Objectives : This study was performed to elucidate the pro-apoptotic effect of Artemisiae capillaris herba extracted with ethanol on biliary tract cancer cells. Materials and Methods : The biliary tract cancer cell line SNU-1196 was used in this study. Cells were treated with different concentrations of Artemisiae capillaris herba for 24, 48 and 72 hours. After the treatment, cell viability, apoptosis, caspase activities and the mRNA expressions of the Bcl-2, Bax, P53, and P21 were measured by using MTT assay, cell cycle analysis, apoptosis assay, and RT-PCR. The cell cycle analysis was done by flow cytometry and apoptosis assay by cell death detection ELISA kit. Results : Artemisiae capillaris herba inhibited proliferation of SNU-1196 in long-time culture group with dose-dependent manner. All cells treated with Artemisiae capillaris herba showed increased apoptosis with dose- and time-dependent manner. Exposure of SNU-1196 to Artemisiae capillaris herba induced caspase-3 activation. However, apeoptosis was blocked when SNU-1196 was treated together with the pan-caspase inhibitor Z-VAD-FMK and the caspase-3 inhibitor Z-DEVE-FMK. After the treatment of Artemisiae capillaris herba, the mRNA expressions of caspase -3, -8, -9, p53, and p21 was increased in all cells. Artemisiae capillaris herba resulted in a significant decrease in Bcl-2 and an increase in Bax mRNA levels. Conclusions : These results suggest that Artemisiae capillaris herba would be beneficial in the treatment of biliary tract cancer.

Purpose: To report a rare case of solitary eyelid schwannoma. Case summary: A 60-year-old female visited our clinic with a small cyst-like mass of the left upper eyelid, which had developed 3 years earlier. A 3×3 mm-sized, nonpigmented, cyst-like mass was located in the lateral area of the left upper eyelid margin. We performed an excisional biopsy under local anesthesia. The tumor was well encapsulated, easily dissected, and could be completely removed. Histopathologic examination showed cellular areas of spindle cells with Verocay bodies forming palisades and less cellular myxoid areas. Immunohistochemical analysis for S-100 protein showed a strongly positive reaction. Based on the histopathologic features, a diagnosis of benign Schwannoma was made. Conclusions: Schwannoma should be also considered in the differential diagnosis of a well-circumscribed mass lesion in the eyelid.

This paper is an illustration attempting approach of comparative culture between literary text and literary anthropology and it analyzes Lee Chung-jun’ 〈The island swallow the mythology〉 quoting the sections of Poyatos’ methodology. This work enhances the inside view of human and the spirit of times and once being a journalist, the aspect of the anthropologist of the writer who completely tries to gather data to find folklore materials and culture of ‘shaman ritual drama(rite)’ and ‘regret(emotion)’ and ‘mythology(symbol)’ ready for the elements of traditional culture in itself, and so it’s enough for the explanation of literary anthropology. In order to interpret literary anthropological on literary text in detail the present writer adopted the necessary part of Poyatos’ methodology, an authority of this field to analyzes the work. Those are ‘intelligible systems’ on ‘anthropological realism’ and ‘[Documentary(Historical) realism]’. And also the material that gives awfully off the emotion of Korean tradition becomes one of the realism of culture on the subject of continuing the writer’s view of human looking at an shaman with the sight of humanity, the view of literature delivering lives of human putting love for mankind, the view of history criticizing of pressing the individual, the view of world(identity) through the experience of traditional culture. Expanding a literary study into the dimension of culturology, that is, mean that it develops the sense of the sight watching literature. On the contrary, there was more or less limitation in closer analyzes the text because literary anthropology is related to the outside the text of culture, not inside the text. In conclusion Lee Chung-jun’ 〈The island swallow the mythology〉, is rather an understanding about the lives of the people within the boundary of Korean history and traditional culture than deeply searches universal human problem of the whole mankind.

폐경 전 중년여성에서 대사증후군과 골밀도의 연관성

연구목적: 폐경 전 중년여성에서 대사증후군의 임상적 지표와 골밀도와의 연관성에 대해 조사하고자 하였다. 연구재료 및 방법: 308명의 여성 중 정상대조군은 268명, 대사증후군 환자는 40명이었다. 신체계측 (키, 체중, 체질량지 수, 허리둘레 등) 및 혈청 지질검사, 공복혈당, 혈압 등의 자료를 수집하여 골밀도와 함께 분석하였다. 결 과: 연구대상에서 정상군과 대사증후군 환자군 간의 대퇴골 경부 및 요추 골밀도의 유의한 차이는 관찰되지 않았다. 반면, 대퇴골경부 골밀도는 체질량지수 (r=0.209, P<0.001) 및 허리둘레 (r=0.142, P=0.012)와 양적 상관성을 보였으며, 요추 골밀도도 역시 체질량지수 (r=0.176, P=0.002) 및 허리둘레 (r=0.113, P=0.048)와 유의한 연관성이 있음을 관찰하였다. 결론: 폐경 전 여성에서 체지방의 증가와 복부비만화는 골밀도의 증가와 연관성을 보였으며, 대사증후군의 다른 임상적 지표들은 유의한 관계가 없었다. 폐경 전 대사증후군에 이환된 여성에서 다양한 임상적 특성들이 골밀도에 어떠한 영향을 미치는지에 대해 장기적 추적관찰을 통한 연구가 필요할 것으로 생각된다. Objectives: To determine the relationship between metabolic syndrome (MS) and bone mineral density (BMD) in middle-aged women. Methods: Three hundred eight premenopausal women were included in this study, including 268 women without MS and 40 women confirmed to have MS according to the National Cholesterol Education Program-Adult Treatment Panel III (NCEP-ATP III) criteria. Clinical markers including anthropometric parameters, the serum lipid profile, and glucose level were determined and analyzed with the BMD at the lumbar spine and femoral neck. Results: There was no significant difference in femoral neck or lumbar BMD between the controls and women with MS. In correlation tests, the body mass index (BMI) showed a significant association with the femoral (r=0.209, P<0.001) and lumbar BMD (r=0.176, P=0.002). In addition, the waist circumference (WC) was also significantly correlated with the femoral (r=0.142, P=0.012) and lumbar BMD (r=0.113, P=0.048). Conclusion: The BMI and WC showed a significant positive association with the femoral and lumbar spine BMD in premenopausal women. Further studies with a large population will be needed to elucidate the combined effect of MS on BMD in middle-aged women. (J Korean Soc Menopause 2010;16:142-147)

Rare complications of distal tibia fracture are claw-toe deformity, cavus deformity and checkrein deformity. These complications may be due to contracture of the muscles of the deep posterior compartment of the leg after a compartment syndrome. These 4 cases were treated by selective tendon lengthening of flexor tendon at the retromalleolar level.

세 명의 산부인과 전문의별 복강경하 전자궁절제술의 학습곡선

목적: 복강경하 전자궁절제술은 전통적인 개복하 전자궁절제술의 대체 방법으로 점차 많이 시행되고 있으며 학습곡선의 변환점을 찾는 것은 훈련 과정을 계획하는 데 도움을 줄 수 있다. 이번 연구에서는 동일 기관에서 세 명의 다른 산부인과 전문의의 복강경하 전자궁절제술의 학습곡선을 비교하여 평균 변환점을 찾아보았다. 연구 방법: A, B, C 세 명의 산부인과 전문의에게 수술받은 첫 140명의 환자에 대해 후향적으로 분석하였다. 각 집도의에게 수술받은 환자들은 수술받은 순서대로 20명씩 7개의 그룹으로 나뉘어졌다. 각 집도의 간에 수술받은 환자의 나이, 자궁 무게, 수술 시간, 수술 전후의 혈색소치 변화를 비교하였다. 각 집도의별로 수술 시간과 수술 전후 혈색소치 변화를 이용하여 학습곡선을 구하였다. 결과: 수술 시간으로 그린 학습곡선은 세 집도의 모두에서 80~100건에서 변환점을 보였다. 수술 전후 혈색소치 변화로 그린학습곡선은 경험에 따라 감소하는 경향을 보이지 않았다. 세 집도의에게 수술받은 환자의 나이와 수술 전후 혈색소치 변화에는 차이가 없었다. 하지만 평균 자궁 무게는 C 집도의에게 수술받은 경우가 나머지 집도의에게 수술받은 경우보다 유의하게 가벼웠고 평균 수술 시간은 C 집도의에서 유의하게 길었다. 결론: 산부인과 전문의가 복강경하 전자궁절제술 학습곡선의 변환점에 다다르기 위해서는 적어도 80~100건의 수술 경험을 필요로 한다. 이러한 결과는 향후 복강경하 전자궁절제술의 훈련 과정에 길잡이로 활용할 수 있겠다. Objective: Total laparoscopic hysterectomy (TLH) is becoming more commonly used as an alternative to traditional abdominal hysterectomy and Analyzing the turning point of a learning curve can be useful in planning training programs. This study was to define the average turning point of a learning curve of TLH by comparing three separate gynecologists in one institute. Methods: Retrospective analysis of the first 140 consecutive cases of TLH performed by three separate gynecologists A, B, and C. Patients of each gynecologist were divided into 7 equal groups of 20 operations classed chronologically. Patient`s age, uterus weight, operation time and pre-post operative hemoglobin difference of the three gynecologists were compared. Operation time and pre-post operative hemoglobin difference were evaluated to build learning curves for each gynecologist. Results: Learning curve built by operation time showed turning point after 80~100 cases in all three gynecologists. Learning curve built by pre-post operative hemoglobin difference did not show a decreasing pattern. There were no statistical differences in patient`s age and pre-post operative hemoglobin difference between the three gynecologists. However, mean uterine weight of gynecologist C was significantly lighter than that of gynecologist A and B. Operation time was significantly longer in cases by gynecologist C than in cases by A and B. Conclusion: At least 80~100 cases of experience in TLH is needed for a gynecologist to reach the turning point of the learning curve. This result can be used as a guide to the training program of TLH.

기능성 표면 구조를 갖는 인공 관절 관절면의 윤활성 향상

목적: 인공 관절 관절면의 마모 특성 향상을 목적으로 하여, 임상에서 사용되고 있는 인공 고관절의 관절면 표면에 요철 패턴을 형성하여 관절면의 윤활 특성을 검토하였다. 대상 및 방법: 요철 패턴의 기능성표면이 부가된 Co-Cr 합금제의 골두와 초고밀도 폴리에틸렌제의 비구컵의 윤활 특성을 검토하기 위하여 마찰 계수의 경시 변화, 표면 조도, 마모량 등을 측정하였다. 기능성 표면은 0.3-0.8 ㎜의 직경을 갖는 구멍이 1.2㎜의 피치로 연속적으로 배열되어 있다. 관절면에 부가된 기능성 표면을 갖는 인공 고관절을 진자식 관절 시 뮬레이터를 이용하여 평가하였다. 결과: 패턴을 부여한 골두 및 무 패턴 골두와 비구컵을 관절 운동시킨 경우 마찰 계수는 거의 같은 값을 나타내었다. 그러나, 마모량을 살펴 보면 무패턴 골두와 비교하여 패턴을 부가한 골두의 경우 비구컵의 마모량을 최대 45%까지 감소되었으며, 골두의 표면 조도는 무 패턴의 경우가 요철 패턴의 경우보다 3배 정도 높은 것으로 나타났다. 결론: 이러한 결과는 인공 고관절에 기능성 표면을 부가함으로써 윤활 특성이 향상되는 것을 나타내었다. 따라서, 패턴의 부가에 의한 마찰 계수의 현저한 감소는 볼 수 없었으나, 관절면의 표면 조도, 마모량에 대해서는 우수한 특성을 나타내고 있어, 패턴부가에 의하여 인공 관절 관절면의 윤활 성능이 향상된 것을 알 수 있었다. Purpose: A new surface structure with a regular patterning of the frictional surfaces of the artificial joints were designed to improve the wear problems. Materials and Methods: The lubrication properties for the cup (ultra high molecular weight polyethylene, UHMWPE) vs. ball (Co-Cr alloy) were examined to evaluate the coefficient of friction, wear and surface roughness. The surface structure is a dent pattern, which has diameter of 0.3-0.8 ㎜, a pitch of 1.2 ㎜. In this study, such surface patterns were applied to artificial hip joints, and the effects of regular patterning on the frictional surface were examined using a pendulum type hip joint simulator. Results: The coefficient of friction between the cup and a patterned ball showed the same value between the cup and the non-patterned ball. However, the amount of wear of the cup, which was in contact with the patterned head, was only 45% that of the cup, which was in contact with the non-patterned head, and the surface roughness of the cup with the non-patterned head was three times greater than that of the cup with a patterned head. Conclusion: These results demonstrate that the lubrication properties can be improved by patterning the frictional surface of the artificial joint. Consequently, patterning on the frictional surfaces can exted the life of an artificial joint.
