        갑상선 수술 후 음성 변화에 대한 비대면 음성언어치료 증례

        Voice therapy is effective in many voice and speech disorders. However, patients have low accessibility to therapeutic facilities due to disease-unrelated reasons such as lack of time and pandemic of COVID-19. Contactless voice therapy could be an alternative and may helpful to all patients with voice and speech problems. We developed contactless voice and speech therapy program on the necessity of improving accessibility. Herein, we report the first case of voice therapy to 30 year-old female patient who complained voice change after total thyroidectomy using contactless voice and speech therapy service platform in Korea.

        요추 추간판 탈출증 치료 중 진단된 다발성 골수종 1례

        Objectives: The aim of this study is to notify incidental diagnosis of multiple myeloma during conservative treatment for lumbar disc herniation, caused by right flank pain. Methods: A 45-year-old female came to our hospital for the control of pain caused by lumbar disc hemiation L5-S1. After using Cox technique, she occured right flank pain and found out multiple fracture by bone scan. For further evaluation, she was diagnosed with multiple myeloma with bone marrow biopsy. Results and Conclusions: This case report suggests that clinicians should consider the possibility of multiple myeloma in the case of multiple fracture without trauma.

        Synchronized Oxygen Delivery and Its Optimization Method: A Bench Study

        Long-term oxygen therapy (LTOT) has been widely used to treat patients with chronic obstructive pulmonary disease. The traditional oxygen delivery methods used for LTOT are continuous flow oxygen (CFO) and demand oxygen delivery (DOD). CFO wastes a considerable amount of oxygen, whereas DOD often makes patients feel uncomfortable because it abruptly supplies a large amount of oxygen at the onset of inhalation. Hence, we developed an algorithm for predicting the onset of inhalation, which allowed oxygen to be supplied smoothly before inhalation. Moreover, we minimized the discomfort index (DI) to offer more comfortable oxygen delivery. By integrating the prediction algorithm and the minimization of DI, the previous synchronized demand oxygen delivery (SDOD) method was modified. We constructed a bench model to validate the modified SDOD. The results showed that the proposed algorithm accurately predicted the onset of inhalation. The difference in the real-time measured and predicted values for the beginning of inhalation was less than 0.10 s. Using the proposed minimization technique, the DI was decreased by 50% under 20 breaths per minute when compared with the DI calculated from a previous study. In conclusion, the modified SDOD could supply oxygen more comfortably while synchronizing with patient breathing patterns.

        The Rolling Scotch Yoke Mechanism Applied to a Small Air Compressor for Oil-Free Operations

        The rolling scotch yoke mechanism (RSYM), a variation of the scotch yoke mechanism, is developed in this work. The RSYM is not a mechanism by rigorous definition because the output position is not uniquely determined by the input position, however has several advantages over other reciprocating mechanisms. Unlike a conventional scotch yoke mechanism whose crank pin slides relative to the yoke, the bearing wrapping crank pin of the RSYM rolls without slipping on the yoke in the most range of operation except two small intervals where it slips. Because the friction force is very small in these brief slip intervals, the mechanism generates a much smaller amount of heat than the conventional scotch yoke mechanism. This enables the RSYM to operate in an oil-free condition while retaining all advantages of the scotch yoke mechanism. A double-cylinder air compressor for an oil-free operation was constructed by adopting the RSYM and tested to demonstrate practicality of the design.

        정유를 구성하는 분자들의 특성에 대한 양자화학적 연구

        In order to understand the characteristics of molecules constituting essential oil, ab intio calculations were carried out for 18 molecules produced by the Mevalonate biosynthetic pathway. As a result of the calculation, we proposed HOMO and LUMO, which are useful for predicting the site of nucleophilic, electrophilic, and radical reaction. And the nucleophilicity of essential oil molecules calculated by HOMO and LUMO energies was greater than the electrophilicity also. It showed that a good linear relationship between electronic chemical potential and electro accepting power. And we have reported the dipole moment and ㅔpolarizability, which are physical properties of essential oil molecules. In the reaction of molecules constituting essential oil with oxygen, the values ​​of the reaction Gibbs energy were –2814.14kcal/mol ~ -2542.33kcal/mol and the reaction enthalpy ranged -2875.03kcal/mol to -2605.94 kcal/mol. From this, we concluded that the reaction with oxygen occurs spontaneously with work productivity and is an exothermic process. These results will be helpful for future molecular studies of essential oils. 정유를 구성하는 분자의 특성을 이해하기 위하여 Mevalonate 생합성 경로로 만들어지는 18종의 분자를 대상으로 ab intio 계산을 하였다. 계산 결과로 친핵, 친전자, 친라디칼 반응의 위치를 예측함에 유용한 HOMO, LUMO를 제안하였으며, HOMO, LUMO 에너지로 계산 한 정유 분자들의 친핵도는 친전자도 보다 크게 나타남을 보였으며, 전자적 화학 퍼텐셜과 전자 받기 세기와는 좋은 선형관계를 보였다. 그리고 정유 분자의 물리적인 성질인 쌍극자 모멘트와 편극도를 보고하였다. 성분 분자와 산소의 반응에서는 반응 깁스 에너지와 반응 엔탈피의 값은 각각 -2814.14kcal/mol ~ -2542.33kcal/mol 범위 값과 -2875.03kcal/ mol ~ -2605.94kcal/mol 범위의 값을 가졌다. 이로부터 산소와의 반응은 일생산성으로 자발적으로 일어나며, 발열 과정임을 알 수 있다. 이러한 결과는 향후 정유 분자 연구에 도움이 될 것이다.

        UAM의 성능기반항법을 위한 통합시스템오차 분석 및 감시/경고에 대한 연구

        The total system error in urban air mobility is examined by monitoring and alerting of issues at the corridor within performance-based navigation. The various components constituting the system should be scrutinized to ascertain the inherent error characteristics. In instances of flight technical errors, alterations in characteristics are identified based on the turbulence intensity. Then, the estimated total system error is employed to calculate the range of the required navigation performance (RNP). The result is aligned with the prerequisites for an accurate performance-based navigation and the maximum operating environment. Actual navigation performance is determined by considering the total system error. The error probability circle, which is calculated based on the actual navigation performance, is applied to establish the actual location. The monitoring and alerting capabilities are ensured to be within the computed RNP range.

        화학 소작술과 전기 소작술을 동시에 활용한 제 4형 새열낭종의 치료

        Background and Objectives:Fourth branchial cleft cyst is a rare congenital anomaly which cause a recurrent cervical abscess. Complete excision of fourth branchial cleft cyst is difficult because of a complicated fistula tract. In addition to attempting chemocauterization with trichloroacetic acid (TCA) to avoid surgical complications, authors performed an electrocauterization to close internal opening of pyriform sinus. Materials and Methods:We reviewed ten patients of fourth branchial cleft cyst underwent TCA chemocauterization and electrocauterization simultaneously. Clinical characteristics including patient informations, medical records, treatment results were analyzed retrospectively. Results:Interval time until diagnosed with fourth branchial cleft cyst was variable from several days to decades. Five patients had a history of incision and drainage. Mean follow up period was 36.1 months and all patients were treated with no recurrence. Conclusion:TCA chemocauterization with electrocauterization can be a effective choice to reduce recurrence rate and ensure safety of patients of fourth branchial cleft cyst.
