyang300’ has slightly opaque seedswith low amylose content (11.2%), unlike cultivars such as Baegjinju and Milky-queen, which have seeds with low amylose but are opaque. ‘Milyang300’ was selected and named ‘Miho’ in 2016. ‘Miho’ is a late-maturing ecotype with an average heading date of August 21st. Itshows mild resistance to leaf blast and strong resistance to rice stripe virus, and bacterial blight (K1-K3); however, it is susceptible to brownplant hopper infestation. The yield capacity of ‘Miho’ determined over 3 years of regional yield trials was 565 kg/10 a. In terms of processingproperties, ‘Miho’ shows no significant differences in textural properties when stored for 6-48 h at 3℃ and -18℃. Moreover, the physicalproperties of ‘Miho’ remain unaltered during freeze-thaw processes. Thus, the low-amylose rice ‘Miho’ may be suitable for the productionof refrigerated and frozen cooked rice (Registration No. 7656).
